  • 최근 생활습관정보는 대사증후군을 진단하기 위한 임상적 진단지표로 중요하게 활용되고 있다. 대사증후군은 심혈관 및 간질환 그리고 당뇨와 같은 여러 합병증을 유발할 수 있는 질환으로 질환 정도에 따른 체계적 관리가 필요하다. 그러나 대사증후군 환자의 생활습관을 수집하기 위한 대부분의 시스템은 자가진단 및 예방 중심의 시스템으로 구성되어 있어 정확한 생활습관을 수집하여 생활습관을 관리하기에는 어려움이 있다. 본 논문에서 제안하는 시스템은 임상적 진단지표에 도움이 될 수 있도록 신뢰성 있는 생활습관 정보를 수집하기 위한 방법을 제시하고 수집된 생활습관정보를 모니터링 하여 환자의 생활습관 개선 여부에 따라 지속적인 피드백을 제공하여 체계적으로 생활습관을 관리할 수 있는 시스템을 제안하고자 한다.

Effects of a Computer-based Cognitive Rehabilitation Therapy on Mild Dementia Patients in a Community (지역사회 경증치매환자를 대상으로 한 전산화 인지재활 치료(COMCOG) 효과)

  • This study aims to understand the effects of a Computer - based Cognitive Rehabilitation Therapy(CBCRT) evidence based on mild dementia patients' ability to activities daily living(ADL), cognitive function and measure of occupational performance and to suggest basic data for a cognitive rehabilitation therapy for dementia patients. Method : A CBCRT was applied two times a week for 5 weeks to 14 mild dementia patients who visited Yongin Center for Managing Dementia in Gyeongi-do between February and August 2009. Based on frame of reference for Visual-Perception a CBCRT was applied at home. Moreover, a one group pretest-post test design was, which is a quasi-experiment and research, also applied in order to verify the effects of the rehabilitation therapy on the subjects' ability to ADL, cognitive function and occupational performance skills. Results: A significant effect was confirmed (p<.05) from the CBCRT which Assessment of Motor and Process Skills(AMPS) processing skills and cognitive function and occupational performance skills. Neither was found any significant effect in improving motor skills from AMPS. Conclusion: It seems that a CBCRT based on evidence and has an effect on the improvement of the ability to ADL and cognitive function of mild dementia patients living in a community. The present author hopes that, in the future, more cognitive rehabilitation programs will be developed to improve the functions of mild dementia patients living in a community.

The Correlation Between Relationship Ability and Activities of Daily Living(ADL) Performance Ability of Patients With Traumatic Brain Injury(TBI) (외상성 뇌손상 환자의 대인관계 능력과 일상생활활동 수행능력과의 상관관계)

  • Objective : To investigate the correlation between relationship ability and ADL performance ability of patients with TBI. Methods : This study was carried out from March 2012 to April 2012 with 20 patients with TBI. Relationship ability assessed using Relationship Change Scale(RCS), ADL performance ability assessed using Functional Independence Measure(FIM). Results : The RCS showed a high level of correlation with marital status and duration of illness. The RCS showed a high level of correlation with the FIM total score and the FIM cognition area. However the RCS did not show a correlation with the FIM motor area. Conclusions : Functional disability of social cognition is a factor that disturbs rehabilitation after TBI and it has a negative influence on the relationship in ADL. If accurate evaluation and treatment on relationship ability after TBI are carried out side by side from the initial stages of onset, we could expect more patients to improve their social cognition and ADL performance ability.

The Effect of Bilateral Arm Training for the Chronic Phase After Stroke in Activities of Daily Living (양측성 상지 활동이 만성 뇌졸중 환자의 일상생활활동에 미치는 영향)

  • Objective : To identify the effect of bilateral arm training for activities of daily living(ADL) of the chronic phase after stroke. Methods : 20 patients with stroke, randomized to an experimental group(n=10) and a control group(n=10). Both groups received the usual occupational therapy. In addition, an experimental group received an bilateral arm training, 30-min sessions per a day for 6 weeks. The Functional Independence Measure(FIM) was used to evaluate ADL for both groups. Results : First, there is a significant difference before and after receiving the bilateral arm training(p <.05). Second, there are significant differences in all ADL areas of Self-care, Transfer, Social cognition after bilateral arm training(p<.05). Third, there are significant differences in all sub-areas of Self-care, only stairs of Locomotion, and only Problem solving of Social cognition by comparing sub-areas as bilateral arm activity(before and after) and occupational therapy intervention(before and after). Conclusion : Using bilateral arm training for the chronic phase after stroke improved their abilities of ADL. Therefore, there are an expectation to use bilateral activity training for the chronic after stroke clinically and a need of further study about bilateral arm training related to ADL in the future.

  • 현재 많은 사람들이 스마트폰을 이용하고 있으며 스마트폰을 활용한 IT 기술들이 나날이 발전하고 있다. 또한 스마트폰을 이용 하여 사람들이 생활하는데 있어서 더욱 더 편리한 생활을 할 수 있도록 서비스 환경을 제공해준다. 그 중 대표적인 기술이 U-Health(Ubiquitous-Health) 기술 이며 U-Health 기술은 건강증진과 고령화 사회를 대비하기 위한 효과적인 문제해결 방안으로 주목받고 있다. U-Health 서비스는 스마트폰의 각종 센서를 활용하여 환자의 건강 상태를 체크하고 실시간으로 업데이트 하여 환자의 상태를 더욱 효율적으로 관리할 수 있다. 그러나 환자와 병원 서버 간에 데이터 단일화, 통신문제 등으로 인해서 아직까지는 많이 활용되고 있지 않는 현실이다. 또한 스마트폰으로 인한 환자의 개인신상 정보가 쉽게 노출될 수 있다는 단점이 있다. 따라서 본 논문에서는 NFC(Near Field Communication) 기반에서 환자와 병원간에 신뢰성 있는 데이터 전송을 위한 인증 시스템을 설계 하였다.

