Clinical Analysis of 1,472 Musculoskeletal Patients Who Visited Emergency Room of Oriental Medicine Hospital (한방병원 응급실에 내원한 근골격계 환자 1,472명에 대한 임상적 분석)

  • Objectives : The purpose of this study is to understand the characteristics of the musculoskeletal patients who visited the emergency room of an oriental medicine hospital and provide better first-aid medical service. Methods : We performed a retrospective study of 1,472 patients who visited the emergency room of an oriental medicine hospital, from June 1st, 2007 to May 31st, 2010. Results : Of 1,472 total patients, the male to female ratio was 1.06:1 and people in their 30s were the peak age group. The majority of patients visited the emergency room between 10:00 to 24:00(midnight). Most patients visited the emergency room from Saturday to Sunday. By monthly distribution, the patients increased slightly during February, April, September and October. The majority of patients visited the emergency room within 24hours of onset. The patients were categorized as follows : low back pain 64.81%, cervical pain 11.82%, ankle pain 10.67%. 27.51% of patients used stretcher cars when visiting the emergency room. The admission rate was 22.96%. Conclusions : Of the patients who visited the emergency room of an Oriental hospital, low back pain, cervical pain and ankle pain was the most frequent, in this order. By meticulously preparing for the continued increase in musculoskeletal treatment demand, we can further assist and speed the specialization and popularization of Oriental medical services.

Studies on Antitumor Effect and Synergistic Action of Natural Products with Anticancer drugs against Hepatic Tumors (생약의 간암세포에 대한 항종양효과와 항암제와의 상승작용)

  • The antitumor effect of 柴胡(Bupleuri Radix : BP), 茵陳(Artemisiae capillaris Herba; ACH) 및 蒲公英(Taraxaci Herba; TH) and 蒲公英 EE層(Ethyl ether layer of TH; EETH) on human hepatocytes such as Hep G2, PLC and Hep 3B, and synergistic action with the anticancer drugs, that is, mitomycin(MMC), cisplatin(CPT) and 5-fluorouracil(5-FU) were studied by the method of MTT. The results were obtained as follows: 1. $IC_{50}$ against Hep G2, PLC and Hep 3B was $15.5{\mu}g/ml$, $25.4{\mu}g/ml$ and 31.25 in MMC, $92.5{\mu}g/ml$, $50.2{\mu}g/ml$ and $62.5{\mu}g/ml$ in CPT and $125{\mu}g/ml$ in 5-FU respectively. 2. Cytotoxic effect on Hep G2 was obvious in BP-treated group, synergistic action was most effective in TH-treated group or with MMC. 3. Cytotoxic effect on Hep 3B was obvious in ACH-treated group, synergistic action was most effective in ACH-treated group or with MMC. 4. Cytotoxic effect on PLC was obvious in ACH-treated group, synergistic action was most effective in TH-treated group or with MMC. From above results it was concluded that ACH showed the best antitumor effect against PLC and Hep 3B, BP aganst Hep G2 and also synergistic effect was most effective with MMC, which indicates that it is necessary to seperate the antitumor substances in ACH.

Quality and Sensory Characteristics of Bulgogi Sauce with Various Amount of Omija Extract Juice (오미자 즙의 첨가량에 따른 불고기 소스의 품질 특성)

  • In order to apply oriental medicine materials, which are applicable to the LOHAS concept demanded recently in society, to sauces, this study added different amounts of Omija, which has been proved to have various efficacies, effects and functions, to Bulgogi sauce with soy sauce as its main ingredient and analyzed the functionality of the prepared sauce through physical and sensory tests. General component analysis showed that, with increase in the Omija content, water content, crude ash and crude protein decreased, and crude fat was not detected As to color, L-value gradually increased with Omija content increased, and a-value indicating redness also increased as Omija extract added increased The higher the Omija content was, the lower pH and salinity were and the higher viscosity was. In the quantitative descriptive analysis of Omija Bulgogi sauce, with increase in the Omija content, color, flavor, taste and aftertaste grew stronger, and in the results of palatability test, the sauce containing Omija 5% was preferred most in all the evaluated items. In the quantitative descriptive analysis of Omija Bulgogi, with increase in the amount of Onija extract added, the strength of color, the flavor of sauce, and the taste of Omija grew stronger and the unpleasant smell, saltiness, sweetness and unpleasant taste grew weaker. Summing up the result of this study, when we tested five specimens of different Omija contents including a control, the specimen of 5% content was preferred most This result suggests that other kinds of fruit juice may be usable in soy sauce Bulgogi sauce and continuous efforts should be made to develop new types of Bulgogi sauce.

