The Relationship among Dementia care Knowledge, Attitudes toward to dementia and Approach to dementia care of Nurses (간호사의 치매케어 관련 지식, 치매에 대한 태도 및 치매간호 접근법과의 관계)

  • The purpose of this study was to examine the relationships among dementia care knowledge, attitudes toward to dementia and approach to dementia care of nurses. The participants were 202 nurses in G hospital of P city. The mean score of dementia care knowledge was 11.02, attitude toward dementia was 40.15 and approach to dementia care was 7.90. The dementia care knowledge was significant difference for sex(t=-2.424, P<.05), education level(F=2.932, P<.05) and work unit(F=2.268, P<.05). The dementia care knowledge was positively correlated with attitude toward to dementia(r=.162,P<.05 ). The predictors of dementia care knowledge were Emergence Room(${\beta}=-1.568$, p<.05) and Intensive Care Unit(${\beta}=-1.682$, p<.05). The result suggest that it is necessary to develop convergence education program to improve dementia care knowledge and attitudes toward to dementia to give high quality nursing care for patients with dementia.

A Comparative Study Concerning the Psychopathologies between the Patients with Peptic Ulcer and Those with Essential Hypertension - Focused on Medically-Ill Out Patients - (소화성 궤양과 본태성 고혈압 환자의 정신병리에 관한 비교 연구 - 내과외래환자를 중심으로 -)

  • Objectives: The authors studied the anxiety, depression and psychological characteristics of medical out-patients, which are diagnosed as peptic ulcer disorder and essential hypertension. We also examined the state of psychiatric consultation. Methods: The subjects were composed of 56 patients with peptic ulcer disorder, 44 patients with essential hypertension and 116 controls. STAI, BDI and SCL-90R was administered to all subjects. Chart review, telephone interview and Self report questionnaire of etiology and severity of illness, drug compliance and so forth were performed in disease groups. Results: Considerable depression was noticed in 39.3% among the patients with peptic ulcer disorder, 27.7% in hypertension and 12.1% in control group by BDI. State anxiety was noticed in 44.6% among the patients with peptic ulcer disorder, 54.5% in hypertension and 18.1% in control group by State anxiety. Trait anxiety was noticed in 42.9% among the patients with peptic ulcer disorder, 34.1% in hypertension and 25.8% in control group by Trait anxiety. The higher SOM, BDI, STAI, the lower drug compliance and rapport. Psychological problems are considered of the most important etiology in 48.2% of peptic ulcer group and 45.7% of hypertensive group. But psychiatric consultation is made only in 1.8% of peptic ulcer group and 2.3% of hypertensive group. Conclusions: Anxiety and depression are common phenomena in medical outpatients. In comparison with the normal control group, peptic ulcer group showed significantly higher trait anxiety and depression and hypertensive group higher state anxiety. These characteristics are related to the drug compliance and doctor-patient relationship. These results suggested the needs of active psychiatric consultation.

Emotional State and Personality Characteristics in Patient with Panic Disorder (공황장애 환자의 정서상태와 성격특성)

  • Objectives: The purpose of the present study was to investigate the emotional state and personality characteristics of patient with panic disorder. Methods: Twenty patients with panic disorder, 21 patients with somatoform disorder, and 20 normal healthy controls were studied. Korean version of Beck Depression Inventory(BDI), State and Trait Anxiety Inventory(STAI) and Korean standardized edition of Cattell's 16 Personality Factors Questionnaire(16-PF) were used for assessment. Statistically, One-way ANOVA with Scheffe test were used by SPSS/PC for windows. Results: 1) Total score of BDI was higher in the panic group than the somatoform group and normal control group(p<0.001). 2) Total score of state anxiety and trait anxiety in the panic group was higher than the somatoform group and normal control group(p<0.001). 3) In 16 PF, there were no definitely abnormal range of scores. But in first-stratum source traits of 16PF, the panic group was higher than the somatoform group and normal healthy group in O-factor (p<0.01) and Q4-factor(p<0.001). In second-stratum source traits of 16PF, the panic group was higher than the somatoform group in ANX-factor(p<0.05), but lower than normal control group in TOUfactor(p<0.05). 4) There were no differences in the panic subgroup according to sex and cutoff points of BDI score 16 and STAI-T score 54. 5) According to cutoff point of STAI-S score 52, the panic subgroup above 52 was higher than the panic subgroup under 52 in G-factor(p<0.001), Q3-factor(p<0.05) and SUP-factor(p<0.001), but lower in L-factor(p<0.05). Conclusion: These results suggest that emotional state of patients with panic disorder are depressive and anxious as compared with patients with somatoform disorder and normal controls. Patients with panic disorder did not show any abnormal personality characteristics but were more guilt-prone, anxious, emotionally sensitive. We propose that the understanding of panic patients' emotional state and personality characteristics will helpful to treat and manage in patient with panic disorder.

