Convergence outpatient medical service patient experience research using data mining (데이터마이닝 기법을 이용한 융복합 외래 의료서비스 환자경험조사 연구)

  • The purpose of this study is to find out specific measures that can help the management strategy of patient-centered medical institutions by conducting research on patient experience surveys of convergence outpatient medical services using data mining techniques according to changes in patient-centered medical culture. Using the raw data of the 2018 Medical Service Experience Survey, 8,843 people over the age of 15 who had patient experience in outpatient medical services were analyzed. Decision tree analysis was performed. The determinants of satisfaction with outpatient medical services patient experience were the doctor's area and patient's rights protection area, and the determinants of intention to recommend outpatient medical services were the doctor's area and facilities comfort. Women evaluated the experience positively in overall satisfaction as compared to men, and those over the age of 60 positively evaluated the overall satisfaction and intention to recommend. It is significant that the outpatient experience decision-making model is presented, and that the doctor's area, patient's rights protection area, and facility comfort are important factors. Long-term research on the 'Medical Service Experience Survey' is needed, and research on the inpatient medical service experience is needed.

Influence of Perception of Patient rights and Ethical Values on Biomedical Ethics Awareness in Nursing Students (간호대학생의 환자권리에 대한 인식과 윤리적 가치관이 생명의료윤리의식에 미치는 영향)

  • Purpose: The purpose of this study was to identify nursing students' perception of patient rights, their ethical values and biomedical ethics awareness, and to examine the factors affecting the nursing students' biomedical ethics awareness. Methods: The participants of this study were 273 nursing students in B and K metropolitan city. Data collection was conducted through the structured questionnaires from March 2 to March 25, 2016. Data were analyzed using t-test, ANOVA, Scheffe's test, Pearson's correlation coefficient, and multiple regression analysis with SPSS WIN v 21.0. Results: The mean scores of nursing students' perception of patient rights, ethical values and biomedical ethics awareness were $4.56{\pm}0.38$, $3.26{\pm}0.31$, $2.91{\pm}0.20$, respectively. Biomedical ethics awareness was positively correlated with the nursing students' perceptions of patient rights (r=.38, p<.001) and ethical values (r=.25, p<.001). Factors affecting the nursing students' biomedical ethics awareness were the perception of patient rights (${\beta}=.36$, p<.001) and ethical values (${\beta}=.13$, p=.023). Conclusion: The results suggest that nursing educational program should include perception of patient rights and ethical values to foster biomedical ethics awareness for nursing students.

Development and Evaluation of a Web-based Simulation Program on Patient Rights Education using Integrated Decision Making Model for Nurse Students (가치통합 의사결정모델을 이용한 간호학생 대상 웹기반 환자권리교육 시뮬레이션 프로그램 개발 및 평가)

  • Purpose: This study was designed to develop and evaluate the a web-based simulation program on patient rights education using integrated decision making model into values clarification for nurse students. Methods: The program was designed based on the Aless & Trollip model and Ford, Trygstad-Durland & Nelms's decision model. Focus groups interviews, surveys on learning needs for patient rights, and specialist interviews were used to develop for simulation scenarios and decision making modules. The simulation program was evaluated between May, 2011 and April, 2012 by 30 student nurses using an application of the web-based program evaluation tools by Chung. Results: Simulation content was composed of two scenarios on patient rights: the rights of patients with HIV and the rights of psychiatric patients. It was composed of two decision making modules which were established for value clarifications, behavioral objective formations, problems identifications, option generations, alternatives analysis, and decision evaluations. The simulation program was composed of screens for teacher and learner. The program was positively evaluated with a mean score of $3.14{\pm}0.33$. Conclusion: These study results make an important contribution to the application of educational simulation programs for nurse students' behavior and their decision making ability in protecting the patient rights.

Nursing Students' Human Rights Sensitivity and Perception of Patients' Rights (간호대학생의 인권감수성과 환자권리에 대한 인식)

  • Purpose: This study aimed to explore human rights sensitivity and the perception of patients' rights among nursing students. For the study, 253 nursing students from K and D universities located in Daegu and the Gyeongbuk region were asked to participate. Methods: Data were analyzed using IBM SPSS Statistics 20.0; in addition, descriptive statistics, a t-test, and a one-way ANOVA were used. Results: The average human rights sensitivity score was 2.40 out of 5 points. Human rights sensitivity was significantly different in education experiences in regards to human rights after getting into nursing school and in nursing courses, as well as experiences of clinical practice. Among the six episodes that measured human rights sensitivity, nursing students reported the highest score on the right to the pursuit of happiness of older people and the lowest score was reported on the right to privacy. The average score of patients' rights perception was 4.53 out of 5 points. In particular, nursing students considered the right to enjoy human worth/dignity and the right to equality to be the most important of patients' rights. Patients' rights perception, in general, was significantly different depending on the number of family members. Conclusion: It is needed to develop an effective nursing curriculum in order to improve nursing students' human rights sensitivity and foster positive perceptions regarding patients' rights.

