A case of Hwabyung (화병 환자 1례에 대한 증례 보고)

  • Hwabyung is a culture-related disease generally known among people in k orea, and various studies have been done by the Psychiatrists and by the Oriental Medical Doctor. In this case, we administrated Chungsimjihwangtang to a patient suffering from hwabyung symptoms. After administration of Chungsimjihwangtang medication clinical symptons improved Chungsimjihwangtang showed desirable effect on hwabyung symptoms.

Cardiovascular Health and Depressive Symptoms and Well-being Status in Community-based Korean Women according to Hwa-byung Symptoms (일 지역사회 한국여성의 화병증상에 따른 심혈관 건강, 우울 및 안녕)

  • Purpose: This study explored cardiovascular health (CVH), depressive symptoms, and well-being among community-based Korean women according to hwa-byung (HB) symptoms. Methods: This cross-sectional study measured HB symptoms, depressive symptoms, quality of life (QoL), and spiritual well-being (SWB) in addition to biophysical indicators of CVH. Results: Fourteen women of the HB group are more likely to live alone with lower income than 36 women in the comparison group. Compared to the comparison group, the HB group has larger mean waist circumference and higher prevalence of abdominal obesity along with more knee arthritis and back pain. Although women in the HB group appeared worse in other CVH than their counterparts, this was not statistically significant. It is noteworthy, that the HB group showed a higher probability of having a hard cardiovascular event (CVE) within 10 years than the comparison group. Women in the HB group reported more depressive symptoms, poor QoL, and low SWB as well as smoking and sleep difficulties. Conclusion: Despite the small sample size, significant relationships of HB symptoms to abdominal obesity, the probability of a hard CVE within 10 years, and psychosocial health were found. Strategies for effective community-based cardiovascular programs for Korean women may include HB screening and/or management.

The Relationship between Anger and Depression level in Hwa-byung Patients (화병환자의 분노와 우울정도에 관한 연구)

  • Purpose : The purpose of study was to extend the understanding and knowledge of hwa-byung by identify the relationship between anger and depression level in hwa-byung patients. Method : The subjects in this study were 69 hwa-byung patients who hwa-byung clinic in oriental hospital. The data were collected through the personal interviews with questionnaires and the period for data collection was from March. to June. 2001. The tools of the measurement were the anger scale that developed by Spielberg and translated by June, et al. The tools of the measurement were the Epidemiologic Studies Depression Scale(CES-D) that developed by Radloff and translated by Shin. The data was analyzed by the SPSS computer program that included the descriptive statistics, $X^2$-test, t-test, ANOVA and pearson correlation. Result : 1. The mean score for anger was 17.4, 24.9, and the mean score of depression was 20.3. 2. There was the significant difference in the score for anger according to the age(t=7.02, p=.000), the marital status(t=3.34, p=.001), the average monthly income(t=5.48, p=.000), the occupation (t=2.18, p=.031). 3. There was the significant difference in the score for depression according to the age(t=3.58, p=.016), education(t=6.35, p=.002), the average monthly income(t=11.37, p=.000), 4. Anger in Hwa-byung patients showed significant correlation with depression level(r=.405, p=.000). Conclusion : From this study, a relationship between anger and depression in hwa-byung patients was identified. Therefore, nursing interventions are needed to assessment anger and depression level with hwa-byung patients.

Study on Correlation between Four Constitution Characteristics and Hwa-byung - Based on Hwa-byung Epidemiologic Study Data - (체질특성과 화병의 상관관계에 대한 연구 - 화병역학연구 자료에 기반하여 -)

  • Objectives : The aim of this study was to find correlations between characteristics of Four constitution and Hwa-byung. Methods : Clinical data were collected from Hwa-byung Epidemiologic study data. we selected 130 self-labelled Hwa-byung patients data and 83 of them were diagnosed as Hwa-byung by SCID. We used Hwa-byung SCID, Hwa-byung scale, CES-D, STAI, STAXI and Personality and Physiological Symptom Questionnaire(PPSQ) as evaluation tools. We performed statistical analysis of each 2 groups, self-labelled Hwa-byung group(N=130) and diagnosed Hwa-byung group(N=83). Results : 1. In self-labelled Hwa-byung group, Greater yin score showed significant negative correlation with 3 indices of Hwa-byung scale and significant positive correlation with 1 index. Lesser yin score showed significant positive correlation with 6 indices. In diagnosed Hwa-byung group, Greater yin score showed significant negaitive correlation with 5 indices. In both groups, Lesser yang score showed no significant correlation with any indices of Hwa-byung scale. 2. In self-labelled Hwa-byung group, Greater yin score showed significant negative correlation with both trait anger score and state anger score of STAXI. Lesser yin score showed significant positive correlation with trait anxiety score, state anxiety score and CES-D score self-labelled Hwa-byung group. Lesser yang score showed significant positive correlation with anger-out score of STAXI. But Lesser yin score showed significant negative correlation with same index diagnosed Hwa-byung group. Conclusions : 1. The higher Greater yin score, suppression or denial, psychological defense mechanism, become more stronger. 2. The higher Lesser yin score, anger-out tendency become more weakened and increased prevalence of Hwa-byung symptoms. 3. The higher Lesser yang score, anger-out tendency become more stronger.