Comparison of the Usefulness of Diagnostic Tests for Rheumatoid Arthritis (류마티스 관절염 진단을 위한 검사의 유용성 비교)

  • Rheumatoid arthritis (RA) is a chronic inflammatory disease, which is mainly characterized by disease of joints affected with synovial hyperplasia, pathological immune response, and progressive destruction; all of which represent an important social health problem. These provide new insights in its pathogenesis of rheumatoid arthritis diagnosis and disease progression in molecular changes. This review focuses on new serological and immunological markers which seem to be useful in early diagnosis and prognosis of rheumatoid arthritis. Therefore, such tests are widely conducted for serological biomarkers and the developments with such immunological factors to identify patients who are at risk for disease progression. This evidence of the disease based on laboratory medicine could provide the best outcome for patients. Finally, data from recent studies will help to refine the ultimate usefulness of this novel approach for early diagnosis, treatment, and helping clinicians to optimize therapy by using this approach.

A Qualitative Study of Physicians' Use of Clinical Information Resources and Barriers (임상의사의 진료목적 정보원 이용과 장애요인에 관한 질적 연구)

  • We analyzed the characteristics of the physicians preferred information sources and barriers through in-depth interviews. Information searches for patient treatment were subdivided into deciding patient treatment methods, understanding the latest treatment trends, and preparing presentation materials for conferences. The variables that affected the search behaviors were identified as being background knowledge on the topic, clinical experience, job title, search skills, user training, and familiarity with the library homepage. PubMed was the most preferred choice because of users' familiarity, reliability, and the vastness of information; Google was also used frequently for easy access and fast search result. The accuracy and the recentness of information were the most significant criteria. Easy interface and convenient access were also considered important due to physicians' time constraints. Searching obstacles were divided into difficulty of searching system, unfamiliar term, too vast resources, difficulty to get fulltext articles and complex advanced search features. The results of this study can be utilized as a basis for improving information service of library and curriculum development for physicians.

  • Bruxism is a much-discussed clinical issue in dentistry. Although bruxism is not a life-threatening disorder, it can influence the quality of human life, especially through dental problems, such as, frequent fractures of dental restorations and pain in the orofacial region. This research has a goal to investigate the diagnostic methods of bruxism, to provide an appropriate information about various treatment in clinical situation, and to evaluate the effect and the usefulness of those methods. There is no certain remedy for bruxism that is a technically efficient and definitely reliable diagnosis and treatment. So, the primary purpose is to prevent the oral and maxilofacial tissue injuries from bruxism and to relieve the pain and symptom. Therefore, Combining various reversible treatments together, such as behavior modification, Oral appliances therapy and physiotherapy, is recommended. For a bruxism treatment in dental field, more researches about the factors influencing on diagnosis and cure are necessary.

Short-term clinical outcome of intentionally replanted posterior molars (의도적 재식술을 시행한 대구치의 단기간의 임상 평가)

  • Objectives: This retrospective study evaluated the therapeutic effects of the intentional replantation (IR) procedure performed on the maxillary and mandibular molars of 35 patients. Materials and Methods: For the subjects, IR was performed due to difficulties in anatomically accessing the lesions and/or close proximity to the thick cortical bone, inferior alveolar nerve, or maxillary sinus, which rendered the ordinary periradicular surgery impossible. The patients'progress was followed for a year and up to 2 years and 4 months. The success of the procedure was evaluated in terms of clinical and radiographic success (%). Results: The results revealed the following: (a) 1 case (3%) of failed tooth extraction during IR; (b) 2 cases (6%) of extraction due to periodontal diseases and inflammatory root resorption; (c) 3 cases (9%) of normally functioning teeth in the oral cavity with minor mobility and apical root resorption, and; (d) 29 cases (82%) of normally functioning teeth without obvious problems. Conclusions: IR was confirmed to be a reliably repeatable, predictable treatment option for those who cannot receive conventional periradicular surgery because of anatomic limitations or patient factors.

