인지 기반 작업수행(Cognitive Orientation to daily Occupational Performance: CO-OP) 그룹 중재가 뇌성마비 아동의 작업수행기술에 미치는 영향: 사례연구 (Effect of Group Cognitive Orientation to Daily Occupational Performance (CO-OP) Intervention on the Occupational Performance Skills of Children With Cerebral Palsy)

  • 목적 : 본 연구는 뇌성마비 아동의 작업수행기술에 인지 기반 작업수행(Cognitive Orientation to daily Occupational Performance; CO-OP) 그룹 중재가 효과가 있는지 알아보고자 한다. 연구 방법 : 연구 선정기준에 부합하는 뇌성마비 아동 3명을 대상으로 2021년 4월부터 6월까지 진행하였으며, 연구 설계는 단일실험연구 ABA를 적용하였다. 기초선 기간(A) 3회기, 중재 기간(B)은 10회기(주 1회, 60분/회), 재기초선 기간(A') 3회기로 구성하여 실시하였다. 중재 전후 비교를 위한 평가로는 운동발달평가, 시지각 평가, 일상생활 동작 평가를 시행하였으며, 매회기의 수행의 질을 평가하기 위해서 수행의 질 평가척도를 사용하였다. 결과 : 3명의 대상자 모두 3가지 공통 목표 활동(양치질하기, 색종이 접기, 옷 입고 벗기)에서 작업수행의 질이 향상되었다. 또한 시지각, 대 운동과 소 운동, 일상생활 동작 평가 점수도 향상되었다. 결론 : CO-OP 그룹 중재가 전 학령기 및 학령기 뇌성마비 아동의 학업 전 기술 및 작업수행 향상에 효과적인 것으로 나타났다.

정상 어린이에서 혈청 인슐린양 성장인자-I과 인슐린양 성장인자 결합단백-2 및 -3의 농도 분석 (Evaluation of Serum Insulin-Like Growth Factor(IGF)-I, Insulin-Like Growth Factor Binding Protein(IGFBP)-2 and IGFBP-3 Levels in Healthy Korean Children)

  • 목 적 : 혈청 IGF-I, IGFBP-2, IGFBP-3 농도 및 IGF-I/IGFBP-3 molar ratio는 소아의 성장 발달, 당뇨병, 비만 및 악성종양의 진단, 예후 예측과 치료후 추적에 유용한 지표이지만 성별, 연령별, 인종별 변이가 있다. 저자들은 우리나라 어린이의 성별 및 연령군별 혈청 IGF-I, IGFBP-2, IGFBP-3 농도 및 IGF-I/IGFBP-3 molar ratio의 정상 표준치 확립을 위하여 본 연구를 시행하였다. 방 법 : 총 92명(남자 42명, 여자 50명)의 건강한 어린이를 대상으로 하였고, 신생아군 9명(남자 3명, 여자 6명), 영아군 19명(남자 9명, 여자 10명), 유아군 24명(남자 13명, 여자 11명), 학령군 30명(남자 12명, 여자 18명), 청소년군 10명(남자 5명, 여자 5명)으로 분류 후, ELISA 방법을 이용하여 혈청 IGF-I, IGFBP-2 및 IGFBP-3 농도를 측정하였고, 각 군간의 IGF-I, IGFBP-2, IGFBP-3 및 IGF-I/IGFBP-3 molar ratio를 비교분석하였다. 결 과 : 1) 남자에서는 IGF-I과 IGFBP-3는 출생과 더불어 증가하여 각각 유아군(P<0.05), 학령군(P<0.05)에 최고값에 도달한 후 청소년군으로 갈수록 감소하였다. IGFBP-2와 IGF-I/IGFBP-3 molar ratio는 각각 학령군과 영아군에 최고값을 보이고 감소하였으나 출생시에 비해 통계적으로 유의한 증가를 보이지 않았다. 각 군간의 비교에서 IGFBP-2만이 유의한 차이를 보였다(P<0.05). 2) 여자에서는 IGF-I과 IGFBP-2, IGFBP-3는 출생과 더불어 증가하여 각각 청소년군, 학령군(P<0.05), 학령군(P<0.05)에 최고값에 도달하였으나 IGF-I은 출생시에 비해 통계적으로 유의한 증가를 보이지 않았다. IGF-I/IGFBP-3 molar ratio는 출생시에 최고값을 보였으나 통계적으로 유의한 차이를 보이지 않았다. 각 군간의 비교에서 IGFBP-2 및 IGFBP-3가 유의한 차이를 보였다(P<0.05). 3) 동일연령군별 남녀 비교에서 IGF-I과 IGFBP-2, IGFBP-3 및 IGF-I/IGFBP-3 molar ratio 어느 것도 남녀 간에 통계적으로 유의한 차이를 보이지 않았다. 4) 상관관계 조사시 IGF-I의 농도가 높을수록 IGFBP-3의 농도 역시 의미있게 증가하였고(r=0.504, P=0.000), IGF-I/IGFBP-3 molar ratio와 비만도와의 상관 관계를 조사해 본 결과 통계적으로 의미있는 음의 상관 관계를 보이지 않았다(r=0.051, P=0.629). 또한 비만도와 IGF-I, IGFBP-2 및 IGFBP-3 사이에 통계적으로 유의한 상관 관계는 없었다. 결 론 : IGF-I과 IGFBP-3는 사춘기 진행에 따라 증가하며, 본 연구에서 IGFBP-3가 학령군에서 최고 수준을 보이는 것은 사춘기가 일찍 오는 요즘 어린이들의 성장 추세를 반영하는 것으로 추정되어진다. IGFBP-2는 신생아군에서 가장 높고 영아군에서 감소하는 데 이것은 IGFBP-2가 출생 전 발달 과정에 중요한 물질임을 뒷받침해주는 소견이다. 향후 더 많은 정상아를 대상으로한 연구를 통하여 보다 정확한 우리나라 어린이의 성별 연령군별 정상 표준치 확립이 필요하겠다.

