DENTAL TREATMENT FOR A PATIENT WITH TREACHER COLLINS SYNDROME : CASE REPORT (Treacher Collins 증후군 환아의 치과 치료: 증례보고)

  • Treacher Collins syndrome(TCS) is an autosomal craniofacial development disorder which results from mutations in the gene TCOF1. Major features include midface hypoplasia, micrognathia, microtia, conductive hearing loss. Oral manifestations are characterized by cleft palate, shortened soft palate, malocclusion, anterior open bite and enamel hypoplasia. The purpose of this presentation is to describe the interesting aspects of dental treatment of a patient with TCS. A 6-year-old boy with TCS visited Seoul National University Dental Hospital for dental caries. Multiple caries was observed from clinical and radiographic examination. Because of multiple caries and behavior management ploblem, dental treatment under general anesthesia was planned. Treatment of posterior teeth was performed and some primary teeth were extracted. General anesthesia was induced and maintained with sevoflurane, nitrous oxide and oxygen. Under general anesthesia, successful dental procedure was done. Considering behavior management problem and medical condition of patient with TCS, general anesthesia can be useful.

  • Wolf-Hirschhorn syndrome(WHS) is a congenital disorder caused by deletions of the short arm of chromosome 4. The most common characteristics are mental and growth retardation, dietary disorder and craniofacial features with a characteristic 'Greek warrior helmet' appearance. The dental characteristic of WHS includes delayed development, tooth agenesis, clefts, microdontia, taurodontism, and severely worn dentition. The purpose of this case report is to describe the dental treatment of a patient with WHS. 3-year-old boy with WHS visited the Seoul National University Dental Hospital for dental treatment. He had difficulty with nasotracheal intubation because of craniofacial anomalies and also had poor oral hygiene due to a limitation of mouth opening and dietary disorder. Due to his airway problem, behavior management and severity of dental conditions, dental treatment was performed under general anesthesia. This case suggests general anesthesia can be chosen with WolfHirschhorn syndrome patients to safely care for their dental problems.

Management of Displaced Maxillary Canines by Extraction of the Primary Canine: Factors Affecting Treatment Outcome (유견치 발치를 통해 변위 상악 견치 치료 시 결과에 영향을 미치는 요인 분석)

  • The purpose of this study was to evaluate the effect of interceptive primary canine extraction in palatally and buccally displaced maxillary permanent canines, and to analyze the clinical and radiographic factors affecting the treatment outcome. 97 maxillary permanent canines from 86 patients whose maxillary permanent canine were in the mesio-occlusal directions and overlapped with the roots of the adjacent teeth were analyzed. In 64 of 97 (66.0%) maxillary permanent canines, the displaced crown was completely deviated from the adjacent lateral incisor root only by extraction of the primary canine. Not only the characteristics of maxillary permanent canines such as bucco-palatal displacement direction, horizontal and vertical position of the crown tip, and presence of apical closure, but also periapical rarefaction on the primary canine and peg-shaped adjacent lateral incisor significantly affected the treatment outcome.

A 20-year Retrospective Study of Pediatric Oral Lesion Biopsy (소아 청소년 환자에서 시행된 구강 병소 생검 : 20년 간의 후향적 연구)

  • The aim of this study was to provide pathological information of pediatric oral lesions by retrospectively analyzing oral biopsy results from pediatric patients at the Seoul National University Dental Hospital. Biopsy results of all oral lesions from pediatric patients, aged 0 - 16 years, were collected from the files of the Department of Oral Pathology, Seoul National University Dental Hospital from January 2000 to April 2020. The patients were divided into 3 age groups: 0 - 5, 6 - 11 and 12 - 16 years. All oral lesions were classified into three main categories: inflammatory and reactive, tumor or tumor-like and cystic lesions. Among the total of 2928 biopsy specimens, tumor or tumor-like lesions(35.66%) were the most common, followed by inflammatory and reactive lesions(34.29%) and cystic lesions(30.05%). Regardless of the categories used in this study, odontoma was the most frequently found lesion, mucocele and dentigerous cyst being the next common. This study was the first retrospective review of pediatric oral pathology in Korea, and the results from this study may assist in providing informative insight into the pediatric oral pathology for pediatric dentists.

Clinical Factors Affecting the Results of Regenerative Endodontic Procedure (재생근관치료 결과에 영향을 미치는 인자의 분석)

  • This study retrospectively analyzed the effect of clinical factors on the outcomes of REP(regenerative endodontic procedure). Patients who received the REP using triple antibiotic paste due to trauma or fracture of dens evaginatus from February, 2011 to January, 2020 were included in the study. Finally, 57 teeth in 54 patients were selected. Investigated clinical factors were as follows: intentional bleeding, etiology, and root development stage. Treatment outcomes evaluated were as follows: improvement of subjective symptoms, changes in the periapical lesion, and the amounts of root development after REP. To compensate for differences in angulation and position between repeated radiographic examinations, images were aligned by Turboreg plugin. To evaluate the amounts of root development, apical diameter, root area, and root length were measured by ImageJ software. Among the aforementioned factors, intentional bleeding had no significant effect on treatment results. Regarding the etiology, the increase in the root area and the root length was significantly less in trauma cases than in dens evaginatus fracture cases. Considering root development stage, more immature teeth presented more increase in the root area.