A Case Study of lower Extremity Amputation Patient (하지 절단 환자의 간호 사례 연구)

  • 본 환자를 간호하면서 절단으로 인해 신체적 고통을 받고 있는 존재로서, 생각과 정서, 욕구를 지닌 존재로서의 환자를 돕고자 시도해 보았다. 신체적인 적응을 돕기 위해서 근력강화와 관절가동역증진, 통증조절, 보조기 착용을 위한 단단의 관리, 잔여관절의 변형예방에 중점을 두었다. 심리적인 적응을 돕기 위해서 환자가 절단 수술과 수술 후 변화된 신체외모를 수용하는 과정을 증시했다. 끝으로 사회적인 면에서는 의미있는 사람들의 집단속에서 존재하는 인간으로서의 환자에게 관심을 두었다. 환자의 우측 슬관절 가동역은 완전히 회복된 상태는 아니지만 목발을 이용하여 걷거나 기타 일상생활을 하는데 불편함은 없으며 앞으로 계속적인 노력을 통해 회복될 수 있을 것이다. 그의 좌측 고관절 가동역은 정상유지되고 있으며 절단부위는 보조기 착용에 적합하게 형성되어 가고 있다. 비슷한 처지에 있는 이씨환자의 도움과 환자의 독립적인 생활태도는 환자가 자신의 불구에 적응하고 일상생활에 적응하는데 많은 도움이 되었다. 협조적이고 동정적인 환자들과 의료진의 도움으로 환자는 보호된 병원환경내에서 신체적 제한점을 주목하고 자신의 불구에 적응하면서 타인들과 원만한 관계를 유지해 왔다. 그는 앞으로 사회에 돌아가서 생활하게 될 것이다. 우리 사회는 아직도 그들에 대해 편견을 가지고 있으며 직업적 교육적 의료치료적 배려가 부족한 상태이다. 사회속에서의 적응은 병원에서의 적응보다 훨씬 더 어려울 것이며 때로는 그를 좌절하게 할 것이다. 그의 신체적 결함이 자기 자신만의 책임은 아니며 어느 누구나 사고에 희생될 가능성은 있다. 사회인들이 그들에게 관심있고 수용적인 태도를 지니고 그들의 정상 요구와 인간으로서 동등하게 살아갈 권리를 인정하고 받아들인다며 좀더 쉽게 자신의 슬픔을 극복할 수 있을 것이다.

  • In the traditional medical field, the patient was a person to receive protection from the doctor because there are vertical relationship between the patient and the doctor. But in modern medical field, patients change their role to health-care consumer to be guaranteed their rights more actively. This study compare patient's rights in doctor's vocational ethics and patient's rights in law, consumer rights. This study analyzes what is type of law-relationship between patients and doctor and how can they act health-care as health-care consumer.

  • This study was to investigate the relationship between safety & quality management changes of patient and changes in management activities based on hospital workers in five mental hospitals and five geriatric hospitals which should be required medical certifying authorities. As a result of the research study, participation whether or not of certification service of mental hospital & geriatric hospital workers was positive correlation to improve change of 'Performance level of Safety Activities for the patient' 'Provide the High Quality Medical Service for the patient' 'Respect the Rights and Responsibility of the patient' 'Performance level of Infection Control Activities' out of contents of Patient Safety & Medical Service Quality.   • 과거 정신지체는 사회의 불가사의한 것이었으나 현재는 이들의 원인과 병리현상의 이해로 예방과 의학적 관리에 괄목할 만한 발전이 이루어져 왔다. 그러나 이들 정신지체인의 치과관리는 과거와 별다른 변화를 이루지 못한 것이 우리의 현실이다 이들에 관심을 갖고 치료하기 원하는 치과의사들도 아직 진단이나 치료방법에 이용이 제한되어있다 정신지체인 들에게도 정상화 개념을 도입해 보다 나은 건강관리 환경을 이루도록 시대가 바뀌어 가고 있다. 환자의 권리를 보장키 위해 수용시설 입소화. 물리적인 강제치료 등 기존의 일반적인 행위에서 탈피하여 이제는 행동조절의 차원에서 약물이용과 치과적용에 필요한 모든 지식의 습득이 필수적인 것으로 변화되고 있다.

A Study of Issuance of Medical Records Using AMOS (구조 방정식을 활용한 의무기록 사본 발급 특성에 관한 연구)

  • This paper investigates the issuance of medical records of patients and caregivers who have obtained medical records from the Medical Record Information Center of "a university" in Daejeon from January through March in 2006. According to the structural equation, "the time zone for issuance of medical records" was -0.01 as a path coefficient against "how medical records are issued" and +0.86 against "ordinary characteristics." As shown above, privacy and confidentiality are what really matters in the management of medical records. Therefore, they must be protected regardless of whether the medical records are hard or electronic copies.
