The Study on Antithrombosis and Inflammation according to The Broth Preparation Method of Gamijoukyungtang (가미조경탕(加味調經湯)의 전탕(煎湯) 방법에 따른 항혈전 및 염증에 관한 연구)

  • Purpose: In this study, we investigated the anti-thrombotic and antiinflammatory efficacy of "Gamijoukyungtang(GJKT)". Methods: We studied inhibitory effects of platelet aggregation, FXa activation, $TXB_2$ and $PGE_2$ biosynthesis and suppressive effects of GPIIb/IIIa activity and oxidative damage, pro-inflammatory cytokine reduction effects of 'GJKT(by press extractor)/GJKT-1(by pressless extractor)' in vitro. Also, we studied suppression of pulmonary embolism, AV shunt model in rats and shortening of Rat tail bleeding time in vivo. Results: GJKT/GJKT-1 extract showed inhibitory effects on GPIIb/IIIaactivities and platelet aggregation induced by ADP, epinephrine, collagen and arachidonic acid. They suppressed biosynthesis of $PGE_2$ but GJKT-1 only supressed biosynthesis of $TXB_2$. In FXa assay, they inhibited activation of FXa. they suppressed pulmonary embolism triggered by collagen and epinephrine. In AV shunt model, they decreased the weights of AV shunt thrombus. they inhibited pro-inflammatory cytokines and decreased oxidative damages caused by DPPH. Conclusion: We confirmed the anti-thrombosis, and ant-inflammatory efficacy of 'GJKT(by press extractor)/GJKT-1(by pressless extractor)'.

  • Pycnogenol is used as a dietary supplement based on various medical research studies suggesting health benefits. In recent years, it has also been used in cosmetic products, but the clinical research on its dermatologic effects has been insufficient. This study was designed to determine the effectiveness of pycnogenol topically applied to skin of individuals with acne in their teens and twenties. Cosmetics containing 0.2% pycnogenol were applied to a group of 11 clinical subjects for 6 weeks and their skin conditions were assessed. The group that used cosmetics with pycnogenol showed decrease in P-Acnes, acne causing bacteria, phorphyrin index, a metabolite, and the pigmentation and redness index. This study confirms that pycnogenol extract is effective dermatologically in decreasing acne bacteria and reducing redness and pigmentation of skin affected by acne.

Current Status and Application Prospects of Anti-Atherosclerotic Active Biomaterials (항동맥경화 활성 바이오소재 개발 연구 동향 및 활용 전망)

  • Atherosclerosis, a disease with high morbidity and mortality worldwide, is a chronic inflammatory disease that is a major cause of cardiovascular diseases such as stroke and myocardial infarction. Atherosclerosis is characterized by the accumulation of lipid deposits in the arteries, forming atheromas. This leads to the narrowing of the arteries and thrombosis. Recently, the need to develop bio-derived anti-atherosclerotic materials has been highlighted with concerns about the side effects of synthetic therapeutics. Accordingly, related research (such as the discovery of biomaterials for the improvement and treatment of atherosclerosis and the identification of mechanisms) has been actively conducted. Biomaterials including polysaccharides, polyphenols, and coenzyme Q10 have been reported to inhibit or delay symptoms by modulating factors involved in the development of atherosclerosis. For biomaterials with superior activity, in vivo anti-atherosclerotic activity has been confirmed. In this review, the pathogenesis of atherosclerosis was investigated, and the current status and application prospects of biomaterials with anti-atherosclerotic activity were proposed.

