Although significant progress has been made in the surgical treatment of esophageal carcinoma as well as in the detection of early stage esophageal carcinoma by diagnostic techniques, the prognosis of the esophageal carcinoma patients remain poor. The p53 gene product is known to regulate cell growth and proliferation. And the nm23 gene was identified originally as an anti-metastatic influence whose expression was correlated inversely with tumor metastatic potential in murine melanoma cell lines. This experiment was intended to know the relationship among the p53 and nm23 gene expression versus clinicopahologic characteristics of the esophageal cancer. Total 40 cases were collected from patients who had undergone esophagectomy at St. Mary's Hospital, Catholic university of Korea. Immunohistochemical stain for p53 mutant-type protein and nm23 protein was graded as <10% positive tumor cells: negative; 10∼30% positive tumor cells: + ; 30∼50% positive tumor cells: ++, and >50% positive tumor cells: +++. The tumor invasion was grades as none:- ; mild:+ ; moderate:++ ; severe: +++. Overexpression of p53 protein and nm23 was not associated with the survival and cliniocopathologic characteristics of the esophageal cancer. Moreover, the combination analysis of p53 and nm23 revealed that there was no relationship between the gene expression and the clinicopatholic characteristics of the esophageal cancer.
Background: Spontaneous pneumomediastinum is an uncommon disorder with few cases reported. It usually occurs in young males and has benign self-limiting course without any apparent concomitant factor. It is seen after intrathoracic pressure changes leading to alveolar rupture. The clinical experiences of two medical centers were reviewed to aid in optimal management. Material and Method: Retrospective review between March 2003 and August 2010 with spontaneous pneumomediastinum patients was performed. Result: 24 patients were identified with a diagnosis of spontaneous pneumomediastinum. These 24 patients were comprised of 18 men and 6 women with mean age 18.9 years (range 10 ~ 33). The major initial complaints were chest pain (79.2%), throat pain (62.5%), and subcutaneous emphysema (41.7%). The triggering events were exercise (16.7%), coughing (12.5%) and vomiting (12.5%). No apparent triggering event was noted in 54.2% of patients. In all cases, chest radiograph and computed tomography was done. Diagnostic computed tomography was required in 25%. White blood cell counts and C-reactive protein (CRP) were checked, and their initial mean values were $9,790{\pm}3,240/{\mu}L$ Land $1.31{\pm}1.71mg/dL$, final mean values were $5,440{\pm}1,665/{\mu}L$ Land $0.72{\pm}0.73mg/dL$, respectively. 23 patients were admitted (average $5.0{\pm}1.8$) and the symptoms were self-limiting in all cases without complications. Conclusion: Spontaneous pneumomediastinum is a benign condition with mild inflammatory signs that often presents with chest or throat pain. Secondary causes must be ruled out to avoid an unfavorable outcome with less invasive study. Because of very rare complications and recurrence, outpatient basis and shortened hospitalization may be feasible.
Acute aortic dissection associated with high mortality rate has an extremely poor prognosis if early diagnosis and treatment are not received. Recently, with advanced computed tomography and echocardiography, diagnostic rate is higher and early operation is possible. Therefore preoperative medical therapy at ER(emergency room) lowered the mortality rate. This study was done to analyze the results with preoperative management at ER and operations, retrospectively. Material and Method: A series of 42 patients treated surgically for acute aortic dissections from 1991 to 2001 were included in this study. There were 18 males and 24 females. Mean age was 51.1 years. The admission course through emergency and outpatient department(OPD) was 34 and 8 respectively. Result: 26 patients underwent ascending aorta replacement-7 combined aortic valve replacements, 7 patients underwent descending aorta replacements and 9 patients received Bentall's operation. At emergency department, 20 patients received antihypertensive drugs and $\beta$-receptor blockers and 6 patients died. 22 patients did not receive antihypertensive and $\beta$-receptor block drugs and 10 patients died. There were 16(38%) overall deaths. Conclusion: Early diagnosis at ER or OPD is essential for acute aortic dissection, and it is important to select the most appropriate noninvasive interventions as possible. Therefore, preoperative drug therapy at ER is suggested according the patient conditions.