Effects of Muscle Relaxation Approach and Joint Movement Approach on Neck Movement and Comfort of Daily Living in Patients with Tension-type Headache of Forward Head Posture (근육 이완 접근과 관절 가동 접근이 긴장성 두통을 가진 두부 전방 전위 자세 환자의 목의 움직임 및 일상생활 편안함에 미치는 영향)

  • Objectives The purpose of this study was to improve the comfort of daily life such as reduction of headache and increase of movement of neck by using muscle relaxation approach and joint movement approach for office worker with tension type headache of foward head posture sitting over 5 hours. Methods For this, 9 male and 15 female participated in the foward head posture with tension type headache. Each group consisted of 3 male and 5 female. Groups are divided into groups, such as muscle relaxation therapy, joint movement therapy, muscle relaxation and joint movement therapy. After intervention for each group for a month, we measured neck movement and head disability index and neck disability index 2 week. SPSS 23.0 (IBM Corp., Armonk, NY, USA) was used for data analysis. The one-way repeated analysis of variance (ANOVA), one-way ANOVA, compared t-test was used for statistical analysis. Results Three intervention groups have brought improvements in neck movement and daily life comfort. There is significant difference in the improvement of neck extension and change in neck disability index between 2 and 4 weeks in the joint movement approach compared to muscle relaxation approach, muscle relaxation and joint movement approach. Conclusions Office workers are exposed to tension type headache. However, muscle relaxation approach and joint movement approach can improve neck movement and daily life comfort.

A Study on the Relationship between Cognitive Processes and Emotion Regulations in Depression and Anxiety Disorder: Focused on the Neurocognitive Networks (우울 및 불안 장애에서의 인지적 처리와 정서조절 고찰: 신경인지 연결망을 중심으로)

  • This review aims to propose a model that can reinterpret the abnormal and functional connections between cognitive processes and emotional regulations based on the neurocognitive networks for a comprehensive understanding of pathologic processes and treatment approach of depression and anxiety disorder. Through the processes of rebuilding the network model for depression and anxiety disorder, it was confirmed that depression can be said to be 'over-immersion in self-referencing' due to hyper-activation of default mode network (DMN), and anxiety disorders to be 'disconnection with self-referencing' due to hypo-activation of DMN. The attempts to link up between abnormal activation and pathological function of DMN which is thought to be involved in self-referential processing associated with self-consciousness and projection among neurocognitive networks may be another starting point that can afford to be suggestive in integrated interpretation and therapeutic approach to depression and anxiety disorder.

Alexithymia in Somatoform Disorder and Diabetes Mellitus (신체형장애 환자와 당뇨병 환자에서의 Alexithymia)

  • Objectives : "Alexithymia" mean literally "no word for mood(or emotion)". It is not only a marked constriction in emotional functioning but a deficit in their cognitive processing. We designed this study to investigate the level of alexithymia, psychopathology and personality factors of patients with somatoform disorder and with diabetes mellitus. Methods : The subjects were consisted of patients with somatoform disorder(N=20), patients with diabetes mellitus(N=20), and normal control(N=20). The level of alexithymia, psychopathology and personality factors were assessed by the Toronto Alexithymia Scale(TAS), the Symptom Checklist 90-Revision(SCL 90-R), and the Sixteen Personality Factor Questionnaire(16-PF). And we compared demographic characteristics, psychopathology and personality factors among three groups, and assessed the relationship between alexithymia and psychopathology, and between alexithymia and personality factors. Results : The results were as follows. 1) Patients with somatoform disorder showed significantly higher TAS scores compared to patients with diabetes mellitus and the normal control group. 2) Patients With somatoform disorder showed significantly higher scores of somatization, anxiety scales than patients with diabetes mellitus and the normal control group, and showed significantly higher scores of obsessive-compulsive, depression, phobic anxiety, psychoticism scales than the normal control group by the SCL-90-R. 3) The normal control group showed high intelligence scores only as compared to patients with somatoform disorder by the 16-PF. 4) A significant relationship was found between TAS scores and psychopathology in patients with somatoform disorder. 5) All three groups did not shown any correlation between TAS and 16-PF. Conclusion : Patients with somatoform disorder showed higher TAS scores and more multiple psychopathology than patients with diabetes mellitus and the normal controls. A significant relationship was found between TAS scores and psychopathology in patients with somatoform disorder. We suggest that the therapeutic approach to patients with somatoform disorder to express emotions and manage psychopathology, and that the treatment methods of patients with diabetes mellitus aims to improve firstly physical conditions are more helpful.
