A clinical Case Report of Spastic Patient After Stroke Treated with Gagamyounjo-tang (뇌경색 후유증으로 발생한 주관절 경직 환자의 가감윤조탕(加減潤燥湯) 치험1례)

  • Many patients have difficulties after stroke. Spasticity is an important one of that difficulties. So in these days, its management is becoming a major issue in rehabilitation. Gagamyounjo-tang was administered to a patient have had spasticity after stroke include Hyulhu(血虛) symptoms. And check the modified Ashworth Scale, Motor grade and VAS for being in the hospital. After treated with Gagamyounjo-tang, Modified Ashworth Scale, Motor grade and VAS have improved, and most of all symptoms about hyulhu(血虛) was disappeared. Based on this result, this study shows that Gagamyounjo-tang was effective in spasticity after stroke especially included Hyulhu(血虛) symptoms.

  • 인터넷의 생활화와 컴퓨터의 대중화로 인해 의료 분야에서도 웹 환경에서의 지식기반 시스템의 필요성이 증대되고 있으며, 의료정보 자원과 지식기반의 응용시스템이 점차 늘어나고 있다. 또한, 지식기반 시스템에서 의료정보를 의미적 처리하기 위한 의료분야의 온톨로지 개발도 많이 시도되고 있다. 한방 온톨로지는 질병, 증상 등 정보를 체계적이고 논리적으로 표현함으로써 의료정보의 효과적 제공 및 의료지원이 가능한 지식 자원이다. 한방 온톨로지 기반의 지능화된 의료 서비스는 한방분야의 질병 및 증상 정보에 정확성을 부여하고, 체계적이고 질적으로 향상된 데이터를 제공할 수 있다. 또한, 환자의 증상에 따른 연관 증상을 제공하여 지능적인 문진이 이루어질 수 있도록 문진시스템에 적용이 가능하다. 본 논문에서는 한방 의료 분야의 질 높은 의료지원 서비스 제공을 위한 한방 온톨로지 설계 및 구현 방법을 제시하고, 구축한 한방 온톨로지를 소개한다.

Effects of Price Discount on the Demand and Revenue of Oriental Medical Clinic (한방의료의 가격인하전략이 한방의료에 대한 수요와 경영수지에 미치는 영향)

  • We have seen a sharp increase in the utilization rate of medical services since the launch of Nation Health Insurance system in 1989. However, the market share of oriental medicine in the Korean health sector has been gradually diminished mainly due to low insurance coverage and high price. Especially high price as an entry barrier to oriental medical services has played a major role in decreasing market share. This paper investigated the effects of price discount on the financial condition and market share of oriental medical institutions. Microeconomic theory and multiple regression analysis were used as a methodology in testing the alternative hypothesis: price discount of enveloped herb drug will result in increases in both the revenue and market share of oriental medical institutions. Data was collected from the statistical yearbook and sample survey. The price elasticity of demand for enveloped herb drug was estimated at 5.8 during 1987-1995 period, which means that a 1% decrease in price will bring about a 5.8% increase in the utilization of oriental medical services. The empirical result shows that a drastic price discount for the enveloped herb drug will eventually improve the financial status of the oriental medical institutions and enlarge the market share of oriental medicine in the Korean health care sector.

  • This paper is proposed that ontology_based Oriental Medicine Customer Relationship Management system is designed with existing OCS, EMR, ERP system for oriental medicine center of efficient customer information management and analysis. As using ontology technique, we support oriental medicine service at grade according to quality and healthy of the customer. Also we designed 4 kind oriental medicine customer relationship management system frame - customer management, public relations/marketing, service management, statistics/analysis - in the customer management process standardization through a medical expert system.

