Three Dimensional Stress Analysis of a Dental Implant with Central Cavity (중공을 가지는 치과용 임플란트의 3차원 응력해석)

  • In this study, we propose a new short dental implant and investigate its bio-mechanical characteristics by using three dimensional finite element analyses. The proposed dental implant has the central cavity which can be integrated with the core of cancellous bone remained by trepanning drill. We take the Bicon short implant as a reference model for studying the effects according to the shape of cavity. The parametric finite element model using ANSYS APDL has been built to determine which length, diameter and thread of central cavity would be effective to dissipate stress. The reduction of undesirable stress in adjacent bone which can suppress bone defects and the eventual failure of implants. The numerical results shows that the cavity of well-determined shape has the beneficial effects on reducing the bone absorption in cancellous bone.

  • A new species of the genus Jaspis (Demospongiae: Astrophorida: Ancorinidae), Jaspis coreana n. sp., is described on the specimen collected from Geomundo Island, Korea during September 1995 to March 2002 by SCUBA diving. This new species is characterized by two sponges association which is completely covered with Poecilastra wondoensis except for oscules. This new species is similar to J. wondoensis in its spicules type, but different in their spicules size, form of oscules and sponge colour. J. coreana has a large oxyasters and longer megascleres than J. wondoensis, oscule cluster and golden yellow colour in life.

  • Osteoma is defined as a benign neoplasm of bony tissue. Osteomas are divided into two groups: central and peripheral by origin, compact and cancellous by histopathologic findings. Solitary osteoma of jaw bone, not related with Gardner syndrom, is relatively uncommon. We present two cases of huge solitary peripheral osteomas: compact type exophyting osteoma on the inferolingual side of mandibular angle and cancellous type on the residual ridge of posterior maxilla, which is extremely rare.

Structural analysis of trabecular bone using Automatic Segmentation in micro-CT images (마이크로 CT 영상에서 자동 분할을 이용한 해면뼈의 형태학적 분석)

  • This paper proposes an automatic segmentation method of cortical bone and trabecular bone and describes an implementation of structural analysis method of trabecular bone in micro-CT images. The proposed segmentation method extract bone region with binarization using a threshold value. Next, it finds adjacent contour lines from outer boundary line into inward direction and sets candidate regions of cortical bone. Next it remove cortical bone region by finding the candidate cortical region of which the average pixel value is maximum. We implemented the method which computes four structural indicators BV/TV, Tb.Th, Tb.Sp, Tb.N by using VTK(Visualization ToolKit) and sphere fitting algorithm. We applied the implemented method to twenty proximal femur of mouses and compared with the manual segmentation method. Experimental result shows that the average error rates between the proposed segmentation method and the manual segmentation method are less than 3% for the four structural indicatiors. This result means that the proposed method can be used instead of the combersome and time consuming manual segmentation method.

Regional Morphological and Mechanical Characteristics in the Human Thoracic Vertebral Trabecular Bones (인체 흉추 해면골의 영역별 형태학적 및 기계적 특성 연구)

  • This study analyzed the regional morphological and mechanical characteristics of vertebrae by using micro-computed tomography (micro-CT) and micro finite element analysis (FEA). For the present study, the $12^{th}$ human thoracic vertebral bones (an 85-years female and a 48-years male) were used. These were scanned by using micro-CT. Structural parameters were evaluated from the acquired 20 image data for fifteen $4{\times}4mm^2$ regions (five regions in respective layers of superior, middle and inferior part) in the thoracic vertebral trabecular bones. $4{\times}4{\times}4mm^3$ cubic finite element models of each regions were created at $70{\mu}m$ voxel resolution to investigate effective modulus ($E^+$). The present study indicated that there were significant differences in morphological and elastic mechanical characteristics of each region. There are close relationship between effective modulus and structural model index (SMI) in the bone of the 48-years male and between effective modulus and bone volume fraction (BV/TV) in the bone of the 85-years female. In addition, the effective modulus of central regions is about 80% stiffer than that of lateral regions at transverse plane. These findings may be likely to explain the previous result that a change of loading distribution of the vertebral trabecular bones is caused by spinal curvature and nucleus pulpous degeneration of the intervertebral disc.

