LOSA는 Line Operations Safety Audit(항공사 운항안전 감사)의 약자이며 기존의 적발 위주의 기존 Line Audit제도와 달리 조종사의 자발적 참여와 철저한 비밀을 유지하며, 처벌 금지 약속을 통하여 참여자가 평소 습관대로 비행할 수 있게 한다. 훈련된 감사관이 이를 소정의 절차서에 의거 조종석에서 관찰하여 실제의 안전취약 및 위협요소, Error를 포착해서 수집하고 텍사스대학 인적요인 연구소에서 분석하여 최종보고서를 작성하여 제도를 개선하는 안전프로그램이다. 제도와 방안을 개선하는 신개념의 선진 운항감사제도로서 안전관리시스템의 대표적인 비행안전 프로그램으로 통상 3${\sim}$4년을 주기로 실시한다. ICAO, IATA, FAA 및 IFALPA 실행 권고사항으로 현재 약30여개의 항공사가 실시하였다. LOSA는 2009년1월부터 ICAO부속서 6에 의거하여 항공사에서 실행해야 되는 SMS(안전관리 시스템)의 가장 효율적인 Hazard 식별 및 위험 관리도구 중의 하나이다. 본 논문에서는 안전관리시스템의 효과적 도구인 LOSA를 설명하고 항공사내 실행방법을 소개하는데 있다.

ICAO(International Civil Aviation Organization) Doc 9803의 국제규정에 의거하여 SMS(Safety Management System-안전관리시스템)의 대표적인 비행안전 모니터링 프로그램으로 인정된 신개념의 운항감사제도인 LOSA(Line Operations Safety Audit-항공운항 안전감사)를 항공사에서 실시하여 정상 운항시의 운항승무원을 관찰하여 실제의 안전취약 및 위협요소, Error를 포착하여 텍사스대학 인적요인 연구소에서 작성한 최종보고서의 분석을 통해 제도와 방안을 개선한다. 본 논문에서는 LOSA실행을 통한 승무원들의 위협 및 error의 유형과 발생율, 관리율 및 관리여부를 분석하여 설명하고 항공사내 개선인 SCP(Safety Change Process)를 소개하는데 있다.

항공 사고는 첨단 항공 기술의 발달로 꾸준히 감소해 왔지만, 여전히 발생하고 있다. 조종사는 목적지 공항까지 안전하고 효율적으로 비행하기 위해 위협과 오류를 관리해야 한다. 조종사에게 영향을 미치는 위협은 환경적 위협과 항공사 위협으로 나눌 수 있으며, 환경사위협에는 객실 위협이 포함된다. 조종사와 객실 승무원은 정상적인 운항 중에는 협력이 필요하며, 비상시에는 안전한 비행을 위해 효과적인 협력이 절대적으로 필요하다. 조종사가 비행 중에 간섭이나 작업 흐름의 중단이 발생할 때 에러를 할 가능성이 높아진다. 항공사들은 안전 운항을 방해할 수 있는 객실위협의 종류를 적극적으로 식별하고 분석하여, 조종사들이 안전한 비행을 유지하기 위해서 위협을 관리하도록 절차 및 훈련을 개선해야 한다. 본 연구는 안전관리시스템(SMS)의 안전도구인 LOSA(Line Operation Safety Audit)를 기반으로 기내위협의 유형을 파악하고, 항공사들에게 조종실위협 관리를 체계적으로 개선하는 데 필요한 객실위협과 간섭에 대한 자료를 제공하고자 한다.

LOSA is spreading rapidly over the world to improve safety of the airlines using threat and error management system. LOSA has shown its success on the major improvement on many areas including the riskiest phase called the blue box, which is the busiest during approach and landing. Asiana Airlines has completed data collection in 2004 and round table for the data cleaning, to implement safety change process. The propose of this paper is to introduce the LOSA, ICAO DOC 9803, and the experience of Asiana.

The purpose of this study is to ensure safety by proactively identifying hazards that could be derived from changes in mission form and environment as the advanced aircraft such as F-35A stealth fighter, KC-330 Multi-role transport and tanker, RQ-4B high altitude unmanned reconnaissance aircraft, etc are introduced that the Republic of Korea Air Force(ROKAF) has never been operated so far. To this end, the safety management methods based on proactive and predictive approaches used in advanced countries(US Air Force, UK Royal Air Force, Royal Australian Air Force) operating aircraft types same or similar things being newly powered by the ROKAF were reviewed. In addition, the direction for improvement of the safety management methods operating in the ROKAF and the measures necessary for establishment of the new safety management techniques to be applied were suggested.

Line Operations Safety Audit (LOSA) is a well known preventive aviation safety program for Threat and Error management (TEM). High quality LOSA data suitable for safety management is obtained when a flight crew flies at the same level of attention as ordinary flight. Factors contributing to LOSA data quality may include flight crew's understanding on LOSA purpose, observer's career, and characteristics of the organization responsible for LOSA operations. This study explored purposes of TEM and LOSA, as well as their relationship. Previous studies mentioned quality of LOSA data can be influenced by heuristic judgment, hawthorne effect, and priming effect. This study recognized the importance of LOSA data quality to be effectively used for preventive safety management. It was confirmed that the level of understanding on LOSA concept, experience of the observer, and the characteristics of the department in charge of LOSA operation could affect the quality of LOSA data.
