A Retrospective Study of Chronic Sinusitis in Children Treated by Korean Medicine - Using Paranasal Sinus Water's View (한방 요법으로 치료된 소아 만성 부비동염 33례에 대한 후향적 연구 - 부비동 단순 X선 촬영을 이용하여)

  • Objectives The purpose of this study is to examine 33 cases of chronic sinusitis in children who were treated with Korean medicine. Methods Herbal medicine, acupuncture, and nebulizer and laser therapy were used to treat chronic sinusitis, and a plain radiography (paranasal sinus water's view) was used to confirm improvement of the children's sinusitis. Then, the data was classified by sex, age, frequency and duration of common cold, severity of sinusitis, duration of treatment, pattern identification and so on. Results There were 72.7% boys and 27.3% girls. The average age of children is $4.48{\pm}2.18years$. 70% of the children had common cold more than once a month, and 85.1% of the children from the study group had common cold lasting more than a week. The sinusitis severity score ranged from 10 to 200. 81.8% were more than 100. Duration of the treatment was $157.21{\pm}83.4day$, duration of herbal medicine treatment was $79.3{\pm}31.23day$, rate of herbal medicine was $62.12{\pm}29.21%$. The frequency of acupuncture, nebulizer and laser therapy ranged from 1 time to 11 times but was much lower than expected. Conclusions 33 cases of chronic sinusitis in children were treated with Korean medicine, and cure of the illness was confirmed by PNS water's view, and clinical characteristic of the patients with chronic sinusitis was analyzed.

A retrospective randomized study of success rates according to abutment selection in DENTIS submerged implant with an internal hex connection (DENTIS 내부연결형 서브머지드 임플란트에서 지대주 선택에 따른 성공률의 후향적 연구)

  • PURPOSE. The purpose of this study is to determine the efficacy of the DENTIS submerged-type implant with an internal hex connection and to build corresponding abutment-selection criteria. MATERIAL AND METHODS. A total of 204 patients received submerged implant fixtures with an internal hex connection at the Dong-A University Hospital Dental clinic in Busan from January 2013 and May 2016. Three specific abutments, UCLA abutments, customized abutments, ready-made abutments, were randomly selected. Implant success was defined as the basis of the International Congress of Oral Implantologists(ICOI, 2007) criteria. The relationship between the implant success rate and the abutment factor was analyzed using the Kruskal-Wallis test(P<.05). RESULTS. A total of 508 implants were placed in 204 patients. After a mean observation period of 38.6 months, 493 out of 508 implants were in normal function, yielding an overall success rate of 97.05%. A total of 15 implants failed: 10 in the maxillary molar area, 4 in the mandibular molar area, and 1 in the mandibular incisal area. All of the implant failures occurred in a single-implant prosthesis, especially high in the maxillary molar area. The Kruskal-Wallis analysis showed that abutment selection has no significant correlation with implant failure(P>.05). CONCLUSION. DENTIS submerged implants with an internal hex connection showed predictable results with a success rate of 97.05%. It is no influence on the success rate in the selection of submerged implant abutment with an internal hex connection.

Retrospective Study on Effect of Chuna Therapy, Also Known as Supine JS Cervical Therapy, Combined with Korean Medicine Treatment on Neck Pain and Headache Caused by Whiplash Injury (편타성 손상에 의한 경항통 및 두통 환자의 추나요법 병행 한방복합치료의 효과: 후향적 관찰연구 - 앙와위 경추 JS 신연 교정기법을 중심으로)

  • Objectives To observe the pain reduction effect of Korean medical treatment combined with supine JS cervical therapy on neck pain and headache caused by whiplash injury. Methods The medical records of 30 patients suffering from neck pain and headache caused by traffic accidents from August 1, 2020 to October 30, 2020 were studied. 15 patients receiving supine JS cervical therapy combined with Korean medical treatment were assigned to group A and another 15 patients receiving Korean medical treatment but without Chuna therapy were assigned to group B. The following patient data were analyzed retrospectively - age, sex, and number of admission days, neck pain and headache visual analogue scale (VAS), and neck disability index (NDI). Statistical analysis was conducted using IBM SPSS 25.0. for Windows. A p-value≤0.05 was considered to be statistically significant. Results Both neck pain and headache VAS score in group A and B showed a statistically significant decrease. The changes in neck pain and headache VAS score in group A was statistically significant higher compared to those of group B. NDI in group A showed a statistically significant decrease, but NDI in group B did not decreased statistically significant. Conclusions Korean medicine treatment effectively reduces neck pain and headache. Especially, the pain reduction effect is better in patients receiving Korean medicine treatment combined with supine JS cervical therapy than in those receiving Korean medicine treatment without supine JS cervical therapy.

