The analysis on the use experience of other medical institution for the same symptoms and the frequent diseases of outpatients of Korean medical institution: Based on the 2017 Korean Medicine Utilization and Herbal Medicine Consumption Survey (한의 외래서비스 이용자의 동일 증상에 대한 타 의료기관 이용 경험과 다빈도 질환 분석: 2017년 한방의료이용 실태조사를 이용하여)

  • Objectives : The aim of this study is to investigate the use experience of other medical institution of outpatients of Korean medical institution. Methods : Cross-sectional study using the 2017 Korean Medicine Utilization and Herbal Medicine Consumption Survey was conducted. Demographic variables and frequent diseases were analyzed according to the experience of using other medical institutions. The proportions of experience of using other medical institutions of patients with frequent diseases of Korean medical outpatient were analyzed. Results : Fifty-three percent of outpatient had experiencing using other medical institutions for the same symptoms. The frequent diseases of the two groups were similar. The proportion of single use of Korean medicine for injury of lumbar and pelvic, rhinitis, and menstrual disorders were relatively high. Conclusions : There was no notable difference in the frequent diseases according to use experience of other medical institution. Further studies on this topic are needed.

Market Segmentation of Patient-Utilization in Oriental Medical Care and Western Medical Care (양.한방 의료서비스 이용환자의 시장 세분화에 관한 연구)

  • The objectives of this study were analysis of patient\`s characteristics and market segmentation in oriental medical care and western medical care. This study focused on medical utilization using Anderson's health utilization model. The source of data was 1998 National Health and Nutrition Survey which Korean Institute For Health and Social Affairs carried out. A stratified multistage probability sampling design was used in this survey. The analysis was conducted using the statistical software package SPSS version 10.0 and Answer Tree 2.1 which is one of data mining methodology. The results were as follows ; 1) 44.9% of respondents reported visiting oriental medical center within recent two weeks. 3.4% of them used oriental medical care. The group of age, kind of disease and medical expenditure are associated with the difference western and oriental medical utilization rate. 2) There were several factors related to utilization of oriental medical care according to decision tree. Especially, important factors that patient chose his medical center were kinds of disease, kinds of common medical use, and expenditure. 3) in the results of CART analysis, market of oriental medical care were classified by seven categories. The major groups who have a preference for oriental medicine were those musculo-skeletal, cerebra-vascular disease, or chronic headache patients, and they had a preference fur oriental medical care in common use. These results show that oriental and western medical market were divided into various areas by market segmentation.

Study on the Selection Factors of Korean and Western Medical Institutions (양한방 의료 서비스 선택요인에 관한 연구)

  • Korean medical system is unique system that has two medical system, Western medical institution and Korean medical institution. In this environment, patients who use each medical institution have different selection factors. The study explores the selection factors' difference of korean/western medical institutions. The empirical analysis of the surveyed data produced the following outcomes. The result of factor analysis, four factors were extracted. That was Human services, Costs and Physical Environment, Prestige and reputation, Public relations and reliability. Overall, the four selection factor importance scores of Korean medical institution were higher than western medical institution's. These findings show that there is a difference between korean and western medical institutions in terms of selection factors. After this study, More study about medical management and healthcare policy including korean medical characteristics is needed.

Patterns of Korean Traditional medical Services Utilization in Daegu (대구지역의 한방 의료서비스 이용 실태)

  • Objectives : Recently, the number of elderly people has been rapidly increasing in Korea, and it led to increase demands of Korean traditional medical services. However, there is a lack of information on Korean traditional service utilization. Thus, this study was intended to identify Korean traditional service utilization patterns such as acupuncture, moxibustion and cupping, since Korean traditional services were specially used in treating chronic diseases for elderly people. It is important to understand health services utilization for quality improvement of healthcare systems. Methods:We used data of 'Survey in Daegu' in 2008 and analyzed characteristics of disease structure and utilization patterns of health care service of 1,097 cases of participated citizens. Results : The proportion of the elderly people (>50 age) in total participators is 52.4%. Male to female ratio is 1:1.65 and most common age group was between 50 and 59 years old. 84.2% of participators were admitted at oriental health care facilities. In addition, the results showed that female were more used the service than man. The most common treated disease is musculoskeletal disorders in both male and female. Conclusions : Acupuncture, moxa and cupping are widely used to treat chronic degenerative diseases such as musculoskeletal disorders at oriental health care facilities. Elders and female prefer to be admitted at oriental health care facilities.

A Study on Relationship between Health Services Quality and Customer's Satisfaction in Oriental Medicine Hospitals (한방 의료 서비스 품질이 환자 만족 및 재이용 의도에 미치는 영향)

  • Purpose : This study was conducted to understand Oriental medical health services and analyze the relationships between the perceived Oriental medical health services and the effect, influencing patient satisfaction as. well as intentions of re-visiting such hospitals. Methods : SPSS 10.0, a statistical processing method, was used to process the study data and frequency and ANOVA analysis were used to analyze the differences. Also, Pearson correlation coefficient was performed to analyze the relationships between work satisfaction and health services quality, as well as patient satisfaction and intentions to re-visit such hospitals. Results : Most patients were satisfied with the service, but it was shown that improvement of facilities and medical equipment was needed. The survey showed higher satisfaction with longer experience of doctors and nurses. Staff recognition of health services quality seemed lower than patient satisfaction, so efforts to improve the health services and relationships between health services quality and patient satisfaction are necessary. Conclusion : Higher quality and satisfaction with Oriental medical health services are going to improve total re-use intention to a significant degree.

