The evaluation for usefulness of the custom made immobilization device for the anteroperitoneal resection patients with rectal cancer (복 회음부 절제술 환자를 위한 고정용구 제작 및 유용성 평가)

  • I. Purpose Patient immobilization is essential factor for successful radiation therapy and major problem is reproducibility to maintain patient position during total radiation therapy period. Purpose of this study is evaluation for usefulness of the custom made immobilization device for the anteroperitoneal resection patients with rectal cancer II. Materials and Methods The object of this study were patients who underwent anteroperitoneal resection and undergo radiation therapy at present with rectal cancer. We made immobilization device for patient individually and analyzed its set up reproducibility, patient position deviation and errors. III. Results There was $5mm{\sim}10mm$ deviation in patient position without individual immobilization device, but we improved the deviation within few mm limitation with individual immobilization device. IV. Conclusion Custom made immobilization device was very helpful for anteroperitoneal resection patient with rectal cancer. We improved the patient position deviation within few mm limitation, shorten the set up time and we could give the comfort to patients.

  • The demand for a better immobilization device has been increased in the radiation oncology field. Especially, it is essential to have a reliable and practical immobilization tool for the whole body radio-surgery and the IMRT(intensity modulated radiation therapy). A new method to immobilize the abdomen for the external beam radiation treatment was developed. The air-injected balloon blanket (AIBB) was designed as an immobilization device. As the air was injected into the AIBB, it pressed down the patient's abdomen and fixed the patient. The AIBB played a useful role to grab the patients' motion. Displacement of the abdomen in the anterior-posterior direction, which showed moving most during the respiration, reduced by more than 5 mm. Patients' movements from the breathing were reduced. The experimental results revealed that the AIBB could be used for the clinic.

The Clinical Effect and Construction of a Stereotactic Whole Body Immobilization Device (전신 정위 고정장치 제작과 임상효과에 대한 연구)

  • Purpose: To develop a whole body frame for the purpose of reducing patient motion and minimizing setup error for extra-cranial stereotactic radiotherapy, and to evaluate the repositioning setup error of a patient in the frame. Materials and Methods: The developed whole body frame is composed of a base plate, immobilizer, vacuum cushion, ruler and belts. The dimension of the base plate is 130 cm in length, 50 cm in width and 1 cm in thickness. The material used in the base plate of the frame was bakelite and the immobilizer was made of acetal. In addition, Radiopaque angio-catheter wires were engraved on the base plate for a coordinate system to determine the target localization. The measurement for radiation transmission and target localization is peformed in order to test the utilization of the frame. Also, a Matlab program analyzed the patients setup error by using the patient's setup images obtained from a CCTV camera and digital record recorder (DVR). Results: A frame that is useful for CT simulation and radiation treatment was fabricated. The frame structure was designed to minimize collisions from the changes in the rotation angle of the gantry and to maximize the transmission rate of the Incident radiation at the lateral or posterior oblique direction. The lightening belts may be used for the further reduction of the patient motion, and the belts can be adjusted so that they are not in the way of beam direction. The radiation transmission rates of this frame were measured as 95% and 96% at 10 and 21 MV, respectively. The position of a test target on the skin of a volunteer is accurately determined by CT simulation using the coordinate system in the frame. The estimated setup errors by Matlab program are shown 3.69$\pm$1.60, 2.14$\pm$0.78 mm at the lateral and central chest, and 7.11 $\pm$2.10, 6.54$\pm$2.22 mm at lateral and central abdomen, respectively. The setup error due to the lateral motion of breast is shown as 6.33$\pm$ 1.55 mm. Conclusion: The development and test of a whole body frame has proven very useful and practical in the radiosurgery for extra-cranial cancers. It may be used in determining target localization, and it can be used as a patient immobilization tool. More experimental data should be obtained in order to improve and confirm the results of the patient setup error.

  • A immobilizing device that is essential for correct lung and lens shielding with homogenous dose distribution in fractionated total body irradiation was developed and it's efficiency was evaluated. The main frame was made of stainless steel bar (5 cm in diameter) to withstand up to 230 cm in height and 100 kg in weight to prevent any injury even in unconsciousness condition. The saddle was designed to adjust the body weight and hight of standing patients. Chest and back supporter were made of 1 cm acryl which could fix the lung block and cassette holder. Leather and sponge pedding were used for head rest to keep patients comfortable. The device was strongly fixed by specially designed bolts on the bottom panel which was made of 1 cm stainless steel and 10 cm thick wooden board. Precise manipulation ($\pm$2 mm) was possible by upper two pulleys and side handles. Average four minutes twenty five seconds were needed for exact setting in fractionated TBI. No significant difference of lung block location on repeated verification films was confirmed and relatively homogeneous dose distribution was measured in rando phantom experiments and patient treatments ($\pm$5%). This immobilizing device was very efficient to keep correct position of patients, which is essential for better result and less complication in fractionated TBI.

