혈관내벽에 홀뮴-166 방사선 분할 조사시 흡수선량 분포

  • 경피적관상동맥성형술(CPTCA)이나 스텐트삽입술 후에 발생하는 재협착을 방지하기 위한 방사선을 조사하는 방법 중에 베타 입자를 방출하는 액체 선원을 catheter풍선 내에 넣어 일정 시간 방사선 조사 시키는 방법이 있다. 조사시킬 혈관의 길이가 길어 한번의 방사선 조사가 어려울 경우 영역을 분할하여 두 번에 나누어 조사할 경우가 있다. 조사영역의 겹치는 부근의 흡수선량이 고선량이나 저선량이 되는가를 알기 위하여 두 풍선간의 접근 거리에 따른 혈관내벽의 흡수선량 분포를 알아보았다. 풍선내의 액체 선원은 Ho-l66을 이용하였고 Ho-l66의 물리적 반감기는 26.8시간이고 최대에너지 1.85 MeV, 평균에너지 0.69 MeV와 최대에너지 1.77 MeV, 평균에너지 0.65 MeV를 갖는 베타 입자를 방출한다. Ho-l66 의 방사선 흡수선량을 측정하기 위하여 GafChromic 필름(Nuclear Associates, Carle Place, NY, USA)을 이용하였고, 방사선이 조사된 필름의 optical density는 videodensitometer(Wellhofer, Schwarzen-bruck, Germany)를 이용하여 값을 읽었다. Catheter 풍선은 직경이 3 mm 이고 길이가 20 mm인 것을 이용하였다. 혈관 내벽의 최대 흡수선량을 표준화하여 겹치는 부분의 흡수선량 분포를 접근 거리에 따라 구하였다. 또한 몬테카를로 시abf레이션으로 확인하였다. 두 풍선의 겹치는 부근의 선량 분포는 풍선 중앙에서 중앙사이의 거리가 21 mm 일 때 중앙에서 20% 증가하였고, 거리가 22 mm일 때와 23 mm일 때 각각 10%와 40%의 감소를 보였다. 풍선 도자의 풍선 안에 베타입자 방출 액체 선원을 넣어 혈관내벽에 방사선 조사하는 방법은 비정거리가 짧아 혈관 내벽 부근에만 방사선을 조사시키고 그 외 중요 장기에는 영향을 덜 미치는 장점이 있다. 그러나 혈관 내벽 표면으로 부터의 거리에 따라 흡수선량이 급격히 떨어지는 분포를 이루기 때문에 두 개의 풍선이 겹치는 부근의 흡수선량은 아주 작은 접근 거리에서도 급격한 변화를 보였다. 따라서 시술 중에 겹치는 부분을 아주 적게 분할하여 정확하게 차례차례로 조사시키기 위해서는 신중한 거리 조정을 하여야 한다.

  • A major concern associated with carotid artery angioplasty and stenting (CAS) is a periprocedural distal cerebral embolization. To prevent distal embolization, embolic protection devices (EPDs) have been developed. However, the risk of cerebral embolism after protected CAS in patents with a vulnerable plaque is controversial and either a silent or a symptomatic stroke can occur despite the use of EPDs. Here, we report a case of a massive cerebral microemboli after a protected CAS using a distal filter EPD for a vulnerable plaque with a lipid rich necrotic core and intraplaque hemorrhage.

Change of Serum Levels of C-Reactive Protein After Coronary Angioplasty and Its Effects on Clinical Restenosis (협심증 환자에서 관동맥중재술에 따른 C-reactive Protein의 변화 및 임상경과)

  • Background: There are many evidences that inflammation is an important determinant of the development of atherosclerosis and one of the systemic markers of inflammation, C-reactive protein(CRP), is associated with extent of coronary artery disease and risk of coronary events. We assessed the time response of CRP response after coronary angioplasty and it's influence on the clinical restenosis in angina patients. Materials and Methods: Patients included 36 angina patients undergoing single vessel angioplasty. Levels of CRP were measured before and 12, 24, 48, and 72 hours after angioplasty. Clinical restenosis was assessed at 6 months after procedure. Results: Baseline CRP level was $0.30{\pm}0.01$ mg/dL in stable and $0.46{\pm}0.28$ mg/dL in unstable angina patients(p<0.05). After angioplasty, CRP level was increased with peak at 24 hour and persisted to 72 hours after angioplasty. At 24 hour after angioplasty, the magnitude of CRP change was $0.32{\pm}0.31$ mg/dL in stable and $0.79{\pm}0.73$ mg/dL in unstable angina patient(p<0.05). The change of CRP level was not associated with troponin-T after angioplasty. In unstable angina patients, clinical restenosis was developed in 8% of patients with low baseline CRP levels and in 50% of those with high baseline CRP levels more than 0.6 mg/dL(p<0.05). Conclusion: In unstable angina patients, inflammatory response is more increased than stable angina patients, and increased inflammatory response effects on the restenosis after coronary angioplasty.

