Blood Pressure of Healthy Newborns in the First Week of Life (분당 차병원에서 출생한 신생아 혈압치 비교)

  • Purpose : This study was peformed to study normative blood pressure data in full-term neonates that may be used to facilitate Identification of neonatal hypertension.Methods : 383 newborns born in our hospital from May 2003 to January 2004 were enrolled in this study Using an oscillometric device(BP-88 NEXT, COLIN Corp.), their blood pressures were measured more than one time within a week after birth. According to each clinical variable such as sex, delivery mode, birth weight, gestational age and presence of maternal disease or perinatal problems, we divided the population into groups and calculated the mean blood pressures of each group. We compared mean blood pressures between the divided groups according to each clinical variable statistically.Results : Mean systolic and diastolic blood pressure of the Population was 70.8$\pm$ 10.9 mmHg and 43.4 $\pm$ 8.0 mmHg, respectively. There was no satistically significant difference in blood pressure according to clinical variables. Mean systolic pressure showed positive correlation with birth weight and gestational age(r=0.1420, 0.03130).Conclusion : Our results are almost in agreement with Zubrow's data from 695 newborns in U.S.A, 1995. Our data may be helpful for early detection and management of neonatal hypertension, thereby maintaining renal function ,and preventing possible complications of renal disease.

  • Blood pressure (BP) is one of the important physiological parameters for diagnosing cardiovascula diseases by means of noninvasive method. Existing noninvasive methods for measuring arterial BP have to use cuff and difficult in measuring arterial BP continuously. Systolic blood pressure (SBP) and pulse transit time (PTT) have a kind of inverse relationship. We acquired PTT data when subjects were in relaxation and also after exercise. We performed the linear regression analysis for making the regression equations for each subject and the regression equation for all subjects. We compared the estimated SBP with the measured SBP to check the accuracy of our regression equations. From the result, the regression equations for each subject was appropriate according to the American National Standards Institute of the Association of the Advancement of Medical Instrument (ANSI/AAMI) which says that BP devices should have ±5mmHg mean of error and 8mmHg standard deviation of error. However, the regression equation for all subjects was not proper to ANSI/AAMI recommendation. The result means that, without cuff, we can continuously estimate each subject's SBP through PTT and indivisual calibration.

  • We propose the tele-monitoring system to real-time medical signal system and implemented the blood-pressure and blood-sugar monitoring system with bluetooth communication system. And, it transmits patient's data blood pressure, blood sugar to doctor and doctor sends the result of medical treatment to patient's phone, hand-phone or PDA. We developed the measurer as portable type in order to non-restrained monitor.

  • This study is to develop automatic extraction system of continuous blood pressure using ECG (Electrocardiogram) and PPG(Photoplethysmography) for u-health care technology. PTT (Pulse Transit Time) was determined from peak difference between ECG and PPG and its inverse made to get blood pressure. Since the peaks were vulnerable to be contaminated from noise and variation of amplitude, this study developed the adaptive algorithm for peak calculation in any noise condition. The developed method of the adaptive peak calculation was proven to make the standard deviations of PPT decrease to 28% and the detection of noise increase to 18%. Also, the correlation model such as blood pressure = -0.044 $\cdot$ PTT + 133.592 has successfully been determined for predicting the continuous pressure measured without using cuff but with using PPG and ECG, only.

  • In this study, we developed a circuit device detecting korotkoff sounds of forearm electronic blood pressure monitor by digital signal. In order to test a circuit detecting signal from korotkoff sounds, systolic and diastolic pressure were compared our developed circuit device with the existing forearm electronic blood pressure monitor (Model: SE-7000, Korea). Devices for an experiment composed of a forearm cuff, a stethoscope, an amplifier, a PC with A/D board, etc. Results of korotkoff sounds was similar to a pattern of oscilometric signals from the existing forearm electronic blood pressure monitor. We thought it is possible to measure blood pressures, if blood pressures were detected precisely using signals of korotkoff sounds.