Influences of Prednisone on Muscle Strength and Instrumental Activities of Daily Living in Patients of Kidney Transplantation from Brainstem Dead Heart-beating Donors in Korea (뇌사자로부터 공여받은 신장이식자의 근력과 일상생활활동에 프레드니손(Prednisone)이 미치는 영향)

  • 목적: 본 연구의 목적은 뇌사자로부터 공여받은 신장을 이식한 대상자들이 프레드니손을 생체이식 대상자들보다 더 많이 투여받는 임상상황에서 프레드리손이 근력과 일상생활활동에 영향을 주는지 규명하기 위함이다. 방법: 연구 대상은 뇌사자로터 신장을 응급으로 수여받은 32명이었으며 모두 혈액투석을 하고 있었다. 이식 수술 전날, 환자의 일반적인 정보, 근력, 일상생활활동에 대해 조사하였으며 이식수술 후 12주째 되는 날 외래에서 근력과 일상생활활동을 다시 조사하였다. 프레드니손 용량은 임상기록지를 통해 수집하였다. 연구결과: 신장이식술 후 12주의 근력은 고관절 신전근력을 제외하고는 유의하게 감소되지 않았다. 그러나 신장이식 후 12주의 일상생활활동은 수술 직전보다 유의하게 감소된 것으로 나타났다. 근력은 혈액투석을 시행한 기간에 의한 영향을 더 받았으며 일상생황활동의 저하는 3개월간 활동을 제한하는 퇴원교육의 영향이 큰 것으로 볼 수 있다. 결론: 본 연구에서 대상자의 근력과 일상생활활동은 프레드니손 투약의 영향을 받지 않았으며 오히려 혈액투석을 시행했던 기간에 따라 영향을 받은 것으로 나타났다. 따라서 혈액투석 환자들의 근력 증진을 위한 간호중재 개발이 필요하다.

Effect of Virtual Reality Training Focus on ADL on Upper Extremity Function and Activities of Daily Living in Stroke Patients (일상생활에 중점을 둔 가상현실 훈련이 뇌졸중 환자의 상지기능과 일상생활활동에 미치는 효과)

  • This study aimed to investigate the effect of virtual reality training focus on ADL on upper extremity function and activities of daily living in stroke patients. 20 patients with chronic stroke were included in this study. The participants were divided into the experimental (n=10) or control (n=10) groups. Both groups received conventional rehabilitation therapy for 30 min a day, 5 times per week for 4 weeks. The experimental group also performed virtual reality training focus on ADL for 30 min a day, and the control group performed general virtual reality training for 30 min a day. The Fugl-Meyer Assessment (FMA) and Functional Independence Measure (FIM) scores were evaluated before and after the intervention, and both groups showed significant improvement (p<.05). Compared to the control group, the experimental group showed significant improvement in the FMA wrist score and the FIM total, self-care, and sphincter control scores (p<.05). These findings suggest that virtual reality training focus on ADL may have a positive effect on wrist function and overall ADLs compared to that using general virtual reality training in stroke patients.

  • 당뇨병의 여러 합병증은 주기적인 검진과 함께 환자들도 인식을 많이 하고 있다. 그러나 상대적으로 생명과는 별 연관이 없는 듯 보이고 환자에 따라서는 일시적일 수 있는 신경병증과 이에 따른 발병변에 관해서는 관심이 덜한 편이다. 하지만 조금만 신경쓰면 별 문제없이 생활할 수 있는 발질환을 대수롭지 않게 여기다 결국 조기진단과 치료가 안되어 고생하고, 심지어 발을 절단해 삶의 질을 떨어뜨리는 환자가 많다.

The Study on the Balance Reaction and Physical Activity of Dementia Patients (치매환자의 균형반응과 신체활동에 관한 연구)

  • The purpose of this study was to investigate the balance reaction characteristics of dementia patients. Under the object of 30 dementia patients, they were divided into moderate group and severe group and examined the correlation among Clinical test of sensory interaction and balance(CTSIB), Berg balance scale(BBS) and Bathel index(BI) according to perceptional function. As a result, CTSIB was no significant difference between severe group and very severe group and BBS was significant difference between severe group ($47.32{\pm}4.80$) and very severe group ($43.09{\pm}4.18$)(p<0.05) and BI was significant difference between severe group ($92.89{\pm}9.33$) and very severe group ($76.82{\pm}6.81$)(p<0.05) and significant correlation among perceptional function, BBS and BI(p<0.05, p<0.01). Therefore, these results should be used the base data of developing rehabilitation program or protection management for dementia patients.

  • This study investigated the effects of dual task training on balance, upper extremity motor function and activities of daily living in patients with chronic stroke. The study subjects were 14 chronic stroke patients. dual task training groups(n=7), and single task training groups(n=7). The intervention period was conducted for 3 times a week, 30 minutes per session, and 4 weeks for all groups. To measure the BT4, JTT, K-MBI were used before and after the intervention. Dual task training groups showed a significant increase in BT 4 and JTT scores(p<.05). The difference between the two groups was significant in the BT4 and JJT scores(p<.05). There was a score improvement in K-MBI, but there was no statistically significant difference(p>.05) Dual task training was effective in facilitating balance, upper extremity function and activities of daily living in patients with chronic stroke.