  • 작약의 메타놀에기스를 작용성분을 추적하면서 분리하였으며 EtOAc 분획으로부터 gallic acid methylester(GA-1) 및 paeonoldmf 혈소판 응집억제 작용 물질로서 분리하였으며 Paeonol은 혈소판 응집억제 작용이 이비 보고된바 있다. GA-1은 오가피로부터 혈소판 응집억제 작용물질로서 분리된 DBA-1과 구조적으로 매우 유사하며 혈소판 응집억제 작용물질로서 알려진 aspirin의 구조와 유사하다. Platelet aggregometer을 이용하여 DBA 및 GA analogs 13종(DBA-1-10 및 GA-1-3)들이 ADP 또는 collagen에 의하여 유도된 혈소판 응집에 대하여 억제작용을 보이는가를 검색하였다. 이들중 aspirin과 유사하게 억제작용을 보인 DBA-1-3-7-9-10, GA-3 및 paeonol에 대하여 mouse를 이용한 in vivotlfgja을 시행하였다. 즉 endotoxin 또는 collagen 과 epinephrine을 정맥주사하여 intravascular thrombosis를 일으켜 혈소판수를 감소시키며 이때 이들 물질들의 혈소판 감소 억제효과를 관찰하였다. 대조약물로서 사용한 aspirin은 collagen과 epinephrine에 의한 혈소판 감소를 현저히 억제한 반면 endotoxin에 의한 혈소판 감소에는 효과가 없었다. DBA-9, -10, GA-3 및 paeonol은 endotoxin에 의한 혈소판감소에 대하여 약한 억제효과를 보였으며, 검색시료 전부가 collagen과 epinephrine에 의한 혈소판 감소에는 aspirin보다 작용이 적었으나, DBA-10, GA-3 및 paeonol은 현저하게 억제효과 있었다. 또한 collagen과 epinephrine 동시 투여에 의한 치사 실험에서는 DBAD-7, GA-3 및 paeonol이 aspirin과 같거나 강한 사망 억제 효과가 있었다.cyclopropyl-7-(2-furanyl) -6-fluoro-1,4-dihydro-4-oxo-3-quinoline carboxylic acid (compound 4), 1-cyclopropyl-7-(2-thiophenyl)-6-fluoro-1,4-dihydro-4-oxo-3-quinoline carboxylic acid (compound 6) ,1-cyclopropyl-7-(3-pyridinyl)-6-fluoro-1,4-dihydro-4-oxo-3-quinoline carboxylic acid (compound 8), 1-cyclopropyl-7-(2-fluoro-3-pyridinyl)-6-fluoro-1,4-dihydro-4-oxo-3-quinoline carboxylic acid (compound 10)를 합성하였다.10^{-7}$ M)에 의한 단백인산화에 대하여는 더 미약한 억제-효과를 나타내었다. 이상의 결과는 PDE-1과 항우울약들의 항혈소판작용은 PKC-기질인 41-43 kD와 20 kD의 인산화를 억제함에 기인되는 것으로 사료된다.다. 것으로 사료된다.다.바와 같이 MCl에서 작은 Dv 값을 갖는데, 이것은 CdCl$_{4}$$^{2-}$ 착이온을 형성하거나 ZnCl$_{4}$$^{2-}$ , ZnCl$_{3}$$^{-}$같은 이온과 MgCl$^{+}$, MgCl$_{2}$같은 이온종을 형성하기 때문인것 같다. 한편 어떠한 용리액에서던지 NH$_{4}$$^{+}$의 경우 Dv값이 제일 작았다. 바. 본 연구의 목적중의 하나인 인체유해 중금속이온인 Hg(II), Cd(II)등이 NaCl같은 염화물이 함유된 시료용액에 공해이온으로 존재할 경우 흡착에 의한 제거가

Solid Fermentation of Medicinal Herb Using Phellinus baumii Mycelium and Anti-thrombin and Anti-oxidation Activity of its Methanol Extract (장수상황버섯 균사체를 이용한 한약재의 고체발효 및 메탄올 추출물의 트롬빈 저해 활성과 항산화 활성)

  • To produce bioactivity-strengthen medicinal herbs, the 36 medicinal herbs which have antioxidation or blood circulation activity, were solid fermented using Phellinus baumii mycelium. Most of medicinal herbs, except Chrysanthemum indicum (flower), Zizyphus jujuba Miller (fructus), Aconitum koreanum R. Raymond (root), Magnolia denu-data (flower), and Polygonatum sibiricum Redt (root bark), showed good fermentation at $25^{\circ}C$ for 20 days under 90% of relative humidity. The poor fermentations of the herbs could be explained by lack of nutrient, structural rigidity, and the content of antifungal substance. After fermentation, the average water content of herbs were increased to $67.21{\pm}11.43%$ from $30.84{\pm}15.67%$, but the average pH and average methanol extraction ratio were slightly decreased to $11.16{\pm}7.06%$ and $4.83{\pm}0.73$ from $13.91{\pm}12.22%$ and $5.06{\pm}0.87$, respectively. The analysis of thrombin inhibition and DPPH scavenging activity of the methanol extracts of herbs showed that thrombin inhibition activities of the fermented Drynaria fortunei Kunze, Melia azedarach var. japonica, Prunus persica and Orostachys japonicus, and DPPH scavenging activities of the fermented Polygala tenuifolia, Scrophularia buergeriana, Angelica dahurica, Drynariafortunei Kunze, Cyperus rotundus, and Boschniakia rossica were increased as compared with those activities of non-fermented its cognate herbs. Our results suggest that the production of bioactivity-strengthen medicinal herbs is possible by solid fermentation of Phellinus baumii mycelium, as fermented Drynaria fortunei Kunze showed increased antioxidant and thrombin inhibitory activities than those of non-fermented herbs.

