Development of a Risk Assessment Tool for Emerging Infectious Diseases (신종감염병의 양적 및 질적 혼합 위험 평가 모델 개발)

  • Background: The emergence of new infectious diseases threatens public health, increasing socioeconomic damage, and national risks. This study aimed to develop an evidence-based risk assessment tool to quickly respond to new infectious diseases. Methods: The risk elements were extracted by reviewing the risk assessment methods of the World Health Organization, United States, Europe, United Kingdom, and Germany, and the validity and priority of elements were determined through expert meetings and Delphi surveys. Then, the scale and level for each risk element were defined and a final score calculation method according to the risk evaluation result was derived. The developed risk assessment tool was verified using data at the time of domestic transmission of an emerging infectious disease. Results: In case of spread of actual infectious diseases, priority is determined based on the criticality of the elements in each area of transmissibility and severity, from which the weighted score of the risk assessment is derived. Then, the risk score for each element was calculated by multiplying the average value of the risk evaluation by its weight and the evaluation risk assessment score for the two areas was calculated. At last, the final score is plotted in a matrix where the x-axis indicates the transmissibility and the y-axis the severity and plotted on the coordinate plane for time series use. Conclusion: With respect to transmissibility and severity, this risk assessment method to respond to new and re-emerging infectious diseases enables rapid and evidence-based evaluation by quantitatively and qualitatively assessing various risk elements.

The Role and Policy of Korean Medicine Doctor for the Prevention and Management of Infectious Diseases: A Mixed-Methods Approach (감염병 예방 및 관리를 위한 한의사의 역할과 정책적 과제: 혼합연구방법론을 적용하여)

  • Objectives: This study investigated the role of Korean medicine doctors in the national quarantine system and the status of their duties through questionnaire surveys and in-depth interviews with Korean medicine staff who participated in COVID-19 work. Methods: We developed a questionnaire survey to investigate the status and perception of Korean medicine doctors' participation in responding to infectious diseases, and the results were derived by conducting frequency analysis and T-test using SPSS program. The measurement variables of this study were the status of COVID-19 related work participation and perception of Korean medicine doctors' role in infectious diseases, and the maximum significance level was 0.05. Results: There were 300 respondents to the survey, and as for the COVID-19 related work, Korean medicine treatment for COVID-19 patients accounted for the most(52.7%), and various preventive and treatment tasks such as epidemiological investigations and specimen collection were conducted. Conclusion: According to this study, it was confirmed that there are sufficient legal and administrative grounds for Korean medicine doctors to perform specimen collection and epidemiological investigations. Therefore, it is necessary to prepare a policy framework to solidify the status of Korean medicine doctors in national infectious diseases in the future.

Effect of Hydrocortisone Aceponate - Gentamicin - Miconazole Topical Otic Combination for Treating Canine Otitis Externa (개 외이염 치료에서 하이드로코티손 아세포네이트-겐타마이신-미코나졸 국소 혼합제제의 효과)

  • Fifty-four dogs with otitis externa were enrolled in the study for the Evaluation of efficacy of a Hydrocortisone aceponate - Gentamicin - Miconazole otic combination ($Easotic^{(R)}$, Virbac, Carros, France). Otitis externa patients were treated by $Easotic^{(R)}$ once daily for 5 days and 2 days off treatment and evaluated on $7^{th}$ day. If otitis externa persisted, additional $Easotic^{(R)}$ treatment was administered once daily for 5 days and rested 2 days and reevaluated on $14^{th}$ day. For the evaluation of efficacy of $Easotic^{(R)}$, eight clinical signs were scored on a severity scale and infectious agents from ear sample were also graded using semi-quantitative scale at each visit. Sum of clinical scores and cytological scores was defined as Global Clinical Score. When $Easotic^{(R)}$ was applied once daily for 5 days, global clinical score was reduced 76.0%. When $Easotic^{(R)}$ was administered for 10 days, during first 5 days administration, 46.6% reduction of global clinical score was detected. During additional 5 days administration, 82.2% reduction of global clinical score was observed compared with Day 0. Any relevant adverse effect was not reported during the study in all cases. Thus, $Easotic^{(R)}$ treatment once daily for 5 days and 10 days appears to be effective and safe treatment for canine otitis externa.

Cross-Protection Effectiveness of Cucumber mosaic virus (CMV) Isolates Associated with Satellite RNA for Prevention of CMV Disease in Pepper Plants (Satellite RNA 보유 Cucumber mosaic virus(CMV)의 고추 CMV병에 대한 교차방어 효과)

