The Effects of Neuromuscular Re-education on Physical Composition, Body Water, and Functional Independence in Stroke Patients (신경근 중재 접근이 뇌졸중 환자의 신체조성, 체수분 및 기능적 독립성에 미치는 영향)

  • Purpose: The purpose of this study is to investigate the effect of neuromuscular re-eduction program and traditional intervention program. Methods: It is focused on difference between changes of experimental before and after on 30 stroke patient's physical composition, body water and functional independence. Results: The obtained results are as follows ; 1. In the comparison of change in physical composition between the experimental group and control group, the between-subjects factors were not a significant difference. 2. In the comparison of change in body water between the experimental group and control group, the between-subjects factors were not a significant difference. 3. In the comparison of change in functional independence between the experimental group and control group, the between-subjects factors were a significant difference in 8 items and total FIM (p<0.01; p<0.05) but, were not a significant difference in 11 items. Conclusion: the neuromuscular re-eduction program was more effect then traditional intervention program in functional independence but, was not a significant difference in body composition and body water.

Combined Repair of Pectus Excavatum and Open Heart Surgery in Marfan's Syndrome (Marfan 증후군 환자에서 개심수술 및 누두흉의 교정)

  • The presense of pectus excavatum in Marfan's syndrome may complicate cardiac operation by making midline sternotomy technically more difficult and limiting the operative exposure of the heart. We operated on a 33 year old male patient with Marfan's syndrome and severe pectus excavatum who had severe mitral regurgitation and moderate aortic regurgitation with 52mm aortic root dilation. The operative field was adequately exposed through a midline sternal incision with two sternal retactors. The patient underwent Bentall operation and mitral valve replacement. The repair of pectus excavatum was performed after completion of CPB and the administration of protamin. Permanent internal stabilization achieved by overlapping of the ends of lower ribs and reinforced with sternal closure wire.

  • This enable system integration for efficient data processing by interconnecting ASP and SQL, on-line consultation between patient and Korean medicine doctor, and semi-eternal use of medical examination data owing to storage and check of medical examination data(accurate medical examination and description using this medical examination data between patient and Korean medicine doctor). This is a Korean medicine remote examination system which converts from existing Korean medicine hospital system being maintained as existing off-line only to medical examination type of both on-line and off-line mode transcending time and space on web in which anyone can participate.

A Study on Information Need and Use Behavior of Doctors (의사들의 정보요구 및 이용행태에 관한 연구)

  • The purpose of this study is to provide for efficient health information service which doctors may use for their better medical practice and scientific research. 177 doctors were surveyed from December 3 to December 21th in 2007. This study mentioned activities of information need and use that were evaluated by information gathering methods used, information source, frequency of use, and degree of satisfaction toward library services. Doctors showed that they need information mainly to know the recent research results in their interesting subject field and to treat a patient. Doctors trended to obtain information through internet search because easy access and easy use, accuracy and up to dateness of information use. Doctors trended to seek information mainly through library, but they showed relatively low frequency of library use because they do not know how to use it.

  • Patients with atopic dermatitis, an intractable disease, suffer uncontrollable psychological distress as well as various symptoms in their skin. Recent findings showed that mental factors such as stress may play a role in the disease and that some techniques for alleviating psychological problems were efficacious in the atopic dermatitis. Therefore, we proposed the mind-body medicine that has been used to decrease the negative influences of stress and to cure psychosomatic disorders as an additional remedy for the atopic dermatitis. A model on the therapeutic effect of mind-body medicine and some qualities of practician also were discussed.

Appearance Management Experiences of Breast Cancer Patients -A Grounded Theory Approach- (유방암 환자의 외모 관리 경험에 대한 근거이론적 접근)

  • This study explored the appearance management experiences of breast cancer patients undergoing mastectomy. For data collection, one-on-one interviews were held with 18 Korean females diagnosed with breast cancer from the first stage to the third stage from 2008 to 2015. The data was analyzed through a three-step analysis of grounded theory. QSR NVivo 11.0 for Mac was applied in order to avoid biased results according to the subjective viewpoint of researchers. From open coding results, six paradigms and 15 categories were drawn. The results of axial coding showed that the psychological changes category was the central phenomenon. Additionally, three categories (skin changes, scalp/hair changes, and general reactions) were found to be causal conditions, and five paradigms (clothing behavior, skin care behavior, makeup behavior, scalp/hair-care behavior, and shopping behavior) were found to be action/interaction strategies. Last, selective coding integrated fragmented qualitative data through open coding and axial coding in order to derive explanatory narratives. A comprehensive examination of the appearance management behaviors of breast cancer patients suffering from extreme stress due to physical/psychological changes enables this study to provide a foundation for related studies to be activated in the clothing and textiles discipline.

