Institutional Approach to Healthcare Information Exchange: Focused on Medical Law (의료법상 진료정보교류를 위한 법제도적 고찰)

  • Compared to penetration of Electronic Medical Record(EMR) system, Healthcare Information Exchange(HIE) has been less active in South Korea. The aim of this study is to explore medical law newly legislated to introduce HIE through the nation. The important insights are that the medical institutions exchange the patient's healthcare information based on the consent of the patient, and it is expected to be set up and managed the medical record exchange support system by the government and a consignment organization. In addition, the certification program for standardization and interoperability on the EMR system would be conducted. Nevertheless, continued policy developments and researches for the promotion of HIE will be urgently needed such as the education for the vendors and developers, developments of the certification programs and the incentive payment programs and the public relations.

A Study on the Improvement of Personal Information Protection in Small and Medium-sized Medical Institutions (중소형 의료기관의 개인정보 보안실태 및 개선방안)

  • Rapid developments of IT technology has been creating new security threats. There have been more attacks to get patients' sensitive personal information, targeting medical institutions that are relatively insufficient to prevent and defend against such attacks. Although the government has required senior general hospitals to get the ISMS certification since 2016, such a requirement has been burdensome for small and medium-sized medical institutions. Therefore, this study was designed to draw measures to identify and improve the privacy status of the medical institution by dividing it into management, physical and cyber areas for small and medium-sized medical institutions. The results of this study showed that the government should provide financial support and managerial supervision for the improvement of personal information protection of small and medium-sized medical institutions. They also suggested that the government should also provide medical security specialists, continuous medical security education, disaster planning, reduction of medical information management regulations not suitable for small and medium sized institutions.

The health and medical statistics survey in Medical Records Offices required by the outside institutions (의무기록부서의 외부기관 통계지원 업무에 관한 연구)

  • This study was conducted in order to examine the quantity of health and medical statistics survey in Medical Records Offices which was required by outside institutions and whether it is profitable for the hospitals or not. The thirteen outside institutions required health and medical statistics of the 24 types of the survey to Medical Records Offices. 16.7% of health and medical statistics of the survey was paid to medical records offices such as the National Cancer Registration Survey, Patient Survey, Tuberculosis Patient Survey, and Hospital Discharge Patients Injury Survey. Medical Records Offices' total length of time for the health and medical statistics survey was over 200 hours per year like the National Cancer Registration Survey, Healthcare Accreditation System and Hospital Discharge Patients Injury Survey. The Medical Record Administrators in the hospitals with fewer than 500 beds work full time from 1 to 3. It is indicated in the study that it is necessary to improve the health and medical statistics survey system in Medical Records Offices required by the outside institutions and to employ additional Medical Record Administrator for more accurate Health and Medical Statistics Survey.

  • Recently, A utilization request of the U-Healthcare services are increasing rapidly. This is because the increase in smartphone users and ubiquitous computing technology was developed. Furthermore, the demand for access to and use of medical information systems is growing rapidly with a smartphone. This system have the advantage such as they can access from anywhere and anytime in the healthcare information system using their smartphone quickly and easily. But this system have various problems that are a privacy issue, the location disclosure issue, and the potential infringement of personal information. this problems are arise very explosive. Therefore, we propose a secure information security system that can solve the security problems in healthcare information systems for healthcare workers using smartphone. Our proposed system, doctors record, store, modify and manage patient medical information and this system would be safer than the existing healthcare information systems. The proposed system allows the doctor to perform further authentication by transmitting using SMS to GOTP message when they accessing medical information systems. So our proposed system can support to more secure system that can protect user individual information stealing and modify attack by two-factor authentication scheme. And this system can support confidentiality, integrity, location information blocking, personal information steal prevent using cryptography algorithm that is easy and fast.

Study on Safety Management Activity of Blood Test Room of Nuclear Medicine Department (핵의학과 혈액검사실의 안전 관리 활동에 대한 고찰)

  • Purpose: The object evaluation method about medical institutes of these days increases credibility of consumers about medical services by conducting a certification system about medical institutes. In addition, as nuclear medicine test rooms and diagnosis test medicine room adopt many kinds of international certification systems, the matters regarding safety management of test rooms are being regarded as important. Since the blood test rooms of nuclear medicine are exposed to many harmful factors such as infection from clinical specimen and radioactive isotope reagent, there is a need to pay lots of attention to the safety management of staff and patients. Therefore, this study discusses safety management activities of staff and patients, which are conducted in the blood test rooms of the nuclear medicine department in Asan Medical Center. Materials and Methods: In the blood test rooms of the nuclear medicine department in Asan Medical Center, the matters regarding comprehensive safety management by the person in charge of safety management are offered and all staff members of the test rooms apply them into work. Safety management education is regularly conducted according to established regulations, and infection is prevented through implementation of wearing personal protectors and hand sanitation during test work. In addition, technical safety guides and accident guides for interruption of electric power are provided against emergencies. Through infection management guides, infection prevention and preparation methods for infection are learned and radioactive isotope management, safety management about reagent use and safety guides about harmful chemical substances are being applied to work. Results: The blood test rooms of the nuclear medicine department apply safety management regulations to work. Under the situation where hand sanitation should be conducted, hands are washed to prevent infection between staff and patients, and for preventing infection from clinical specimen, personal protectors are worn. The reagent, which is classified as harmful substance, is separately stored to be easily recognized, radioactive wastes and general medical wastes are also safely managed. Through these lots of safety management activities, safety management awareness of staff members is enhanced, and patients are protected from many dangers. Conclusion: Staff members of the blood test rooms of the nuclear medicine department should be fully aware of safety management regulations and apply them to work. When better safety management suggestions are made, they need to be examined and applied for increasing quality of safety management for staff and patients.