  • The brachial systolic blood pressure and pulse pressure are the predictors of cardiovascular disease in individuals over 50 years of age. As the stiffness increases, the reflex amplitude and pressure in the late systole increase, resulting in an increase in left ventricular load and myocardial oxygen demand. Therefore, it is necessary to study how stiffness affects blood pressure. In this study, the blood pressure pulse waves were measured before and after taking the drug, and the blood pressure pulse wave was measured before and after myocardial heart transplantation in patients with heart failure. The correlation between R, L, and C components of the Windkessel model was estimated by increasing blood pressure. As a result of modeling the parameters of the Windkessel model using the curve fitting method, the increase in blood pressure and decrease in systolic rise time were due to the increase in the L component in the RLC Windkessel model. Among the various mechanical characteristics of blood vessels, the most important parameter affecting high BP waveform is the inertance.

Effect of Green Tea and Coffee Drinking after Meal on the Postprandial Fall of Blood Pressure in Elderly Person with Postprandial Hypotension (식후저혈압 노인에서 식후의 녹차와 커피 섭취가 혈압하강에 미치는 효과)

  • The purpose of this study was to compare the preventive effects of green tea and coffee drinking on postprandial hypotension in the elderly. A total of 30 women ($81.2{\pm}6.8years$) who had experience of postprandial hypotension participated 3 interventions by random order allocation. During the experimental interventions, each participant had a cup of instant coffee (60mg caffeine), or green tea (15.8mg caffeine) after lunch, and control group had only their lunch. Systolic blood pressure (SBP), diastolic blood pressure (DBP), and heart rate (HR) were measured with 15 minute interval from 30 minutes before the meal to 120 minutes after the meal. The preprandial SBP and DBP were not significantly different between three groups. Postprandial SBP reduction measured at the 1 hour after meal was significantly smaller in coffee group compared to the control group, whereas green tea group was not ($-15.1{\pm}17.9mmHg$ vs $-13.4{\pm}22.4mmHg$ vs. $-4.1{\pm}18.6mmHg$ for control, green tea, and coffee group, respectively, p=.032). Likewise, DBP decrement at the 1 hour after meal was significantly smaller in coffee than control group ($-12.8{\pm}13.0mmHg$ vs $-6.9{\pm}16.6mmHg$ vs $-0.8{\pm}13.4mmHg$ respectively, p=.033). The HR response of the three groups were not significantly different throughout the intervention. Coffee drinking was potentially beneficial for preventing postprandial hypotension and resultant falling event.

Estimated Blood Pressure Algorithm of Wrist Wearable Pulsimeter Using by Hall Device (홀소자를 이용한 손목착용 맥진기의 혈압추정 알고리즘)

  • In order to get the precise blood pressure and pulse number in the cuffless status, the wrist wearable pulsimeter with a portable and small size apparatus using by Hall device is developed. The regression analysis of the pulse wave measured by the testing product of pulsimeter is conducted two equations of the blood pressure algorithm. The estimated values of blood pressure obtained by the cuffless pulsimeter during 5 s are compared with the practical values measured by electronic or mercury liquid blood pressure meters. The standard deviation of the estimated value and the practical value for the high blood pressure and the low blood pressure were 12.1 and 5.9, respectively, which have the neighborhood values of BP International Standard. The detail analysis of a pulse wave measured by cuffless wrist wearable detecting the changes of the magnetic field can be used to develop a new diagnostic algorithm of blood pressure applying for oriental medical apparatus like as the wrist wearable pulsimeter.

