  • This paper presents the content regarding electronic medical examination chart and data processing for efficient medical examination and fast treatment by realizing remote medical examination system of mutual conversation type among 3 parties(patient, doctor, pharmacist) on internet base, and establishment of database enabled system integration for efficient data processing in both on-line and off-line mode by interconnecting ASP and SQL on IIS 4.0 web server, consultation between patient and doctor, medical examination on off-line mode, transmission of prescription sheet to the pharmacist designated by patient, preparation of medicine, semieternal storage of medical examination data owing to storage and check of medical examination data, more accurate medical examination and prescription using this medical examination data by patient and doctor, and so on. And, data processing between doctor and pharmacist is differently performed based on class such as general member and charge member, and service access right pursuant to this is endowed, so that certification of each member must follow by all means.

  • The constitution of traditional medicine is distinguished from modern medicine. Sasang medicine describe the constitution which is classified as functional intensity of the lung, spleen, liver and kidney. The functional strength of each part of the human body is not the same in each constitution, and this imbalance is considered as a major factor of the disease. Ayurveda is identified the balance of the three dosha in our body is the basis of health maintenance. The reason for the illness is due to the imbalance among vata which express energy metabolism, pitta which plays a role in metabolism and digestion, and kapha which controls body temperature. The two theories which include knowing the characteristics and trends of diseases according to their constitution, the approach of treatment and the health care of daily life. They are also considered as convergent area about treatment methods in the modern society where individual characteristics and lifestyle are emphasized. The purpose of this study is to understand the concept of constitutional basis for increasing patient satisfaction and to help diversity of treatment in preventive medical area by comparing and analyzing the theory of Sasang and Ayurveda.

A Convergent Approach to Gain a Better Understanding of Alzheimer's Disease: Stereoscopic Vision (융복합적 접근을 통한 알츠하이머형 치매의 이해 증진 : 양안 지각)

  • The present study investigated the effects of AD on stereoscopic vision. Sixty participants (20 AD patients, 20 mild cognitive impairment patients, 20 healthy elderly controls) participated in the study. Two cubes, one on the left and the other on the right of the center of the monitor, appeared at varying distances from the observer with their relative distances controlled in two disparity conditions (absolute vs relative disparity) combined with two disparity directions (crossed vs uncrossed disparity). Participants identified the object that appeared closer to them. Results demonstrated comparable performance with all three groups performing accurately, suggesting that the effect of AD on stereopsis is negligible. Discussion focused on brain pathology affected by AD involving high level visual processing.

Health Approaches for Weight Perception and Weight Loss Efforts in Hypertensive Patients with Obesity: The 2016-2019 Korea National Health and Nutrition Examination Survey

  • This study was conducted to identify factors influencing weight loss efforts in hypertensive patients with obesity in Korea using data from the 2016-2019 National Health and Nutrition Examination Survey. Weight perception, weight loss efforts, and weight control methods were investigated for 1,910 subjects. Data were analyzed using descriptive analysis, cross-tabulation analysis and logistic regression. Among obese hypertensive patients, 12.6% perceived their weight as normal. Weight loss efforts were 2.03 times (95% CI: 1.48 to 2.78) higher in people with overweight perception than those with normal weight perception, and 1.74 times (95% CI: 1.33 to 2.26) higher in women than in men. In addition, those with class 1 obesity were 1.50 times (95% CI: 0.85 to 2.65) higher than those with class 3 obesity, and those with class 2 obesity were 2.16 times (95% CI: 1.16 to 4.00) higher than those with class 3 obesity. These results suggest that weight management approaches for hypertensive patients with obesity should be individually designed according to weight perception, gender, and obesity class.

  • There are various approaches to treatment for patients with fully edentulous jaw. In maxilla, it is easy to obtain sufficient stability, support, and retention with the traditional full denture method. In mandible, however, there are difficulties caused by anatomical limitations. In this case, treatment with implants can be considered. If it is difficult to place implants in posterior area, a fixed complete denture can be considered, using 4 - 5 implants anterior to mental foramen. This approach has changed in many ways, such as surgical methods and materials used, over the years. This case report compares two approaches and their changes over time, especially in the cases of mandibular fixed prostheses using implants, paired with maxillary complete denture.

A Qualitative Study on Reducing Dental Anxiety through Cognitive Behavioral Therapy (인지행동치료를 통한 치과불안 감소에 대한 질적 연구)

  • Many people in Korea suffer from a fear of dental treatments; however, only a few studies have focused on this population. This study assessed the effectiveness of cognitive behavioral therapy (CBT) and its interventions in reducing dental anxiety, especially when administered by trained dental staff. Using case studies, the authors analyzed the content of each session and examined the process of reducing dental anxiety. First, the authors observed that the results of both dental fear scale and subjective unit of disturbance decreased significantly with changes in cognitive distortions related to dental anxiety and that rapid improvements were noted after two therapy sessions. Second, the study reported that practicing relaxation (deep-breathing) and muscle relaxation techniques; using hand signals to indicate anxiety or discomfort; establishing an empathic relationship with the therapist; making an anxiety list; and graded insensitive and exposure along with rehearsal, were effective in coping with dental anxiety and in changing negative thoughts towards the dentist. Third, a simple cognitive behavioral intervention can be effective in reducing dental anxiety. In summary, a short-term, five-session psychological intervention with CBT produced a sustained decrease in the symptoms of dental anxiety, allowing the patients to receive the required procedures. The authors discussed the study limitations and the implication of their results on future research.

