A Case of Pedunculated Schwannoma of Aryepiglottic fold (피열후두개주름에 발생한 유경성 신경초종 1예)

  • Schwannoma, also known as a neurilemmomas or neurinomas, is benign peripheral nerve sheath tumors arising from the Schwann cells. About 25~45 % of all schwannomas are found in the head and neck region, and the majority are found in the parapharyngeal space. Schwannoma of the larynx is very rare with an incidence of 0.1~1.5% in all benign laryngeal tumors. Recently, we experienced a case of a pedunculated schwannoma arising from the left aryepiglottic fold in a 80-year-old patient. The tumor was completely excised under direct laryngoscopy with the use of a $CO_2$ laser and subsequently confirmed as schwannoma. So, we report this case with a review of the literatures.

  • Verrucous carcinoma was first described by Ackerman who reported a case of verrucous carcinoma of the oral cavity. While it is primarily occurred in oral cavity, larynx is the second most affected site, accounting for nearly 35% of cases. It is a rare variant of well-differentiated squamous cell carcinoma and comprises 1% to 4% of all laryngeal carcinomas. It is characterized by locally invasive, slow growth and the surface shows papillary fronds with prominent hyperkeratosis. Because of the benign appearance and difficulty of the tissue diagnosis, it is often undiagnosed or misdiagnosed. The primary treatment modality is surgery and the extent of surgical resection may be guided by T stage. We experienced a case of verrucous carcinoma of larynx in a patient with hoarseness lasted for about 2 years and had difficult to diagnosis and treatment. Therefore we report this case with a brief review of literature.

A Case of Left Recurrent Inferior Laryngeal Nerve with Right Sided Aortic Arch (우측 대동맥활이 동반된 좌측 반회하후두신경 1예)

  • A non-recurrent laryngeal nerve on the left side is a rare anomaly which is reported in 0.04% and it is associated with abnormal developments of the aortic arch during embryogenesis. Although the possibility is extremely low, it is important to consider the possible existence of a non-recurrent laryngeal nerve to prevent a nerve injury during thyroidectomy. We experienced a 42 year-old male with left thyroid papillary cancer who had right side aortic arch and aberrant left subclavian artery. Even though we found that this patient had a recurrent laryngeal nerve, we present this case of the right aortic arch with an aberrant left subclavian artery variation with a brief review of literature.

  • Schwannoma is a benign soft tissue tumor arising from the schwann cells of the nerve sheath. Although 25 to 45% of schwannomas arise in the head and neck region, larynx is one of the rarest sites of involvement. Recently, we experienced a case of a laryngeal schwannoma causing voice change in a 53-years-old woman. Laryngeal schwannoma is located right false vocal fold with bulging, which was completely removed by microlaryngeal surgical excision. We report a rare case with a review of literature.

  • Spindle epithelial tumor with thymus-like differentiation (SETTLE) is an extremely rare type of thyroid tumor, with spares reports on its clinical course. SETTLE is derived from branchial pouch or thymic remnants, and classified as a low grade, malignant neoplasm due to its indolent growth and tendency to develop delayed metastasis. Therefore, stringent initial workups and long-term follow-up are required to prevent misdiagnosis. We present a case of recurrent spindle epithelial tumor with thymus-like differentiation (SETTLE) in the glottis of 57 year-old male patient, who had a history of surgical excision for the SETTLE from the thyroid gland 5 years ago. Transoral CO2 laser surgery was performed to remove the glottic tumor and there was no evidence of local recurrence at 11 months postoperatively.

Technical Review of Target Volume Delineation on the Posterior Fossa Tumor : An Optimal Head and Neck Position (후두와 종양의 방사선치료 시 표적용적의 결정을 위한 적절한 치료자세 연구)

  • Purpose : To explore a 3D conformal radiotherapy technique for a posterior fossa boost, and the potential advantages of a prone position for such radiotherapy. Materials and Methods :A CT simulator and 3D conformal radiotherapy Planning system was used for the posterior fossa boost treatment on a 13-year-old medulloblastoma patient. He was placed In the prone position and Immobilized with an aquaplast mask and immobilization mold. CT scans were obtained of the brain from the top of the skull to the lower neck, with IV contrast enhancement. The target volume and normal structures were delineated on each slice, with treatment planning peformed using non-coplanar conformal beams. Results : The CT scans, and treatment In the prone position, were peformed successfully. In the prone position, the definition of the target volume was made easier due to the well enhanced tentorium, In audition, the posterior fossa was located anteriorly, and with the greater choice of beam arrangements, more accurate treatment planning was possible as the primary beams were not obstructed by the treatment table. Conclusion : .A posterior fossa boost, in the prone position, Is feasible in cooperating patients, but further evaluation is needed to define the optimal and most comfortable treatment positions.

재발성 흡인성 폐렴을 동반한 편측성대마비 환자의 제I형 갑상연골 성형술 1례

  • 편측성대마비에 대한 치료방법으로 외측에 위치한 성대를 수술적으로 교정하려는 시도는 최근 활발하게 진행되어 Teflon 등을 이용한 성대내 이물주입술, 제Ⅰ형 갑상연골 성형술 및 피열연골 내전술등의 시술들이 시행되어 왔다. 저자들은 최근 후두와 종양으로 개두술을 시행받은 후 발생한 재발성 흡인성 폐렴 및 애성을 주소로 내원한 49세 남자환자를 원인 미상의 우측 편측성 성대마비로 진단하고 제Ⅰ형 갑상연골 성형술을 시행한 후 흡인성 폐렴 및 애성 등의 증상의 현저한 호전을 관찰할 수 있었으며, 술전 Maximal comfortable phonation time(MPT)이 6.24초에서 술후 17초로 증가하는 등 좋은 결과를 얻은 1례를 치험 하였기에 보고하는 바이다.