Background: Non-invasive interventional therapy has been performed for main bronchial obstruction by endobronchial tuberculosis because of the risk of main bronchial reconstruction regardless of the pulmonary function. But, effects of the inteeventional therapy are attacked by arguments. This study was aimed at interpreting the risk and effectiveness of bronchoplasty for benign bronchial stenosis over the last ten years in our hospital by reviewing the results based on clinical progression. Material and Method: We retrospectively reviewed the clinical records and out-patient medical records including 2f consecutive patients who underwent main bronchial reconstruction for obstruction by endobronchial tuberculosis. All of them had past medical history of anti-tuberculosis medication. They were preoperatively evaluated by bronchoscopy and chest computed tomography. Result: There were no incidences of postoperative mortality and signifcant morbidity. There were 2 cases of retained secretions but these problems were resolved by therapeutic bronchoscopy or intubation. All of the patients are still alive without obstructive airway problem. Conclusion: Bronchoplasty should be considered as one of the primary treatment modalities, if it is anatomically feasible.
The purpose of this study is to evaluate the usefulness of coronary computed tomography angiography of coronary artery disease for the purpose fo health screening according to gender and age. In addition the association between hematological factors (Glucose, total cholesterol, visceral fat, body mass index, triglycerides, high density lipoprotein (HDL), low density lipoprotein (LDL)) and coronary artery disease is investigated. A retrospective analysis of coronary computed tomography angiography with 299 subjects showed that the detection rate of coronary artery disease was higher in men over 50 years of age and it was statistically significant. In particular, the prevalence rate of men (37.9%) was about 2 times higher than that of women (17.0%). Glucose and HDL as hematological factors associated with coronary artery disease were statistically significant. The prevention and management of coronary artery disease seems to require the control of glucose and high density lipoprotein (HDL). Although it is not statistically significant with other hematological factors, the need for management of coronary artery disease was identified. the coronary computed tomography angiography of coronary artery has higher radiation doses than other CT scans. Therefore, for the purpose of screening, coronary computed tomography angiography should be considered in consideration of the sex and age of the examinee, and detection of coronary artery disease through other non-invasive tests should be prioritized over coronary computed tomography angiography.
Video-assisted thoracic surgery(VATS) has been widely used in the treatment of spontaneous pneumothorax in spite of the absence of definitive data regarding the relative safety and long term results of this procedure. We reviewed 34 patients (group I) who underwent )8 video-assisted surgical procedures for spontaneous pneumothorax from June 1994 to December 1995 and compared the results of these patients with the results of another 14 patients (group ll) who underwent bullectomy through axillary Oho- racotomy during the same period. Average age, sex distribution, site and extent of pneumothorax, surgical indications, and complication rate showed no differences between the two groups. In group ll patients, th number and sites of bullae tend to be multiple compared to patients in group 1. The mean number of ends-GIA used for stapling of bullae was 2. 6 per patient with the range from 2 to 4 in group 1. The mean duration of chest tube drainage was not different between the two groups ().7 days and 3.9 days), but the mean time to discharge was significantly shorter in group I (5.6 days) than in group ll (8.9 days). Mean follow-up time was 12 and 11 months in each groups and ranged from 2 to 21 months. Pneumothorax recurred after three of 38 procedures in group I (7.9 %) with no recurrence in group ll. These data suggest that video-assisted thoracic surgery is a viable alternative to thoracotomy for the treatment of spontaneous pneumothorax with low morbidity and shorter hospital stay. However, it should be applied cautiously to patients with spontaneous pneumothorax because of the relatively high incidence of recurrence compared to axillary thoracotomy.