Changes in the characteristics of patients transferred to the emergency room through private institutions during inter-hospital transport after the COVID-19 outbreak : A retrospective study (COVID-19 발현 이후 병원간 이송 시 민간 이송업체를 통해 응급실로 전원된 환자들의 특성 변화 : 후향적 연구)

  • Purpose: The purposes of this study were to determine the changes in the pattern of patients who were transferred to the emergency room through inter-hospital private institutions and to determine how long transport takes following the COVID-19 outbreak. Methods: This retrospective observational study analyzed the emergency medical services reports of private institutions following the COVID-19 outbreak in South Korea. The study was conducted in Busan between January 19, 2019 and January 18, 2020, and between January 19, 2020 and January 18, 2021. Results: Upon comparing the patient transport times during the "Pre-COVID-19 period" and "COVID-19 period," a significant delay was noticed in the preparation for transfer of patients during the "COVID-19 period" (p<.05). There were significantly more patients with respiratory infections and patients who complained of general symptoms during the "COVID-19 period." Moreover, there was a higher frequency of patients who were transferred to a 'Level I' emergency room during the "COVID-19 period" compared to during the "Pre-COVID-19 period" (p<.05). Conclusion: Following the COVID-19 outbreak, there is a delay in patient transport to the emergency room through private institution inter-hospital transport and an increase in the number of patients complaining of respiratory infection symptoms. Thus, emergency medical services need additional administrative and economic support to transport infected patients.

Data Processing and Visualization Method for Retrospective Data Analysis and Research Using Patient Vital Signs (환자의 활력 징후를 이용한 후향적 데이터의 분석과 연구를 위한 데이터 가공 및 시각화 방법)

  • Purpose: Vital sign are used to help assess the general physical health of a person, give clues to possible diseases, and show progress toward recovery. Researchers are using vital sign data and AI(artificial intelligence) to manage a variety of diseases and predict mortality. In order to analyze vital sign data using AI, it is important to select and extract vital sign data suitable for research purposes. Methods: We developed a method to visualize vital sign and early warning scores by processing retrospective vital sign data collected from EMR(electronic medical records) and patient monitoring devices. The vital sign data used for development were obtained using the open EMR big data MIMIC-III and the wearable patient monitoring device(CareTaker). Data processing and visualization were developed using Python. We used the development results with machine learning to process the prediction of mortality in ICU patients. Results: We calculated NEWS(National Early Warning Score) to understand the patient's condition. Vital sign data with different measurement times and frequencies were sampled at equal time intervals, and missing data were interpolated to reconstruct data. The normal and abnormal states of vital sign were visualized as color-coded graphs. Mortality prediction result with processed data and machine learning was AUC of 0.892. Conclusion: This visualization method will help researchers to easily understand a patient's vital sign status over time and extract the necessary data.

Cordyceps Militaris Complex extract Treatment for Rheumatoid Arthritis : A Retrospective Observational Study (동충하초 복합추출물로 치료받은 류마티스 관절염 환자군에 대한 후향적 관찰 연구)

  • The purpose of this study is to observe the effect of Cordyceps Militaris(CM) complex extract treatment on patients diagnosed with rheumatoid arthritis(RA). We reviewed medical records of 9 rheumatoid arthritis patients who visited the pain & rehabilitation center, Daejeon Korean Medicine Hospital of Daejeon University and were treated with CM complex extract from October 14, 2019 to October 14, 2020. We analyzed the records of DAS 28(Disease Activity Score 28), CDAI(Clinical Disease Activity Index), SDAI(Simplified Disease Activity Index), VAS(Visual analogue Scale), and KHAQ-D(Korean Health Assesment Questionnaire-Disability Index) before and after treatment to evaluate the effects of CM complex extract on RA. As a result, the scores of DAS 28(p<0.05), CDAI(p<0.01), SDAI(p<0.01) and VAS(p<0.01) were statistically significantly decreased, after 8 weeks of taking CM complex extract in RA patients. In conclusion, CM complex extract treatment may have potential effects of reducing disease activity and pain and improving quality of life in patients diagnosed with RA.

Treating COVID-19 with Qingfei Paidu Decoction with and without Ephedra Herba: A Retrospective Case Series (청폐배독탕 및 청폐배독탕거마황을 사용해 COVID-19 증상의 개선을 보인 증례 보고 : 후향적 사례군연구)

  • The purpose of this report is to describe the effects of Qingfei Paidu decoction (QPD) and QPD without Ephedra Herba on COVID-19 patients. One COVID-19 patient was treated with QPD and two others who had been sensitive to ephedra were treated with QPD without it. The fever, cough, and sore throat of all three were monitored three times a day through non-face-to-face assessment for a total of five to seven days, and cough and throat pain changes were evaluated with a numeric rating scale (NRS). After treatment, the fever and NRS scores were improved for all three patients. These results indicate that both QPD and QPD without Ephedra Herba is an effective treatment for COVID-19 symptoms.

