Cognitive Behavioral Therapy in Childhood Tic Disorder : A Case of Report (틱 장애 아동의 인지행동치료 증례: 5회기 습관뒤집기 훈련과 호흡훈련을 중심으로)

  • Chronic tic disorder or Tourette syndrome is known to be a chronic neuro-behavioral disease for which cognitive behavioral therapy (CBT) strategies have recently been introduced. Here, we report the effectiveness of CBT in a case of childhood chronic tic disorder, which is very common in clinical settings. The DSM-IV clinical diagnosis was applied by a child psychiatrist. The Yale Global Tic Severity Scale, Kovac's children's depression inventory, Spielberger State-Trait Anxiety Inventory, Abbreviated Conners' Rating Scales, and the Dupaul ADHD Rating Scales were used. This case involved a pharmacological treatment-refractory patient over the previous year. Thus, psychiatric consultation was undertaken. Subsequently, we administered five sessions of CBT for four weeks, consisting of symptom evaluation and planning, habit reversal training, and ventilation training. Following four weeks of CBT administration, there were improvements in the scores of the Yale Global Tic Severity Scale and the Clinical Global Improvement scale. Our observations indicate that CBT may be effective in the treatment of childhood tic disorder.

The Effects of Respiratory Muscle Training on Respiratory Function, Respiratory Muscle Strength, and Cough Capacity in Stroke Patients (호흡근 강화 훈련이 뇌졸중 환자의 호흡기능, 호흡근력과 기침능력에 미치는 영향)

  • PURPOSE: The purpose of this study was to examine the effects of respiratory muscle training on respiratory function, respiratory muscle strength, and cough capacity in stroke patients. METHODS: This study used a nonequivalent control group pre-post test design. We recruited thirty-four stroke patients(16male, 18female), who were assigned to intervention (n=17), or control (n=17) groups. Both groups participated in a conventional stroke rehabilitation program, with the intervention groups also receiving respiratory muscle training 20 minutes a day, three times a week, for 4 weeks. Respiratory function (forced vital capacity) and respiratory muscle strength (maximal inspiratory pressure, maximal expiratory pressure) were assessed by spirometry. Cough capacity (peak expiratory flow) was assessed using a peak flow meter. The collected data were analyzed by independent and paired t-tests. RESULTS: The intervention group showed a significant increase in the forced vital capacity (FVC), maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP) and peak expiratory flow (PEF) at the end of the program, while the control group showed no significant changes. CONCLUSION: This study showed that respiratory muscle training increased respiratory function, respiratory muscle strength, and cough capacity in stroke patients and prevented a decrease in cough capacity. These findings suggest that respiratory muscle training effect on respiratory function, respiratory muscle strength and cough capacity for rehabilitation in patients with stroke.

  • The purpose of this research is to investigate the usage of voice scripting for speech characterization. The ultimate goal of acting is for an actor to create one's character both in physical and vocal sense, and to present them on stage. Toward this goal, actors train themselves with various methods and techniques as well as character analysis. Most of their efforts are put into for better physical and vocal expression. The vocal characterization on stage is heavily governed by the proper speech based on the effective respiration and voice. This paper provides the way of how to use sound scoring for effective vocal characterization on stage.

Inspiratory Muscle Strengthening Training Method to Improve Respiratory Function : Comparison of the Effects of Diaphragmatic Breathing with Upper Arm Exercise and Power-Breathe Breathing (호흡 기능 향상을 위한 들숨근 강화 훈련 방법 : 위팔운동을 동반한 가로막 호흡과 파워브리드 호흡의 효과 비교)

  • Purpose : It was to compare changes in respiratory function (pulmonary function, inspiratory function) after four weeks of inspiratory muscle strengthening training (diaphragmatic breathing with upper arm exercise, Power-Breathe breathing) for 36 healthy people. Methods : Subjects were randomly assigned to diaphragmatic breathing with upper arm exercise (Group I) and Power-breathe breathing (Group II) was conducted by the protocol for four weeks five times per week. As the main measurement method for comparison between groups For pulmonary function, Forced Vital Capacity (FVC) and Forced Expiratory Volume at One second (FEV1) were used, and for inspiratory function, Maximum Inspiratory Capacity (MIC), Maximum Inspiratory Pressure (MIP), and Maximum Inspiratory Flow Rate (MIFR) were used. Results : In changes in pulmonary function between groups, FVC and FEV1 showed no significant difference, and in inspiratory function changes, MIC showed no significant difference, but in MIP and MIFR, Group B significantly improved over Group A. Conclusion : The progressive resistance training using the Power-breath device applied to the inspiratory muscle did not show a significant difference in the increase in the amount of air in the lungs and chest cage compared to the diaphragmatic breathing training accompanied by the upper arm exercise. However, by increasing the air inflow rate and pressure, it showed a more excellent effect on improving respiratory function.

