The Treatment Choice for Patients with Papillary Thyroid Carcinoma (유두상 갑상선암의 치료 방법 선택에 대한 고찰)

  • 유두상 갑상선암의 대부분은 서서히 진행하는 양호한 임상경과를 보이나, 일부는 저등급 혹은 미분화 갑상선암으로 전환되는 생물학적 특성을 보인다. 저자는 유두상 갑상선암 환자 358예를 적용된 치료방법과 치료결과를 분석하여 논란이 되고 있는 치료방법들 중에서 적절한 치료방법을 찾고자 본 연구를 시도하였다.% 568예중 동측엽절제술 및 협부 절제술 혹은 갑상선 아전 절제술 220예, 갑상선 전 절제술 혹은 근전절제술 143예, 종양적출술 5예였고, 이중 115예는 여러가지 형태의 경부 곽청술이 추가되었고, 150예는 수술후 방사성 동위원소 치료까지 추가되었다. 또 전 환자에서 TSH 억제를 위한 갑상선 호르몬 투여를 하였다. 추적 기간은 5년에서 12년까지 평균 8.5년이었다. Cady와 Rossia AMES Scoring system에 따라 저위험군과 고위험군으로 나누어 재발율과 사망율을 산출한 결과 저위험군 255예중 재발율 11.0%, 사망율 0.4%, 고위험군 65예중 재발율 36.9%, 사망율 20.0%을 보여 고위험군에서 현격히 불량한 예후를 나타내었다. 따라서 저자들은 유두상 갑상선암의 치료방법 선택은 일률적인 치료방법 보다는 환자 개인의 임상적 특성, 병기 등을 고려하여 저위험군에서는 소극적 수술을, 고위험군에서는 적극적인 수술과 수술후 보조치료를 시행하는 것이 바람직하다고 사료되었다.

  • Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a disease characterized by jaw necrosis and delayed wound healing in patients who had received bisphosphonates. Buccal fat pad (BFP) can be used as a pedicled flap in the posterior region of the oral cavity. BFP pedicle flap needs simple surgical technique and it shows less donor site morbidity and aesthetic problem than other vascularized flap. BFP pedicled flap was fed by 3 arteries-facial, internal maxillary, and transverse facial artery. Osteomyelitis was generally related with poor blood supply. Thus, rich blood supply of BFP pedicle flap can have a potential advantage to BRONJ patients. In this case report, we presented 3 BRONJ patients treated by BFP pedicle flap after sequestrectomy.

  • There are many challenges for reconstruction after intraoral tumor resection. Especially, palatomaxillary reconstruction has two primary goals: closure of the oronasal communication and re-creation of proper myomucosal function. Prosthodontic treatment using obturator and several surgical procedures are selected depending on the size and site of the defect, the difficulty of operative procedure, operation time and donor site problem. Above all, it is considered that radial forearm free flap is the first choice for palatal reconstruction. Our department introduces a novel method using tunnelized-facial artery myomucosal island flap for palatomaxillary defect reconstruction, which can successfully reduce donor-site morbidity, and duration of surgery and hospitalization.

  • The arterial stenosis related to the intimal thickening of the arterial wall is the main cause of many diseases in human cardiovascular system. Hemodynamic behavior of the blood flow is influenced by the presence of the arterial stenosis. In this study, effects of the pulsatile flow, caused by the periodic motion of the heart, on the blood flow and its interaction with the arterial stenosis are analyzed by the FEM-based computational fluid dynamics. As a result, it was found that the characteristics of the pulsatile flow in the artery with stenosis are quite different from those of the steady flow. And, the pulsatile flow condition affects the wall shear stress, which is one of the most important physiological parameters in the hemodynamics.