Legal Issues on Pharmacopunture (약침의 한방의료행위성에 대한 검토)

  • Pharmacopunture is a new combined method of acupunture and oriental drugs. Recently, this method is widely used to treat traffic accident patients in oriental medicine. However, there is no evidences of treatment, no information of effects and side-effects of this method, and no information of drugs used. In South Korea, western medicine and oriental medicine are regulated differently. When a new technology is invented in the area of western medicine, that method should pass several stages of clinical trials. After that processes, that method can be done as a medical practice. However, in the area of oriental medicine, there is no process like that. According to in South Korea, medical practice without license are composed of two behaviors. First type is that medical practice is done by a person who has no medical license. Second type is that medical practice is done by a person who has a medical license but the area of the license is different. Because of this reason, the distinction between the western medical practices and the oriental medical practices is very important. Medical practices are protected by license mainly because they can harm human life or body. When we invented new medical practice and try to practice it to the patients, we should consider the risk of that method whether it is western medical practice or oriental medical practice. It is not clear that the pharmcopunture which has been done is satisfied the standard of medical treatment.

  • Until now, many researchers have studied and raised issues on medicine tourism. However, the studies for invigoration of medicine tourism from chinese medicine perspective have not been sufficient. The chinese medicine tourism is a new generated concept that combined health medical service of chinese treatment with health-related tour activity as a kind of health tourism. In this paper, we try to search developmental suggestions and propose alternatives for vitalization of chinese medicine tourism from aerial service view.

Application of the Principle of Trust to the Medical Service Division between Oriental and Western Medicine (한·양방 의료 사이에서 신뢰의 원칙이 적용되는 경우에 관한 고찰)

  • South Korea's medical system has dual systems-that is, Oriental and Western Medicine. Both are different from method for diagnosis or treatment of diseases and Scientific principle. Of the characteristics of Oriental medical practice in comparison with Western medical practice, notably, it is difficult to acknowledge specific constitutions, Oriental medical doctors' discretion is broad, and Oriental medical practice has a low invasiveness. Thus, it is difficult to acknowledge human specific constitutions when grounded on Oriental medical principles, thereby making it difficult for Oriental medical doctors to argue such specific constitutions as a means of defending against their medical negligence. And, it is difficult to prove Oriental medical doctors' negligence because Oriental medical doctors' scope of discretion is broad. Collaboration of Oriental medicine and western medicine can diagnose and treat the patient's diseases from a different viewpoint, making both medicines complementary. Oriental medicine and western medicine are independent of each other, equal, thus making them divided horizontally. Horizontal medical service division involves the principle of trust, but the principle of trust does not always apply to Oriental medicine and western medicine, because if patients shift from one area of medicine to another, the scientific principle, diagnostic method and treatment method of that medical area should be different. Application of the principle of trust to both of them needs to be analyzed according to types of medical institutions where transfers occur, and to the scope of work division between them.

Variation Analysis of Medical Service Utilization in Oriental Medicine Frequent Disease of Rural Area (농어촌지역 한방 외래 다빈도 상병의 의료이용 변이분석)

  • The objectives of this study are to identify whether the small area variation also exists in the oriental medicine and, if it exists, what causes, to expand our boundary of research interests on the small area variation observed at the western medicine toward the oriental medicine as one of the fundamental research foundations and to provide any fundamental findings from this study results to the healthcare politicians to promote consumer's rational behaviors for the use of healthcare. This study analyzed the health insurance claim data (2010, 2011) which were the patients of western medicine and the outpatients of the oriental medicine with the top 10 most frequent diseases and looked into the variation of healthcare utilization among the areas after grouping resident area into an 86-area category. The study result shows that the small area variation was also observed at the part of the oriental medicine in which the characteristics of patients critically affect the healthcare expenditure per visit day rather than those of providers and the characteristics of both patients and providers equally affect the healthcare expenditure per patient. Therefore, this study suggests that government set up healthcare policies on the standardization of oriental medicine to prevent its over-utilization and unmet need, enforcing the roles of oriental medicine in the markets, enhancing the appropriate health care utilization, and expanding provision and sharing the health care information to reduce unnecessary health care utilization.
