Hypopharyngeal Foreign Body Migration Induced Retropharyngeal Abscess (후인두공간으로 이동하여 농양을 유발한 하인두 이물 1예)

  • Foreign bodies in upper aerodigestive tract migrating into retropharyngeal space are rarely encountered emergent cases in otolaryngologic fields. A 60-years-old female presented throat pain and lump sense after a meal. Computer tomography showed metallic foreign body impacted in the retropharyngeal space. A hypopharyngeal perforation was suspected by through flexible laryngoscopy and gastrointestinal endoscopy. It was successfully removed by external cervical approach, and we report this case with a review of the related literatures.

Thyroid Papillary Carcinoma Presenting as Posterior Pharyngeal Mass : A Case Report (후인두 종물로 나타난 갑상선 유두상 암종 1예)

  • Papillary thyroid carcinoma frequently invades the lymph node, trachea, esophagus and perithyroid tissue. However, direct extension to posterior pharyngeal area is known to be rare. A 64-year-old male was referred to our clinic presenting as posterior pharyngeal mass during gastrofiberscopy. The neck CT scan showed soft tissue mass in retropharynx and lymph node in right level III with calcifications. We performed the total thyroidectomy with selective(level II, III, IV) and anterior compartment neck dissection. In operative findings, the right thyroid mass were connected to the retropharynx through the posterior portion of inferior constrictor muscle. Histopathologic findings revealed the papillary thyroid carcinoma extended to retropharynx. We report a unique case with a literature review.

  • Tuberculosis of the retropharynx is extremely rare. The diagnosis is frequently delayed because of its anatomical location and atypical symptom. It would be crucial to consider tuberculosis infection as a possible source of abscess and should be mindful about the tests to diagnose it. We experienced a 23-year-old man with retropharyngeal abscess caused by tuberculosis presenting hypernasality and hoarseness in the throat. In this article, we reviewed the etiology, diagnosis, and treatment of this case, with a review of literatures.

  • 목적 : 본 연구는 경부의 후인과 회전 운동 범위에 있어 운동 감각 역치의 차이를 비교하기 위해 수행되었다. 방법 : 본 연구의 대상자는 19명(10명 남자, 9명 여자 나이 19~30세 평균 23.2, 표준편차 3.3)이 참가하였다. 이 연구에서 운동 감각 역치는 편안히 앉은 자세에서 시각 차단시와 시각 허용시에, constant stimuli의 방법을 사용하여 반복 측정되었으며, 움직임을 수행 하는 과정에서 정지 상태를 유도하여 그 정지동작에서 두 동작의 차이를 평가하였다. 결과 : 본 연구의 결과 후인 운동 감각이 오른쪽 또는 왼쪽 회전 운동 감각 보다 더 민감하게 나타났다. 결론 : 중간 범위의 두 움직임간의 차이를 비교할 때 시각차단 유무는 각 운동 감각에 영향을 미치지 않았다.

A case of postoperative nasopharyngeal reflux associated with retropharyngeal lymphangioma in newborn infant (수술 후 비인두 역류가 동반된 신생아의 후인두림프관종 1례)

  • Lymphangioma is a rare benign congenital tumor of the lymphatic system, which is commonly diagnosed before 2 years of age. In the natronal report, cystic lymphangioma was usually reported as a huge translucent mass located in the head and neck area. It's occurrence in retropharyngeal space with respiratory obstruction and swallowing difficulty in neonate is extremely rare and postoperative nasopharyngeal reflux has rarely been reported. Complete resection is the standard therapy. However, involvement of the upper airway may be determining prognosis in the extensive lymphangiomas because of the difficulty of complete excision. We present a case of cystic lymphangioma in neonate which was initially asymptomatic but gradually progressed to cause respiratory obstruction due to enlargement. After resection, nasopharyngeal reflux developed with dysfunction of the soft palate and gradually improved with conservative care over 5 months.

Dysphagia after Retropharyngeal Abscess Treated with Transcutaneous Injection of Botulinum Toxin at Cricopharyngeus Muscle (경피적 윤상인두근 보툴리눔 독소 주입으로 치료한 후인두농양 후 발생한 연하곤란증)

  • Recently, negative pressure wound therapy (NPWT) has been reported to be effective for the treatment of cervical infections including retropharyngeal abscess. The 71-year-old woman with retropharyngeal abscess presented in this case showed improvement of infection through long-term NPWT. She continued to complain of swallowing difficulties after recovery. In this case, we performed the transcutaneous injection of botulinum toxin at the cricopharyngeus muscle for the patient who developed dysphagia after treatment for retropharyngeal abscess and observed improvement in swallowing.

  • This study investigates the relationship between dysphonia severity index(DSI) and voice handicap index(VHI). Seventeen patients with a vocal nodule and twenty patients with laryngopharyngeal reflux(LPR) patients participated in this study. Results showed that there is no significant difference in either DSI or VHI between vocal nodule and LPR patients, with a weak negative correlation between DSI and VHI. Results also showed that there is significant difference only in both MPT and Fhi of all DSI parameters between vocal nodule and LPR patients. These results suggest that voice evaluation should be conducted both objectively in terms of acoustical and aerodynamic parameters and subjectively in terms of GRBAS and VHI.

  • A 73-year-old man with sudden sensory neural hearing loss received a stellate ganglion block. Two hours after the block, the patient complained of newly developed neck discomfort. After an additional two hours, the neck swelled up gradually and neck pain and dyspnea developed. A plain radiograph of neck revealed narrowing of the upper airway; a tracheostomy was performed and the dyspnea was improved. On the next day, the pain site extended to the right scapula and a CT image revealed a huge retropharyngeal hematoma. Hematoma evacuation and bleeder ligation were then performed and the patient was discharged on the fourth day after admission without any complications. A practitioner should always remember to educate the patients about possible complications and undertake intensive observation when performing procedures, even in patients who do not initially present with a compromised airway.

  • Hypopharyngeal carcinoma is an aggressive malignancy that usually presents at a late stage, thereby resulting in an overall poor prognosis fir these unfortunate patients. The most effective treatment of pyriform sinus carcinoma combines surgery and radiation. Total laryngopharyngectomy is necessary in advanced tumors, but may not be appropriate in limited lesions. Therefore several procedure for partial laryngopharyngectomy have been proposed. Supracricoid hemilaryngopharyngectomy is the one of these procedure. This report presents one case of supracricoid hemilaryngopharyngectomy in a patient with pyriform sinus medial wall cancer and analyses aerodynamic study before and after operation.