  • Two attenuated Cucumber mosaic virus (CMV) isolates, Paf-CMV and Rs2-CMV that had been selected from CMV isolates associated with satellite RNA (satRNA) were tested for cross-protection effect in pepper plants. The viruses selected as attenuated strains appeared to be identical serologically and physically to the challenge virus (Mf-CMV), but they were lower in the dilution end-point of infectivity of crude sap than Mf-CMV When symptoms were observed in several indicator plants after inoculation, Paf-CMV and Rs2-CMV were symptomless or showed mild mosaic symptoms while another satRNA isolate Ap-CMV developed severe mosaic symptoms on the leaves as Mf-CMV. The nucleotide sequences of the satRNAs were determined by sequencing full-length cDNA clones. Paf-, Rs2- and Ap-satRNAs were 386, 335, and 347 nucleotides long, respectively, The sequences were then compared with the other known Y-satRNA, revealing that nucleotide sequences of the satRNAs consisted of 5'- and 3'-terminal conserved regions. However variations occurred on the middle regions of the sequences, especially those related to symptom interference, showing significant differences between Paf-satRNA and other isolates. Infectious transcripts of Paf-satRNA and Rs2-satRNA induced mild mosaic symptoms in pepper plants when supported by genomic RNAs of Mf-CMV. Under greenhouse conditions, Paf-CMV and Rs2-CMV were tested for cross-protection effect in pepper and tobacco (Nicotiana tabacum cv, Xanthi nc) plants against Mf-CMV. No symptoms were developed on the plants vaccinated with Paf-CMV until 3 weeks after inoculation with the virulent strain; however another attenuated isolate, Rs2-CMV, showed less effectiveness in cross-protection. Depending on the concentration of the challenged virus, symptoms sometimes appeared later in the upper leaves. However, in plants challenged with low concentrations (below 0.2 mg/ml) of the challenge inoculum, symptoms caused by the virulent strain did not develop on the plants vaccinated with Paf-CMV. In the field experiments, the number of pepper plants with severe mosaic symptoms in the control plots was progressively increased after transplanting and reached approximately 50% after 50 days. On the other hand, the incidence of mosaic disease appeared very low on the plants that had received the protective inoculation with Paf-CMV.

The Diagnosis of Pneumoniae Following Bone Marrow Transplantation by Bronchoscopy (골수이식후 기관지내시경을 이용한 폐렴의 진단)

  • Background : Pulmonary complications following bonemarrow transplantation (BMT) are common and associated with a high mortality rate. We investigated the yield, safety, and impact of fiberoptic bronchoscopy (FOB) for diagnosis of postBMT pneumoniae. Methods : From May 1997 to April 2000, 56 FOBs were performed in 52 post BMT patients for clinical pneumoniae. BMT patients with respiratory symptoms and/or pulmonary infiltrates had a thoracic HRCT(high resolution computed tomography) and bronchoscopic examination including BAL (bronchoalveolar lavage), TBLB (transbronchial lung biopsy), PSB (protected specimen brush). Results : The characteristics of the subjects were as follows : 37 males, 15 females, mean age of 31.3 years(l7-45), 35 sibling donor allogenic BMTs, 15 nonrelated donor allogenic BMTs, and 2 autologous BMTs. Fiftynine percent of FOBs (33 FOBs, 31 patients) were diagnostic. Isolated pathogens included the following : 12 cytomegalovirus (CMV) (21.4 %), 7 pneumocystis carinii (PC) (12.5 %), 11 CMV with PC (19.6 %), 2 Mycobacaterium tuberculosis (3.6%), and 1 streptococcus (1.8%). Most of the radiographic findings were diffuse interstitial lesions. CMV pneumoniae had mainly diffuse interstitial nodular lesion, and PC pneumoniae had diffuse, interstitial ground glass opacity(GGO). When CMV was accompanied by PC, a combined pattern of nodular and GGO was present. Of the 56 cases (23.2%), 13 died of CMV pneumoniae (n=2), PCP (n=2),mixed infection with CMV and PC (n=3), underlying GVHD (n=1), underlying leukemia progression (n=1), or respiratory failure of unknown origin (n=4). There was no major complication by bronchoscopy. Only 3 cases developed minor bleeding and 1 episode temporary hypoxemia. Conclusion : Based on our findings, CMV and PC are the major causes of postBMT pneumoniae. In addition, BAL can be considered a safe and accurate procedure for the evaluation of pulmonary complications after BMT.

  • This study was conducted to investigate the effects of bifidobacteria on the growth and Caco-2 cell-adherence of Escherichia coli O157:H7 .Dur-ing momo-culture of E. coli O157:H7 and mixed culture with Bifidobacterium infantis K9, pH viable cell count, and ammonia concentration were measured Co-cultivation of E. coli O157:H7 with bifidobacteria. producing acidic metabolites rapidly decreased the viable cell count of E. coli O157:H7 In addition rapid decrease of ammo- nia concentration was observed during mixed culture after 8 hrs incubation compared to single culture of E. coli O157:H7 Therefore it is likely that bifidobacteria assimilate ammonia produced by E. coli O157:H7 P4 B, infantis K9 showed quite similar adherence on the Caco-2 cells in either case. On the other hand adherence of E. coli O157:H7 decreased from 2.6% to 1.86% when B infantis K9 was adhered to Caco-2 cell 2 hrs prior to the application of E. coli O157:H7 In conclusion in adherence of E coli O157:H7 to Caco -2 cell was inhibited by competition of its binding to the adherence site with bifidobacteria. In addition inhibitory effects of bifidobacteria on E coli O157:H7 appeared to be much higher with increae of the number of bifidobacteria and its ability of adherence to Caco-2 cells.