Manifestation of Cognitive Function in Geriatric Patient with Subjective Memory Complaint (주관적 기억력 저하를 호소하는 노인 환자의 인지기능 양상)

  • Purpose : The purpose of this study was to find out cognitive function of the patients with subjective memory complaint. Material and Methods : From March 1st 2005 to May 31st 2009, 155 normal individuals without any medical illness who visited Yeungnam University Hospital to undergo medical checkup with neurocognitive test was enrolled, and checked by using Cognitive Assessment & Reference Diagnostic System. Results : 107 of the patients had normal cognitive function, 21 patients (about 15%) were diagnosed with dementia, and 10 patients (about 7%) were diagnosed with considerable psychiatric illness, such as depression, anxiety disorder, adjustment disorder. In amnesia, agnosia, aphasia, attention, calculation, dysexecution, Dementia group and Psychiatric illness group has worse score than Normal individuals group. But, in apraxia, Dementia group has worse score than Psychiatric illness group and Normal individual group. Conclusion : Because the patients with subjective memory complaint can be diagnosed as any psychiatric illness as well as dementia, sensitive screening test and early psychiatric approach is needed.

Gross Alpha Analysis of Nasal Smear Samples and Internal Dose Assessment Procedure in Radiation Emergency (방사선비상시 비강스메어 시료의 전알파 분석 및 내부피폭선량평가 절차)

  • The gross alpha analysis of nasal smear samples for the radiation emergency and the additional follow-up steps were established. Cotton swab sticks using in local hospitals for nasal smear in Korea were used for the verification. The measurement results of standard samples spiked with certified reference source were well agreed within ${\pm}20%$ compared with reference values. The clearance ratio of smear samples conducted with wet smear condition showed higher value. To eliminate the quenching effect of liquid scintillation samples, dry of smear samples should be followed up before counting samples. Based on the measurement results, medical decision levels and internal dose assessment were established for the victims in the beginning of radiation emergency.

  • QR code (abbreviated from Quick Response Code) is code system that is strong in against to apply image processing techniques (skew, warp, blur, and rotate) as QR codes can store several hundred times the amount of information carried by ordinary bar codes. For this reason, QR code is used in various fields, e.g., air ticket (boarding control system), food(vegetables, meat etc.) tracking system, contact lenses management, prescription management, patient wrist band (patient management) etc. In this paper, we proposed reversible data hiding for binary images. A reversible data hiding algorithm, which can recover the original image without any distortion from the marked (stego) image after the hidden data have been extracted, because it is possible to use various kinds of purposes. QR code can be used to generate by anyone so it can be easily used for crime. In order to prevent crimes related QR code, reversible data hiding can confirm if QR code is counterfeit or not as including authentication information. In this paper, we proved proposed method as experiments.

  • In telemedicaine, people can make their health checked at anywhere from temporal and spatial constraints and It's environment can provide continuous health information regardless of the location of customers through PCs and smart phones. In addition, personal health information collected utilizing the BYOD(bring your own device) is the most important factor data security and guaranteed personal privacy because it's directly connected to the individual's health and life. In this paper, we provide a signature of the private key encryption system and method for providing the security of personal health information data collected utilizing the BYOD. Against replay attacks and man-in-the-middle attacks on security issues that are emerging as a smart environmental health was used as the timestamp and signature methods. Proposed method provides encryption overhead, while a communication was lower compared to the pre-encrypted with a mean 1.499mJ 1.212mJ shown by simulation to respond quickly in an emergency situation to be about 59%.

  • In distributed computing paradigm, mobile surrogate systems have not gained wide acceptance because of security concerns that have not been suitably addressed yet. Currently even the widely used Grid middleware, Globus, does not have a host authentication mechanism, which makes it hard to provide high performance Grid computing capabilities to mobile clients. In this paper, a mobile Grid service framework is designed and implemented in order to provide Grid services to mobile host such as PDA. The surrogate host-based system allows mobile hosts to be provided with Grid services while moving. The implemented mobile Grid services system has been applied to mobile healthcare system which utilizes Grid computing for ECG signal processing. The presented system framework can be used as a secure enterprise mobile healthcare system for hospital physicians.
