Pulmonary Resection for Invasive Pulmonary Aspergillosis in Hematological Malignancy Patients (혈액암 환자에서 합병된 침습성 폐아스페르길루스증의 수술적 치료)

  • Background: Invasive pulmonary aspergillosis, a frequent fungal infection in immunocompromised patients, is known to have a poor prognosis despite the use of antifungal therapy in leukemic patients. We studied the outcome of surgical resection of invasive pulmonary aspergillosis where bleeding tendency, localized recurrence of infection, and incidence could be reduced. Material and Method: We retrospectively reviewed 14 patients with a hematological malignancy where invasive pulmonary aspergillosis was diagnosed during the 10 years between 1998 and 2007. From the medical records, we reviewed the type and treatment of the hematological malignancy, including the diagnostic methods of invasive pulmonary aspergillosis, the preoperative hematological conditions and their management, and the surgical methods and records. We also analyzed the development of postoperative complications and patient mortality, the recurrence of postoperative invasive pulmonary aspergillosis, and if the patients had a bone marrow transplant. Result: Fourteen patients with invasive pulmonary aspergillosis and a hematological malignancy underwent a pulmonary lobectomy. One patient had a complication of bronchopleural fistula, but there were no other serious complications such as bleeding or wound infection, and none of the patients died postoperatively. Conclusion: We have shown that pulmonary lobectomy is a safe and effective therapy for invasive pulmonary aspergillosis in patients with hematological malignancies that allow further treatment of the hematological malignancy.

Intracellular Monokine Levels in Different Types of Cancer (암의 유형에 따른 모노카인(monokine) 비교)

  • 목적 : 본 연구는 암환자 및 암의 유형에 따라 중요한 종양억제 조절인자로 알려져 있는 모노카인을 flowcytometry를 이용하여 분석, 비교하고자 하였다. 방법 : 연구대상은 고형종양(solid tumor)으로 진단받은 33세에서 76세 사이의 암환자 30명(유방암, 난소암, 폐암, 위암)을 대상으로 말초혈액 단구의 intracellular monokine 중 $TNF{\alpha}$, MIG, MIP를 분석한 유사실험설계 연구이다. 연구결과 : 암환자 군에서의 $TNF{\alpha}$, MIG, MIP 수치는 대조 군인 정상 군에 비해서 유의하게 증가되었으며 특히, 유방암과 난소암 환자 군에서의 $TNF{\alpha}$ 수치는 폐암과 위암의 대상자에 비해 의미 있는 차이를 보여주었다. 논의 : 본 연구에서 제시된 암환자 군에서의 모노카인 수치는 선행연구의 결과와 통일하게 종양 대상자의 면역에 중요한 역할을 하는 것으로 규명 되었으나, $TNF{\alpha}$는 고형종양 중에서도 여성생식기계 암환자 군에서 더 증가하였다. 이에 따라 종양 유형에 따른 모노카인의 역할과 호르몬과의 상호작용기전 규명에 대한 추후 연구가 필요하다.

  • 주치의에게는 암환자의 암 전이 여부가 초미의 관심사다. 암환자의 10명 중 9명이 전이암으로 사망하기 때문이다. 암의 전이 초기에 그 전이 여부를 방사선 진단법으로 가능하지 않다. 혈액을 채취하여 암의 전이 유무를 진단 하는 기술이 개발되고 있다. 이 혈중 암세포는 혈구세포 10억개당 1~100개 정도의 극히 미량이 존재하여 암세포 분리기술이 특별히 잘 개발되어야 한다. 최근 마이크로바이오칩 형태의 분리기술이 큰 기술적 진화를 보이고 있어 본고에 소개하고자 한다. 이 기술은 한 가지 큰 의미를 갖는다. 그것은 암환자의 암 전이 모니터링에 필요한 도구가 될 수 있기 때문이다. 전이암세포 검출 키트로 전이암세포를 계수 하여 환자에게 투약한 항암제가 적합한지에 대한 답을 의사는 얻을 수 있다. 전이암세포 진단용 마이크로바이오칩 기술이 기존의 영상진단법만큼 중요한 임상 수단이 될 것으로 전망된다.