The Effect of Nonpharmacologic Interventions on Behavioral and Psychological Symptoms of Dementia : A Meta-Analysis (치매환자의 행동심리증상에 비약물적 중재가 미치는 효과 - 메타분석)

  • To investigate a variety of nonpharmacologic interventions have confirmed what the symptoms and effects, mainly interventions by acting on behavioral and psychological symptoms of dementia was conducted a meta-analysis. Data were collected from online search engines using search words from domestic and foreign article database. The researcher independently and among the pre-post experimental studies published from January 2000 to June 2016, recalled applying for the elderly with dementia presents the effect of treatment group and the control group RCT in this study were included in the study. The results of this meta-analysis showed that, effect size of the nonpharmacologic interventions of total -0.33, occupational therapy - 0.26, multi-sensory stimulation intervention -0.65 was significant beneficial effects to elderly with dementia(p<.05). According to interventions as a major affected the symptoms associated with behavioral problems, mainly aggression, memory-related problem behavior in the home-based program. Also, reminiscence therapy and occupational therapy is generally apathy, multi-sensory stimulation and music therapy was confirmed that there was a major change in behavior anxiety or agitation. The results of this study confirmed that various nonpharmacologic interventions were effective on behavioral psychological symptoms of dementia patients and confirmed the main symptoms of intervention.

A Study of Optimal Location and Allocation to Improve Accessibility of Automated External Defibrillator (자동제세동기의 접근성 향상을 위한 배치 적정성 연구)

  • On account of population aging and increasing cardiovascular disorders, acute cardiac arrest cases are escalating each year. In order to increase the survival rate of the patients, rapid cardiopulmonary resuscitation is necessary. For this reason, the government is expanding the supply of Automated External Defibrillators(AED). However, the AEDs cannot be effectively deployed to the incident cases that occur outdoors, for the installed AEDs are mostly located indoors. After analyzing the distribution of incident cases within the study area, about 11% of cardiac arrest incidents occurred in open spaces including sidewalks and residential areas. This study was conducted to increase the survival rate of the patients by allocating 41 additional AEDs in the study area using a GIS based location-allocation method. To conduct a feasibility study, the density of a senior floating population covered by the same number of AEDs placed at random and the density covered by the experiment were compared. In conclusion, having excluded outliers caused by geological and social factors, results showed that AEDs placed through GIS based location-allocation covered 5% more of the senior floating population density.

  • This study was conducted to identify the factors affecting the quality of life of cancer patients based on demographic characteristics, health status, and mental health using the results of the 6th National Health and Nutrition Survey. The research tool used the demographic characteristics, health status, mental health, and quality of life of the KNHANES VI-2. Data analysis was conducted using SAS 9.3 version. There were significant differences in the quality of life in the study results by age, gender, economic activity, individual income quartile, subjective health status, mental health status, activity limitation, stress perception, and experience of depression symptoms. Factors influencing the quality of life of cancer patients included age, sex, income quintiles(individual), subjective health status, activity limitation, and experience of depression symptoms, which together explained 39.1% of the quality of life. To improve the quality of life of cancer patients, comprehensive management is required from diagnosis to treatment to return to society. It is also necessary to develop and apply long-term programs including multidimensional approaches and improvement of quality of life, early screening of cancer and cancer prevention education, management of depression symptoms, and social support.

Sleep Disorders and Restless Legs Syndrome in Alcohol Dependence Patients (알코올 의존 환자의 수면 장애와 하지불안증후군)

  • Objectives: We review the sleep problems of the alcohol dependence patients. Especially we are interested in the prevalence, the severity of symptoms, anxiety, depression, and sleep quality of restless legs syndrome (RLS) in the alcohol dependence patients. Methods: We recruit 86 alcohol dependence patients who were admitted from October 6th, 2008 to October 17th, 2008. We interviewed each patient and evaluated sleep questionnaires such as the Sleep Disorder Questionnaire (SDQ), the Pittsburgh Sleep Quality Index (PSQI) and the International Restless Legs Syndrome Study Group (IRLSSG) rating scale. The presence of RLS and its severity were assessed using the IRLSSG diagnostic criteria and the IRLSSG severity scale, respectively. Depression and anxiety were evaluated by the Beck Depression Inventory (BDI) and the Beck anxiety inventory (BAI). Results: Of all 86 patients, 59 patients have insomnia, 33 patients have RLS, 30 patients have Periodic limb movement disorder (PLMD), 29 patients have nightmare. RLS patients have more high score in the BAI ($21.70{\pm}10.36$ vs $14.67{\pm}11.98$), and their sleep quality was poor in the PSQI ($11.09{\pm}4.08$ vs $7.92{\pm}3.91$) than non-RLS patients. Conclusion: This study shows that alcohol dependence patients show many sleep problems such as insomnia and RLS. So we should notice that the sleep problems of alcohol dependence patients are important in clinical approach and treatment.