학령전기 장애 아동 부모와 비장애 아동 부모의 놀이 인식과 놀이 참여 비교 연구 (Comparison of Play Perception and Play Participation of Parents of Disabled Children and Non-disabled Children in Preschool Age)

  • 목적 : 본 연구의 목적은 학령기 전 장애아동과 비장애아동 부모들에게 자녀의 놀이에 관한 포괄적인 설문을 시행하여 놀이 인식을 알아보고 각 부모에게 실질적으로 필요한 놀이의 방향성을 제시하는 것이다. 연구 방법 : 선행연구들을 토대로 제작된 설문지를 설문요청에 동의한 대상자 700명에게 발송하였다. 미회수 및 불성실한 설문을 제외하고 총 596부의 설문지가 분석되었으며 이 중 106부가 장애아동 부모의 설문지였다. 자료 분석을 위해 SPSS Window 23 프로그램을 활용하였으며, 빈도분석과 독립표본 T 검정이 시행되었다. 결과 : 장애 아동 부모는 비장애 아동 부모에 비해 자녀와의 놀이를 더 중요하게 인식하고 있었으며 통계적으로 유의한 차이를 보였다(p<0.01). 자녀와의 놀이에 대한 즐거움 인식은 장애 아동과 비장애 아동 부모 간에 큰 점수 차이를 보였지만 통계적으로 유의하지는 않았다(p=0.053). 자녀와의 놀이 참여시간은 장애아동과 비장애 아동 모두 부 보다 모와의 참여시간이 많았다. 모의 경우 장애아동의 모가 놀이참여 시간이 더 많았고, 부의 경우 비장애 아동의 부가 더 많았다. 장애아동과 비장애 아동 부모 모두 장난감을 사주는 빈도는 '보통' 정도가 가장 많았으며 두 집단 간에 큰 차이를 보이지 않았다. 장애아동과 비장애 아동 부모 모두 놀이 정보를 습득하기 위해 인터넷을 주로 이용하였으며, 놀잇감을 살 때 고려사항은 흥미 유발, 발달수준, 안전 순이었다. 결론 : 본 연구를 통해 학령전기 장애 아동과 비장애 아동 부모의 놀이 인식 및 참여 실태를 비교할 수 있었다. 본 연구를 토대로 부모들이 놀이 시 실질적으로 필요한 부분을 알 수 있고, 추후 아동을 대상으로 한 놀이 연구의 기초자료로 제공될 수 있을 것이다.

