  • 이 연구는 통계청의 치매 사망자수 및 사망률 자료와 공공데이터포털의 서울시 치매안심센터를 분석하여 치매 문제에 초점을 맞추어 치매 환자와 그 가족들에게 지원을 제공하는 치매안심센터의 역할을 조명합니다. 연구 목적은 서울시 각 자치구에 설치된 치매안심센터의 자원과 인프라가 치매 사망률에 미치는 영향을 분석하여, 치매 예방 및 관리 정책의 개선 방향을 제시하는 것입니다. 데이터는 한국통계정보원(KOSIS)과 사망원인통계에서 수집되었으며, 2022 년에 보고된 치매로 인한 사망자 수와 치매관리센터의 자원 정보를 포함합니다. 데이터 전처리 과정에서는 정확성 검증, 정규화, 표준화, 분류 작업이 이루어졌습니다.분석 방법으로는 통계적 접근과 예측 모델링을 통해 치매 치료 센터 자원과 치매 사망률의 관계를 다각적으로 조사했습니다. 본 연구의 결론은 치매 안심 센터의 자원이 치매 사망률에 미치는 영향을 이해하는 데 중요한 정보를 제공하며, 이는 치매 관리와 예방에 있어서 인력의 중요성을 강조합니다.

Surgical Management of Neuropathic Arthropathy due to Rheumatoid Arthritis: A Case Report (류마티스 관절염 환자에서 발생한 신경병성 관절병증의 수술적 접근: 증례 보고)

  • Neuropathic arthropathy (Charcot arthropathy) is a progressive joint disease often associated with conditions such as diabetes, leading to severe joint deformity and pain. However, its occurrence in patients with rheumatoid arthritis (RA) is rare and not well documented. This case report describes a 48-year-old woman with a long history of RA who developed a severe deformity of her right ankle, identified as neuropathic arthropathy extending to the subtalar joint (Brodsky classification type 3A). After excluding other potential causes, the condition was determined to be secondary to RA-associated peripheral neuropathy. Surgery, including allograft and autograft fixation, was performed to correct the deformity. Post-operative complications, such as wound infection, were treated with negative pressure wound therapy and skin grafting. At the 18-month follow-up, the patient was able to walk without pain, demonstrating successful joint fusion.

  • A dentigerous cyst is the most common developmental odontogenic cyst. Patients with dentigerous cyst usually feel no pain or discomfort. If it is not treated, tooth eruption might not only be impeded, but also let the tooth translocate to the unusual area. So, early detection and appropriate treatment are important. Enucleation & marsupialization are the best options to treat a dentigerous cyst. Treatment plan depends on patient's age, health, preserve & protect of important structures, and sort of cyst. In these dentigerous cysts cases, by marsupialization and using obturator, affected tooth could be achieved spontaneous eruption into the dental arch even though they were badly dislocated.

Evaluation of Hypertension Prevention and Management Programs Conducted by Community Health Nurse Practitioners in Rural Areas of Korea (농촌지역 보건진료소의 고혈압 예방 및 관리사업 평가)

  • Objectives: This study aims to evaluate effectiveness of hypertension prevention and management programs (HPMPs) in rural Korea. Methods: A questionnaire survey was conducted with 700 community health practitioners working at primary health care posts (PHCPs). The questionnaire had four domains, which were hypertension management, hypertension prevention, the PHCP environment, and evaluation system, each with different maximum weighted values (WVs). Weighted values of 100 indicate the best effectiveness for the HPMP in all four domains. Results: The average WVs and percent effectiveness of all four domains combined was 61.65 (62%); the hypertension management domain was 28.81 (72%); the hypertension prevention domain was 23.44 (67%); the PHCP environment was4.29(43%); and the evaluation system was5.10(34%). Conclusion: The HPMPs were generally effective, and hypertension management was the most effective. The environment of PHCPs and the evaluation system of HPMPs should be improved to increase the effectiveness of the HPMPs.

Clinical evaluation of a new extraction method for intentional replantation (의도적 재식술을 위한 새로운 발치법의 임상 평가)

  • Purpose: Intentional replantation (IR) is a suitable treatment option when nonsurgical retreatment and periradicular surgery are unfeasible. For successful IR, fracture-free safe extraction is crucial step. Recently, a new extraction method of atraumatic safe extraction (ASE) for IR has been introduced. Patients and Methods: Ninety-six patients with the following conditions who underwent IR at the Department of Conservative Dentistry, Seoul National University Bundang Hospital, in 2010 were enrolled in this study: failed nonsurgical retreatment and periradicular surgery not recommended because of anatomical limitations or when rejected by the patient. Preoperative orthodontic extrusive force was applied for 2-3 weeks to increase mobility and periodontal ligament volume. A Physics Forceps was used for extraction and the success rate of ASE was assessed. Results: Ninety-six premolars and molars were treated by IR. The complete success rate (no crown and root fracture) was 93% (n = 89); the limited success rates because of partial root tip fracture and partial osteotomy were 2% (n = 2) and 5% (n = 5), respectively. The clinical and overall success rates of ASE were 95% and 100%, respectively; no failure was observed. Conclusions: ASE can be regarded as a reproducible, predictable method of extraction for IR.