THE EFFECTS OF DIETARY CONSISTENCY ON THE TRABECULAR BONE ARCHITECTURE IN GROWING MOUSE MANDIBULAR CONDYLE : A STUDY USING MICRO-CONFUTED TOMOGRAPHY (성장 중인 쥐에서 음식물의 경도가 하악 과두의 해면골에 미치는 영향 : 미세전산화 단층촬영을 이용한 연구)

  • The development and proliferation of the mandibular condyle can be altered by changes in the biomechanical environment of the temporomandibular joint. The biomechanical loads were varied by feeding diets of different consistencies. The purpose of the present study was to determine whether changes of masticatory forces by feeding a soft diet can alter the trabecular bone morphology of the growing mouse mandibular condyle, by means of micro-computed tomography. Thirty-six female, 21 days old, C57BL/6 mice were randomly divided into two groups. Mice in the hard-diet control group were fed standard hard rodent pellets for 8 weeks. The soft-diet group mice were given soft ground diets for 8 weeks and their lower incisors were shortened by cutting with a wire cutter twice a week to reduce incision. After 8 weeks all animals were killed after they were weighed. Following sacrifice, the right mandibular condyle was removed. High spatial resolution tomography was done with a Skyscan Micro-CT 1072. Cross-sections were scanned and three-dimensional images were reconstructed from 2D sections. Morphometric and nonmetric parameters such as bone volume(BV), bone surface(BS), total volume(TV), bone volume fraction(BV/TV), surface to volume ratio(BS/BV), trabecular thickness(Tb. Th.), structure model index(SMI) and degree of anisotropy(DA) were directly determined by means of the software package at the micro-CT system. From directly determined indices the trabecular number(Tb. N.) and trabecular separation(Tb. Sp.) were calculated according to parallel plate model of Parfitt et al.. After micro-tomographic imaging, the samples were decalcified, dehydrated, embedded and sectioned for histological observation. The results were as follow: 1. The bone volume fraction, trabecular thickness(Tb. Th.) and trabecular number(Tb. N.) were significantly decreased in the soft-diet group compared with that of the control group (p<0.05). 2. The trabecular separation(Tb. Sp.) was significantly increased in the soft-diet group(p<0.05). 3. There was no significant differences in the surface to volume ratio(BS/BV), structure model index(SMI) and degree of anisotropy(DA) between the soft-diet group and hard-diet control group (p>0.05). 4. Histological sections showed that the thickness of the proliferative layer and total cartilage thickness were significantly reduced in the soft-diet group.

Automatic Segmentation of Trabecular Bone Based on Sphere Fitting for Micro-CT Bone Analysis (마이크로-CT 뼈 영상 분석을 위한 구 정합 기반 해면뼈의 자동 분할)

  • In this study, a new method that automatically segments trabecular bone for its morphological analysis using micro-computed tomography imaging was proposed. In the proposed method, the bone region was extracted using a threshold value, and the outer boundary of the bone was detected. The sphere of maximum size with the corresponding voxel as the center was obtained by applying the sphere-fitting method to each voxel of the bone region. If this sphere includes the outer boundary of the bone, the voxels included in the sphere are classified as cortical bone; otherwise, they are classified as trabecular bone. The proposed method was applied to images of the distal femurs of 15 mice, and comparative experiments, with results manually divided by a person, were performed. Four morphological parameters-BV/TV, Tb.Th, Tb.Sp, and Tb.N-for the segmented trabecular bone were measured. The results were compared by regression analysis and the Bland-Altman method; BV/TV, Tb.Th, Tb.Sp, and Tb.N were all in the credible range. In addition, not only can the sphere-fitting method be simply implemented, but trabecular bone can also be divided precisely by using the three-dimensional information.