Analysis of Child protection system from a preventive Perspective : Focusing on the German case (예방적 관점에서 살펴 본 아동학대 대응체계 분석 - 독일 사례를 중심으로 -)

  • This study analyzed the child protection system in Germany from a preventive perspective and attempts to find the applicable implications to Korea. The research method was analyzed in terms of legal, policy and professionalism. The result is as follows. First, Child and Youth Support Act in Germany stipulated a preventive support system to restore the function of the family. Second, according to the Civil Act, it was stipulated that the family court could intervene early. Third, the federal Child Protection Act stipulated community cooperation for thd child protection system. Fourth, the Youth Agency as the general authority made it possible to provide preventive support and intervention at the same time. Firth, qualification standards were specified in the Child and Youth Support Act. Child protection specialists are granted to public officials who have worked for more than three years. The implications are as follows. First, the child protection system, which operates as a child abuse reporting system, should be expanded to a preventive support system. Second, it is necessary to expand monitoring by establishing an early warning system between networks in order to establish a support system for potentially at-risk children. Third, local governments should support children and parents flexibly and comprehensively for dysfunction caused by difficulties at home. Fourth, it is necessary to enact the Child Protection System Cooperation Act for a network cooperation system.

Comparison of the amount of bacteria according to the number of holes between bristles (칫솔모 사이에 형성한 구멍 수에 따른 세균 양 변화)

  • The purpose of this study was to determine the level of toothbrush bacterial growth, whether the dryness of the toothbrush head differs depending on the number of holes in the head, and to use these results as a reference for future toothbrush design. Two-millimeter holes were created on the head of the toothbrushes in groups of three, one, or zero holes. We made the solution with Streptococcus mutans, and the toothbrushes were placed in the solution and agitated. The toothbrushes were shaken to remove moisture and allowed to air-dry. The toothbrush heads were swabbed with saline and then placed in two inoculation groups. The first group was inoculated with a 102 dilution of the S. mutans culture and the second was inoculated with the original culture. After incubation, bacterial colony numbers were measured. The number of holes on the toothbrush head correlated with a decrease in number of cultured bacterial colonies. Our model of a toothbrush head with holes indicated that these holes in the toothbrush head were effective in reducing the level of microbial contamination and that a greater number of holes creates an improved toothbrush sanitation effect. The average number of colonies on the head of toothbrush by number of holes was high, followed by the number of holes 0, 1 and 3, and the average number of colony among toothbrush heads was same. The use of a toothbrush with holes between the toothbrush head indicates that it is effective in reducing the level of microbial contamination between the toothbrush head and toothbrush and the higher the number of holes, the better the effect.

A Case of Concurrent Chemoradiotherapy After Endoscopic Resection For Early Esophageal Cancer (조기 식도암에서 내시경점막하박리술 시행 후 항암방사선동시요법을 시행한 1예)

  • 62-year-old patient who had past history of endoscopic submucosal dissection for early gastric cancer at September 2008, underwent endoscopic submucosal dissection of esophagus for early esophageal cancer at mid esophagus during health screening service. Because there was a high risk of lymph node metastasis at biopsy results, concurrent chemoradiotherapy was added to endoscopic submucosal dissection. There was a metachronous cancer at mid-esophagus at March 2013. He underwent endoscopic mucosal resection and photodynamic therapy. Concurrentchemoradiotherapy after endoscopic submucosal dissection is an effective treatment method.

A Study on Mother's Experience in Disclosure of Incestuous Sexual Abuse (어머니의 근친성학대 드러냄(disclosure) 경험에 관한 현상학적 연구)

  • The incestuous assault tends to remain a family secret and not easily be disclosed. Mothers of the victims are usually regarded as the responsible person holding the key to decide whether it should be disclosed or not. Sometimes, therefore, they are blamed with the charge of connivance. The purpose of this study was to explore the essence of mothers' experience of disclosing and to understand the subjective meaning given to this experience. Seven mothers of the female victims of incestuous assault were recruited and interviewed. The collected data was analyzed by hermeneutic phenomenological method. The essential theme of the mothers' disclosing experiences could be summarized as "overcoming pain by going through pain". This major theme contained three sub-themes: "wanting to vent the deepest tormenting pain", "getting hurt again by disregard and indifference from others", "attempting to tear open the festering wound and to feel recovered". Mothers experienced the disclosing process as revisiting the hidden wound to find the meaning of the incident and to cultivate the necessary skill and power to overcome the pain. Especially, mother's disclosing process was characterized by constant restructuring of past trauma through mother's will and knowledge, acknowledging the unmodifiable past, interpreting other people's responses, and, finally, constantly interacting with sociocultural context. The study result stressed the importance of considering the whole array of difficulties and meanings experienced by mothers in the disclosing process. Social work services should provide more professional and sensitive care when mothers try to disclose the dark secret of incestuous assault.
