중소병원 간호사의 환자안전문화 인식과 표준주의지침 수행도의 관계에서 비판적 사고성향의 매개효과 (Mediating effects of Critical Thinking Propensity between Perception of Patient Safety Culture and Practice of Standard Precaution of Nurses in Small and Medium Hospitals)

  • 본 연구는 중소병원 간호사의 환자안전문화 인식, 비판적 사고성향과 표준주의지침 수행도와의 관계를 확인하고자 수행하였다. 연구대상자는 S시, C시 소재 3개 중소병원의 간호사로 연구목적을 이해하고 함여 의사를 표시한 대상자는 158명이었다. 자료수집 기간은 2020년 1월 5일부터 2020년 3월 31일까지이었고, 수집된 자료의 기술통계, t-test, one-way ANOVA, 상관관계, 회귀분석, Sobel test는 SPSS/WIN26.0 Program을 이용하여 실시하였다. 연구결과 간호사의 표준주의 지침 수행도는 환자안전문화인식(r=.524, p<.001) 및 비판적 사고성향(r=.471, p<.001)과 유의한 양의 상관관계를 보였다. 비판적 사고성향은 환자안전문화 인식과 표준주의지침 수행도의 관계에서 부분매개효과가 있는 것으로 나타났다. 향후 본 연구결과를 토대로 반복연구가 이루어지고 간호사의 표준주의지침 수행도를 증진시킬 수 있는 교육프로그램의 개발 및 적용에 활용될 수 있을 것이다.

  • 문화마케팅이 가장 활발하게 펼쳐지고 있는 곳은 어디일까. 백화점 스포츠센터 웨딩홀 이제 그 대답을 병원에서 찾아야 할 것 같다. 건국대학교를 비롯해서 현대아산병원, 삼성병원, 세브란스병원, 국립의료원, 경희의료원 등이 음악회를 여는가 하면 미술전시 공간을 마련해 놓고 환자는 물론 지역사회를 위한 문화공간을 제공하는데 앞장서고 있다. 우리가 살아가는 데 그리 익숙하지는 않은 곳이나 몇 번씩은 꼭 찾게 되는 곳 "병원"의 변신. 우리에겐 매우 반가운 현상이 아니겠는가.

토픽 모델링과 동시출현 단어 분석을 활용한 환자안전 관련 사회적 이슈의 변화 (An Analysis of Changes in Social Issues Related to Patient Safety Using Topic Modeling and Word Co-occurrence Analysis)

  • 본 연구의 목적은 온라인 뉴스 기사를 분석하여 환자안전에 대한 사회적 이슈를 확인하고, 환자안전법 시행 이전과 시행 이후 사회적 이슈의 변화를 확인하기 위함이다. R 프로그램을 이용하여 2010년 1월 1일부터 2020년 3월 5일까지 총 7600건의 온라인 뉴스 기사를 수집하였으며, 키워드 분석, 토픽 모델링, 동시출현 네트워크 분석을 시행하였다. 2609개의 키워드는 다음의 8가지 주제로 범주화되었다 : "의료행위", "의료인력", "감염 및 시설", "간호·간병통합서비스", "의약품", "개선을 위한 시스템 개발 및 구축", "환자안전법", "의료기관 인증". 그리고 환자안전법 시행 이전에는 환자안전 인식, 감염관리, 의료기관 인증 등의 키워드가 등장하였으나 시행 이후에는 환자안전 문화, 투약 등의 키워드가 등장하였으며 간호의 중요도 순위가 상승하였다. 의료계뿐 아니라 대중에게도 환자안전에 관한 관심은 높아지고 있으며, 환자안전 향상에 간호의 역할은 중요하다. 따라서 환자안전을 간호의 핵심 역량으로 삼고 지속적인 교육을 해나가야 할 것이다.

군 병원 간호사의 의사소통능력, 비판적 사고성향, 환자안전문화 인식이 환자안전역량에 미치는 영향 (The Influence of Nurses' Communication Competency, Critical Thinking Disposition, and Perception of Patient Safety Culture on Patient Safety Competency in Armed Forces Hospitals)

  • Purpose: This study examined the influence of nurses' communication competency, critical thinking disposition, and perception of patient safety culture on patient safety competency in armed forces hospitals. Methods: A crosssectional design was employed with a convenience sample of 204 nurse officers from four armed forces hospitals in South Korea. Data were collected through a self-administered questionnaire from August 10~23, 2019, and analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and multiple regression. All analyses were conducted using SPSS for Windows 21.0. Results: Patient safety competency scores were 4.35±0.40 for patient safety attitudes, 3.63±0.64 for patient safety knowledge, and 3.94±0.51 for patient safety skill. Communication competency, critical thinking disposition, and perception of patient safety culture were positively correlated with patient safety competency. Communication competency (β=.30, p=.002) and perception of patient safety culture (β=.24, p=.001) were identified as particularly important factors influencing patient safety competency. Conclusion: This study's results suggest that education programs to enhance communication competency and patient safety culture could increase patient safety competency among nurses in armed forces hospitals.

요양병원 간호사의 공감만족 관련요인-환자안전문화 중심으로 (Factors Associated with Compassion Satisfaction of Nurses in Long-term Care Hospitals: Focused on Patient Safety Culture)

  • Purpose: The study investigated the factors associated with compassion satisfaction of nurses in long-term care hospitals with a focus on the culture of patient safety. Methods: This study was conducted using a descriptive survey. The participants were nurses in 1 metropolitan city and 2 other cities (N=164). The data was collected in October, 2018 and analyzed using the SPSS Win 25.0 program. Results: The factors associated with compassion satisfaction were job satisfaction, work attitude, management practice, leadership of the managers of patient safety culture, and clinical career (F=21.66, p<.001). These variables explained 49.3% of the variance in compassion satisfaction of long-term care hospital nurses. Conclusion: These results highlight the importance of long-term care hospital nurses' compassion satisfaction and can be used as meaningful data for intervention to promote a culture of patient safety, and nurses' job satisfaction.

