A Comparative Study of Image Quality and Radiation Dose according to Variable Added Filter and Radiation Exposure in Diagnostic X-Ray Radiography (진단용 X-선 촬영시 부가 필터 및 노출의 변화에 따른 피폭선량 및 영상 화질 비교 연구)

  • To know which parameters were acceptable for achieving lowest radiation exposure to the patients and highest image quality at the diagnostic X-ray radiography, we measured the patient radiation dose and image quality in transmitted PACS (Picture Archiving and Communication System) at variable combinations of the added filters. As a result, the Dose Area Product (DAP: $mGy{\cdot}cm^2$) and Entrance Surface Doses (ESDs: $mGy$) was lowest at 1 mmAl + 0.2 mmCu and highest at 0 mmAl. The histogram of the image quality by transmitted PACS was not significantly different at variable combinations of exposure parameters on the MATLAB. In conclusion, this study can be helpful for expecting radiation dose-exposure and control exposure parameters for the diagnostic X-ray radiography.

Treatment Results of Esophageal Carcinoma Treated by Radiation Therapy (식도암의 방사선치료 성적)

  • Purpose : To determine treatment or이ecol for inoperable esophageal cancer patients, 껜e evaluated survival rate and prognostic factors. Materials and Methods : We evaluated esophageal cancer treated by curative or palliative am in KCCH from 1992 to 1996, retrospectively. Recurrent or underdose case below 40 Gy were excluded. The number of male and female were 35 and 5, respectively. Thirty-eight patients were squamous carcinoma and 2 patients were not biopsy proven. Ten patients were treated with radiation therapy and chemotherapy Median dose of radiation therapy was 59.4 Gy and the range was $40\~60$ Gy. Results : The median survival is 6.5 months and 1-year survival rate was $28.3\%$. Age, location, radiation dose and chemotherapy were not significant prognostic factors. Median survivals of patients with below stage III and over stage IVA were 7.6 and 6.2 months respectively, but it is not significant. Conclusions : The survival for esophageal cancer is very poor. For patients with curative aim, chemotherapy must be considered. For patients with palliative aim, short-term external beam radiation therapy and/or brachytherapy must be considered.

구내 방사선 촬영에 있어서 Film과 Digital sensor에 따른 피폭선량의 차이에 대한 비교

  • 목적 : 기존 구내 방사선 촬영에서 사용되었던 Film에서의 노출선량과 Digital Sensor를 이용한 구내 디지털 촬영에서의 노출선량을 비교하여 현재 광범위 하게 사용 되어지고 있는 Digital Sensor가 환자의 피폭선량을 감소하는데 기여하는 정도를 알아본다. 대상 및 방법 : 치아 우식증이 없는 5개의 구치부 치아를 선택하여 석고 블럭에 매식한 후 교합면과 인접면에 우식병소들을 형성하였다. 이를 필름(Kodak Insight; IS, Kodak Co, USA)과 Digital Sensor(Kodak RVG 6000; Kodak Co, USA)에 XCP Instrument(Rinn Co, USA)를 사용하고, 전면에는 조직등가물질인 Acrylic Resin Block 20mm를 설치하였다. Acrylic Resin Block의 관구 측면 에서는 조사선량계를 부착하여 단계별로 변화시키는 노출조건에 대한 조사선량을 측정하였다. 그리고 이렇게 얻어진 영상을 3명의 방사선학 전공의와 1명 의 보존과 전공의가 평가를 하였다. 결과 : Film과 Digital Sensor를 가지고 촬영한 영상을 분석한 결과 노출선량에서도 Digital Sensor는 Film 노출선량과 비슷한 결과를 도출해 낼 수 있었다. 그러나 Digital Sensor로 촬영된 영상의 경우에는 Film 경우보다 좀더 효과적으로 조사선량을 판단 할 수 있었으며, 또한 영상 판독시 필름보다 폭 넓게 응용 할 수 있었다. 결론 : 본 연구에서는 선량 변화에만 의존하는 것이 아니라 Digital Sensor로 촬영시 영상 조절을 병행한다면 좀더 정확하고 효과적인 진단 활동에 도움이 될 수 있을 것으로 사료된다.