Therapeutic Approach for Stroke Patients based on Central Pattern Generator (중추유형발생기에 근거한 뇌졸중 환자의 치료적 접근)

  • In the last years, it has become possible to regain some locomotor activity in patients with incomplete spinal cord injury (SCI) through intense training on a treadmill. The ideas behind this approach owe much to insights derived from animal studies. Many studies showed that cats with complete spinal cord transection(spinalized animals) can recover locomotor function. These observations were at the basis of the concept of the central pattern generator located at spinal level. The neural system responsible for the locomotor restoration in both cats and humans is thought to be located at spinal level and is referred to as the central pattern generator(CPG). The evidence for such a spinal CPG in human is emphasis on some recent developments which support the view that there is a human spinal CPG for locomotion. An important element in afferent inputs for both spinal injured cats and humans is the provision of adequate sensory input related locomotor, which can possibly activate and/or regulate the spinal locomotor circuitry This review article deals with the afferent control of the central pattern generator. Furthermore, the application of adequate afferent inputs related locomotor for stroke patients will be able to facilitate locomotion ability, which is automatic, cyclic, rhythmic. These insights can possibly contribute to a better therapeutic approach for the rehabilitation of gait in patients with stroke.

  • The advent of personal healthcare record(PHR) technology has been changing the uses as well as the paradigm of internet services, and emphasizing the importance of services being personalization. But the problem of user's privacy infringement and leaking user's sensitive medical information is increasing with the fusion of PHR technology and healthcare. In this paper, we propose a security labeling scheme for privacy protection in PHR system. In the proposed scheme, PHR data can be labeled also manually based on patient's request or the security labelling rules. The proposed scheme can be used to control access, specify protective measures, and determine additional handling restrictions required by a communications security policy.

  • Conventional antipsychotics are commonly used to treat children and adolescents suffered from schizophrenia to other neuropsychiatric conditions. Regrettably, studies for typical antipsychotics report high rates of sedation, orthostatic hypotension, and extrapyramidal side effects. Over the past few years, atypical antipsychotics have been prescribed for use in adults with psychotic symptoms. Child psychiatrists have begun using these drugs to children and adolescents hoping safe and better alternatives to the conventional antipsychotics. However, there is not enough short-term and almost no long-term data about atypical antipsychotics for pediatric patients. Therefore, the purpose of this article is to review what is known about the use of the atypical antipsychotics in young patients. To do so, an appropriate approach to the use of these drugs in child and adolescent patients my be offered.

  • IHE(Integrating the Healthcare Enterprise)에서는 환자의 전자진료기록 공유를 위하여 XDS.b(Cross-Enterprise Document Sharing.b) 프로파일을 개발하였다. 그러나 XDS.b 프로파일의 Document Source와 Document Consumer는 모바일 환경에서는 사용하기에는 부적당하기 때문에 모바일 환경에서 XDS.b infrastructure에 쉽게 접근할 수 있는 MHD(Mobile access to Health Documents) 프로파일을 개발하였다. 본 논문에서는 전자진료기록 표준인 HL7 CDA 문서 등록을 위한 XDS.b Document Source의 Provide and Register Document Set-b[ITI-41] 트랜잭션과 MHD Document Source의 Put Document Dossier[ITI-65] 트랜잭션을 모바일 환경에서 구현하고 메시지의 크기를 비교하였다. 구현된 결과를 통하여 MHD ITI-65 트랜잭션의 메시지 크기가 XDS.b ITI-41 트랜잭션 메시지 보다 80% 정도 감소함을 알 수 있었다.
