대한관절경학회지의 학술진흥재단 등재를 위한 준비 -대한관절경학회지의 현재-

  • 정형외과학회 내에도 다양한 세부 분과가 존재하여 다양한 분과 학회가 설립되었다. 대한관절경학회는 1994년 12월 설립되었고, 그 동안 회원들의 많은 논문이 게재되었으나, 연구 업적 평가 시 한국연구재단에 등재되어있는 학술지 중심으로 논문 평가가 이루어지면서 투고 논문이 감소였다. 2009년 현재 한국 연구 재단에 등재된 학회지는 총 1158개 이며, 등재 후부 학회지는 총 573 개 이다. 이중에 의약학 계열의 등재 학회지는 124개이며, 등재 후보 학회지는 63개 이다. 정형외과 내에서는 대한정형외과학회지, 대한골절학회지, 대한스포츠학회지가 등재되었으며, 대한견주관적학회지, 대한고관절학회지, 대한족부족관절학회지, 대한척추외과학회지가 등재 후보 학술지로 선정되어있다. 대한관절경학회지를 한국연구재단의 등재지로 추진하기 위하여 대한관절경학회지 현 상태를 등재후보지 평가기준으로 평가하여, 그 결과를 보고하고 대한관절경학회지의 발전을 위한 문제제기를 하고자 한다.

Effects of the Modified Casual Shoes on Foot Pressure Distribution during Standing: Pilot study (기능성 신사화가 선자세의 족저압력 분포에 미치는 영향: 예비연구)

  • 본 연구는 기능성 신사화 착용이 즉각적인 선자세 족저압력 분포 변화에 미치는 영향을 확인하고자 하였다. 남성 물리치료사 8명을 대상으로 하루 간격을 두고 동일한 대상에게 일반적 신사화와 기능성 신사화를 착용하고 선 자세 동안 높은 족저압력의 발과 낮은 족저압력의 발간에 족저압력 분포를 비교하였다. 비교 분석 결과, 일반화를 착용한 선 자세에서 높은 족저압력의 발과 낮은 족저압력의 발간에 족저압력 비율이 유의한 차이를 보였지만 기능성 신사화에서 유의한 차이를 보이지 않았다. 그리고 높은 족저압력 분포 발에서 기능성 및 일반적 신사화간에 전족부와 후족부의 압력이 차이를 보였다. 선 자세에서 기능성 신사화가 일반적 신사화 보다 즉각적으로 정상 생리적 자세를 유도하는 것으로 보였다.

  • Mesenchymal chondrosarcoma is very rare and mostly occurs in lower extremity, especially femur, head and face, and pelvis in order. An eleven-year old male patient suffered from left heel pain for several months and underwent biopsy at another hospital. A Malignant bone tumor was suspected, so the patient was transferred to our hospital. We reviewed the outside pathologic slide, which revealed the diagnosis of mesenchymal chondrosarcoma. It was treated by a below knee amputation. Three months later, a chest computed tomography (CT) revealed multiple metastatic nodules in both lungs. All metastatic nodules were surgically excised and the patient received postoperative chemotherapy. As far as our knowledge, mesenchymal chondrosarcoma in the calcaneus is the first case in the world. Thus, we report a case of mesenchymal chondrosarcoma in the calcaneus with thorough review of documents.

Fractures & Dislocations Related to Middle and High School Taekwondo Athletes in Competition (중.고교 태권도 선수의 경기 중 발생한 골절과 탈구)

