Squamous Cell Carcinoma in the Base of the Tongue in a Dog (개의 혀 뿌리에서 발생한 편평세포암종)

  • It is difficult to detect and diagnose a mass at the tongue base, particularly if the mass is located under normal appearing mucosa. Computed tomography (CT), particularly post-contrast images, is useful to evaluate a tongue mass and adjacent structures including cervical lymph nodes. Nevertheless, a definitive diagnosis of a mass is obtained by histopathological examination. In this case, we describe a mass arising from the tongue body to the root in a dog. The patient was referred with ptyalism and an eating disorder. An oral examination was performed, and decreased tongue motility was observed. CT scanning was performed, and a lingual mass was detected. A lingual mass sample was collected by biopsy at the postmortem 2 months later. A histopathological examination was performed, and the lingual mass was diagnosed as squamous cell carcinoma.

Evaluating the usefulness of BinkieRTTM (oral positioning stent) for Head and Neck Radiotherapy (두경부암 환자 방사선 치료 시 BinkieRTTM(구강용 고정장치)에 대한 유용성 평가)

  • Purpose: The purpose of this study is to evaluate the effectiveness of oral positioning stent, the BinkieRTTM in radiation treatment for head and neck cancer patients in terms of tongue positions reproducibility, tongue doses and material properties. Materials and Methods: 24 cases using BinkieRTTM during radiation treatments were enrolled. The tongue was contoured on planning CT and CBCT images taken every 3 days during treatment, and then the DSC and center of tongue shift values were analyzed to evaluate the reproducibility of the tongue. The tongue dose was compared in terms of dose distribution when using BinkieRTTM and different type of oral stents (mouthpiece, paraffin wax). Randomly selected respective 10 patients were measured tongue doses of initial treatment plan for nasal cavity and unilateral parotid cancer. Finally, In terms of material evaluation, HU and relative electron density were identified in RTPS. Results: As a result of DSC analysis, it was 0.8 ± 0.07, skewness -0.8, kurtosis 0.61, and 95% CI was 0.79~0.82. To analyze the deviation of the central tongue shift during the treatment period, a 95% confidence interval for shift in the LR, SI, and AP directions were indicated, and a one-sample t-test for 0, which is an ideal value in the deviation(n=144). As a result of the t-test, the mean and SD in the LR and SI directions were 0.01 ± 0.14 cm (p→.05), 0.03 ± 0.25 cm (p→.05), and -0.08 ± 0.25 cm (p ←.05) in the AP direction. In the case of unilateral parotid cancer patients, the Dmean to the tongue of patients using BinkieRTTM was 16.92% ± 3.58% compared to the prescribed dose, and 23.99% ± 10.86% of patients with Paraffin Wax, indicating that the tongue dose was relatively lower when using BinkieRTTM (p←.05). On the other hand, among nasal cavity cancer patients, the Dmean of tongue dose for patients who used BinkieRTTM was 4.4% ± 5.6%, and for those who used mouthpiece, 5.9% ± 6.8%, but it was not statistically significant (p→.05). The relative electron density of Paraffin Wax, BinkieRTTM and Putty is 0.94, 0.99, 1.26 and the mass density is 0.95, 0.99 and 1.32 (g/cc), Transmission Factor is 0.99, 0.98, 0.96 respectively. Conclusion: The result of the tongue DSC analysis over the treatment period was about 0.8 and Deviation of the center of tongue shifts were within 0.2 cm, the reproducibility was more likely excellent. In the case of unilateral head and neck cancer patients, it was found that the use of BinkieRTTM rather than Paraffin Wax or Putty can reduce the unnecessary dose irradiated to the tongue. This study might be useful to understand of BinkieRTTM's properties and advantages. And also it could be another considered option as oral stent to keep the reproducibility of tongue and reducing dose during head and neck radiation treatments.

  • Lymphangioma is a benign tumor of lymphatic vessels. The majority of cases are present at birth and 95% of the tumors aroused before the age of 10 years. Oral lymphangioma may occur at various sites but are most frequent on the anterior two thirds of the tongue, where they often result in macroglossia. The lesions present nodularity with gray and pink projections. In the present cases, all the patients who had the macroglossia combined with lymphangioma showed openbite and mandibular prognathism. The purpose of this paper was to report the dental and craniofacial findings of macroglossia combined with lymphangioma and review the pertinent literature through the cases.

  • Malignant tumours of the oral cavity that require resection of the tongue result in severe deficiencies in speech and deglutition. In such patients, improvements in mastication, swallowing, and speech may be reasonable goals for treatment. The viability of a prosthodontic approach to treatment depends on the type and extent of surgery. In a total glossectomy, a mandibular tongue prosthesis is the treatment of choice. Mandibular tongue prosthesis occupies the space in the floor of the oral cavity. It provides the patients with a platform for directing food into the esophagus and aids in speaking. This type of prosthesis can achieve that protection of the underlying fragile tissue and improvement in appearance and psychosocial adjustment. This case report describes the technical steps involved in prosthetic rehabilitation for a glossectomy patient.