산삼의 배양 및 그 응용에 관한 연구

  • Korea mountain ginseng known as oriental miracle drug is an important medicinal plant. The effect of mountain ginseng adventitious roots extract has been described. The valuable root of mountain ginseng contained several kinds of ginsenosides that have been confirmed to have many active functions for the human body. However, the study of mountain ginseng has a limit because the price of wild ginseng is very expensive and rare. The mountain ginseng adventitious roots were derived from mountain ginseng callus that were induced from mountain ginseng roots. Adventitious roots were separated from callus and grown in solid media(Murachige and stoog media). It was cultured in a 20L bioreactor. After culturing for 40days, adventitious roots were harvested. Afterwards the harvested mountain ginseng adventitious roots were dryed and extracted. We examined the effect on melanogenesis of mountain ginseng adventitious roots extract. Here, we report the inhibitory effect of melanin biosynthesis on the adventitious roots extract of In vitro test. Also, we assessed the safety of adventitious roots extract. In vitro, cytotoxicity of adventitious roots extract was assessed in mouse fibroblast using two method: The neutral red uptake assay and the MTT assay. In vivo, the allergic and irritant were Patch teated in 30 patients. Consequently, extract of mountain ginseng adventitious roots have inhibitory effect on melanin biosynthesis in B-16 melanoma cell test, tyrosinase inhibitory test and DOPA auto-oxidation test. There were decreased 86%(0.5% concentration), 45%(1% concentration) and 61%(1% concentration), respectively.

A Study of the Therapeutic Effects of Ginseng on Chi-deficiency Syndrome with Sphygmography (맥파계법을 이용한 인삼의 기부족 증상 치료효과에 관한 연구)

  • Since ancient times, records pertaining to chi-reinforecenent of ginseng have been documented in books of Chinese nedicine. We, therefore, assessed the therapeutic effects of ginseng on patients with chi-deficiency syndrome. Eigheen cases of chi-deficiency, screened at the outpatient service in the internal department of Chinse medicine at the China Medical College Hospital. were prescribed two grams of Korean red ginseng powder three times daily for a two week period. For evaluation, a questionnaire and diagnosis by sphygmography were conducted prior to and after administration of the prescription. For control purposes, fourteen volunteers fasted for more than five days (seven had access only to ginseng. and the other seven were given only mineral water) and were subsequently evaluated by the same criteria. Three results were obtained for the ginseng treated group: (1) improvements in symptoms of chi-deficiency such as fatigue and dizziness; (2) changes in pulse waves with time domains showing alternate P-waves, strengthened dicrotic waves, and some minute waves in the end-diastolic period; (3) frequency domains with enhanced amplitudes around 12Hz. These three findings, which are consistent with each other, suggest the reinforcement of the effect of ginseng on 'heart-spleen chi' in Chinese medicine.

A clinical study of Soyangin Yangkyuksanwhatang (양격산화탕(凉膈散火湯) 활용(活用)에 대(對)한 임상적(臨床的) 연구(硏究))

  • Background and Purpose : We intended to be helpful to understand constitutional symptoms of a disease and an application of the prescription through a clinical study of Soyangin Yangkyuksanwhatang. Methods : We studied 65 patients(40 men and 25 women) treated with Soyangin Yangkyuksanwhatang in constitutional clinic of Dongguk Pundang Oriental Hospital. And then we investigated the characteristics of Yangkyuksanwhatang treated patients, remedial values, onset of action and side effects. Results: Soyangin Yangkyuksanwhatang was used in all age groups both men and women. There were 37 kinds of chief complaints including chest discomfort, numbness of hands and feet, motor disturbance and skin disease etc. The distributions of remedial values were slightly improved(58.5%), much improved(12.3%), no improved(29.3%) and there was no aggravated patient. Using the prescription, mostly I prescribed only Soyangin Yangkyuksanwhatang without any changing, and added some herbs to the prescription according to the symptoms of a disease. Soyangin Yangkyuksanwhatang was affected in remedial values by the addition and subtraction of herbs. The onset of action in young age group was shorter than old age group. Side effects were palpitation, abdominal pain and indigestion etc.

