Clinical Investigation of 11 Cases of Chronic Eosinophilic Pneumonia Reported in Korea (국내에서 보고된 만성 호산구성 폐렴 11예에 대한 임상적 고찰)

  • Background: Chronic eosinophilic pneumonia(CEP) presents with profound systemic symptoms, including fever, malaise, night sweats, weight loss, and anorexia together with localized pulmonary manifestations such as cough, wheeze, and sputum. It is an illness occurring predominantly in women. The chest radiogragh shows fluffy opacities that often have a characteristic peripheral configuration. The hallmark of CEP is the peripheral blood eosinophilia and a prompt response to oral corticosteroid therapy. We investigated characteristics of eleven patients of chronic eosinophilic pneumonia, reported in Korea. Method: There were eleven reports of CEP from 1980 to 1996, including three cases experienced in our hospital. The journals were analysed in respects of clinical history, laboratory, and radiographic findings. Results: 1) Male vs. female ratio is 3 : 8. The peak incidence occurred in forty and fifty decades. The atopic diseases were present in 6 cases. Asthma was the commonest manifestation 2) The presenting symptoms were as follows: cough, dyspnea, sputum, weight loss, fever, general weakness, night sweats, urticaria with the descending incidence. 3) Peripheral blood eosinophilia was present in all patients(mean ; 38.4%) and serum IgE level was elevated in nine patients(mean ; 880IU/ml). Conclusion: The diagnosis of chronic eosinophilic pneumonia is based on classic symptoms, including fever, night sweats, weight loss with a typical roentgenogram of peripheral pulmonary infiltrates and peripheral blood eosinophilia, and that is confimed by lung biopsy and/or bronchoalveolar lavage. Chronic eosinophilic pneumonia is responsive to corticosteroid promptly and recommended at least 6 months of therapy to prevent relapse.

Effects of Non-ionic or Zwitterionic Surfactant on in vitro Digestibility of Rice Straw and Growth of Rumen Mixed Microorganisms. (비이온성 및 양쪽 이온성 계면활성제 첨가가 반추위 혼합 미생물의 성장과 볏짚의 in vitro 소화에 미치는 영향)

  • This experiment was conducted to investigate effects of non-ionic or zwitterionic (+/-) surfactants on digestibility of rice straw, and changes of growth of rumen mixed microbes, pH, and gas production during in vitro fermentation. Also, during in vitro ruminal fermentation, microbial attachment on rice straw was investigated using scanning electron microscopy (SEM). Tween 80 or SOLFA-850 for non-ionic surfactant (NIS), and 3-(Dodecyldimethylammonio) propanesulfanate (DDAP) for zwitterionic surfactant (ZIS) was supplemented by 0.05% and 0.1% in Dehority's artificial medium containing Holtein rumen fluid, respectively, and the substrate for fermentation was rice straw passed through 1 mm screen. The experiment was composed of 7 treatments (two levels of two NISs, two levels of a ZIS) including the control, and 6, 12, 24, 48 and 72 hr of fermentation time with 3 replications per treatment. Treatment of Tween 80 increased in vitro DM digestibilities during 48 hr and 72 h post fermentations compared to the other treatments, whereas treatment of DDAP as a ZIS resulted in decreased DM digestibility than that of the control from 24 hr post fermentation (P<0.05). Gas production in vitro was greater (P<0.05) with addition of NIS than the control or ZIS, and increased as fermentation time elapsed. Rumen mixed microbial growth was greatest with addition of Tween 80 as NIS, and lowest when DDAP as ZIS was supplemented to the fermentation tube (P<0.05). In SEM observation, rumen microbial population attached on rice straw particle was greater with addition of NIS, but was less with addition ZIS compared with the control. In conclusion we could not found any positive effects of ZIS surfactants on rumunal fermentation characteristics and rumen microbial growth rates.

Effects of Nitrate-rich Plant Extracts on the in vitro Ruminal Fermentation and Methane Production (질산염 화합물 함유 식물 추출물이 in vitro 반추위 발효성상과 메탄 발생에 미치는 영향)

  • This study was conducted to evaluate the effects of nitrate-rich plants extracts on the in vitro rumen fermentation characteristics and rumen methane production. The extracts of nitrate-rich plants, as potato, carrot, chinese cabbage, lettuce and spinach were used in this study. The ruminal fluid was collected from a cannulated Hanwoo cow fed concentrate and timothy in the ratio of 6 to 4. The 20mL of mixture, comparing McDougall's buffer and rumen fluid in the ratio 2 to 1, was dispensed anaerobically 50mL serum bottles containing 0.3g of timothy substrate and extracts of nitrogen-rich plants. The serum bottles were incubated 39℃ for 9, 12, 24, 48 hours. The pH value was decreased by increased incubation times and normal range to 6.31 to 6.96. The dry matter digestibility was significantly(p<0.05) lower in chinese cabbage than in control at 9h incubation time. Ammonia concentration was significantly(p<0.05) lower in potato, chinese cabbage, lettuce than in control and the rumen microbial growth rate was significantly(p<0.05) higher in carrot than in control at 24h incubation time. The concentrations of acetate and propionate was significantly(p<0.05) lower in treatment than in control. The concentration of butyrate was showed a different pattern depending on treatments. Total gas emissions was significantly(p<0.05) lower in chinese cabbage, lettuce, spinach than in control at 12h, 24h incubation time. Methane production was significantly(p<0.05) lower in potato, chinese cabbage, spinach than in control, carbon dioxide production was significantly(p<0.05) lower in treatment than in control. In conclusion, supplementation of the nitrate-rich plant extracts in ruminal fermentation in vitro resulted in decreasing the methane production without adversely affecting the fermentation characteristics. Particularly the chinese cabbage extract was regard as a potential candidate for reducing the methane emission in ruminants.