  • Purpose: We intended to classify the pattern and to seek the cause of fractures and/or dislocations related to middle and high school Taekwondo athletes in competition and try to get the ways for preventing them. Materials and Methods: We studied 50 fractures and dislocations of the athletes in competition, which were surveyed in 2005-2007 national middle and high school Taekwondo tournament annually. We analyzed the type and the cause of the injury, performed survey at postinjury l year in 32 cases, sought proper treatments and methods for prevention. Results: The 26 cases (52%) of foot fracture, 15 cases (30%) of hand fracture and 9 cases (18%) of forearm fracture are occurred in this study. The 9 cases (28%) of satisfaction in treatment, 15 cases (47%) of average and 8 cases (25%) of dissatisfaction are surveyed in the 32 cases. The causes of dissatisfaction are lack of rest (7 cases), excessive training (5 cases), immoderate participation of match (6 cases) and the others. The athletes were returned to the match before complete recovery in 21 cases (67%). Conclusion: The causes of fractures and dislocations in middle and high school Taekwondo athletes were stroke on upper and lower extremities and high-speed hyperflexion injury that was occurred during spraining of great toe on the floor. We consider that the athletes need the development of effective protector, improvement of equipment in stadium and enough rest and education.

Analysis of the Recurrence after Surgical Treatment of the Hemangioma in the Extremities (사지에 발생한 혈관종의 수술적 치료 후 재발에 대한 분석)

  • Purpose: To analyse the risk factors for recurrence of hemangiomas in extremities after surgical treatment and to compare with those of trunk. Materials and Methods: 120 cases of hemangioma with surgical treatments from June 1998 to September 2009 were analysed. 53 cases with surgical treatment on trunk in the same period were set to be the control group. We analyze several factors: age, location, site, size, histologic types and correlation between recurrence and each risk factor using logistic regression analysis. Results: Recurrence rate was 11.7% in extremities and 9.4% in trunk. There were no correlation between recurrence and age, site, size, histologic type. But, there was stastically significant correlation between recurrence rate and location, especially hand, forearm, feet in extremities and head and neck in trunk. Conclusion: Recurrence after surgical treatment of hemangioma is highly prevalent in anatomical location such as, hand, foot and forearm those are difficult to achieve complete resection because of close to neurovascular structures. Careful observation should be needed owing to incomplete resection can occurs recurrence.

Mid-Term Results of Modified Hoffman Procedure for Rheumatoid Forefoot Deformity (류마티스 전족부 변형에 대한 변형 Hoffman 술식의 중기 결과)

  • Purpose: A rheumatoid forefoot deformity includes severe hallux valgus and claw toe of the four lateral toes. The authors intended to analyze the mid-term results of the modified Hoffman procedure for a rheumatoid forefoot deformity. Materials and Methods: Twenty-two feet of eighteen patients were followed up for more than two years after the modified Hoffman procedure. The mean age was 54.7 years, and the mean follow-up period was three years and four months. Clinically visual analogue scale (VAS) and American Orthopaedic Foot and Ankle Society (AOFAS) MP-IP score were analyzed preoperatively and postoperatively. The postoperative complications, the satisfaction of patients, and the presence of plantar callosity were also analyzed. Radiographically, the hallux valgus angle, the 1st intermetatarsal angle, the 1st interphalangeal angle, the 1st interphalangeal joint arthritic change after the operation, and the dorsiflexion angle and the time to union of the 1st metatarsophalangeal joint were analyzed. Results: Clinically, the VAS and AOFAS score were respectively improved from 7.1 points and 30.3 points preoperatively to 1.5 points and 83.1 points postoperatively (p<0.001). All patients were satisfied with the results. Plantar callosity disappeared in all cases. Radiographically, the mean hallux valgus angle, the 1st intermetatarsal angle, and the 1st interphalangeal angle changed from 52.8°, 13.3°, and 7.5° preoperatively to 16.2°, 8.7°, and 14.6° postoperatively (p<0.001). The mean dorsiflexion angle of the 1st metatarsophalangeal joint was 17.2° postoperatively. The mean time to radiographic union of the 1st metatarsophalangeal joint was 11.1 weeks. There were two cock-up deformities of the lesser toe, one wound problem, and two hallux interphalangeal joint arthritis as a complication. There were no cases of nonunion of the 1st metatarsophalangeal joint. Conclusion: The modified Hoffman procedure appears to be a safe and satisfactory procedure for a rheumatoid forefoot deformity.