중등 정도 고혈압인 사람에서, 정어리 근육 가수분해물로부터 유도된 단쇄 펩타이드인 Valyl-Tyrosine의 항고혈압 효과

  • 본 연구는 선천선고혈압쥐(SHR)에서와 마찬가지로, 높은 정상혈압 및 중등 정도의 본태성고혈압 환자에서 Valyl-Tyrosine(VY)의 항고혈압 효과 여부를 결정하기 위해 수행되었다. 29명의 지원자에 대하여 무작위 이중맹검 위약검사를 실시하였으며, 3 mg의 VY를 포함하는 100-ml 음료와 100ml-위약 음료를 조제하여 사용하였다. 연구대상은 VY (남16/여1, 45.5${\pm}$3.2세, 146.4${\pm}$2.3/90.5${\pm}$1.8 mmHg) 집단과 위약(P) (남11/여1, 48.8${\pm}$3.0 세, 145.5${\pm}$2.4/92.3${\pm}$1.8 mmHg) 집단으로 나누었다. 대조(C)기간 3주 째에 VY-음료 또는 P-음료를 하루 2번씩 투여하기 시작하여 4주 간의 실험(E)기간(BP)은 매주 측정되었으며, 아침에 앉은 자세에서 측정하였다. 혈액 표본은 C기간과 E기간의 마지막 날 채취하였다. VY집단에서, 수축기(S) 및 이완기(D) 혈압의 감소는 첫째 주에 각각 9.7 및 5.3 mmHg (P < 0.001)이었으며, E기간의 시작에 따른 넷째 주에는 각각 9.3 및 5.2 mmHg (P < 0.001)이었다. P 집단에서는 SBP와 DBP 모두 변하지 않았다. VY 집단의 혈압은 회복기간의 끝까지 점진적으로 증가하였다. VY집단에서, 혈장 안지오텐신(Ang) I과 VY의 농도는 현저히 증가한 반면 Ang II와 알도스테론은 VY 투여 후에 현저히 감소하였다. VY는 SHR에서와 마찬가지로 중등 정도의 고혈압 환자에서도 Ang I-전환효소 억제를 통하여 현저한 항고혈압 효과를 가지는 것으로 보이며, 어떤 부작용도 검출되지 않았다.

EFFECT OF KOREAN RED GINSENG POWDER (GP), ADMINISTERED ORALLY, ON BLOOD PRESSURE IN HYPERTENSIVE RATS (홍삼분말이 고혈압 쥐에 미치는 영향)

  • 이미 발표된, 혈압에 미치는 인삼의 효과에 관한 연구들의 보고는 서로 상반된 견해들이 많이 있다. 인삼에는 혈압상승작용과 혈압을 내려주는 효과가 모두 있다고 보고하였다. 본 저자들은 고혈압 쥐에 미치는 인삼의 급${\cdot}$만성효과를 연구하기 위해 일련의 2가지 실험을 하였으며, 실험재료로 사용한 홍삼은 일한홍삼주식회사에서 구입하였다. (실험 1) 홍삼분말을 일반 쥐사료에 혼합하여 (5g/kg), 임의로 사료를 섭취하게 하여 11주간 쥐를 사육하였으며, 하루에 섭취한 인삼의 양은 체중 kg당 $250{\~}750$mg이었다. 홍삼분말의 투여는 DON쥐, SHR쥐, 그리고 한개의 신장을 협자시킨 고혈압 쥐의 경우 혈압에 영향을 미치지 않았다. 홍삼분말의 투여는 DOC 쥐에 있어 혈압을 약간 상승시켰다. (실험 2) 홍삼분말을 체중 kg당 350mg씩 경구투여한 후 복대동맥에 카뉼제를 삽입하여 혈압을 24시간 직접 측정하였다. 그 결과 DOC쥐와 SHRSP쥐에 있어 혈압에 영향이 없었다. 이 실험에서는 마취를 시키지 않았으며, 자유로이 풀어 놓았다. 실험 1과 같이 매일 체중 kg당 $300{\~}500$mg씩 11주간 홍삼분말을 만성적으로 투여하였을 때, SHRSP쥐의 혈압에 영향이 없었다. DOC 고혈압 쥐의 경우 약간의 혈압 강하가 있었으며, DOC 고혈압의 정도는 (실험 2)에서 약간 낮았다.
