Evaluation of factors affecting sensory neural hearing loss (감각신경성 청력손실에 영향을 미치는 제 요인에 관한 연구)

  • Reports on a potential relationship between sensory neural hearing loss(SNHL) and cardiovascular or hematologic factor show that the results are controversial. A detailed analysis of risk factors in the development of SNHL was carried out in 3,050 non-noise exposed healthy worker. The mean hearing threshold of both ears at 500, 1000, 2000, 4000, 8000Hz was measured and the effect of age, possible cardiovascular risk factor and hematologic factor (blood viscosity and hemostatic factors) on SNHL were evaluated. first, each of these were associated with loss of hearing sensitivity when univariately and multivariatively analysed. In a multiple regression model, age, sex, body mass index, WBC and total cholesterol level were independently associated with the mean of hearing sensitivity decrease at 4000 and 8000Hz. Second, study subjects were divided into two group (normal vs SNHL) and we compare the possible risk between both groups, and analysed univariate and multivariative logistic model. In a multiple logistic regression model, age, sex, body mass index, WBC and total cholesterol level, total protein, platelet were independently associated with SNHL. Our results show that we have some reliable indices of susceptibility to SNHL using cardiovascular measures or biochemical factor, but future, more extensive studies are required.

Changes of Coagulability after Off-pump CABG: Comparison with On-pump CABG (심폐바이패스 없이 시행하는 관상동맥우회술 후 혈액응고 기늠의 변화: 심폐바이패스하 관상동맥우회술 후 혈액응고 상태와의 비교)

  • Hemostatic function is regarded to be preserved after an off-pump coronary artery bypass grafting (CABG), compared to conventional CABG, and the preserved hemostatic function may increase thrombotic occlusion of the graft. We studied the changes of hemostatic variables in patients undergoing off-pump CABG, and compared to those of on-pump CABG. We studied the changes of coagulation function in 11 patients who underwent off-pump CABG (group I), and compared them with those of 11 patients who underwent on-pump CABG and Dor procedure (group II). Coagulation status was evaluated by thromboelastography and blood coagulation test preoperatively, postoperative 1$^{st}$ day, 2$^{nd}$ day, 3$^{rd}$ day, and 5$^{th}$ day, respectively. Among the variables measured by thromboelastography (such as r time, k time, $\alpha$ angle, and MA value) and blood coagulation test (such as factor Ⅶ, protein S, protein C, antithrombin III, activated protein C resistance test, plasminogen, D-dimer, prothrombin time, activated partial thromboplastin time, platelet count, hemoglobin, and fibrinogen), there were significant differences in the MA value, $\alpha$ angle, and platelet counts between the two groups. MA values were 140$\pm$72% and 153$\pm$98% in group I, and 87$\pm$27% and 78$\pm$28% in group II, at postoperative 3$^{rd}$ and 5$^{th}$ days, respectively (p<0.05). $\alpha$ angle was 122$\pm$92% in group I and 69$\pm$23% in group II at postoperative 3$^{rd}$ day (p=0.09). Platelet count was 63$\pm$55% in group I and 33$\pm$13% in group II at postoperative 3$^{rd}$ day (p<0.05). Patients who underwent off-pump CABG showed increased coagulability during postoperative periods, compared to those who underwent on-pump CABG. Our data suggest that aggressive perioperative anticoagulation therapy is warranted in patients undergoing off-pump CABG. CABG.

Relationships of Peripheral Inflammatory Marker With Suicide Attempt History (자살시도력과 말초혈액의 염증 지표 간의 연관성)

  • Objectives : The purpose of this study is to determine whether there is a significant difference in the neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio according to the history of suicide attempt in patients with psychiatric diseases. Methods : A medical record review was conducted on patients who had been hospitalized in Konyang University Hospital since 2021-03-01 to 2023-02-28 to collect demographic and clinical characteristics. T-test for continuous variables and Chi-square test for categorical variables were performed to determine demographic differences according to the history of suicide attempt, and the ANCOVA test was performed to compare the average value of peripheral inflammatory marker according to the history of suicide attempt with gender and age as covariates. One-way variance analysis was performed to determine whether the number of suicide attempt causes significant difference of the peripheral inflammatory marker. Results : The final analysis target of this study was 266 patients, 101 had history of suicide attempt, and 165 had no history of suicide attempt. The neutrophil-to-lymphocyte ratio (p<0.001) and platelet-to-lymphocyte ratio (p<0.001) were higher in patients with the history of suicide attempt than patients without the history of suicide attempt, but the neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio were not significantly increased depending on the serial increase of number of suicide attempts. Conclusions : This study suggests that peripheral inflammatory markers are meaningful and easily accessible indicators for predicting the risk of suicide attempt in psychiatric patients. We expect that prospective follow-up study will be conducted with more subjects and controlled potential confounding variables.