Development of Immobilization Devices for Patients with Pelvic Malignancies and a Feasibility Evaluation during Radiotherapy (골반부 암 환자를 위한 고정기구 개발 및 방사선치료 시 효용성 평가)

  • [ $\underline{Purpose}$ ]: Immobilization devices that improve the setup reproducibility of pelvic cancer patients and that provide comfort to patients during radiotherapy were designed and the feasibility of the devices was evaluated. $\underline{Materials\;and\;Methods}$: A customized device was designed to immobilize a knee, thigh, and foot of a patient. Sixty-one patients with prostate cancer were selected and were divided into two groups-with or without devices. The setup errors were measured with respect to bony landmarks. The difference between digitally reconstructed radiographs (DRR) and simulation films, and the differences between DRR and portal films were measured. $\underline{Results}$: The left-right (LR), anterior-posterior (AP) and craniocaudal (CC) errors between the DRR and simulation films were $1.5{\pm}0.9\;mm$, $3.0{\pm}3.6\;mm$, and $1.6{\pm}0.9\;mm$, respectively without devices. The errors were reduced to $1.3{\pm}1.9\;mm$, $1.8{\pm}1.5\;mm$ and $1.1{\pm}1.1\;mm$, respectively with the devices. The errors between DRR and portal films were $1.6{\pm}1.2\;mm$, $4.0{\pm}4.1\;mm$, and $4.2{\pm}5.5\;mm$, respectively without the devices and were reduced to $1.0{\pm}1.8\;mm$, $1.2{\pm}0.9\;mm$, and $1.2{\pm}0.8\;mm$, respectively, with the devices. The standard deviations among the portal films were 1.1 mm, 2.1 mm, and 1.0 mm at each axis without the devices and 0.9 mm, 1.6 mm and 0.8 mm with the devices. The percentage of setup errors larger than 3 mm and 5 mm were significantly reduced by use of the immobilization devices. $\underline{Conclusion}$: The designed devices improved the setup reproducibility for all three directions and significantly reduced critical setup errors.

Clinical Implementation of an Eye Fixing and Monitoring System with Head Mount Display (Head Mount Display (HMD)를 이용한 안구의 고정 및 감시장치의 임상사용 가능성 확인)

  • A system to non-invasively fix and monitor eye by a head mounted display (HMD) with a CCD camera for stereotactic radiotherapy (SRS) of uveal melanoma has been developed and implemented clinically. The eye fixing and monitoring system consists of a HMD showing patient a screen for fixing eyeball, a CCD camera monitoring patient's eyeball, and an immobilization mask. At flrst, patient's head was immobilized with a mask. Then, patient was Instructed to wear HMD, to which CCD camera was attached, on the mask and see the given reference point on its screen. While patient stared at the given point in order to fix eyeball, the camera monitored Its motion. Four volunteers and one patient of uveal melanoma for SRS came into this study. For the volunteers, setup errors and the motion of eyeball were analyzed. For the patient, CT scans were peformed, with patient's wearing HMD and fixing the eye to the given point. To treat patient under the same condition, daily CT scans were also peformed before every treatment and the motion of lens was compared to the planning CT Setup errors for four volunteers were within 1mm and the motion of eyeball was fixed within the clinically acceptable ranges. For the patient with uveal melanoma, the motion of lens was fixed within 2mm from daily CT scans. An eye fixing and monitoring system allowed Immobilizing patient as well as monitoring eyeball and was successfully implemented in the treatment of uveal melanoma for SRS.

  • In the case of single tooth replacement, a fixed prosthesis or a dental implant is the treatment option commonly selected as first choice. However, any amount of sound tooth structure should be removed to prepare the abutment teeth for full coverage retainer. The adjacent tooth damage can be avoided placing a dental implant. However, depending on the patient's oral condition and any other circumstances, it may be impossible or delayed. In this case resin bonded fixed partial denture was selected as an alternative that can restore single tooth loss without much tooth structure removal to the young patients and the patients with gingival recession. We report these patients were satisfied with esthetic and function.

  • Anchorage control in orthodontic treatment is an important factor affecting treatment results. In the conventional approach, intra-oral anchorage such as application of differential force and moment, Nance holding arch and lingual arch, as well as extra-oral anchorage such as head gear were used for anchorage reinforcement. However, these anchorages may result in undesired tooth movement and require patient cooperation. To overcome these disadvantages, skeletal anchorage system was introduced as orthodontic anchorage. Types of skeletal anchorage include implant, onplant, miniplate and miniscrew. Especially, miniscrew has many advantages such as reduced patient cooperation, low cost and easy placement. Recently, it is successfully used in orthodontic treatment. This cases were treated using orthodontic miniscrews for retraction of ectopically erupting maxillary canine and impacted mandibular canine and intrusion of maxillary incisors.

  • Our objective was to develop and evaluate a non-invasive device for rigid immobilization and surface disease non-contact of the table in the lower extremity during CT angiography. The immobilization device consists of two components. The patient's lower limb device stabilizing elements made of polyethylene resin soft materials, and pelvis parts foam pad is used for non-contact surface. In a Prospective study the lower extremity device was used in patients who underwent a CT angiography of the lower limb. Immobilization with our device was well tolerated by all patients. The quality of the resulting images in the popliteal and infrapopliteal region was rated by five-point scale. The rigid immobilization resulted in a complete absence of motion artifacts. The new device is an effective, well tolerated and easily used immobilization that is suitable of use in 3D lower extremity CT angiography.

  • 급성기 허혈성 뇌졸중 증상이 있는 뇌 관류 CT검사를 시행한 환자를 대상으로 장비사가 제시한 고정 시간 기법(fixed time technique)과 조영제 추적 기법(bolus tracking technique)을 비교하여 환자의 피폭선량을 분석하고자 하였으며 조영제 추적 기법의 유용성과 최적의 조영증강 구간을 구현하는 Time graph를 알아보기 위한 것이다.