A Study on the Utilization of Diagnostic Equipments and Patient Dose for Diagnostic Radiological Procedures in Korea (진단방사선영역에서 방사선장치의 이용실태 및 환자피폭선량에 관한 조사연구)

  • IAEA's guidance levels have been provided for western people to the end. Guidance levels lower than the IAEA'S will be necessary in view of Korean people's proportions. Therefore, we need to develope the standard doses for Korean people. And we conducted a nationwide survey of patient dose from x-ray examinations in Korea. In this study, the 278 institutions were selected from Members Book of Korean Hospital Association. The valid response rate was approximately 57.9%. Doses were calculated from the questionnaires by NDD method. We obtained the results were as follows; 1) General radiographic equipments were distributed for 42.0%, fluoroscopic equipments 29.4%, dental equipments 13.2%, CT units 8.1 % and mamographic units 7.2%. 2) According to classification by rectification, three-phase equipments were 29.9%, inverter-type generators 29.5%, single-phase equipments 25.5%, constant voltage units 9.0% and unknown units 6.0%. 3) According to classification by receptor system, film-screen types were 46.8%, CR types 26.8%, DR types 17.7% and unknown types 8.9%. 4) The number of examinations were chest 49.2%, spine 16.8% and abdomen 12.7%. 5) Patient doses were head AP 3.44 mGy, abdomen AP 4.25 mGy and chest PA 0.39 mGy.

A Study on Dose Reduction in Infant Skull Radiography (유아 두개골 방사선촬영에서 피폭선량 감쇄에 관한 연구)

  • When an infant has visited a hospital due to skull fracture, the rupture of a blood vessel, or skin wounds on the head resulted from an incident, accident, traffic accident, or disease, he/she becomes to undergo anterior/posterior and lateral skull imaging, which is a head test at the department of radiology. In the head test, if the adult skull imaging grid is applied to the imaging, the secondary radiation will be removed to enhance the contrast of the image. However, among the radiation exposure conditions, the tube voltage should be enhanced by 8~10 kVp leading to an increase in the patient exposure. The present study was conducted under assumption that if the same images can be obtained from infant skull imaging without using the skull imaging grid, the exposure dose will be reduced and the artifacts due to grid cut off can be prevented. The researcher measured the radiation dosage using a radiation meter and conducted the subjective evaluation (ROC, receiver operating characteristic) among medical image evaluation methods. Based on the results, when the images were taken without using the grid, the exposure dose was reduced by 0.019 mGy in the anterior/posterior imaging and by 0.02 mGy in the lateral imaging and the image evaluation score was higher by 4 points. In conclusion, if the images of the skulls of infants that visited the hospital are taken with out using the grid, the exposure dose can be reduced, the image artifacts due to grid cut off can be prevented, and the lifespan of the X-ray tube will be extended.

A Study of Injection Dose for Patients and Exposure Dose for Technologists from the PET/CT Systems (PET/CT 장비 특성에 따른 방사성 의약품 주입량이 방사선 종사자에게 미치는 영향)

  • Purpose: It appears the different value when the injection dose is calculating for patients on each PET/CT systems. It directly affects the technologists' radiation exposed dose. We studied the effect of the variable injection doses from several PET/CT systems to exposure dose for technologists. Materials and Methods: Six technologists have worked for 5 months through unit rotations with 3 PET/CT systems {Scanner 1 (S1): 0.15 mCi/kg, Scanner 2 (S2): 0.17 mCi/kg, Scanner 3 (S3): 0.12 mCi/kg}. Eighteen to 19 patients have had examinations per a day on each PET/CT systems. Examination parameters were adjusted to the same. TLDs were used for checking the exposure dose of technologists. Results: Each technologists' the monthly average exposure dose was as follows; S1: 0.76 mSv, S2: 0.93 mSv, S3: 0.47 mSv. The maximum exposure dose was 1.12 mSv, and minimum was 0.42 mSv. The results showed significance in the correlation between the PET/CT system and the exposure dose (p<0.005). Conclusion: When the amount of injection dose was small, the exposure dose was decreased not only the patients but also the technologists. The exposure dose was decreased by the individual proficiency of technologists. However, the low injection dose can highly reduce the exposure dose for technologist so that there will be needed to following studies.

