Interdisciplinary approach on oral rehabilitation of an adolescent patient with multiple missing teeth: a clinical report (타과와의 협진을 통한 다수치 결손을 동반한 성장기 환자의 보철 수복 증례)

  • In case of missing of permanent teeth by trauma or innate defect, the decision of treatment modalities and application timing have an important effect on the prognosis of oral rehabilitation. In this case report, interdisciplinary approach between the orthodontic and prosthodontic treatment, the way to re-establish the collapsed occlusal vertical dimension, and implant prosthetic considerations will be discussed. Proper diagnosis on teeth and craniofacial skeleton was made prior to treatment and provisional restorations were used in regard of growth patterns of the patient. Finally, the edentulous areas were restored with fixed implant prostheses. Diagnosis, treatment rationale and prognosis will be discussed thoroughly.

  • This study aims to examine a special type of hospital known as Western Oriental medicine treatment designed to provide the best treatment by combining the advantages of Western and Oriental medicine, The actual conditions of the organizations and operations of O.P.D and how the cooperative treatment between Western and Oriental medicine is conducted at general hospitals that operate the cooperative treatment by applying a drawing analysis and an area analysis, and provide materials that help design hospitals that offers the cooperative treatment based on the information. According to the results of this study, the forms of the cooperative treatment between Western and Oriental medicine included several methods; to operate the cooperative center separately between an Oriental hospital and a general hospital, to operate the cooperative hospital, using several floors for an Oriental hospital inside a general hospital, and to operate the cooperative center in specialized centers inside a general hospital and the type of cluster that offers the cooperative treatment in specialized centers is known as the best appropriate treatment.

Analysis of Risk Factors for Complication after Hip Fracture Surgery in the Elderly According to Geriatric Interdisciplinary Team Care and Orthopedic Care (노년내과와 정형외과의 협진 여부에 따른 노인의 고관절 골절수술 후 합병증 발생 위험 요인분석)

  • Purpose: The study was to analyze clinical outcomes and risk factors of for complications associated with the hip fracture surgery in the elderly before and after interdisciplinary treatment. Methods: A retrospective method was used to investigate the general and therapeutic characteristics, frequency of complications and clinical outcomes. The subjects of the study were 553 patients who underwent hip fracture surgery from January, 2009 to December, 2014. Results: The interdisciplinary group was older and less likely to walk independently even before the fracture than a usual care group. The incidence of complications was higher in the interdisciplinary group than the usual care group. The prevalence of complications in both groups was 66.5%. Multivariate logistic regression analysis showsed that the risk factors for complications of hip fracture surgery were as follows: advanced age, stroke, Parkinson disease, time interval from emergency room to operation, pre & post ambulatory status, American Society of Anesthesiologists (ASA) classification quality of postop intensive care unit (ICU) care and foley indwelling. Conclusion: This study has implications in that it recognized the necessity for interdisciplinary treatment and provided the basic base data for nursing intervention of the elderly patients who underwent hip fracture surgery.

Radiation prosthetic stents applied to oral cancer patients during the radiation therapy: case reports (효율적 방사선요법을 위한 구강 방사선스텐트의 적용: 증례보고)

  • Radiation prosthetic stent is defined as the customized oral devices which serve for an efficient administration of radiation dose to the affected areas or a minimizing the unnecessary irradiation to surrounding normal tissues during maxillofacial cancer radiotherapy. Since the use of stents is individualized, a close collaboration among radiotherapist, surgeon and prosthodontist is essential thereby which helps in limiting the post-therapy morbidity as well as the stable oral rehabilitation. In this report, two customized stents (bolus carrier and tongue depressing) were fabricated and applied to patient undergone irradiation for soft palate and tongue carcinoma selectively. Multidisciplinary approach can be a proper strategy and recommended for control the sequel of post-irradiation.

A Study on the Characteristics and Consultation Request Type of Inpatients Referred for Depressive Symptoms (우울 증상으로 의뢰된 입원환자의 임상적 특징 및 자문 의뢰 형태에 관한 연구)

  • Objectives : The purpose of this study is to investigate the characteristics of depressive patients who admitted to general hospital. We examined the clinical characteristics of patients who were referred to the Department of Psychiatry as depressive symptoms, according to the type of consultation request, and comparing 'with re-consultation' and 'without re-consultation' groups. Methods : We performed a retrospective chart review of 4,966 inpatients who were referred to the Department of Psychiatry from August 2005 to December 2011. Results : For about 6 years, among the inpatients referred for psychiatric consultation, a total of 647 patients were referred for depressive symptoms, accounting for 13.82% of the total consultations. The average age of depressive patients was 58.6 years, which was higher than the average of 56.4 years of overall patients. Among the depressive patients, 275 patients were included in 'with re-consultation' group and there was no statistically significant difference when comparing 'with re-consultation' group and 'without re-consultation' group. However, there was a difference in the tendency of the two groups in the type of consultation request. 'With re-consultation' group was in the order of frequency of consultation type 3-2-1, whereas the 'without re-consultation' group was in the order of frequency of consultation type 2-3-1. Conclusions : The group of inpatients who were referred for depressive symptoms in general hospital showed the largest proportion of the group of patients referred to the Department of Psychiatry. 'With re-consultation' group had a higher rate of re-consultation due to the occurrence of new symptoms after hospitalization compared to 'without re-consultation' group. Therefore, doctors in each department and psychiatrists should pay attention to the depressive symptoms of inpatients and actively discuss treatment plans to improve the quality of medical services, identify risk factors, and make efforts to intervene early if necessary.

  • 다양한 치과 전문분야의 협진 치료가 필요한 환자에서는 심미적인 만족뿐 아니라 기능적이며 장기적 안정을 가져올 수 있는 치료가 전제되어야 한다. 이를 위해서는 분명한 치료목표와 그 목표를 이루기 위한 치료계획을 세우는 것이 선행되어야 한다. 환자와 술자 모두가 만족하는 치료결과를 갖기 위하여 어떠한 치료목표가 필요하며 그 목표를 위하여 치료계획을 세우는 방법과 그 치료과정에 대해 실제 임상증례를 통하여 각 과정을 설명하였다.

  • Lesch-Nyhan syndrome is rare X-linked genetic disorder involving absence of the enzyme hypoxanthine guanine phosphoribosyl transferase (HGPRT) related to purine metabolism. The deficiency of HGPRT activity leads to an excesscive uric acid production and consequent hyperuricemia. It occurs almost exclusively in males, and the incidence is estimate to be 1/100,000~380,000. Clinical presentation is characterized by developmental delay, mental retardation, choreoathetosis, spastic cerebral palsy, nephrolithiasis, obstructive nephropathy and acute gouty arthritis. A characteristic feature of Lesch-Nyhan syndrome is the appearance of intractable self-mutilation behavior. Self-mutilation behavior is complicated by secondary infection and tissue loss as well as pain. The dental management of self-mutilation includes presertive methods of using appliances such as lip bumper or soft mouthguard and radical methods such as extraction of all teeth or orthognathic surgery. A case of Lesch-Nyhan syndrome patient with self-mutilation and severe lower lip injury is presented. He was treated successfully with soft mouthguard.