Analysis of thermal changes in bone by various insertion torques with different implant designs (서로 다른 형태의 임플란트의 식립토크가 골에 미치는 열변화에 관한 연구)

  • Purpose: This study aims at investigating the influence of various insertion torques on thermal changes of bone. A proper insertion torque is derived based on the thermal analysis with two different implant designs. Materials and methods: For implant materials, bovine scapula bone of 15 - 20 mm thickness was cut into 35 mm by 40 - 50 mm pieces. Of these, the pieces having 2 - 3 mm thickness cortical bone were used as samples. Then, the half of the sample was immersed in a bath of $36.5^{\circ}C$ and the other half was exposed to ambient temperature of $25^{\circ}C$, so that the inner and surface temperatures reached $36.5^{\circ}C$ and $28^{\circ}C$, respectively. Two types of implants ($4.5{\times}10\;mm$ Br${\aa}$nemark type, $4.8{\times}10\;mm$ Microthread type) were inserted into bovine scapula bone and the temperature was measured by a thermocouple at 0.2 mm from the measuring point. Finite element method (FEM) was used to analyze the thermal changes at contacting surface assuming that the sample is a cube of $4\;cm{\times}4\;cm{\times}2\;cm$ and a layer up to 2 mm from the top is cortical bone and below is a cancellous bone. Boundary conditions were set on the basis of the shape of cavity after implants. SolidWorks was used as a CAD program with the help of Abaqus 6.9-1. Results: In the in-vitro experiment, the Microhead type implant gives a higher maximum temperature than that of the Br${\aa}$nemark type, which is attributed to high frictional heat that is associated with the implant shape. In both types, an Eriksson threshold was observed at torques of 50 Ncm (Br${\aa}$nemark) and 35 Ncm (Microthread type), respectively. Based on these findings, the Microthread type implant is more affected by insertion torques. Conclusion: This study demonstrate that a proper choice of insertion torque is important when using a specific type of implant. In particular, for the Microthread type implant, possible bone damage may be expected as a result of frictional heat, which compensates for initial high success rate of fixation. Therefore, the insertion torque should be adjusted for each implant design. Furthermore, the operation skills should be carefully chosen for each implant type and insertion torque.

  • Granulocytic sarcoma is a manifestation of myelogenous leukemia, which means a solid mass consisting of primitive precursors of the granulocytic series of white blood cells. We present CT and MR imaging findings of bilateral sino-orbital granulocytic sarcoma in a 22-month-old boy. The mass involved bilateral orbital fossa which resulted in bilateral proptosis. Moreover, the mass extended to the almost skull base including paranasal sinuses, maxilla, temporal bone, zygomatic bone, sphenoid bone, ethmoid, and palatine bone. The adjacent dura was continuously thickened and the lower half of cavernous sinus was also involved. The patient was diagnosed as AML (M5) with t(8,21) translocation through a chromosome study from the bone marrow.

Effect of Vibration on Trabecualr Bone of OVX Rats (기계적 자극이 난소 제거한 쥐 해면골에 미치는 영향 분석)

  • Some researchers proposed positive effects of whole body vibration (WBV) on osteoporotic trabecular bones of animals. In the present study, the correlation between the improvement of morphological characteristics and the effect of WBV was investigated and analyzed in OVX rats. The rats were randomly assigned to 5 groups: control, sham, WBV 17Hz, WBV 30Hz and WBV 45Hz. The WBV groups were exercised on a vibration platform (magnitude : $1mm_{peak-peak}$, frequency : 17Hz, 30Hz and 45Hz, 30 minutes/day for 5 days/week). The 4th lumbar on rats was scanned by In-vivo Micro-CT at the week 0(just before WBV) and the week 8(after WBV). Structural parameters of the 4th lumbar, based on two dimensional (2D) scan image data, were investigated and analyzed. The quantitative decrement rate of trabecular bone on WBV groups with 30Hz and 45Hz were lower than control and sham groups. The results showed the positive effect of WBV on osteoporotic bones of OVX rats.