Prognosis of Colorectal Cancer and Apoptosis Activation (대장암의 예후와 Apoptosis 활성)

  • We studied on the expression of apoptosis in colorectal cancer, lymph node, their corresponding normal mucosa and colorectal cancer patient's blood by genomic DNA electrophoresis and TUNEL labeling method. From 7 cases among 37, 20 cases among 47 and 5 cases among 15, DNA ladders were expressed in normal tissues, colorectal tissues and Iymph node tissues, respectively. A DNA ladder was not observed in 7 cases of colorectal cancer patients blood. In case of TUNEL labeling, we could observe TUNEL color espression in colorectal cancer and lymph node tissues. As these result suggest that apoptotic index may be associated with the colorectal cancer development, and mat be used as a prognostic indicator but further evaluations will be needed.

Stomach Cancer Secondary to Hematologic Diseases (혈액질환에 속발하는 이차성 위암)

  • Purpose: Patients with hematologic diseases such as chronic myeloid leukemia (CML) or chronic lymphoid leukemia (CLL) are known to have an increased chance of acquiring a secondary neoplasm. Stomach cancer is one of the most common malignant diseases in Korea, and we investigated whether the incidence of secondary stomach cancer in patients with a hematologic disease increases, in order to determine if a more intensive screening program for detecting secondary gastric cancer was required. We also investigated the safety of performing a gastrectomy in hematologic disease patients. Materials and Methods: From 1992 to 2006, the medical records of 8376 patients diagnosed with one of the six common hematologic diseases were reviewed. Results: Nine secondary stomach cancers were found among the 8376 patients during the 15-year observation period. No surgical-related complications occurred, and there was no recurrence of stomach cancer if detected early. Conclusion: It seems that a more intensive screening program for detecting secondary gastric cancer in hematologic disease patients is not required, and surgery is not risky in these patients.

  • 유전자에 대한 정보를 획득하는 기술적인 문제가 해결되면서, 질병 진단을 위한 새로운 접근 방법으로 혈액 속에 있는 모든 단백질(proteome)의 구성을 분석하는 프로테오믹스(proteomics)에 대한 연구가 최근 들어 활발하게 진행되고 있다. 본 논문은 암 진단을 위하여 혈액 중의 단백질의 구성을 측정한 2차원 전기영동 (2D electrophoresis) 젤 데이터를 해석하는 새로운 방법을 제시하였다. 우선 측정된 많은 단백질 스팟(spot) 중에서 T-statistics 방법으로 단백질 스팟들을 선택하였다. 선택된 단백질 스팟들로 이루어진 암 환자와 정상인 두 샘플들의 확률 분포를 각 집단에 따로 적용된 PCA 영역에서 계산하였다. 최종적으로 조건부 확률의 차이에 근거한 베이즈 분류(Bayes classification) 이론을 적용하여 암 진단을 하였다.

  • The current population of elderly is increasing and the with an extended average lifespan, the frequency of cancerous occurrences have also increased, with these increases and the increase in radiotherapy for cancer patients, recognitions of harm and importance have become known. This article was known tumor treatment of patients with hematopoietic disorder by doing a comparative study on the changes in blood cells caused by the acute effects of trace dose to high dose of radiation exposed to mice. According to the sensitizer injection may give rise to harm to the components of peripheral blood. This material needs to be considered when for treating tumor patients and the risks of hematopoietic harm and believe that radiation therapy will be reasonable.

Relationship Between Spiritual Health and Depression of Patients with Hematologic Malignancy (혈액 암 환자의 영적건강과 우울의 관계연구)

  • Purpose: This study was designed to identify the relationship between spiritual health and depression with the hematological malignancies patients in an isolated room. Method: The obtained data were analyzed using SPSS Win 12.0. For the statistical anayses, Pearson correlation coefficients, multiple linear regression analysis, t-test and ANOVA were calculated. Results: The mean scores of the status of spiritual health, depression were 111.62 and 29.78, respectively. The score of spiritual health was significant differences by the faith (F=19.65, p=0.000). Depression score was significant differences by age (F=4.561, p=0.002) and spiritual state (F=4.843, p=0.004). Spiritual health and depression was moderately correlated (r=-.681, p=.000). Conclusion: From the above results, oncology nurse should consider spiritual health and depression when caring patients with hematological malignancies.