마취·회복실 간호사의 환자안전문화와 환자안전역량이 안전간호활동에 미치는 영향 (Effect of Patient Safety Culture and Patient Safety Competence on Safety Nursing Activity among Nurses working in Anesthetic and Recovery Rooms)

  • Purpose: This study was to investigate the relationships among patient safety culture, safety competence and safety nursing activity among nurses in anesthetic and recovery rooms, and to identify the factors contributing to safety nursing activity. Methods: A descriptive correlational study was conducted. Participants were 156 nurses from 13 hospitals. Data were collected from February 11 to March 15th, 2019, and analyzed using descriptive statistics, t-test, Mann-Whitney U test, one-way ANOVA, Pearson's correlation and multiple regression analysis with SPSS statistics 24.0 Program. Results: Safety nursing activity was significantly different in relation to nurses' level of education, position at work, clinical career, clinical career at anesthetic and recovery rooms, and work experience in patient safety. Safety nursing activity demonstrated a significant positive correlation with patient safety culture and patient safety competence. Factors contributing to safety nursing activity were patient safety knowledge, skill and attitude, clinical career, clinical career at anesthetic and recovery rooms, and the patient safety improvement system which explained 57.0% of total variance of safety nursing activity. Conclusion: To improve safety nursing activities at anesthetic and recovery rooms, it is necessary to develop patient safety programs with enhanced knowledge, skill and attitude to take patient safety as a top priority.

  • 본 연구는 간호사가 지각하는 공유리더십, 조직의사소통, 간호서비스 질을 파악하여 환자안전관리활동에 영향을 미치는 요인을 분석하여 이론적, 실천적 정보와 중재방안을 위한 기초자료를 마련하기 위한 서술적 조사연구이다. 본 연구는 G도 소재의 C지역 임상 간호사 155명을 대상으로 2023년 7월 17일부터 7월 28일까지 자료를 수집하였으며, 총 154부를 최종 분석하였다. SPSS Win. 25.0 프로그램을 사용하여 기술통계, t-test, one-way ANOVA, Pearson correlation coefficient, hierarchial multiple regression으로 분석하였다. 대상자의 환자안전관리활동에 영향을 미치는 변수들을 위계적 선택법을 이용한 다중회귀로 분석한 결과 공유리더십이 높을수록, 조직의사소통과 간호서비스 질이 높을수록 환자안전관리활동에 영향을 미쳤으며, 설명력은 45.9%이었다. 간호사의 환자안전관리활동에 대한 효율적인 간호수행을 위하여 환자안전관리활동을 지지할 수 있는 구체적인 중재 프로그램을 개발하고, 그 효과를 검증하는 연구가 지속적으로 시행되어야 할 것이다.

치과위생사의 환자안전문화 인식이 환자안전관리활동에 미치는 영향 (Influence of perceived patient safety culture on patient safety management activity in the dental hygienists)

  • The study aimed to investigate the influence of perceived patient safety culture on patient safety management activity in the dental hygienists. Methods: A self-reported questionnaire was completed by 292 dental hygienists in Seoul, Incheon and Gyeonggido from March 1 to April 8, 2016. The questionnaire consisted of general characteristics of the subjects (9 items), patient safety culture (44 items), and patient safety management activity (25 items) by Likert 5 point scale. Data were analyzed by t test, one way ANOVA, stepwise multiple regression test, and post-hoc Tukey test using SPSS 18.0 program. Results: The perceived patient safety culture was 3.50 on average. Entire organization was the highest score (3.68) and followed the communication process (3.55), the environment of work unit (3.47), the attitude of supervisor/manager (3.45), and the frequency of events reported (2.98). The average of patient safety management activity was 3.71. As for the factors of patient safety culture on patient safety management activity, communication process was the most influential factor (${\beta}=0.268$), and followed the entire organization (${\beta}=0.265$), the environment of work unit (${\beta} =0.166$), the frequency of events reported (${\beta}=0.104$), and among the control variables. Work place proved to be the only significant variable. Conclusions: In order to promote patient safety management activity of dental institutions, the patient safety culture should be created and established. The influence of communication process and patient safety culture at entire organization level was huge. So the environment of work unit and the perceived patient safety culture in the process of reported events were influencing factors. The strategy for patient safety management activity should be considered because of low level of perceived patient safety culture.

병원 간호사가 지각하는 환자안전문화와 안전간호활동과의 관계 (Relationship between Hospital Nurses' Perceived Patient Safety Culture and Their Safety Care Activities)

  • Purpose: This study was performed to measure hospital nurses' perceived patient safety culture and their safety care activities, and to investigate the relationship between these two factors. Method: This study was a cross-sectional survey. The participants were 301 nurses working at 4 general hospitals, and data collection was done from June 22 to June 30, 2009 by self-administrated questionnaires. Results: With a possible score of 5 points, the average score for nurses' perceived patient safety culture was 3.34, and for their safety care activities, 4.25. There were perceived differences in patient safety culture and safety care activities according to age, position, length of work experience and number of patient safety education sessions attended. All sub-factors in patient safety culture had a positive relationship with safety care activities. Factors influencing nurses' safety care activities were number of patient safety education sessions attended, hospital environment, and supervisor/manager. These factors explained 58.2% of the variance. Conclusion: The findings indicate that patient safety education is very important to improve nurses' safety care activity. So nursing supervisors/ managers should develop strategies encourage patient safety education, and make nurses' working environment safer.
