MRI Evaluation for the Histologic Components of Soft-tissue Tumors: Comparison of MEDIC and Fast SE T2-weighted Imaging (연조직종양의 조직 성분 평가를 위한 자기공명영상: MEDIC 과 지방억제 T2 영상의 비교)

  • Purpose : To compare Multi Echo Data Image Combination (MEDIC) and fast SE T2-weighted images with fat saturation (T2FS) to suggest more accurate evaluation of the histologic components of soft-tissue tumors. Materials and Methods : The experimental group included 25 histologic tissues (5 vascular, 4 neural, 4 fibrous, 4 hypercellular, 2 hemorrhagic necroses, 2 cystic, 2 lipoid, 1 myxoid stroma, and 1 thrombus) in 10 patients who had pathologically confirmed schwannoma (n = 3), hemangioma (n = 2), lipoma (n = 1), angiokeratoma (n = 1), synovial sarcoma (n = 1), liposarcoma (n = 1), and malignant fibrous histiocytoma (n = 1). The inhomogeneity values were measured using the standard deviation value (SD) divided by the mean value as SD presents an error amount similar to that of imaging heterogeneity. Results : The inhomogeneity values of 25 histologic components were lower on MEDIC than those on T2FS (p < .001). Conclusion : We conclude that MEDIC is more accurate than T2FS for evaluating the tissue components of soft-tissue tumors using digitalized data because MEDIC images have far lower inhomogeneity.

  • Soft tissue tumor classifications should be an important part of radiology, oncology and, for clinicians and pathologists, they provide diagnostic instruction and prognostic guidelines. In soft tissue tumor classification systems, the World Health Organization (WHO) classifications have become dominant, enabled by the timely publication of new 'blue books' which included detailed text and numerous good illustrations. The new WHO classification of soft tissue tumors was introduced in 2002. Because the classification represents a broad consensus concept, it has gained widespread acceptance around the globe. This article reviews the changes which were introduced the adipose tumors, fibroblastic/myofibroblastic tumors, so-called fibrohistiocytic tumors, smooth muscle tumors, pericytic tumors and skeletal muscle tumors which have been first recognized or properly classified during the past decade.

Image Recognition and Its Application to Radiograph (화상인식과 X선 영상에의 응용에 관한 연구)

  • In this study, we propose a method for quantifying the degree of advance of pulmonary emphysema by using chest X-ray images. With this method, we devise two schemes for this purpose. One is for detecting blood vessels by using a deformable model with the tree-like structure and using an evaluation function specialized by knowledge about blood vessels appeared in chest X-ray images, and the other is for quantifying the degree of advance by using several features, which were extracted from blood vessels, and the equation of quantitative evaluation. In order to evaluate the performance, we applied the proposed method to 189 ROIs(Regions of Interest) of ten chest X-ray images and compared the values by the proposed method with those by a medical doctor.

Primary Malignant Melanoma Presenting as an Anterior Mediastinal Mass (전종격동에 원발한 악성 흑색종)

  • Primary malignant melanoma of the mediastinum is extremely rare : To date, there have been only 8 cases reported in the literature. We report a case of a 50-year-old woman presented with an anterior mediastinal mass. The mass invaded the lung parenchyme of right upper lobe, which was wedge-resected with the mass. Microscopically, spindle or polygonal tumor cells with prominent nucleoli and intracytoplasmic melanin pigments are characterized and Fontana stain confirmed the presence of melanin pigment. Immunohistochemical studies showed diffuse positive staining for HMB45, S100 protein and vimentin, which further suggested melanoma.

  • 병변의 양상에 따른 감별진단 재발성 이하선 종대를 보이는 경우 타액선조영술이 영상진단방법중 1차적인 선택이 된다. Stensen 씨관이나 중심선관(central glandular duct)의 확장을 보이는 경우 만성 타액선염으로 진단할 수 있다. 소아에서의 단일성 양성종괴의 감별진단은 임파절, 혈관종, 양성혼합종양, 저급점액상피암, 임파관종 등이다. 성인에서의 단일성, 양성으로 관찰되는 종괴는 양성혼합종양, Warthin씨 종양, 저급점액상피암, 선상낭성암, 소포상세포암 등이다. 여러개의 종괴를 보이는 경우 Warthin씨 종양, 소포상세포암, 임파종, 육아종, 전이암 등이며 단일성 낭성종괴의 경우는 branchial cleft cyst,, Warthin씨 종양, 상피낭포 등이다.

  • Hemangioblastoma is a benign tumor of vascular origin that develops usually in the posterior cranial fossa. We report a case of supratentorial leptomeningeal hemangioblastoma occurring in a 45-year-old man who received total removal of recurrent cerebellar hemangioblastoma four years ago. He was admitted for the evaluation of severe headache and magnetic resonance image showed a well-enhanced, extra-axial mass in the right parietal region. A presumptive diagnosis was meningioma. It was completely removed with the attached dura. Histological examination including immunohistochemical study showed typical findings of hemangioblastoma. It is emphasized that close observation may be necessary for hemangioblastoma, even after total removal.

Malignant Hemangiopericytoma of the lung (폐에서 발생한 악성 혈관외피세포종 1례 보고)

  • Hemangiopericytoma is a rare tumor of vascular origin, first described by Stout and Murray in 1942. It is characterized by proliferation of capillaries surrounded by pericytes. There is no characteristic clinical or radiological finding. Wide excision is the treatment of choice. A 21 year-old man was admitted with one year history of productive cough. On admission, chest film showed large lobulated mass and Right lower lobe atelectasis. Rt. pneumonectomy was performed and the tumor was confirmed as malignant hemangiopericytoma of the lung. Local recurrence didn`t occur until now.

  • Spinal cavernous hemangiomas of the cauda equina are extremely rare vascular malformations. We report a case of intradural extramedullary cavernous hemangioma of the cauda equina with it's clinical, radiologic and surgical findings. This is the twelveth case of cavernous hemangioma of the cauda equina in the literature. The pertinent literatures are reviewed.