  • Percutaneous vertebroplasty consists of the percutaneous injection of polymethylmethacrylate (PMMA) cement into a collapsed vertebral body in order to obtain pain relief and mechanically strengthen the vertebral body. This procedure is now extensively used in treating osteoporotic vertebral compression fracture. It is an efficient treatment, but it is not free of complications. Most complications after vertebroplasty are associated with PMMA leakage. Pulmonary embolism of PMMA is rare, but this can occur when there is a failure to recognize venous migration of cement early during the procedure. We encountered a case of a patient with asymptomatic pulmonary embolism because of PMMA after percutaneous vertebroplasty. Chest X-ray and CT scanning revealed numerous tubular branching opacities that corresponded to the pulmonary vessels at the segmental and subsegmental levels.

  • Iatrogenic foreign bodies are a challenging complication to both the interventional radiologist and patient, resulting in impaired quality of life and substantial financial cost. The case report describes a successful percutaneous transhepatic removal of an intra-abdominal foreign body. A 72-year-old man underwent surgery for placement of a retrievable covered stent for refractory bile leakage after left hemihepatectomy. Three days after placement, stent folding and migration into a chronic biloma cavity occurred via the bile leakage site. By using a balloon catheter technique, the folded stent could be straightened and repositioned into the bile duct to minimize stent-strut injury during retrieval. The interventional approach could be a valid treatment option for intra-abdominal foreign bodies, as well as intravascular foreign bodies. A thorough understanding of devices and techniques can provide the interventional radiologist with valuable information regarding procedural planning and the management of iatrogenic foreign bodies.

  • Aortic anastomotic leak is an uncommon complication after ascending aortic replacement for acute aortic dissection. Redo-surgery is the traditional standard treatment despite high mortality and morbidity. Recently, endovascular treatment has been attempted as an alternative approach in a few case reports. Here, we present two cases of aortic anastomotic leak due to suture line dehiscence after aortic graft replacement for type A aortic dissection, which were successfully treated by coil with subsequent N-butyl cyanoacrylate embolization and an AmplatzerTM vascular plug.

  • Due to recent changes in living conditions, people who suffer from vascular disease have been increasing. As a result, several kinds of procedures to treat diseases of the blood vessels are being carried out and the epidemiological analysis and interpretation is needed. In this paper, the mechanical behavior of blood vessels based on hyperelastic model were evaluated. The stress distributions in the arterial walls subjected to both normal blood pressure and high blood pressure are studied along with different opening angles representing as the effect of the residual stress. As a result, when applied to residual stress effects in blood vessels to act maximum stress compared to as the absence of residual stress effect about 50% stresses can be reduced. When high blood pressure was the normal blood pressure acting on the blood vessel wall that twice stress can be confirmed.

  • A 10-month-old intact male Maltese dog (body weight, 1.6 kg) presented with intermittent cough and abnormal heart sounds. The dog was diagnosed with a left-to-right patent ductus arteriosus (PDA) on the basis of the results of transthoracic echocardiography, color flow Doppler examination, and angiography. Transcatheter occlusion of the PDA was performed using the $Amplatz^{(R)}$ Canine Duct Occluder (ACDO) via the femoral artery. The device was placed under fluoroscopic guidance and successfully occluded the PDA. The continuous heart murmur disappeared immediately after the correction and no residual flow was detected. The dog was discharged the following day and regularly examined for complications and safety. This is the first report demonstrating the clinical application of ACDO for the transcatheter closure of PDA via the femoral artery in a small-breed dog in Korea.

  • Stents are frequently used throughout the human body. They keep pathways open in vascular or nonvascular duct for a long time. Therefore its stability is very important factor. In recent years, aconsiderable amount of research has been carried out in order to estimate mechanical properties of the stent such as expansion pressure behavior, radial recoil and longitudinal recoil using FEM (Finite element analyses). However, published works on simulation of stent fatigue behavior using FEM are relatively rare. In this paper, a nonlinear finite-element methodwas employed to analyses the compression of a stent using external pressure and fatigue behavior. Finite element analyses for the stent system were performed using NASTRAN FX. In conclusion this paper shows how the stent is behaved in the body, and its fatigue behavior.

  • A 74-year-old woman presented at our hospital with hemoptysis. Three months ago, she had endovascular stent-grafting done by a general surgeon for a saccular thoracic aneurysm that was found accidentally following an episode of fever and chills. Despite a lasting fever after the procedure, she was discharged without further treatment and follow-up. She was subsequently admitted to the hospital for evaluation and several exams were performed. Chest CT scans and an esophagoscopy identified an aorto-esophageal fistula at the level of the aorta that was covered by a previous stent-graft. After extensive administration of antibiotics, surgery was done - esophagectomy, cervical esophago-gastrostomy and replacement of the thoracic aorta. She was later discharged uneventfully.