Clinical significance of follow-up laboratory tests, performed at 6 weeks after the onset of Kawasaki disease (가와사끼병에서 발병 6주째에 시행하는 추적검사의 임상적 유용성에 관한 고찰)

  • Purpose : Low dose aspirin is used due to its antiplatelet effect for the subacute phase of Kawasaki disease(KD). It is usually used for 6-8 weeks, then various hematologic laboratory tests and follow up echocardiography for evaluating coronary abnormalities are performed. Our review investigated the usefulness of various follow up laboratory tests performed at 6 weeks after the onset of KD. Methods : Two hundred eighty-two children diagnosed and hospitalized with KD were identified by reviewing patient's charts. Cases which were diagnosed between January 1997 and December 2004 were included in this study. We reviewed laboratory data including leukocytes, platelet counts, antistreptolysin O(ASO), erythrocyte sedimentation rate(ESR), C-reactive protein(CRP), aspartate aminotransferase(AST), alanine aminotransferase(ALT), urinalysis, and echocardiograms performed at admission and 6 weeks after the onset of KD. Paired t-test and Fisher's exact test, as well as logistic regression tests, were used for the statistical analysis. Results : At 6th week data, ESR and CRP were still elevated in 35(12.4 percent) and 12(4.3 percent) patients, respectively. Sterile pyuria were all normalized. But, 36 patients(12.8 percent) showed thrombocytosis, 22(7.8 percent) elevated AST, 15(5.3 percent) leukocytosis, and 6(2.1 percent) coronary abnormalities. Coronary abnormalities at the 6th week were only shown in patients with initial abnormalites. Younger age and initial thrombocytosis were risk factors for thrombocytosis at the 6th week. Conclusion : All children with initial coronary abonormalites should have an echocardiogram at 6 weeks after the onset of fever. In view of case-effectiveness, additional echocardiographic studies are justified only if abnormalities are present at admission. ESR, CRP, and urinalysis performed at the 6 weeks after onset of KD is not significant for clinical information of progression. Platelet count should be estimated at 6th week for a judgement of continuous antiplatelet therapy.

Evaluation of Anti-oxidant, Anti-microbial and Anti-thrombosis Activities of Fruit, Seed and Pomace of Schizandra chinensis Baillon (오미자 열매, 씨, 착즙 후 박의 항산화, 항균 및 항혈전 활성 평가)

  • In this study, for the efficient use of the byproduct of the omija (Schizandra chinensis Baillon: SC) processing industry, the ethanol extracts of the fruit (F), seed (S), and pomace (P) of SC were prepared, and their useful bioactivities were evaluated. For F-SC, S-SC, and P-SC, the extraction yields were 28.3%, 22.1%, and 7.2%, respectively, and the polyphenol contents were 8.81, 37.22, and 9.20 mg/g, respectively. The total flavonoid content in P-SC (4.31 mg/g) was 3.5-fold higher than that in F-SC (0.76 mg/g). In an antioxidation activity assay, P-SC showed stronger radical scavenging activities against DPPH anion, ABTS cation, and nitrite and stronger reducing power activities than the other extracts. The calculated concentration required for 50% radical scavenging activity, $RC_{50}s$, of P-SC for DPPH anion, ABTS cation, and nitrite was 226.2, 192.5, and $92.5{\mu}g/ml$, respectively. In an antimicrobial activity assay, F-SC, S-SC, and P-SC showed similarly strong growth inhibitions against Bacillus subtilis and P. vulgaris at a concentration of 0.5 mg/disc. F-SC and P-SC showed 15-fold extended time in thrombin, prothrombin, and activated partial thromboplastin time assays at a concentration of 5 mg/ml. The anticoagulation activity of P-SC (2.5 mg/ml) was comparable to that of aspirin (1.5 mg/ml). Furthermore, F-SC and S-SC showed very good platelet aggregation inhibitory activities. F-SC, S-SC, and P-SC did not show significant hemolysis against human red blood cell up to a concentration of 0.5 mg/ml. These results suggest that S-SC and P-SC, both of which are byproducts of the omija processing industry, show strong potential as novel antioxidant, antimicrobial, and antithrombosis agents.