Elimination of Grapevine fleck virus from infected grapevines 'Kyoho' through meristem-tip culture of dormant buds (휴면아 경정 배양법을 통한 포도 '거봉' 에서 Grapevine fleck virus의 제거)

  • Herein, we report the meristem-tip culture from dormant buds of grape 'Kyoho' single-infected with Grapevine fleck virus (GFkV), which is phloem-limited and transmitted by graft inoculation. We produced GFkV-free shoots without thermo- or chemotherapy using meristem-tip explants approximately 0.3 mm (73 explants) and 0.8 mm long (five explants) including shoot apical meristem, 2-5 leaf primordia, and 1-4 uncommitted primordia from dormant buds of the infected woody cuttings (stored at $4^{\circ}C$). Explants were cultured on Murashige and Skoog (MS) medium supplemented with 3% sucrose, 3.0 mg/L benzyladenine (BA) and 0.1 mg/L indole-3-butyric acid (IBA). After 16 weeks of culture, shoot (10-mm long) regeneration frequency achieved from 0.3-mm explants was 4.1% and that obtained from 0.8-mm explants was 40.0%. Virus-free efficiency (expressed as the percentage of RT-PCR negative shoots regenerated) from 0.3- and 0.8-mm explants was 100% and 50%, respectively. Following in vitro multiplication, RT-PCR assays revealed identical results to assays of the first regenerated shoots. Our new methodological approach could be applied for eliminating other viruses in grapevines, as well as for producing virus-free plants in many other deciduous tree species, including fruit trees.

  • Immune response to two methods of Newcastle disease virus vaccine, one inactivated and the other attenuated was observed and the data presented. (1) Administration of inactivated virus vaccine in an amount of 1.0 ml. by intramuscular route gave an appreciable immunity to Newcastle disease for a period of at least three-and-half months. (2) The chickens given attenuated virus vaccine in the drinking water produced satisfactory immunity as manifested by the fact that immunized birds showed resistance when challenged 105 days after the vaccination and maintained high degree of HI titer for a period of 75 days. (3) Vaccination with the attenuated virus vaccine in drinking water is very simple and time saving in procedure, although the duration of immunity seems to be slightly shorter than that proced by inactivated virus vaccine. Antimicrobial Activity of Korean Herbal Complex Extract and Clay Mineral Mixture against Escherichia coli O157:H7 (한약재 복합추출물과 점토 광물질 혼합제의 Escherichio coli O157:H7에 대한 항균효과)

  • The present study was evaluated the antibacterial effect of the combination of Coptidis rhizoma, Lonicerae Flos, and Paeonia japonica (1:1:1) extracts (CLP1000). Also, the effectiveness of CLP1000, dioctahedral smectite (DHS), and the combination of CLP1000 and DHS (CLPS1000) against E. coli O157:H7 infection was studied using ICR female mice. During the incubation period, the dose of 10% and 20% CLP1000 were inhibited the growth of E. coli O157:H7 by 30% and 47%, respectively. For 7 days after single challenge with E. coli O157:H7, forty female ICR mice were divided into four experimental groups which were orally administered with saline, 10% CLP1000, 10% DHS, and 10% CLPS1000, respectively. On the 3rd day, the number of E. coli O157:H7 in mouse feces was significantly decreased by administration of CLP1000 (p < 0.05), DHS (p < 0.05) and CLPS1000 (p < 0.001). On the 7th day, CLP1000 (p < 0.05) and CLPS1000 p < 0.001) administration significantly decreased the number of E. coli O157:H7. According to the results of the present study, administration of CLPS1000 to mice can reduce the severity of E. coli O157:H7 infection. Also, it is suggested that CLPS100 represents a good candidate for the treatment of enteric infections in domestic animals.

Inhibitory Effect of Metal Surface on the Antimicrobial Resistance Microorganism (금속표면이 항생제 내성균주의 생육억제에 미치는 영향)

  • Background: The aim of this study was to comparatively evaluate the bactericidal effects of copper, brass (copper 78%, tin 22%), and stainless steel against methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant Enterococcus faecium (VREFM), and multidrug-resistant Pseudomonas aeruginosa (MRPA). Methods: The isolates (MRSA, VREFM, MRPA) used in this study were mixed wild type 3 strains isolated from patients treated at Uijeongbu St. Mary's Hospital in 2017. These strains showed patterns of multidrug resistance. The lyophilized strains were inoculated into and incubated for 24 hr in tryptic soy broth at $35^{\circ}C$. The initial bacterial inoculum concentration was adjusted to $10^5CFU/mL$. A 100-mL bacterial suspension was incubated in containers made of brass (copper 78%, tin 22%), copper (above 99% purity), and stainless steel at $35^{\circ}C$. Viable counts of bacteria strains were measured for 9 days. Results: In this study, the bactericidal effects of copper and brass on MRSA, VREFM, and MRPA were verified. The bactericidal effect of stainless steel was much weaker than those of copper and brass. The bactericidal effect was stronger on MRPA than on MRSA or VREFM. Conclusion: To prevent cross infection of multidrug resistant bacteria in hospitals, further studies of longer duration are needed for testing of copper materials on objects such as door knobs, faucets, and bed rails.