Designing the Training Scenarios that Assuming Chemical Accidents in the Buildings and Analyzing Activities of Special Rescue Crew (건물 내부 화학사고를 가정한 모의 훈련 시나리오 설계 및 특수구조대원의 활동성 분석)

  • This study examined the activity of the simulated training scenarios and special rescue crew assuming a chemical accident inside the building. To improve the response ability and safety of rescue personnel in the event of a chemical accident, a total of eight stages of simulated training scenarios were conducted and the activity was measured. As a result of the activity measurement according to the simulation training scenario, the average maximum heart rate of the rescuers was the highest at 177.50 ppm in the 4th stage, and then 176.67 ppm for the 5th step, 171.17 ppm for the 2nd step, 169.33 ppm for the 3rd step, 162.17 ppm for the 6th step, 159.58 ppm for the 7th step, 148.75 ppm for the 8th step, and 139.33 ppm for the 1st step. The mean maximum respiratory rate was the highest at 38.17 rpm in the 4th stage, followed by 37.58 rpm in the 5th stage, 32.00 rpm in the 6th stage, 31.58 ppm in the 7th stage, 31.33 rpm in the 3rd stage, 31.08 rpm in the 2nd stage, and 28.00 rpm in the 8th stage. The 1st stage was 22.67 rpm. In addition, body water reduction measurements were reduced by up to 0.7 g due to the training intensity.

The Review of Breathing Pattern Training for The Spinal Stabilization. (척추 안정화를 위한 호흡패턴 훈련에 대한 고찰)

  • Purpose : The purpose of this study was carried out to review for the importance of breathing pattern training for the spinal stabilization. Methods : This is a literature study with books and thesis. Results : Breathing with normal respiratory mechanics has a potent role in neuro-musculo-skeletal system. The evaluation of respiratory mechanics should be a routine part of every physical examination. And respiratory mechanics must be intact for both normal posture and spinal stabilization to be possible. Conclusion : The spinal stabilization exercise with the breathing pattern training is more efficient therapeutic exercise program for the patient with neuro-musculo-skeletal system disorder.

Effects of Abdominal Breathing Training Using Biofeedback on Stress, Immune Response and Quality of Life in Patients with a Mastectomy for Breast Cancer (바이오휘드백을 이용한 복식호흡훈련이 유방암절제술 환자의 스트레스, 면역반응 및 삶의 질에 미치는 효과)

  • Purpose: This study was to determine the effects of abdominal breathing training using biofeedback on stress, immune response, and quality of life. Method: The study design was a nonequivalent control group pretest-posttest, quasi-experimental design. Twenty-five breast cancer patients who had completed adjuvant chemotherapy were enrolled. The experimental group(n=12) was provided with abdominal breathing training using biofeedback once a week for 4 weeks. State anxiety, cancer physical symptoms, serum cortisol, T cell subsets(T3, T4, T8), NK cell and quality of life were measured both before and after the intervention. Result: Though state anxiety, cancer physical symptoms, and serum cortisol were reduced after 4 weeks of abdominal breathing training using biofeedback, there was no statistical significance. It showed, however, improvement in quality of life (p=.02), and T3(p=.04). Conclusion: Abdominal breathing training using biofeedback improves quality of life in breast cancer patients after a mastectomy. However, the mechanism of this beneficial effect and stress response requires further investigation with special consideration in subject selection and frequency of measurement. Nurses should consider this strategy as a standard nursing intervention for people living with cancer.

Is There Any Immediate Difference between Pulmonary Function and Respiratory Muscle, with or without Vibration Stimulation in Respiratory Resistance Training? (진동 자극 유무에 따른 호흡 저항 훈련 시 폐 기능과 호흡근의 즉각적인 차이가 있을까?)