Eighteen patients operated on for thymoma from 1985 to 1994 were evaluated, 3 with myasthenia gravis and 15 without. Masaoka's clinical staging revealed stage I disease in 5(27.8%), stage ll in 6 (33.3%), stage 111 in 6(33.3%), and stage IV in 1 (5.6%). Of the 18 patients, 10 had surgical resection as the only treatment and the remaining 8 (stage II : 1 patient, stage III : 6 patients, stage IV: 1 patient) had surgical resection with adjuvant preoperative or postoperative radiotherapy and/or chemotherapy There was no operative mortality. Currently, 15 patients are alive, 3 are dead. The mean follow up time for all patients was 3.4 years. Acturlal survival at 5 years was 82.2 $\pm$ 9% for all patients' 100% for those in stage I and II, and 62.5% for those in stage III. Pleural and mediastinal recurrence developed in one patient in stage II which was considered to have noninvasive disease on the operating field. Two patients underwent reoperation for recurrence of thymoma a intervals of 14 months and 52 months. Clinical stage and resectability had a significant prognostic value(p < 0.05). The presence of myasthenia gravis is no longer considered as an adverse factor in survival.
Background: Pleural drainage following video-assisted thoracic surgery has traditionally been achieved with largebore, semi-rigid chest tubes. Recent trends in thoracic surgery have been toward less invasive approaches for a variety of diseases. The purpose of this study was to evaluate the safety and efficacy of drainage by means of small, soft, and flexible 14Fr Blake drains. Material and Method: Between December 2007 and March 2008, 14Fr silastic Blake drains were used for drainage of the pleural cavity in 37 patients who underwent a variety of video-assisted thoracic surgical procedures at our institution. Result: The average postoperative length of hospital stay was 3.26 days (range, 2~12 days), Blake drains were left in the pleural space for an average of 3.15 days (range, 1~7 days), and the average amount of drainage was 43.8 ml/day. The maximal amount of blood removed daily by a Brake drain was as much as 290 mL. There were no drain-related complications. Blake drains seemed to cause less pain while in place, and particularly at the time of removal. Conclusion: The use of a Blake drain following minor thoracic surgery appeared to be safe and effective in drainage of fluid or air in the pleural space, and were associated with minimal discomfort.

Purpose : To find sensitivity of MRI imaging methods to slow flow phantom study was performed with conventional Spin-Echo, gradient echo based Phase Contrast, fast GRASS, and heavily T2-weighted Fast Spin Echo pulse sequences. Materials and Methods : A siphon driven flow phantom was constructed with a ventriculo-peritoneal shunt catheter and a GE phantom to achieve continuous variable flow. Four different pulse sequences including Spin-Echo, Phase Contrast, GRASS and Heavily T2-weighted Fast Spin Echo were evaluated to depict slow flow in the range from 0.08 ml/min to 1.7 ml/min and to compare signal intensities between static fluid and flowing fluid. Results : In the slow flow above 0.17 ml/min conventional Spin-Echo showed superior apparent contrast between static and flowing fluid while GRASS was more sensitive to the very slow flow below 0.17 ml/mim. It was not accurate to calculate flow and velocity below 0.1 ml/min with a modified PC imaging. Conclusion : Four different MR pulse sequences demonstrated different sensitivity to the range of slow flow from 0.08 ml/min to 1.7 ml/min. This finding may be clinically useful to measure CSF shunt flow or detecting CSF collection and thrombosis.
Malignant mesenchymoma is a very rare tumor presented during the embryonic and infant period and malignant mesenchymoma in the adult is extremely rare. Tumor is composed of two or more unrelated mesenchymal derivatives apart from fibrous tissue. These tumors are thought to be originated from embryonic mesenchyme capable of differentiating into any type of connective tissue. A 61 years old man with complaints of cough and copious sputum of onset of two months was admitted after initial examinations, showing a very huge mass over the right upper lobe. Right pneumonectomy with partial rib resection of 3rd, 4th, and 5th ribs was performed due to the initial diagnostic impression of squamous cell carcinoma by the fine needle aspiration biopsy. The operative field presented a mass locating across the interlobal fissure with severe adhesions to the chest wall. Postoperatively, the patient received 5,000 rads of radiotherapy and presently, 6 months later, has shown no signs of recurrence.