학령 전 아동에서 Lacto-ovo Vegetarian과 Non-vegetarian의 식생활 습관과 영양소 섭취 차이에 관한 연구 (A Study on the Differences in the Dietary Pattern and Nutrient Intake of Lacto-ovo Vegetarian and Non-vegetarian in Korean Preschool Children)

  • Korea used to be a country where individuals ate diets high in vegetables and rice, but in recent years, it has become a country where western-style diets are increasingly popular. Consequently, the age for chronicle disease, including obesity and diabetes, has become lower. The purpose of this study was to compare the dietary habits of kindergarten children between lacto-ovo vegetarian and non-vegetarian families. The data were collected from the children(n=300) at Sahmyook University Kindergarten. The results showed that non-vegetarians had higher intakes of cookies, cakes, processed foods, ice cream, and fast foods, while vegetarian children consumed high amounts of vegetables, fruits, and kimchi. The non-vegetarian children tended to have irregular meals, and ate high-fat meats such as kalbi and samgyupsal. However, the results show it is important that many children among both vegetarian and non-vegetarian families should be encouraged to have appropriate essential nutrients, because a high percentage of children in both groups appeared to have low intakes of energy, vitamin $B_2$, vitamin C, calcium, zinc, iron, etc.

부산 시내 일부 지역의 소득수준별 학령전 아동의 식생활 태도 및 영양상태에 관한 비교 (A Comparative Study on the Dietary Attitudes and Nutritional Status of Preschool Children in Different Income Levels in Busan)

  • This study was carried out to investigate the dietary attitudes and nutritional status of preschool children in different income levels in Busan. The survey was conducted from November 1 to November 30, 2004 by questionnaires mid data analyzed by the SPSS program. The results are summarized as follows: The Kaup index of the upper income class group was higher than that of the other income classes. In the middle income class group, nutrition knowledge scores were higher than in any other income class. Sixty-four point nine percent of the upper income class children, 25.7% of the middle income class children and 15.7% of the low income class children had nutrition education experiences. Intake of protein was higher than that of the Korean recommended dietary allowances (RDAs) for children. In the upper income class group, intake of calcium was higher than in any other income class. Intakes of calcium and iron were lower than those of the RDAs in all children. Dietary attitude had positive correlation with frequency of breakfast, time for outdoor activity, and height and weight, and had negative correlation with frequency of fast food and time for watching television. Frequency of snack had positive correlation with frequency of fast food and time for watching television, but frequency of breakfast had negative correlation with those. These results indicate that nutrition education programs linking parents to children should be carried out to improve their nutritional status.

학령기 아동의 자기효능감, 사회적 지지가 우울에 미치는 영향 (The Effects of Self-efficacy and Social Support on Depression in Elementary School Children)

  • Purpose: The purpose of this study was to investigate the effects of self-efficacy, social support, and depression in elementary school children. Methods: The participants of this study were 619 students from seven elementary schools located in Jollabuk-do Province, Korea. The methods included data analysis, descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficients and stepwise multiple regression using SPSS/WIN version 15.0. Results: The mean scores of self-efficacy, social support, and depression were $2.9{\pm}0.58$, $3.2{\pm}0.44$ and $14.0{\pm}7.32$, respectively. There was a significant positive correlation between self-efficacy and social support (r=.517, p<.001). Significant negative correlations were observed between self-efficacy and depression (r=-.557, p<.001) and between social support and depression (r=-.571, p<.001). The variables of depression in elementary school children were self-efficacy and social support, which explained 42.7% of the cases. Conclusion: The findings from this study suggest that current intervention programs are not sufficient to curb depression in elementary school children and therefore it is needed to develop nursing intervention programs to promote self-efficacy and social support.

체질량 지수로 판정된 학령전 남아의 비만에 영향을 미치는 요인 (Risk Factors of Obesity by Body Mass Index in Preschool Boys)

  • This study was carried out to identify dietary risk factors for childhood obesity. The study subjects were male children, aged 2-6years, who were enrolled in nursery schools and kindergartens. We obtained anthropometric measurements from 949 boys and dietary questionnaires from 253 boys. The subjects were classified into two groups using the BMI criteria, normal (n=200) group with the BMI below 85th percentile and the obese group with the BMI at 85th percentile and above (n=53). The logistic regression analysis showed that the odds ratios (OR) for obesity were, birth weight greater than 4.0㎏ (3.27) , increased meal speed (4.98) , the frequency of overeating more than twice a week (2.63), the consumption of cooked rice more than one bowl per meal (2.77), the consumption of milk and dairy products less than 5 times a week (2.81), and increased maternal meal speed (1.98). Stepwise multiple logistic regression analyses showed that the variables which contribute to obesity significantly were birth weight greater than 4.0 ㎏ (OR 5.19), the frequency of overeating more than twice a week (OR 2.51) , the consumption of cooked rice more than one bowl per meal (OR 2.74), and the consumption of milk and dairy products less than 5 times a week (OR 4.43) (p<0.0001) . The results of this study indicate that dietary habits of both children and of their mothers as well as child's birth weight are important variables that contribute to childhood obesity. 부산지역 학령전 아동의 식품섭취평가에 관한 연구 (A Studyo on the Evaluation of Food Intake of Preschool Children in Pusan)