A Clinical Study for 157 Cases of CVA by Sasang Constitutional Medicine (사상의학적(四象醫學的) 중풍치험(中風治驗) 157례(例)에 대(對)한 임상적(臨床的) 연구(硏究))

Background and Purpose: We intended to make the standards of CVA management of Sasang Constitutional Medicine and be helpful to understand the constitutional symptoms through a clinical study of CVA. 2. Methods: We studied 157 CVA inpatients(79 men and 78 women) who were in constitutional clinic of Dongguk Pundang Oriental Hospital during 1 year from March 1997 to February 1998. We investigated and analysed the characteristics, past histories, symptoms and progresses of patients. 3. Results: The results of constitutional analysis showed 88 Taeumin(56.1%), 48 Soyangin(30.6%) and 21 Soeumin(13.4%). The frequency of strokes was much the same between male and female and the greater part of cases were 50-70 of age. The CVA cases were classified into 119 cerebral infarction(75.8%), 11 cerebral hemorrhage(7.0%), 1 subarachnoid hemorrhage(0.6%) and 26 trasient ischemic attack. From the standards of symptom management, there were abnormality of a coated tongue(63.1%) in most cases, sleeping disorders(48.4%), excremental disorders(44.7%) and urinary disorders(36.9%) on admission. With regard to the rate of improvement in the standards of CVA management, excremental disorders improved 87.1% and emotional disorder 81.0%, swallowing disorder 78.7% and so on.

Investigation of Antimicrobial Activity and Stability of Ethanol Extracts of Licorice Root (Glycyrrhiza glabra) (감초의 에탄올 추출물의 항균활성 및 안정성 조사)

  • Conditions for extraction of antimicrobial materials from licorice root, Glycyrrhiza glabra, were optimized. Among solvents tested, 95% ethanol gave highest antimicrobial activity, and was chosen as optimal extracting solvent. Extraction temperature and time were optimal at room temperature and for 12 hr, respectively. Minimal inhibitory concentration (MIC) of 95% ethanol extracts was determined against 14 microorganisms. Reference microorganisms included 6 Gram(-) bacteria, 4 Gram(+) bacteria, and 4 yeast strains. Ethanol extract exerted very strong growth inhibition on Gram(+) bacteria, while was moderately effective for Gram(-) bacteria and yeasts. Treatment at $180^{\circ}C$ for 30 min or extreme pHs merely destroyed antimicrobial activity of ethanol extract. These findings suggest ethanol extract of G glabra may be useful as natural preservative.

Identification of Antioxidative Components from Ethanol Extract of Rhus verniciflua STOKES (옻나무 에탄올 추출물로부터 항산화 활성 물질의 구조동정)

  • The free phenolic acid fraction of the chloroform extract from 75% ethanol extract of Rhus verniciflua STOKES (RCF) showed stronger antioxidative activity than BHT, BHA and ${\delta}-tocopherol$ at the same concentration. RCF components were isolated and identified by silica gel column chromatography, thin layer chromatography, mass spectrometer and $^1H-NMR\;and\;^{13}C-NMR$. The antioxidative activity was confirmed by electron donating activity, Rancimat method and thiobarbituric acid test in liposome system. RCF-11 could be further separated into three fractions. The antioxidative active compounds were purified and identified as gallic acid, butin and butein. The RCF-13 was purified and identified as sulfuretin.

  • In order to determine if the mRNA and protein expression levels of 3T3-L1 adipocytes are influenced by oriental medicines, adipocytes were treated with $100\;{\mu}g/ml$ of G-Re and aqueous extract of a Coix lachrymajobi var. mayuen (AEC) every other day for 12 days, respectively. The tumor necrosis factor alpha ($TNF-{\alpha}$). mRNA and protein expressions were suppressed markedly in treated mature adipocytes. Those of lipoprotein lipase (LPL) levels were found to increase gradually in preadipocytes differentiating into mature adipocytes. Those were higher than that of the untreated mature adipocytes. The treated adipocytes showed reduction of leptin expression levels, while in untreated mature adipocytes cell, those of levels were significantly higher after the conversion of preadipocytes into mature adipocytes. The resistin levels in the treated adipocytes were significantly decreased comparing to that of the untreated mature adipocytes. In conclusion, the expression levels of LPL, $TNF-{\alpha}$, leptin and resistin mRNA and proteins are shown to be regulated by G-Re and AEC, making them potential candidates for controlling fat mass related obesity.