  • 현재까지 NSAID 및 SAID의 사용으로 급성염증의 경우는 잘 조절되고 있으나, 류마티스 관절염과 같은 만성염증성 질환은 극복하지 못하였다. 뿐 만 아니라, 상기의 약물들의 장기간 사용으로 인한 부작용이 문제되고 있다. 그러므로, 만성염증성 질환의 치료를 위한 새로운 계열의 항염증제 개발이 시급하며, 많은 연구자들이 여러 가지 식물추출물을 이용하여 신약개발의 가능성을 타진하고 있다. 이의 일환으로, 본 연구에서는 고전문헌에서 사용된 식물들을 대상으로 하여 Rat의 류마티스 관절염 model을 이용하여 그들의 항염증작용을 연구하였다. 여정자 및 등줄나무를 포함한 27종의 식물을 이용하여 각 methanol 추출물을 조제하고, 매일 경구로 투여하였다 (200 mg/kg/day). 류마티스성 관절염은 rat의 족부에 Mycobacterium butyricum (0.6 mg/rat)을 주사하여 유발시켰고, 2차부종의 억제를 추출물의 활성으로 판정하였다. 그 결과, 27종의 식물중 목통, 마황 및 산두근이 2차부종을 유의성있게 억제하였으며, adjuvant 주사 후 16일에 억제율이 각각 22%, 36%, 13%로 나타났다. 산두근을 분획하여 재검정한 결과 50 mg/kg/day의 용량으로 투여시 EtOAc 및 n-butanol 분획에서 억제능이 나타나, 이들 분획을 대상으로 활성물질의 분리를 계속하고 있다.

A Case Report of Sequela of Operation of Hallux Valgus (족부 무지외반증 수술 후 후유증 치험 1례)

  • Objectives : This study aims to report the effect of Korean medicine treatments on a sequela of operation of hallux valgus. Methods : The patient was treated with Dangkisoosan-gami, Acupuncture treatment including Herbal-Acupuncture and physical therapy. We evaluated treatment effects by circumference of foot and ankle, Edema index score by Body Composition Analyzer, Ankle-Brachial Index (ABI) and Visual Analogue Scale (VAS). Results : After 25 days of treatment, the symptoms such as lower extremity edema, foot pain and ankle pain were improved. Conclusions : In this case, Korean medicine treatments for a sequela of operation of hallux valgus was effective. But further studies are required to confirm the effect of these methods.

  • The disc internal disruption syndrome is not well known to us, but the following hypothesis is widely accepted in clinical practice. The disc internal disruption syndrome may develop intractable back pain with aggrevated of pain, loss of spinal motion with any physical exercise, leg pain, loss of energy, marked weight loss, and profound depression. The patient with this syndrome will be found to have normal plain roentgenograms, myelograms, CT scans, results of blood examination and neurologic findings. For these reasons, this syndrome was frequently diagnosed by abnormal discographic findings. We had experience with two cases of disc internal disruption syndrome with clinical, roentgenographic and discographic evaluations. Thus We present these cases with a brief review of the concerned literature.

Postoperative Rehabilitation of Foot Fracture in Korean Medicine Treatment: Clinical Case Report (족부 골절수술 후 한방 재활치료의 효과: 증례보고)

  • The objective of this study is to propose postoperative rehabilitation for foot fracture in Korean Medicine and to report its effectiveness. There were three patients who were got foot fracture surgery received korean medicine with acupuncture, herbal medicine, cupping and exercise treatment. Patients were evaluated numeric rating scale (NRS), range of motion (ROM), American Orthopaedic Foot and Ankle Society (AOFAS) ankle hindfoot score, foot function index (FFI) and EuroQol-5 dimension (EQ-5D) index. Case 1 and 2 were improved NRS from 6 to 3. Case 3 was improved NRS from 5 to 2. These cases were improved ROM in all directions, AOFAS score, FFI, EQ-5D and walking status. This study suggest that Korean medicine rehabilitation could be effective for patients who have received foot fracture operation.