Relationship of Knowledge about Disease, Illness Attitude, and Quality of Life for Patients with Chronic Obstructive Pulmonary Disease(COPD) (만성 폐쇄성 폐질환자의 질병관련 지식, 질병태도, 삶의 질에 관한 연구)

  • The purpose of this study was to investigate the correlation between knowledge related to disease(KRD), illness attitude, and quality of life(QOL) in relation to the Chronic Obstructive Pulmonary Disease(COPD). According to the study result, there were significant differences with regard to KRD in accordance with subjects' education, monthly income, mMRC(modified Medical Research Council) Dyspnea Scale, inhaled bronchodilator and antibiotic treatment. In addition, there were significant differences with respect to illness attitude according to monthly income and mMRC. In case of the QOL, there were significant differences in accordance with age, marital status, monthly income, inpatient status, history of hospital admission, oxygen use, comorbidity, mMRC, and inhaled steroid. It has shown that there was a significant correlation between KRD and QOL(r=-.438, p<.001), illness attitude and QOL(r=.279, p=.001). Thus, it is important to grasp monthly income and mMRC, which are correlated with the three elements in common. As per assessment about COPD patients in the clinical situation, it is necessary to develop a multi-disciplinary approach, health coaching program for improving KRD and having a positive attitude based on the low-income group with taking interest in the circumstances of mMRC as well as objective test results.

Need Survey of Home Modification on Outpatient with Stroke and Spinal Cord Injury in Rehabilitation Center (재활치료를 받는 뇌졸중과 척추손상 외래환자들의 주거환경개선에 대한 요구도 조사)

  • The purpose of this study was to investigate the need of home modification for outpatient with stroke and spinal cord injury. The subjects of this study were 54 outpatients who have experience in rehabilitation therapy at hospitals in Seoul and Gyeonggi-do. A questionnaire was distributed and collected form January 2th to February 4th, 2012. Descriptive statistics and $x^2$-test were used to analyze data. There were statistically significant difference in stairs of access, in width of front door, in bathroom, space of room according diagnosis. There were statistically significant difference in light of sensor, tissue holder and socket cover in bathroom for the comparison of demands for home modification by living type. There were statistically significant difference in width and threshold of front door, tissue holder and towel bar, nonslip floor in bathroom, space and table, chair of kitchen, space and threshold of porch, light for the comparison of demands for home modification according score of MBI. Based on the results of the survey, needs of home modification is different according disability level, diagnosis, living type, ADL performance. The results of this study show a need for the further occupational therapy of home modification and supporting policies in home modification.

Study on Development of Patient Effective Dose Calculation Program of Nuclear Medicine Examination (핵의학검사의 환자 유효선량 계산 프로그램 제작에 관한 연구)

  • The aim of this study was to develop and distribute a dedicated program that can easily calculate the effective dose of a patient undergoing nuclear medicine examinations, and assist in the study of dose of nuclear medicine examinations and information disclosure. The program produced a database of the effective dose per unit activity administered (mSv/MBq) of the radiopharmaceuticals listed in ICRP 80, 106 Report and the fourth addendum, was designed through Microsoft Visual Basic (In Excel) to take the effect of 5 different (Area, Clark, Solomon(=Fried), Webster, Young) of pediatric dose calculation methods and 7 different body surface area calculation methods. The program calculates the effective dose (mSv) when the age, radionuclide, substance, and amount injected in the human body is inputted. In pediatric cases, when the age is entered, the pediatric method is activated and the pediatric method to be applied can be selected. When the BSA (Body Surface Area) formula is selected in the pediatric calculation method, a selection window for selecting the body surface area calculation method is activated. When the adult dose is input, the infant dose and the effective dose (mSv) are calculated automatically. The patient effective dose calculation program of the nuclear medicine examinations produced in this study is meaningful as a tool for calculating the internal exposure dose of the human body that is most likely to be obtained in nuclear medicine examinations, even though it is not the actual measurement dose. In the future, to increase the utilization of the program, it will be produced as an application that can be used in mobile devices, so that the public can access it easily.

  • We treated synchronous double primary lung cancers, where one site resulted from CIS disease, with lobectomy and argon plasma coagulation (APC) in a patient who couldn't tolerate pneumonectomy, which resulted in a reduction of the extent of surgery. APC could be a reasonable alternative for CIS disease of lung in inoperable patients.

Oncologic Outcome of Sacral Chordoma (천골에 발생한 척색종의 치료결과)

  • Purpose: We analyzed treatment result to examine the outcome for patients with sacral chordoma and to determine relevant prognostic factors. Materials and Methods: We retrospectively reviewed 19 patients with sacral chordoma seen at out institution between 1990 and 2010. There were 9 men and 10 women with mean age of 56 years. The average follow up was 63 months (range, 25-144 months). 15 patient received surgical treatment, six of these patient had wide, eight had marginal, one had intralesional margin and 4 patient treated with Radiation therapy only. Results: The disease free and overall survival rate for all 19 patients was 34.7% and 79.7% at 5-years, respectively. Statistical analysis using the log-rank test revealed no significant difference between the surgery and radiation therapy groups in overall survival (p=0.54). Nine of 19 patients had local recurrence at a median of 2.5 years postoperatively. Seven of these 9 patients had distant metastasis at a median of 4.5 years postoperatively. Among the variables, tumor size (p=0.033) and tumor involvement of above S3 (p=0.032) were independent prognostic factor for overall survival. Nine of 15 patients who received surgical treatment had postoperative complication such as voiding difficulty and incontinence. Conclusion: Careful consideration of the patient general condition and predictable complication of the treatment might be the best way to improve patient's survival and quality of life.