  • To assess the food intake and diet quality of preschool children in Pusan, dietary survey was conducted with 176 subjects using 24hr recall method. Diet quality was assessed by food group pattern, dietary diversity score(DDS), dietary variety score(DVS), and dietary frequency score(DFS). The total number of food items consumed was 307. The foods consumed most frequently were rice, Korean cabbage kimchi, milk and green onion. When investigating the consumption pattern of major six food groups, 38.1% of subjects consumed all six groups. Food group was missed in order of fruit(41.5%), sweet(19.9%), dairy(16.5), vegetable(4.5%) and meat(1.7%). The mean values of DDS, DVS, and DFS were 5.16, 13.82, and 16.07 re spectively. Persons who had higher DDSs also had higher DVSs(p<0.001). DDS, DVS and DFS correlated positively with NAR(nutrient adequacy ratio) and MAR(mean adequacy ratio) significantly. Associations between the NAR of most nutrients and MAR with DVS and DFS were higher than those between the NAR of most nutrients and MAR with DDS. People with DDS of above 6 or DVS of above 16 or DFS of above 21 met two thirds of recommended dietary allowance for nutrients. Based on the result of the food group intake, the food intake of subjects was not adequate, especially the fruit and dairy groups. When assessing the dietary quality of subjects using DDS, DVS and DFS, many people appeared not to have a desirable food intake.

학령전 아동의 전염병 개념에 관한 연구 (A Study on the Concept about Communicable Disease of Preschool Children)

  • It is the base of the health through life, the health of children is very important. The health education is the most effective measure for disease prevention and health promotion. To educate children for the health, we must consider their concept of the health before. Therefore I attempted this study for investigation for the concept about respiratory communicable disease which is one of the most important health problems. The design of this study was descriptive investigational study. The subjects of this study are 4-6 aged 65 children who are attending district nursery school. The tools of this study constituted 3 parts as an etiological agents, environmental agents and host agents. The data were collected by an individual interview and cartoon cards. The results of this study are as followed: 1. The preschool children tend to think contamination along the physical distance, nearer things more relational. 2. The preschool children tend to have their experience centered thinking. 3. There was a highly significant correlation between the children's age and knowledge scores(r = 0.431, P= 0.0003). 4. There wasn't a significant correaltion between sex group. In conclusion, it wad founded that the preschool have a tendancy to think communicable disease along the physical distance and their daily life experiences. This results support the Piaget's cognitive developmental theory. Therefore, to develop the health education program for preschool students, above results must be referenced.

학령전 아동의 식습관과 식이섭취평가에 관한 연구 (A Study on the Food Habit and Dietary Intake of Preschool Children)

  • This study was conducted to investigate the dietary habits and assess the dietary intake of preschool children. Food habit, preference and nutritional supplement status were investigated using a questionnaire answered by the mothers of 453 subjects aged 3 to 6 years old. Also, a dietary intake survey using a 24-hour recall method was performed by mothers of the children. It was found that 81.2% of subjects had milk, dairy products, cookies, fruit and bread between meals once or twice per day. As well, 60.3% of subjects had an unbalanced diet and 20.7% had an overeating habit. Thus, unbalanced diet was a serious problem for many of the subjects. Due to weight controls, digestion problems and allergies, 11.7% of subjects had special dietary consideration. And 26.4% of subjects were using nutritional supplements. From the 24-recall survey, it was found that all nutrient intakes were higher than the Korean RDA except calcium and vitamin A. Nutrient intakes for protein, calcium, phosphorus and vitamin B2 were significantly different by sex, and also increased with age but not significantly. Children received 35% of daily energy, 44% of daily fat and 52% of daily calcium from snacks, so snacks clearly play an important role in dietary intake. The average number of foods consumed per day by subjects was 17.6 and that dishes was 11.0. Most children consumed 4 or 5 food groups per day. In conclusion, the dietary intake of children aged 3 to 6 were deemed adequate judging from nutrient intake and dietary diversity. More attention should be paid to the nutritional value of snacks in this age group.