Evaluation of in-vitro Antithrombosis Activity of Lees of Korean Traditional Wine (전통주 주박의 항혈전 활성 평가)

  • In this study, ethanol and hot water extracts of lees from Korean traditional wine (J-B, J-S, J-Y, J-H, and J-W) were prepared, and their effects on blood coagulation, platelet aggregation, and hemolysis of human red blood cells (hRBCs) were investigated to develop functional food ingredients from lees. The pH and brix of the lees ranged from 3.90 to 4.29 and 5.0 to $27.0^{\circ}$, respectively, and there was a huge difference in the water and ethanol content among the lees. The nuruk and additives used affected the color and physicochemical properties of lees. The J-W takju made from only rice and traditional nuruk, which has $13^{\circ}$ brix and 1.8% of alcohol, has potential as functional food ingredient. With regard to the extraction yields of lees, higher yields were obtained from J-H, which contains different medicinal plants, in ethanol, followed by J-W, J-B, J-S, and J-Y. Higher extraction yields of lees were obtained from J-S in hot water, followed by J-B, J-W, J-H, and J-Y, respectively. The ethanol extract of J-H and the hot water extract of J-Y had the highest contents of total polyphenol and total flavonoids among the lees extracts. The 10 lees extracts did not show hemolysis activity against hRBCs up to 5 mg/ml. In an anticoagulation activity assay, the ethanol extracts of three yakju lees (J-B, J-S, and J-Y) and the hot water extract of J-W inhibited thrombin activity, whereas the hot water extract of J-B, J-S, and J-H inhibited blood coagulation factors. In an antiplatelet aggregation activity assay, only the J-W takju lees showed significant inhibition activity. Our results suggest that lees from traditional wine had high potential as a novel antithrombosis agent.

Antithrombotic Activity and Protective Effects of hexane fraction of Kamihyulbuchukeotang (KHBCT) on brain injury by KCN and MCA occlusion (가미혈부축어탕 Hexane층의 항혈전활성과 뇌손상 보호효과)

  • This study was performed to investigate the antithrombotic activity and protective effect of hexane fraction of Kamihyulbuchukeotang (KHCTH) on brain injury by KCN and MCA occlusion a prescription of HCT added with Lumbricus and Notoginseng Radix. Experiemental parameters are brain ischemia by MCA occlusion assay, KCN-induced brain injury, pulmonary thrombosis and platelet aggregation assay. The results were summarized as follows; 1. KHCTH extracts significantly inhibited the duration of KCN-induced coma (67%) and mortality (80%). 2. KHCTH extracts significantly suppressed brain ischemic area and edema following MCA occlusion and protected neuron cells as compared with control data. 3. KHCTH extracts inhibited pulmonary thrombosis induced by collagen and epinephrine. 4. KHCTH extracts inhibited platelet aggregation induced by collagen, ADP as agonist up to 76.9% and 32.3% respectivey at 1 mg/ml more effective than water extract of KHCT These data suggested that KHCTH could be applied as the protector of brain ischemia and injury and antithrombotic agent.

The Surgical Treatment of Atrial Fibrillation in Patients Undergoing Simultaneous Open Heart Surgery (심장세동의 수술요법)

  • .Itrial fibrillation is one of the most common cardiac arrhythmias requiring treatment. About 60% of patients with mitral valvular disease have atrial fibrillation and one third of patients with atrial fibrillation may have the past history of thromboembolic events. Between April 1994 and June 1995, 20 patients with organic heart diseases combined with atrial fibrillation underwent open heart surgery including Cox-maze 111 procedure. There were 6 men and 14 women with an average age of 48 years (range, 31 to 66 years). Nineteen patients had valvular heart diseases and 1 ventricular septal defEct (VSD). Mean duration of atrial fibrillation was 36 months (:42 months) (range, 1 to 132 months). T e past medical history of thromboembolic events was positive in 7 patients (35%) and left atrial thrombus was detected in 9 patients (45%). The concomitant procedures were mitral valve replacement (MVR) and aortic valve replacement (AVR) in 5 patients, MVR in 4, MVd and tricuspid annuloplasty(TAP) in 4, mitral valvuloplasty(Mln) in 3, Mln and Tln in 1, MIW and coronary artery bypass surgery in 1, AVR in 1, and patch closure of VSD in 1. Mean aortic cross-clamping time was 175 minutes (range, 116 to 270 minutes). Atrial fibrillation recurred in 16 patients (80%) during the early postoperative period, but, recurrent atrial fibrillation was converted to regular rhythm at postoperative forty-first day in average. There was no early or late death in this series of 20 patients and postoperative complications were inappropriate tachycardia in 5 patients (25%), low cardiac output syndrome in 3 (15%), aggravated hemiplegic in 1, and acute renal failure in 1. Mean follow-up interval of patient was 16.5 months (range, 10.5 to 24 months) and all patients are currently in regular rhythm. Seventeen patients (85%) are in sinus rhythm and 3 (15%) in junctional rhythm. Right atrial contraction was detected in 95% of patients and left atrial contraction in 63% on postoperative transthoracic echocardiogram. The surgical treatment of atrial fibrillation concomitant with open heart surgery is warranted in the recent clinical setting of improved myocardial protection technique, considering the untoward side-effects of atrial fibrillation.