  • Purpose: The purpose of this study was to investigate the effect of whole body vibration combined breathing resistance on lung capacity and respiratory muscle and to suggest a mediation method for improvement of respiratory function and lung function in the future. Methods: This study was a preliminary study design of two groups of 54 healthy young adults who were randomly assigned to an experimental group (n=27) with core exercise combined with respiratory resistance and whole body vibration and a control group with respiratory resistance and core exercise (n=27). All interventions consisted of 6 core exercises every 40 seconds and rest for 20 seconds. To compare the effects of intervention, we measured spirometry and respiratory muscle strength. Results: Both the experimental group and the control group showed a significant increase in Forced vital capacity (FVC) and Maximum voluntary ventilation (MVV) (p<.05). However, FEV1 and FEV1% were significantly increase only in the experimental group (p<.05). FVC, FEV1%, Maximum Inspiratory Pressure (MIP), Maximum Expiratory Pressure (MEP) showed more significant increase in the experimental group than the control group. Conclusion: These findings indicate that whole-body vibration combined breathing resistance is an effective intervention for people, with FVC, FEV1%, MIP, MEP increase.

The Study of Functional Independence and Bone Mineral Density in Athletes With Spinal Cord Injury (척수 손상을 가진 운동선수와 비운동선수의 일상생활 기능과 골밀도 비교연구)

  • 목적 : 본 연구는 척수손상환자들 중 신체활동 정도가 급격하게 차이 나는 운동 선수군과 비선수군의 일상생활동작과 골밀도를 비교 분석하는데 그 목적이 있다. 방법 : 본 연구를 위해 20명(선수 10명, 비선수 10명)의 SCI 환자가 실험에 참가하였다. 골밀도 측정을 위해 이중에너지 방사선골밀도 측정기(Lunar Prodigy, GE Healthcare. England)를 이용하여 종골부위(calcaneus)의 골밀도를 측정하였다. 척수손상환자의 일상 생활 기능을 측정하기 위해 자조관리(self care), 호흡과 괄약근 조절 (respiration and sphincter management task), 이동(transfer)의 세 영역으로 나누어진 SCIM II(Spinal Cord Injury Measurement II)을 이용하였다. 척수손상환자들 중 운동 선수군과 비운동 선수군의 일상생활 동작과 골밀도를 비교 하기 위해 SPSS 14.0 통계 프로그램의 independent t-test를 이용하여 통계분석을 실시하였고 일상생활 동작과 골밀도의 상관관계는 Pearson correlation을 실시하였다. 유의수준은 ${\alpha}$=.05로 통계처리 하였다. 결과 : 검사 결과 선수군이 비선수군보다 통계적으로 유의하게 큰 SCIM 점수와 T-score를 보여주었다. 결론 : 일상생활동작을 측정하기 위하여 SCIM II(SpinalCordIndependenceMeasureII) 척도를 사용하였는데 선수군이 비선수군에 비해 유의하게 높은 SCIM II 총점을 나타내어 운동을 통한 훈련이 척수손상환자의 기능적 활동을 향상시킬 수 있다고 사료된다. 그리고 골밀도 측정 결과 선수군이 비선수군에 비해 통계적으로 유의하게 높은 골밀도를 나타내었다. 이 역시 강도 높은 훈련이 척수손상환자에게 나타날 수 있는 신체구성을 강화시켜줄 수 있을 것으로 사료된다. 운동을 통한 재활 중에서 그 강도가 높을수록 신체의 구성적, 기능적 측면 뿐 아니라 신경의 가소성 측면에서도 증가를 기대해 볼 수 있을 것으로 판단된다.

  • Purpose : The purpose of this study was to examine the Compare the effects of inspiratory muscle strengthening training and expiratory muscle strengthening training of normal adult respiratory function. Method : In this study, we want to compare the effect of inspiratory muscle strengthening training(n=8) and expiratory muscle strengthening training(n=8) to target the normal adult 16 people. expiratory muscle strengthening training, was 25 minutes of training on the basis of the breathing image program that has been pre-recorded. inspiratory muscle strengthening training, use the power-breathe plus on the measured resistance value, was carried out for 25 minutes. Using the spirometer in order to examine the ability to breathe, FVC, FEV1, FEV1 / FVC, MVV was measured. Result : The results showd that in the breath muscle strengthening training FVC, FEV1, MVV increased statistically significantly. The inspiration muscle strength training FVC, FEV1, MVV was a statistically significant increase, FEV1/FVC decreased. There was no statistically significant difference between. Conclusion : In conclusion, both methods give the result of increasing the effective respiratory function. Inspiratory muscle strengthening training, the function of the lung is very limited to be used when and by us effectively and expiratory muscle strengthening training to increase the capacity of the lung is an effective way that will increase the volume.
