Suggestion of The Manual Exposure Condition Guideline for Reducing Patient Dose in Digital Breast Tomosynthesis (디지털 유방단층촬영의 피폭선량 경감을 위한 수동 촬영조건의 가이드라인 제시)

  • The conditions after exposure to digital mammography and digital breast tomosynthesis were analyzed. The examinations for the ACR phantom were done using manual exposure, not auto exposure, to examine image discrimination and patient dose. As a result, the following results were derived: In the CC exposure, the kVp was 2kVp higher while mAs decreased to 58.6% for the 3D tomography. Such result showed an approximate decrease of 60mAs. At that time, the patients' Average Glandular Dose (AGD) was 1.65mGy in 2D and 1.87mGy in 3D; thus, AGD of 3D was shown to have about 1.13times higher. The result of the manual exposure revealed a reduced mAs of up to 80%; there was no effect in the assessment standard in terms of image discrimination, resulting in more than 10 points. When mAs was reduced to 80% in the manual exposure for ACR phantom, AGD was decreased to 0.66mGy. The diagnostic values of images were maintained and patients dose was reduced in the manual exposure in the AEC condition for 3D. Since the use of 3D has recently increased, using the manual exposure has been recommended in this study to improve the diagnostic value, while, simultaneously reducing patients dose.

Utilization-Focused Reduction of Radiation Exposure with XCP-DS FIT Sensor Holder by Measuring Dose of Dental X-ray Generator (구내 방사선발생기의 선량 분포측정을 통한 필름유지기구(XCP-DS FIT)의 피폭선량감소에 대한 유용성)

  • In this study, three dimensional X-ray dose distribution from dental X-ray generator system was measured by ALOKA PDM-117 dosimeter. The X-ray dose distribution will be change with XCP-DS FIT in oral shot, because the distance between X-ray generator and the dosimeter. The X-ray dose change affects on patient exposure and radiograph image quality. Therefore, it is important to obtain relation between the X-ray dose and the distance. The X-ray dose at the central position was decreased with increasing the distance. Furthermore, the dose at the edge of the X-ray flux was increased with increasing the distance. The increased dose affects on the patient radiation exposure. The present results will provide for good dental radiograph image and reducing radiation over-exposure on patient.

Analysis of Exposure Dose According to Chest and Abdomen Combine CT Exam Method (CT 흉·복부 통합검사 시 선량분석)

  • The purpose of this study is confirmed to usefulness between division exam and combine exam of chest and abdomen according to comparing chest and abdomen radiation dose of division exam and combine exam in CT exam method. This study was conducted on patients who were admitted to the E hospital from July 2013 to March 2014 underwent CT studies for the diagnosis of chest and abdomen disease. In study result, male dose were more higher than female dose according to gender analysis of exposure dose that combine exam effective dose were male $33.10{\pm}2.75mSv$, female $31.66{\pm}3.12mSv$ and chest exam effective dose were male $9.07{\pm}2.62mSv$, female $8.30{\pm}2.18mSv$(p<0.05). And, division exam dose and combine exam dose were similar in gender comparison (p>0.05). And, combine exam effective dose, only chest exam effective dose, only abdomen exam effective dose were more higher than DRL(Diagnostic Reference Level) in comparison of patient exposure dose with DRL (p<0.05). In conclusion, chest-abdomen combine exam dose and division exam dose were similar. The chest-abdomen combine study can be used as follow-up and emergency trauma patients. That study will be reduce exam time and the occurrence risk of side effect of the contrast medium.

The Usefulness of Al Face Block Fabrication for Reducing Exposure Dose of Thyroid Glands in Mammography (유방촬영 시 갑상샘의 피폭선량 경감을 위한 Al Face Block 제작 및 유용성에 관한 연구)

  • Currently, there are many studies being conducted around the world to reduce exposure dose to radiation for patients to receive medical treatments in a safe environment. We developed and fabricated of this shield that the patients are protected from the radiation and are need of safety control during breast imaging. In this study, for breast imaging, GE Senography 2000D were used and set at SID 65cm, 28kVp, and 63mAs. The measuring instrument was Fluke's Victoreen 6000-529. And we performed Face Block on with 30 patients. The chamber on the actual thyroid glands to take CC and MLO and measure the dosage before and after wearing the Face Block. For the results, after wearing the Face Block, exposure was decreased by 53.8%-100% and 65.8% in average in CC View and by 50%-100% and 60.7% in average in MLO View. The development of the Face Block that practically decreased the exposure dose of thyroid glands, crystalline eyes during breast imaging and reduced the patients' anxiety during breast imaging. The Face Block is expected to improve patients' satisfaction and contribute to reducing patients' exposure dose, but more efforts should be made to reduce exposure dose to medical radiation.

  • I made inquires about mammographic equipments and circumstances of mammography rooms in the 64 medical facilities in areas of Seoul and Kyong Gi Do. Moreover I had experments about exposure dose with patients and radiologic technologists. so there is the data indicated follows. 1. There are inclined to improve in quality and function of mammographic equipments, it has been proven that s/f system exchanged to DR system. 2. It is certain that the number of examinations are becoming increasingly significant. 3. The Space Scattered Dose of mammography rooms are much more larger than portable equipments. 4. I worry about the affection of expose dose about Space Scattered Dose of mammography room. 5. There is need of study how to cope with the situation about increasing exposure dose of radiologic technologists in small space and numeruous number of examinations.