  • Purpose: Liposarcoma is the most common soft tissue sarcoma, and usually occurs on the thigh or in the retroperitoneal space, but rarely in the oral region. This report presents a case of liposarcoma of the cheek and includes a review of the literatures. Methods: A 21-year-old woman was admitted with a palpable mass in her cheek of about two years duration, which increased in size gradually initially, but had increased rapidly over the three months. There was no particular pain or tenderness. MRI showed a well-enhanced, well-defined mass, which suspected to be hemangioma. Results: The spherical, well-encapsulated mass was surgically excised. Biopsy results revealed myxoid liposarcoma. FDG PET-CT on the seventh postoperative day, revealed a minimal to mild FDG-uptake soft tissue lesion around the mass defect area without evidence of distant metastasis. The patient is being observed and undergoing radiation therapy. Conclusion: Liposarcoma in the head and neck region is a rare disease, and can be overlooked as a benign tumor without a pathologic diagnosis. Therefore, proper treatment and follow-up are required based on an understanding of this disease.

  • Pilomatricoma(or calcifying epithelioma) is a not common benign solitary tumor originated from outer root sheath cell of hair follicle or hair follicle of sebaceous glands. The tumor usually presents as an asymptomatic, hard, superficial located, and skin colored to reddish blue cutaneous mass. Most of the tumors are less than 10mm in diameter and adherent to the skin. Recently, 48-year-old man presented with cheek mass. The tumor was 2.6cm sized and located at the subcutaneous layer of cheek on CT scan. The tumor was clearly removed via transoral approach with buccal incision leaving no wound on face. The mass was confirmed as pilomatricoma on pathologic examination. Herein, we report our experience with literature review.

  • Based on four specimens (50.9~69.0 mm SL) collected from a small stream of southern coast of Jeju Island, Eleotris acanthopoma was described as the second Eleotris species as well as the first record from Korea. They were characterized by having two dorsal fins, well-separated pelvic fins, separation of two rows of sensory papillae on opercle and extending the fourth row of sensory papillae beyond the 13th longitudinal sensory-papillae row on cheek. We proposed a new Korean name, "Geom-eun-gu-gul-mu-chi", for the species.

  • Kabuki syndrome is characterized by typical facial features (elongated palpebral fissures with eversion of the lateral third of the lower eyelid; arched and broad eyebrows; short columella with depressed nasal tip; large, prominent, or cupped ears), minor skeletal anomalies, persistence of fetal fingertip pads, mild to moderate intellectual disability, and postnatal growth deficiency. A 6-year-old male with kabuki syndrome was referred from the local dental clinic for left facial swelling and dental caries on all primary molars. He was treated for acute periapical abscess with incision and drainage under physical restraint, and left maxillary second primary molar was extracted. Other caries treatment was performed under general anesthesia. As the syndrome involves many different medical problems, special cares should be considered. Dental treatment should be carried out in comprehensive consultation system.

  • The bulging of mandibular angle area, frequently, make some difficulties in social life due to its reckless appearence, especially in oriental nations. So, many authors had reported its causes and surgical techniques for correction since late 19th century with diagnosis of masseter hypertrophy. But in spite of its muscular origin, major surgical techniques, in general, should aim complete bony reduction or osteotomy and supplemental myotomy as in hemimandibular hypertrophy or mandibular elongation. We used ultrasonogram for soft tissue depth estimation and expearenced some complications such as incomplete lingual cortical bony reduction, condylar fracture in mandibular angle reduction via intraoral circumvestibular approach. So we reported our surgical technique for angle reduction with possible complications.

Redescription of the Hawaiian Ladyfish Elops hawaiensis from Korea (한국산 당멸치, Elops hawaiensis의 재기재)

  • Redescription of the Hawaiian ladyfish Elops hawaiensis was carried out based on four specimens collected from Busan and Gwangyang between 2008 and 2010. This species has a gular plate between the symphysis and isthmus, and the entire premaxillary tooth band is exposed when the mouth is closed. Our Korean specimens of E. hawaiensis are characterized by having 65~67 vertebrae, 14 anal fin rays and 95~101 lateral line scales, which values differed slightly from those given in previous studies. Molecular analysis using 4S0 base pairs of mitochondrial DNA cytochrome b sequences showed that our four specimens corresponded well with E. hawaiensis (99.8~100.0%). Therefore, our results suggest that there may exist regional differences in vertebrae and anal fin ray counts for E. hawaiensis.