  • Benign symmetric lipomatosis, also known as Madelung's disease, Launois-Bensaude syndrome and multiple symmetric lipomatosis is a cutaneous condition characterized by extensive symmetric fat deposits in the neck, shoulders, and upper trunk. But symmetrical lipomatosis of the tongue is very rare and characterized by involvement exclusively of the tongue, invasiveness, and absence of encapsulation of the adipose tissue. A 75-year-old man presented with about a year history of painless tongue masses on both lateral border. Bilateral partial glossectomy was performed and subsequently was confirmed as symmetrical lipomatosis pathologically. We report this case with a review of the literatures.

Primary Tuberculosis of the Oral Tongue: A Case Report and Literature Review (혀에서 발생한 일차성 결핵)

  • Tuberculosis is primarily a pulmonary disease and extrapulmonary involvement of the oral cavity is an infrequent occurrence accounting for 0.2 - 1.5% of all extrapulmonary sites. The authors report a case of primary tuberculosis of the oral tongue in a 60-years-old male patient who visited the clinic for ongoing tongue pain that he experienced for several months. The lesion was initially indistinguishable from the malignancy, but differential diagnosis was performed on the biopsy with Ziehl-Neelseen stain. After treatment with anti-tuberculosis, the oral lesion healed completely without surgery.

  • Schwannoma is a relatively uncommon, slow-growing benign tumor that is derived apparently from the Schwann cell. Tongue originated schwannomas are exceedingly rare within 1% and therefore often are not immediately included in the differential diagnosis and oral cavity tumors. In the current study, authors report a case of a schwannoma located at the tongue of a 49 years old patient treated by complete surgical excision. The patient is under clinical control, with no signs of recurrence even after 6 months.

  • Lipoma is a benign tumor composed of mature adipocytes and the most common soft tissue mesenchymal neoplasm but relatively rare in the oral cavity. Lipoma of the tongue is uncommon and especially multiple lipomas are quite rare and only several cases of those were reported. We describe an unusual case of multiple lipomas of the tongue.

Effect of ω3-Fatty Acid Desaturase Gene Expression on Invasion and Tumorigenicity in Human Tongue Squamous Cell Carcinoma Cells (인체 혀의 편평세포암 세포에서 ω3-fatty acid desaturase 유전자 발현이 침윤 및 종양형성에 미치는 영향)

  • Omega-3 polyunsaturated fatty acids (${\omega}3$-fatty acid) have been found to possess anticancer properties in a variety of cancer cell lines and animal models, but their effects in human tongue squamous cell carcinomas (SCCs) remain unclear. This study was designed to examine the effect of ${\omega}3$-fatty acid desaturase (fat-1) gene expression on invasion and tumorigenicity in human tongue SCC cells and the molecular mechanism of its action. Docosahexaenoic acid (DHA) treatment inhibited in vitro invasion in a dose-dependent manner. In zymography, matrix metalloproteinase-9 (MMP-9) and Matrix metallopeptidase-2 (MMP-2) activities were reduced, and MMP-9 and MMP-2 promoter activities were inhibited by the DHA treatment. In addition, cyclooxygenase-2 (COX-2) and vascular endothelial growth factor (VEGF) promoter reporter activities were inhibited in SCC-4 and SCC-9 cells after the DHA treatment. To investigate the effect of a high level of endogenous ${\omega}3$ fatty acids, a stable SCC-9 cell line expressing the ${\omega}3$-desaturase gene (fSCC-9sc) was generated. The growth rate and colony-forming capacity of fSCC-9sc were remarkably decreased as compared with those of fSCC-9cc. Likewise, the tumor size and volume of fSCC-9sc implanted into nude mice were significantly inhibited, with increases in the cell death index. Furthermore, a transwell chamber invasion assay showed a reduction in cell invasion of the fSCC-9sc lines when compared with that of the fSCC-9cc line. These findings suggested that fat-1 gene expression inhibited tumorigenicity, as well as invasion in human tongue SCC cells. Thus, utilization of ${\omega}3$ fatty acids may represent a promising therapeutic approach for chemoprevention and the treatment of human tongue SCCs.

종양성 병변으로 인한 구강점막질환

  • 구강점막에 발생하는 종양성 병변으로는 대표적으로 구강점막암(oral mucous membrane cancer), 유두종(papilloma),혈관종(hemangioma), 임파관종(lymphangioma),섬유종(fibroma)등이 있는데 이들 병변들은 구강내 협점막, 치은, 경구개, 연구개, 구강저, 혀 등에 궤양이나 증식성 병소로 나타나서 동통, 출혈, 치아동요, 종창 등의 증상을 유발시킨다. 이러한 병변들에 대한 명확한 치료와 예후를 결정하기 위해서는 무엇보다도 이들 병변들에 대한 정확한 소견에 근거한 진단이 우선되어야 할 것이다. 따라서 본 장에서는 정확한 진단과 치료를 위해 이들 병변들에 대해 소개하고자 한다.