Effects of Dose Reduction Fiber Shielding Cloth on Scattering Rays in Off-target Site during Angiography (선량저감섬유(Dose Reduction Fiber) 차폐포의 혈관조영술(Angiography) 시술 시 비 시술 부위의 산란선 차폐 효과)

  • Unlike conventional radiographic examinations, angiointerventional procedures have a high risk of radiation exposure to patients or operators due to prolonged radiation exposure time. This study was undertaken to examine effects of reducing the radiation risk by applying dose reduction fiber (DRF) shielding cloth during angiography. To investigate the properties of DRF shielding cloth, we measured the scattered radiation below and above a human phantom using a glass dosimeter, at site distances 10 cm away from the irradiated field. The results obtained reveal a 15 ~ 31% reduction of scattered radiation in the irradiation field, and 53 ~ 70% reduced radiation measured after phantom transmission. Taken together, our data indicate that application of DRF shielding cloth for radiation reduction at non-procedural sites during interventional procedure results in reduction of scattered doses to patients and operators, without affecting the medical examinations. We propose the use of DRF shielding during angiointerventional procedures, in order to reduce the risk of radiation exposure of patients and operators.

Evaluation of Radiation Exposure Dose for Examination Purposes other than the Critical Organ from Computed Tomography: A base on the Dose Reference Level (DRL) (전산화단층촬영에서 촬영 목적 부위와 주변 결정장기에 대한 피폭선량 평가: 선량 권고량 중심으로)

  • In this study measured patient exposure dose for purpose exposure area and peripheral critical organs by using optically stimulated luminescence dosimeters (OSLDs) from computed tomography (CT), based on the measurement results, we predicted the radiobiological effects, and would like to advised ways of reduction strategies. In order to experiment, OSLDs received calibration factor were attached at left and right lens, thyroid, field center, and sexual gland in human body standard phantom that is recommended in ICRP, and we simulated exposure dose of patients in same condition that equal exposure condition according to examination area. Average calibration factor of OSLDs were $1.0058{\pm}0.0074$. In case of left and right lens, equivalent dose was measure in 50.49 mGy in skull examination, 0.24 mGy in chest, under standard value in abdomen, lumbar spine and pelvis. In case of thyroid, equivalent dose was measured in 10.89 mGy in skull examination, 7.75 mGy in chest, 0.06 mGy in abdomen, under standard value in lumber spine and pelvis. In case of sexual gland, equivalent dose was measured in 21.98 mGy, 2.37 mGy in lumber spine, 6.29 mGy in abdomen, under standard value in skull examination. Reduction strategies about diagnosis reference level (DRL) in CT examination needed fair interpretation and institutional support recommending international organization. So, we met validity for minimize exposure of patients, systematize influence about exposure dose of patients and minimize unnecessary exposure of tissue.

Consideration on Measured Patients Dose of Three-Dimensional and Four-Dimensional Computer Tomography when CT-Simulation to Radiation Therapy (방사선치료를 위한 CT 검사 시 3DCT와 4DCT에 대한 피폭선량 고찰)

  • This study was to measure the patient dose difference between 3D treatment planning CT and 4D respiratory gating CT. Study was performed with each 10 patients who have lung and liver cancer for measured patient exposure dose by using SOMATON SENSATION OPEN(SIMENS, GERMANY). CTDIvol and DLP value was used to analyze patient dose, and actual dose was measured in the location of liver and kidney for abdominal examination and lung, heart and spinal cord for chest examination. Rando phantom were used for the experiment. OSLD was used for in-vitro and in-vivo dosimetry. Increasing overall actual dose in 4D respiratory gated CT-simulation using OSLD increase the dose by 5.5 times for liver cancer patients and 6 times for lung cancer patients. In CT simulation of 10 lung cancer patients, CTDIvol value was increased by 5.7 times and DLP 2.4 times. For liver cancer patients, CTDIvol was risen by 3.8 times and DLP 1.6 times. The accuracy of treatment volume could be increased in 4D CT planning for position change due to the breaths of patient in the radiation therapy. However, patients dose was increased in 4D CT than 3D CT. In conclusion, constant efforts is required to reduce patients dose by reducing scan time and scan range.

  • This interest in radiation exposure makes increasing doctor's awareness and knowledge of radiation dose in patients during X-ray test important in reducing patient's uneasiness. However, very few facilities are equipped with measurement instruments. Therefore, an intensive study to find out patient dose using computational method has been initiated. This study used special features of the bit system and NDD-M and directly measured the output dose of diagnostic X-ray instruments used in Korea to create tables. Two different methods were found to be adequate when applied to cases when X-ray outputs were both known and unknown, and comparative experiments with real measurement doses were carried out. Presented methods were found to provide more accurate results compared to the bit system and NDD-M. Therefore, patient dose during clinical trials were found to be more easily acceptable to medical personnel in the radiation field in terms of radiation exposure and reduction of medical X ray dose.

  • 유방촬영은 날로 증가추세에 있으며 압박을통해 영상을 얻는 것이 화질을 향상시키고 피폭선량을 줄이는데 필수적이다. 그러나 압박대 자체의 두께로 인해 산란선과 피폭선량을 증가시킬 수 있으므로 압박대 재질에 대한 고찰이 필요하다. 현재 임상에서 쓰이고 있는 재질은 폴리카보네이트이며 플라스틱 계열이다. 환자의 피폭선량을 줄이기위해 노력한다면 이보다 더 좋은 재질에 대해 고려해볼 필요가 있기에 본 연구에서는 플라스틱 계열 물질 중 비결정 플라스틱에 대한 방사선투과성에 대해 비교해 보고자 한다.