The Concept of Hwa-Byung in Nursing (화병의 간호학적 개념)

  • The purpose of this study was to study how nurses interpreted the meaning of Hwa-Byung. It was carried through a review of literature, and the result is expected to explore ways that nursing intervention can promote the understanding of Hwa-Byung. The literature review focused on cultural psychology and psychiatrists' views toward Hwa-Byung. After that, it was reconfirmed concept of Hwa-Byung in nursing devived from historical analysis of the examples of Hwa-Byung from a true record of the Cho-Sun Dynasty(CD ROM, 1997). Characteristics of patients with Hwa-Byung include 'Hwa', an aspect of somatization, and self-diagnosing. It also could find that Hwa-Byung is characterized as 'Hwa', 'somatization' and 'self-diagnosis(subjectiveness)' according to a true record of the Cho-Sun Dynasty(CD ROM, 1997). The conceptual definition of 'Hwa', 'somatization' and 'self-diagnosing (subjectiveness)' are as follows. The core concept of Hwa-Byung, 'Hwa,' has the property of 'fire', and equals the feeling of injustice. Hwa-Byung means congestion of 'Hwa'. Therefore Hwa-Byung is the accumulation of being mistreated and mortified. The feeling of mistreatment comes from subjective experiences, which cannot be in harmony with the values, beliefs and rights of the patients. The situations that they have to endure again and again, though they are the sufferers, connote suppressed aggression and powerlessness endured over time. Suppressed aggression subordinated hostility, hatred and revengeful thoughts; powerlessness subordinates frustration, resignation, and fatalism. Somatization is another form of expressing 'Hwa' through physical symptoms. The somatization of clients with Hwa-Byung plays a role in expressing non-verbalized and suppressed emotions within themselves. The clients who experiences Hwa-Byung think that they know the cause of their illness and self-diagnose their problem as Hwa-Byung. Therefore, the feeling of unfairness which is the premise of Hwa-Byung infers 'subjectiveness' In conclusion, nursing's concept of Hwa-Byung is the accumulation of feelings of being mistreated and mortified. Hwa-Byung is the internalized 'Hwa' from enduring again and again. The feeling of being mistreated comes from subjective judgements about unfairness that cannot be harmonized with the clients' values, beliefs and rights. Those who can express their 'Hwa' only through their bodies imply suppressed aggression and powerlessness.

  • Hwabyung is a culture-bound anger syndrome in Korea. It manifests as one or more of a wide range of physical symptoms, in response to emotional stress. This case describes a 26-year old woman diagnosed as Hwabyung by HBDIS. She complained chiefly of binge eating and sleep disorder. She also complained of chest discomfort, abdominal discomfort, foreign body sensation in the throat, burning sensation and so on. We treated the patient with herbal medication, acupuncture, cupping, and moxa. After treatment, her condition improved. Standardized patients (SP) were used in teaching and practicing interviewing and physical examination skills for This report suggests that traditional Korean treatment such as herbal medication, acupuncture, cupping, moxa is effective for the treatment of Hwabyung patients.

  • Today, if you don't reflect trends in developing new content crafts, you can't succeed in marketing. This is because the trend is the driving force that leads to the needs of consumers that come and go temporarily. The current market is changing and wants new lifestyle trends. As a trend of such latent desire, the purpose is to develop high-value-added interior vases using rice cake patterns and ceramic complexes. In the manufacturing method, the first rice cake patterns writing tool is stamped at a different angle from the top of the toremi board (a new term created above) written in the process of the pot work. A cylinder is made of a second potter's wheel, and after the outer wall is double-flavored, the cylinder is quickly rotated to be extended by pushing it from inside to outside by hand. The purpose of this vase is to develop a decorative vase with high added value by experimenting with various geometric patterns that are dismantled vertically and horizontally at a high speed with the static rice cake pattern stamped on the third, and then utilizing the various rice cake patterns developed in this way as the shape of a jar.

  • Generally, Hwabyung is more common in older women, in low educational group. Many psychiatrists explain it as the illness originated from a series of psychological stresses. And they think that Hwabyung patients have somatization disorder, anxiety disorder, and major depression. But, many of oriental medical doctors explain it as symptoms having the character of fire. In this case, we described a 67-years old woman who diagnosed as Hwabyung. She complained flush face with surging feeling, headache, dizziness and depression, and her condition was improved through oriental medical treatment such as acupuncture and herbal medicine about 5 weeks.

  • Hwabyung is an illness due to stagnated emotion accumulated for a long time and is not rare in the area of neuropsyhchiatric clinics in Korea. It has been widely used among Korean people, and was introduced as culture bound syndrome in DSM-IV. The purpose of this study was to investigate the origin of Hwabyung, and concepts using at past. Hwabyung is described in 'Choson Wangjo Silok' in 1603 (Sonjo 37th year) at first. The explanation about Hwabyung in 'Choson Wangjo Silok' contain etiology, pathology, symptoms and treatment, is closely related with the meaning through Korean Traditional Medicine.

  • Objectives : This study was designed to research the effect of oriental treatments for Hwabyung with Blepharoptosis. Methods : A 51 year-old female patient has been suffered by Hwabyung for 5 months and by Blepharoptosis for 2 weeks. We treated the patient with Herbal medication, oriental psychotherapy and acupuncture. Results : As a result of oriental treatments, Blepharoptosis was disappeared, and the symptoms of Hwabyung was improved gradually. Conclusions : These result suggest that oriental treatments have an effect on Hwabyung with Blepharoptosis.