Comparison of Ultrasound Guided Soyeom Pharmacopuncture Therapy Effect and Unguided Soyeom Pharmacopuncture Therapy Effect on Cervical Facet Joint of Acute Cervical Pain Patient Caused by Traffic Accidents: A Retrospective Study (교통사고로 유발된 급성 경추통 환자의 경추 후관절에 시행한 초음파 유도하 치료군과 비유도 치료군의 소염 약침 치료 효과 비교: 후향적 관찰 연구)

  • Objectives To compare the therapeutic effect of ultrasound guided soyeom pharmacopuncture therapy and unguided soyeom pharmacopuncture therapy on cervical facet joint of patient who have cervical pain caused by traffic accidents. Methods The study was planned as a retrospective study that analyzes the patient's medical records. We analyzed medical records of 20 patients with acute cervical pain caused by traffic accidents from April 1, 2022 to May 31, 2022. 10 patients (Group A) were treated ultrasound guided soyeom pharmacopuncture therapy on cervical facet joint, 10 patients (Group B) were treated unguided soyeom pharmacopuncture therapy on cervical facet joint. We used visual analogue scale (VAS) and neck disability index (NDI) to evaluate improvements in cervical functions and pain, five point Likert scale to estimate patient's satisfaction. We used the IBM SPSS Statistics version 25 program (IBM Co., Armonk, NY, USA) for statistical analysis. Results Group A showed a significant decrease in the VAS on the 5th day of the soyeom pharmacopuncture therapy (p=0.001). However, there was no statistically significant difference of VAS between two groups (p=0.142). Group A showed significant decrease in the NDI on the 5th day of the therapy (p=0.002). Also, there was statistically significant difference on NDI between two groups (p=0.018). Difference of five point Likert scale between two groups was not statistically significant (p=0.407). Conclusions In patients of acute cervical pain caused by traffic accidents, ultrasound guided soyeom pharmacopuncture therapy on cervical facet joint showed significant efficacy on reduction of neck disability compared with unguided therapy.

Musculoskeletal Ultrasound Findings of Knee Pain Patients: A Retrospective Observational Study (근초음파 진단기기를 활용한 슬관절 통증 환자의 초음파 소견: 후향적 관찰연구)

  • The study aimed to understand the knee condition of patients with knee joint pain who visited an Korean Medicine hospital by performing musculoskeletal ultrasound. From July 1, 2021 to October 31, 2021, information on the gender, age, diagnosis, and ultrasound findings of knee joint pain patients were retrospectively collected. To examine the condition of the knee joint, images were taken in a total of three postures. All images were analyzed by an examiner. 7 cases of osteoarthritis, 1 case of medial collateral ligament rupture, and 1 case of quadriceps tendon calcification were collected. It was able to check the condition of articular cartilage, lipoma arborescens, and the formation of osteophytes through musculoskeletal ultrasound. The implementation of musculoskeletal ultrasound can enable Korean medicine doctors to give more active treatment for patients through accurate and scientific diagnosis. Based on more evidence, it is thought that the use of medical devices by Korean medicine doctors will need to be made in the future.

Retrospective Study on the Characteristics of Patients with Scoliosis at the Korean Medicine Hospital (한방병원에 내원한 척추측만증 환자의 특성에 대한 후향적 연구)

  • Objectives This study was conducted to characterize scoliosis patients visiting Korean medicine hospital and to analyze the demands and factors affecting discomfort. Methods This retrospective study analyzed 33 scoliosis patients who visited Korean medicine hospital from March, 2021 to October, 2021. The data analysis consisted of three factors: (1) demographic characteristics, (2) characteristics of demands on Korean medicine (reasons for choosing Korean medical treatment, preferred treatment methods, most uncomfortable part, treatment priorities) and (3) discomfort factors (treatment experiences, diagnosed age and Cobb's angle). Statistical analyses were performed and a p-value≤0.05 was considered to be statistically significant. Results 43.75% of the patients chose 'effectiveness' for the reason why they preferred Korean medicine treatment. 'Chuna treatment' was the most preferred treatment method. The patients chose 'lower back' for the most uncomfortable part and 'pain' for the highest priority of improvement. The Cobb's angle of included patients was 16.02±7.65° and it is not much differ to average of Cobb's angle in Korean. Discomfort was more severe in the patients with treatment-experienced than treatment-naive. The score of discomfort in appearance and psychological were higher in the patients diagnosed in childhood or adolescent period than who were diagnosed after adult. Classification based on Cobb's angle showed no statistical difference. Conclusions Not only Cobb's angle but also other clinical factors should be considered for effective treatment in scoliosis. Also, It is necessary to pay attention to adult scoliosis patients.