A Study on the Reduction of Absorbed Dose through the Insertion of a Shielding Material in the Intraoralsensor of Dental Radiography (치과 방사선촬영 시 구내 센서 내 차폐체 삽입을 통한 피폭선량 감소 연구)

  • In order to reduce the absorbed dose given to the patient during dental radiography, a sensor that inserts a shield into the intraoralsensor was designed. Using the designed sensor, the change in absorbed dose depending on whether or not a shield was used was evaluated. The system used to evaluate the absorbed dose is VEX-S300C from Vatech, and the energy spectrum of X-rays was obtained through SPEKTR simulation based on the irradiation conditions of 65 kV, 3 mA, and 0.15 sec, and the number of photons for each energy was derived. After designing the system through Genat4 Application for Tomographic Emission(GATE) simulation, the energy spectrum obtained was used as a radiation source to calculate the absorbed dose. Lead was used for the shield, and simulations were performed at 0.1 mm thickness intervals from 0.1 mm to 0.5 mm was evaluated. In the case of using an X-ray field with a diameter of 60 mm, the decrease in absorbed dose according to the presence or absence of a shield decreased exponentially as the thickness of the shield increased. In addition, when a 20 mm × 30 mm field was used, the absorbed dose was significantly reduced even when no shield was used, and it was confirmed that the absorbed dose was further reduced when a shield was used.

A Study on the Environmental Radiation Dose Measurement in the Nuclear Medicine Department (핵의학과에서 환경방사선량 측정에 대한 연구)

  • Korean individual occupational exposure control is focused on the retrospective service to the over-exposed person by the reading of personal dosimeter. Since the radiophamaceuticals using in the nuclear medicine department are uncontained radiation sources, the potential exposure at working environment is very high. Moreover, a patient remains radioactive for hours or even days after the administration of a radiopharmaceutical for diagnosis or treatment. Thus, the proper working environmental exposure control must be established and executed to protect not only the affiliated employees, but also guardians accompanying patients and temporarily visiting public from the exposure by the patients. Japanese radiation protection law regulates working environmental radiation exposure by regularly measuring and filing the environmental dose for years. This study was aimed at measuring working environmental radiation dose in the nuclear medicine department of an university hospital located in Daejeon, Korea. We measured the accumulation radiation dose in air at 8 locations in the nuclear medicine department by using the same method as in Japan with glass dosimeters. The highest dose rate, 0.23 mSv per month, was measured at the waiting room, and the second one is at reception desk. Even though the doses were lower than the Korean constraint dose rate (0.3 mSv/week) at the boundary of the radiation controlled area, it was over the dose limit of public (1 mSv/y) and environment (0.25 mSv/y). Conclusionally, it was found that the new or additional procedure was necessary to less the exposure dose to the receptionist and guardians by the environmental radiation dose in the nuclear medicine department.

Measurement of Patient Dose from Computed Tomography Using Physical Anthropomorphic Phantom (물리적 팬텀을 이용한 CT 촬영 환자의 피폭 선량 측정 및 평가)

  • The computed tomogrpahy(CT) provides a high quality in images of human body but contributes to the relatively high patient dose. The frequency of CT examination is increasing and, therefore, the concerns about the patient dose are also increasing. In this study the experimental determination of patient dose was performed by using a physical anthropomorphic phantom and thermoluminescent dosimeter(TLD). The measurements were done for the both axial and spiral scan mode. As a result the effective doses for each scan mode were 17.78mSv and 10.01 mSv respectively and the fact that the degree of the reduction in the patient dose depends on the pitch scan parameter was confirmed. The measurement methods suggested in this study can be applied for the reassessment of the patient dose when the technique in CT equipment is developed or the protocol for CT scanning is changed.
