극지 운항선박에서의 한랭손상 치료 가이드라인에 관한 고찰

  • 최근 지구 온난화의 환경 변화에 극지 항로의 개척의 기회가 열렸다. 우리나라 선박은 극지항해 중 한랭손상의 경험이 매우 낮을 뿐만 아니라 열악한 선내의료 환경에서 손상 환자의 발생 시 정확한 응급처치는 어려울 수 있다. 한랭손상 중 저체온증과 동창, 동상에 대해 선내에서 정확한 환자평가와 응급처치를 위해서 국내 외의 응급처치 분야의 교육 자료를 조사하고 선내 한랭손상 응급처치의 기초 자료를 제공하기 위한 목적으로 수행하였다.

Effects of Theratainment Physical Factors Application on Blood Pressure, Angle and Circumference in an Female University Students Calf (테라테인먼트 물리적 요소 적용이 대학생 여성 종아리의 혈압, 각도, 둘레에 미치는 영향)

  • This study examined the effect of the application of Theratainment physical factors on the calf blood pressure, ankle angle, and calf circumference. A research sample consisting of normal female university students in their twenties were divided into hot therapy group (n=20), cold therapy group (n=20), and compression therapy group (n=20). By referring to the previous study, 20-minute hot therapy, 15-minute cold therapy, and 30-minute compression therapy with 100mmHg intensity was applied. The therapy was applied to left-side lower limb, followed by the application on the right-side lower limb after an hour of resting time. The calf blood pressure, ankle angle, and calf circumference of each group was assessed. In the study results, hot therapy and compression therapy showed a statistically significant difference on the decrease of blood pressure and increase of ankle angle (p<.05), but cold therapy produced no statistically significant difference. None of the three groups showed a statistically significant difference on the calf circumference. Hence, hot therapy and compression therapy is recommended for reducing calf blood pressure and increasing ankle angle related to the calf. These therapies are expected to be used for solving diverse problems of lower limbs.

  • Open heart surgery with hypothermia in patients with cold agglutinin can cause severe complications by hemolysis and hemagglutination of red blood cells. A 41 year-old male patient with mitral stenosis was admitted due to fever and cough. After antibiotics treatment, he was scheduled to undergo mitral valve replacement. In the operation room, we found agglutination of blood cardioplegia during lowering temperature of cardioplegia. And then, the cardioplegia was changed to warm cardioplegia and the operation was performed under normothermia due to the suspicion of the cold reactive protein. The operation was performed uneventfully. Postoperatively, cold agglutinin was confirmed by immunochemistry of the patient\`s serum.

The Effect of Cryotherapy on Release of Inflamation in Patient with Rheumatoid Arthritis (한랭치료가 급성 류마티스 관절염 환자의 염증완화에 미치는 효과)

  • The purpose of this study was to investigate the effect of the cold air application for release of inflamation and pain in patient with acute rheumatoid arthritis who had taken medication(NSAIDs, DMARDs, corticosteroids). The participants had knee joint pain and ESR score over the 30. They were randomly assigned to three groups with each 15. Control group(group I ) were applied cold air and medication, experimental group II were applied hot pack and medication. and experimental group III were applied only medication. They were evaluated from April 1, 2000 to september 1,2000 ,and it was examined by ESR and VAS(visual analoge scale) in Lee Bang Hoon rehabilitation medicine clinic in Jeju Korea. The result of this study were as follow: 1. It was found that group 1, group II and group III had the statistically significant reduction of the ESR score according to treatment time(p<.05). 2. There were not statistically significant differences in ESR change pattern of each group according to treatment time(p>.05). 3. There were not statistically significant differences in the change of ESR according to treatment time between group I and group II , group I and group III , and group II and group III (p<.05). 4. It was found that group I . group II and group III had the statistically significant reduction of the VAS(Visual Analoge Scale) score according to treatment time(p<.05). 5. There were statistically significant differences in VAS score according to treatment time between control group(group I ) and experimental groups(group II and group III)(p<.05), but there were not stsiistically significant differences between experimental groups(p>.05).

  • Summary of background data: It is researched until now and the researches against the effect of position cold treatment the effect which pain threshold, change of skin temperature, researches the effective stiffness, the coldness in the patient it applied and relax an inflammation do the researches against were insufficient. Purpose: This project was to see how much it gives effect by using the cold air therapy an inflammation relief of rheumatoid arthritis patient. Methods: The medical subjects were 10 people from standard to diagnosis category of American College of Rheumatology(ACR) who had rheumatoid arthritis and to inspect informations about inflammation relief, have inspected ESR(erythrocyte sedimentation rate) and WBC(white blood cells). The experiment method was to let the patient bend the joint and lay down flat and treat with cold air from 10 cm away from joint line repeating 5 minutes treatment 1 minute rest and 3 minutes treatment. ESR and WBC cold air therapy reported with 5 days gap and each patient has been applied for 3 times. Result: The 5th day and 10th days ESR and WBC value shows when using cold air therapy, there was a reduction on rejected group but didn't last long, but 15th day it showed only on WBC it had its attention. Conclusions: So the cold air therapy shows it has positive effects on inflammation relief of rheumatoid arthritis patient.