The Effects of Health Conservation Program in Community-dwelling Vulnerable Diabetic Elderly (취약계층 당뇨병 노인을 위한 건강보존프로그램의 효과)

  • This study was conducted to develop and apply the health conversation program to determine its effectiveness. The study employed a pretest-posttest nonequivalent control group design. There were 25 subjects in one intervention group and 22 subjects in the control group. The program included a 4-week, 12-session education program in health conversation for professionally retired elderly mentors and an 8-week, 24-session program for vulnerable elderly diabetics provided by mentors who were older persons trained in diabetes management. Data were collected from December 2015 to May 2016 and subsequently analyzed by an independent t-test using SPSS 20.0. Biochemical variables and physical variables were assessed by blood test and Inbody 230 (Biospace). Psychosocial variables were assessed by questionnaires. The health conservation program led to significantly decreased FBS (p<0.01), TC (p<0.01), TG (p=0.04), SBP (p=0.03), waist (p<0.01) and symptoms of depression (p=0.01), and significantly increased social support (p=0.02). These results indicated that the health conservation program is effective at increasing social support and decreasing FBS, TC, TG, SBP, waist, and symptoms of depression in community-dwelling vulnerable diabetic elderly; accordingly, health conservation programs should be applied to improve quality of life of vulnerable diabetic elderly.

A Study on Clinical Variables Contributing to Differentiation of Delirium and Non-Delirium Patients in the ICU (중환자실 섬망 환자와 비섬망 환자 구분에 기여하는 임상 지표에 관한 연구)

  • Objectives : It is not clear which clinical variables are most closely associated with delirium in the Intensive Care Unit (ICU). By comparing clinical data of ICU delirium and non-delirium patients, we sought to identify variables that most effectively differentiate delirium from non-delirium. Methods : Medical records of 6,386 ICU patients were reviewed. Random Subset Feature Selection and Principal Component Analysis were utilized to select a set of clinical variables with the highest discriminatory capacity. Statistical analyses were employed to determine the separation capacity of two models-one using just the selected few clinical variables and the other using all clinical variables associated with delirium. Results : There was a significant difference between delirium and non-delirium individuals across 32 clinical variables. Richmond Agitation Sedation Scale (RASS), urinary catheterization, vascular catheterization, Hamilton Anxiety Rating Scale (HAM-A), Blood urea nitrogen, and Acute Physiology and Chronic Health Examination II most effectively differentiated delirium from non-delirium. Multivariable logistic regression analysis showed that, with the exception of vascular catheterization, these clinical variables were independent risk factors associated with delirium. Separation capacity of the logistic regression model using just 6 clinical variables was measured with Receiver Operating Characteristic curve, with Area Under the Curve (AUC) of 0.818. Same analyses were performed using all 32 clinical variables;the AUC was 0.881, denoting a very high separation capacity. Conclusions : The six aforementioned variables most effectively separate delirium from non-delirium. This highlights the importance of close monitoring of patients who received invasive medical procedures and were rated with very low RASS and HAM-A scores.

Penalized logistic regression models for determining the discharge of dyspnea patients (호흡곤란 환자 퇴원 결정을 위한 벌점 로지스틱 회귀모형)

  • In this paper, penalized binary logistic regression models are employed as statistical models for determining the discharge of 668 patients with a chief complaint of dyspnea based on 11 blood tests results. Specifically, the ridge model based on $L^2$ penalty and the Lasso model based on $L^1$ penalty are considered in this paper. In the comparison of prediction accuracy, our models are compared with the logistic regression models with all 11 explanatory variables and the selected variables by variable selection method. The results show that the prediction accuracy of the ridge logistic regression model is the best among 4 models based on 10-fold cross-validation.