Endoscopic Laser Surgery in Laryngeal Carcinoma (후두 악성종양에 대한 내시경적 레이저 수술의 적용)

  • Background and Objectives: To determine if laser endoscopic microsurgery is a reliable and appropriate approach in the treatment of laryngeal carcinomas. Materials and Methods: Retrospective study of 62 patients treated with CO2 laser from June 1988 to November 2000 at Seoul National University Hospital for laryngeal squamous cell carcinoma. All patients were treated with curative intention. Fifty three untreated patients with laryngeal carcinoma (39 glottic and 14 supraglottic carcinoma patients) had primary carbon dioxide laser microsurgery. Nine radiation failure patients were treated. Postoperative radiotheray was done for 17 patients. Neck dissection was performed simultaneously for 4 supraglottic cases with cervical nodal metastasis. Mean follow-up duration was 40 months. Results: In primary laser surgery group, distribution of tumors (American Joint Committee on Cancer, 1997) were 38 cases with Tl, 13 cases with T2, 2 cases with T3. Cure rate was 88.7%(47/53) and local control rate was 92.5%(49/53). Larynx was preserved in 94%(50/53) of patients. The overall 5-year survival rate(Kaplan-Meier) was 81.5%. In radiation failure group, 56% of patients were recurred after laser surgery. Conclusion: Laser surgery could be a better treatment modality for early laryngeal cancers and selected advanced cases. Additional radiation therapy should be considered if resection margin is not satisfactory.

A Clinical Study of Hoarseness (애성을 주소로 한 후두질환의 임상 통계적 고찰)

  • Hoarseness is the most common and early symptom in laryngeal diseases. A clinico - statistical analysis was performed on 228 cases with chief complaint of hoarseness at the department of otolaryngology, Jeonbug National University during the past 3 years from January 1980 to December 1980. 1) The number of the patient with hoarseness were 228 cases (2.3 %) among total outpatient of 10110 cases. 2) Among the 228 cases with hoarseness, male were 115 cases and female were 113 cases, so sex ratio was nearly same. 3) The underlying diseases causing hoarseness in order of frequency were acute laryngitis 43 cases (18.9 %), chronic laryngitis 36 cases (15.8 %), vocal nodule 30 cases(13.2 %), vocal polyp 30 cases (13.2 %), vocal cord paralysis 26 cases (11.4 %), laryngeal carcinoma 18 cases (7.9 %) and laryngeal tuberculosis 15 cases (6.6 %). There were other diseases of larynx in 30 cases (13.2 %). 4) The incidence of age distribution in order of frequency were 4th decade (26.8 % ), 3rd decade (18.9%), 5th decade (17.1 %), 2nd decade (15.8 %) and 6th decade (9.6 %). 5) The duration from onset to consultation in order of frequency were 11 days-1 month (22.8 %), 1 month-3 months(19.7%), 3 months -6 months (18.9 %), within 10 days (13.6 %), 6 months-l year (13.2 % ), 1 year -3 years(7. 9 %) and 3 years over. 6) The duration from onset to consultation for underlying diseases were as follows ; acute laryngitis was within 10 days, chronic laryngitis was 3 months - 6 months, vocal nodule was 1 month-3 months, vocal polyp was 3 months -1 year, vocal cord paralysis was 11 days -1 month. 7) Associated symptoms with hoarseness in order of frequency were sore throat (25 %), sputum (8.8 %), swallowing difficulty (7.9 %) and dyspnea (6.1 %). But 84 cases (36.8 %), there were no other symptoms except hoarseness.

Laryngeal Measurement on Neck CT (경부 전산화단층촬영상에서 후두계측)

  • The normal larynx locates to the front of the neck symmetrically and the thyroid notch lies in the center of the neck, but practically the larynx is not symmetric in all people. From a clinical point of views, there are vague cases to decide whether a disordered laryngeal structure is within normal variations or a pathologic condition. The purpose of this study is to investigate the anatomy of the laryngeal framework in normal population. Authors investigate various measures of normal laryngeal framework, such as symmetry and length of the larynx, levels of the hyoid bone and vocal cord and angle of thyroid cartilage by using calipers and protractor on 45 cases of neck CT. The results are summerized as follows. 1. The laryngeal framework was asymmetric to a greater or lesser extent in most cases with directional preponderance to the right side. The degree of asymmetry did not differ among different age groups and between seres. 2. The level of the hyoid bone ranged from C2-C3 vertebrae to C5-C6 intervertebral space with most frequent level of C5. 3. The level of the vocal cord ranged from 01 vertebra to C6-C7 intervertebral space with most frequent level of C5. 4. The angle of thyroid cartilage ranged from 58 degree to 100 degree with average of 81.5 and mean angle between both thyroid alae were 77.24 degree in male and 87.88 degree in female.