  • A 75-year-old male patient without any significant medical and habitual risk factors for acute atherosclerosis obliterans except for hypertension was diagnosed with coronavirus disease 2019 with dyspnea, coughing, and mild fever. After a week of hospitalization, he complained of right foot pain and numbness. The symptoms were aggravated during the next week, resulting in a complete toe color change and loss of dorsalis artery pulse. Enhanced 3-dimensional computed tomography angiography revealed thrombus formation in the right common iliac artery and a loss of blood flow below the popliteal artery on both sides. The patient underwent percutaneous balloon angioplasty with stent insertion followed by medical therapy for anticoagulation. The clinical symptoms immediately were improved after the intervention, but the great toe necrosis was not recovered. Finally, amputation of the great toe was performed.

  • Objective : Cold Urticaria is a symptom of edema and pruritus that appear when a patient is exposed to the coldness. The effect of Bee-venom Acpuncture Therapy is antiinflammatory, pain-killing, and blood circulation promotion. The aim of this study is to report the effect of Bee-venom Acpuncture Therapy to Cold Urticaria. Methods : The Cold Urticaria patients were treated with Bee-venom Acpuncture Therapy. Result and Conclusion : Edema and Pruritus of Cold Urticaria patient improved after Bee-venom Acpuncture Therapy.

  • Objectives: The purpose of this case is to investigate and report the effectiveness of korean Medicine treatment and Harpagophytum radix. Pharmacopuncture for the type 1 complex regional pain syndrome. Methods: One patient is admitted at Bu-Chun Jaseng Hospital of Korean Medicine, diagnosed as type 1 complex regional pain syndrome and treated with korean Medicine treatment and Harpagophytum radix. pharmacopuncture. This case is measured and assessed by Neumerical Rating Scale(NRS), Oswestry Low-back Pain Disability Index(ODI) and Manual Muscle Test(MMT). Results: After treatment, the intensity and frequency of complex regional pain is gradually decreased and the result of NRS, ODI, MMT showed improvement. Conclusions: As seen in this cases of type 1 complex regional pain syndrome, korean Medicine treatment and Harpagophytum radix. pharmacopuncture appear to be effective for the pain relief and musculoskeletal function recovery.

A Study on the Mechanisms by Which the Aqueous Extract of Inonotus obliquus Induces Apoptosis and Inhibits Proliferation in HT-29 Human Colon Cancer Cells (차가버섯 물추출물의 대장암세포 증식억제 및 Apoptosis 유도기전 연구)

  • The mushroom Inonotus obliquue (IO) has been traditionally used for the treatment of gastrointestinal cancer in Russia, Poland, and most of Baltic countries. To explore the possibility that IO has chemoprevention effects, we examined whether or not the aqueous extract of IO inhibits HT-29 cell growth and investigated tile mechanism for this effect. Cells were incubated in the presence of increasing concentrations of the aqueous extract of IO. The extract substantially inhibited the viable HT-29 cell number in a dose-dependent manner and inhibited 5-bromo-2'-deoxyuridine incorporation into DNA of HT-29 cells. Annexin-V staining followed by flow cytometry revealed that the extract induced apoptosis of HT-29 cells in a dose-dependent manner. Western blot analysis of total cell lysates revealed that the extract induced cleavage of caspase-8, -9 and -3 and poly (ADP-ribose) polymerase, but did not affect the protein levels of Bax and Bcl-2. In addition, the extract dose-dependently increased the activity of caspase-8, -9 and -3. We have demonstrated that the aqueous extract of IO inhibits cell proliferation and induces apoptosis in HT-29 cells, which may be mediated by its ability to activate the caspase pathway.

Herbal Medicine Treatment for Cold Urticaria: A Systematic Review and Meta-Analysis (한랭 두드러기에 대한 한약 치료: 체계적 문헌 고찰 및 메타분석)

  • Objectives : The purpose of this study is to evaluate the effects of herbal medicine treatment in patients with cold urticaria. Methods : We searched randomized controlled trials(RCTs) reporting the effects of herbal medicine for cold urticaria through domestic and international databases from their inception to September 2023. The results were summarized in tables. We assessed the risk of bias in included RCTs through Cochrane risk of bias tool and the data synthesis was conducted through RevMan version 5.4. Results : A total of 12 RCTs were included in this review and all trials compared herbal medicine alone treatment(treatment group) with western medicine alone treatment(control group). The total effective rate(TER) of treatment group was statistically higher than that of control group(RR: 1.49, 95% CI: 1.38 to 1.62, p<0.00001, I2=65%). On the other hand, when comparing except for 1 trial with different evaluation period, the TER of treatment group was statistically higher than that of control group and heterogeneity was very low(RR: 1.36, 95% CI: 1.26 to 1.47, p<0.00001, I2=0%). And when comparing 8 trials using the total symptom score(TSS) change index as an indicator of TER, the TER of treatment group was statistically higher than that of control group and heterogeneity was very low(RR: 1.38, 95% CI: 1.26 to 1.51, p<0.00001, I2=0%). The treatment group showed more statistically significant decrease compared to the control group in TSS(MD: -2.51, 95% CI: -2.63 to -2.40, p<0.00001, I2=99%). The relapse rate of treatment group was statistically lower than that of control group(RR: 0.19, 95% CI: 0.10 to 0.40, p<0.00001, I2=0%). Mild adverse events such as sleepiness, dizziness were reported in control group and gastric discomfort was reported in treatment group. In the risk of bias assessment, many cases were evaluated as 'Unclear risk'. Conclusions : This review found that herbal medicine alone treatment could more effective and safe than western medicine alone treatment for cold urticaria. But further well-designed researches are needed because of heterogeneity between trials and the quality of the included trials.