Determining Factors for the Use of Oriental Healthcare Services for Survey Subjects with Chronic Illnesses : 2005 National Health and Nutrition Examination Survey (만성질환자의 한방의료서비스 이용 결정요인 : 2005년도 국민건강영양조사)

  • Objectives : The aim of this study is to investigate the relevant factors which determine the use of oriental healthcare services among subjects with chronic illnesses. Method : This study utilized the data from the Korean National Health and Nutrition Examination Survey in 2005. Out of all the participants of the survey, 11,665 individuals who are older than 19 years old and have one or more chronic diseases were included in this study. Results : The factors that affect utilization of oriental healthcare services were significantly associated with gender, educational level, job, personal income, the number of chronic illnesses, experiences of nontreatment or delayed treatment and admission experiences for the last one year(p<.001). Although some of these factors need further studies, the determining factors for the use of oriental healthcare services are the presence of chronic illness and the number of chronic disease. That is, the chronically ill are more likely to seek oriental healthcare services. And the more chronic diseases the clients have the higher probability of seeking oriental healthcare services was found. Conclusions : The results suggest that the national integrated care services should be established for diverse development of policy regarding the quality of care of chronic illness and cost-effectiveness.

  • Objective : This study was designed to health-Care services recognition comparison of patient between Korean Medicine 00 Hospital and Seoul 00 Health Center. Methods : 277 subjects were participated in this study. After a treatment, we analyzed a patient type by using a questionnaire. After all treatment, we conducted a survey about Medical Trust Scale, Medical Happiness Index Scale, Service Quality Scale. To evaluate a satisfaction degree, we analyzed results of survey statistically. Results : The results of the analysis, Korean Medicine 00 Hospital group got a higher score than Seoul 00 Health Center group statistical significantly on Medical Trust Scale, Medical Happiness Index Scale, Service Quality Scale. Conclusion : Korean Medicine 00 Hospital group got a higher score than Seoul 00 Health Center group. It was more effective in Korean Medicine 00 Hospital group than Seoul 00 Health Center group.

Systematic Review on the Customers' Use of and Satisfaction with Oriental Medical Services (한방의료서비스 이용과 만족도의 영향요인에 대한 계통적 고찰)

  • Objectives: This study aimed to find determinants of customers' use of and satisfaction with oriental medical services in Korea. Methods: A total of 33 articles which consist of 9 articles published in academic journals, 20 master's theses, and 4 doctoral dissertations were included in the systematic review. Among the articles used in the study, 29 used primary survey data, while 4 used secondary data produced by the Korean National Health and Nutrition Evaluation Survey. Results: First, it was found that people who prefer to use oriental medical services usually have one or more of the following diseases: musculoskeletal disorders, paralysis, or diseases or injuries without clear diagnosis. Other variables of gender, age, expectation of treatment outcome, and kindness of doctor and other staff were found to be significant determinants of using the oriental medical services. Second, the determinants of customers' satisfaction with oriental medical services were found to be age, gender, level of education, chronic disease with long length of stay, kindness of staff, medical cost, clinical environment, doctor's reputation, and public image of the institutions. Conclusion: The results of this study can be used to develop marketing strategies for oriental medical institutions in Korea.

Choice of Health Care and Traditional Medicine (양.한방의료 서비스 선택에 관한 연구)

  • This study is to investigate patient's choice of health care and the demand for Korean traditional medicine care in rural areas in 1995. It tried to evaluate the effect of out-of-pocket expenditure, travel time, and waiting time on improving care-seeking and substituting clinical medicine for pharmacy care and Korean traditional medicine care in rural areas. The statistical model of this study is conditional logit to estimate effects of choice-specific and individual-specific characteristics on the choice of type of services. This study used, as explanatory variables, average out-of-pocket payment, travel time, and waiting time of services required to use the services. The model was empirically tested using data from 1995 Korean National Health Survery. The results showed that rural Koreans responded to out-of pocket payment and travel time. Increases of out-of-pocket payment and travel time decreased the probability to choose care in rural Korea. Rural Koreans were more likely to seek care than others with low out-of-pocket payment and travel time. The probability of choosing Korean traditional medicine were higher among the members of the households with higher education level and older persons, while they were lower in the households with large family than others compared with the probabilities of choosing public health facilities. The result of this study implies that policy on use of health care in rural Korea can be focused in managing travel time and out-of-pocket payment.

A Study on Oriental Medical Fee Demanded by Supplier of Medical Services (의료서비스 제공자가 기대하는 한방 시술 수가수준에 대한 연구)

  • Objectives : The Purposes of this study were to investigate payment system for oriental medical treatment as supplier of medical services, and to estimate reasonable levels of medical fee. Methods : This study made these following results by reviewing the answers which were given by 172 Korea traditional doctors from March 1 to April 15, 2006. Results : General satisfaction of payment system for oriental health insurance was $2.17{\pm}1.01$ $(mean{\pm}SD)$ on a 1-7 scale (median 4) as very low level. Reasonable medical fees which were answered by 172 Korea traditional doctor are higher than present fees, thus Korea traditional doctors think that present fees should be increased. And according to the survey, current insurance fees have a problem of disparity between each treatment fee. Conclusions : According to results of this study, current fees of oriental medical treatment are not rational. And this problem leads to distortion of medical treatment. Additional studies in thls field are needed.