Influence of Social Support and Health Literacy on Treatment Adherence in Hemodialysis Patients (혈액투석환자의 사회적 지지와 건강정보이해능력이 치료순응도에 미치는 영향)

  • This study was a descriptive research study for hemodialysis patients to survey the effects of social support and health literacy on treatment adherence. The subjects of this study were 140 hemodialysis patients aged over 40 years who had been receiving treatment for more 1 year in artificial kidney rooms at two general hospitals in Y city. The data were collected from November 1, 2019 to December 31, 2019 and were analyzed using descriptive statistics, t-tests, one-way ANOVA, Scheffe test, Pearson's correlation coefficients, and multiple regression with the SPSS/WIN 26.0 statistical program. The results of this study show that social support (family, friends, medical staff) and health literacy (functional, communication, critical) were positively correlated with treatment adherence. The variables affecting treatment adherence in hemodialysis patients were identified by social support and health literacy with 69.6% explanatory power. To improve the treatment adherence of hemodialysis patients, it is necessary to develop education programs to improve health literacy based on social support.

Exposure to Blood and Body Fluids and Influencing Factors on Compliance with Standard Precautions among Nurses in General Hospital (종합병원 간호사의 혈액 및 체액 노출과 표준주의지침 이행도에 대한 영향 요인)

  • The purpose of this study was to identify blood and body fluids exposure, and factors influencing compliance with standard precautions among nurses. Participants consisted of 144 nurses in three general hospitals located in C province. Data were collected using a questionnaire that consisted of awareness, compliance, safety environment and attitude toward standard precautions. A total of 40.3% of participants had been injured by syringes or sharp tools that had already been used for patients within the last 1 year 1.01 times. Moreover, 22.9% of participants had been exposed to blood and body fluid of patients 1.06 times within the last 1 year. The mean score for compliance with standard precautions was lower than its awareness. There was a significant correlation between compliance and awareness and a safe environment. Multiple regression revealed an adjusted $R^2$ of 0.166 with the awareness score serving as the major predictor variable for compliance of standard precautions. Therefore, it is necessary to develop strategies to apply to systemic and continuous education regarding standard precautions and to foster a safe environment to enhance nurses' compliance with standard precautions.

Extracts of Housefly Maggot Reduces Blood Cholesterol in Hypercholesterolemic Rats (고콜레스테롤 랫드에서 파리유충 추출물의 혈액지질 감소기전)

  • The aim of this study was to evaluate the biological mechanism of orally administered ethanolic extract of fly maggot(EM) on hypocholesterolemic rats fed a high-cholesterol diet. Sprague Dawley male rats were divided into four groups (EM dose control=0, 5.0, 7.0, and 9.0 mg/100 g BW) and were treated for 6 weeks. EM groups revealed a significant reduction in serum triglyceride, total cholesterol, and LDL-C when compared with the control group(p<0.05). HMG-CoA reductase activity in EM groups were lower than those of the control group, but total sterol, neutral sterol, and bile acid excretion were increased in EM groups when compared with the control group(p<0.05). To identify the biological mechanism of EM towards the hypocholesterolemic effect, sterol response element binding proteins (SREBPs) and the peroxisome proliferator-activated receptors ($PPAR{\alpha}$ transcription system were determined in rats fed a high-cholesterol diet. It was discovered that EM suppress the expression of SREBP-$1{\alpha}$ and SREBP-2 mRNA in the liver tissues of high-cholesterol diet fed rats, while simultaneously increasing the expression of $PPAR{\alpha}$ mRNA(p<0.05). This finding indicates that EM may have hypocholesterolemic effects in rats fed a high-cholesterol diet, by regulating cholesterol metabolism-related biochemical parameters and SREBP-$1{\alpha}$ SREPB-2 and $PPAR{\alpha}$gene expression.

The Evaluation of Artificial Lung Using Blood Substitutes (대체혈액을 이용한 인공폐의 평가에 관한 연구)

  • In this paper a newly designed oxygenator module was used to perform the experiments for pressure drop and oxygen transport in order to evaluate the efficiency of the artificial lung. Also, distilled water. sodium sulfite solutions used in this experiment were evaluated for the performance of other artificial lungs. Substituted bloods have many advantages over whole blood in studying pressure drop and oxygen uptake. They are relatively inexpensive, and require fewer variables to be controlled. Furthermore, deoxygenation is not necessary when those solutions are used. In addition to these advantages. assays and interpretation of the experimental results are relatively easy. In the case of using the sodium sulfite solution having the same oxygen partial pressure as whole blood. the oxygen transfer rate of the sodium sulfite solution was basically same as that of whole blood. It was concluded in evaluating the function of artificial lungs that the sodium sulfite solution was suited for measuring oxygen transfer rate. In our module, artificial blood was flowed into the outside of hollow fiber membrane. The artificial lung created in this experiment showed that pressure drop was reduced to $1/3\~1/6$ of that compared to the inside-flow case.
