Antisnoring Effects of Adjustable Anterior Positioner: Case Study (조절성 전방이동형 코골이 방지장치의 효과)

  • Oral appliances are a primary treatment option for snoring and mild to moderate obstructive sleep apnea (OSA) and are considered as an alternative for patients with severe OSA who have failed continuous positive airway pressure (CPAP) or upper airway surgery. But it is true that OAs have varying degrees of effectiveness depending on different oral appliances and the efficacy of OAs are established in some but not all patients. New oral appliance, which is one of Adjustable Anterior Positioners, was developed by Dept of Oral Medicine, Dental School, Dankook University. This is a report for treating severe OSA patient with a failure of previous uvulopalatopharyngoplasty using a new Adjustable Anterior Positioner, followed by significant success of controlling OSA.

  • 경사진 안창은 안창의 경사로 발의 자세를 교정함으로써 하지 통증을 완화시키는 데 사용되는 보조치료기구로 내측 경사진 안창(MWI: medially wedged insole)과 외측 경사진 안창(LWI: laterlly wedged insole)으로 나뉜다. 본 연구는 내/외측 경사진 안창이 족부압력에 미치는 영향을 정량적으로 분석하고자 한다. 분석변수는 압력중심(COP: center of pressure)의 궤적 변화, 발바닥 내 세 개 구획에 상대적으로 작용하는 수직 힘(RVF: relative vertical force) 그리고 여덟 개 해부학적 지점의 압력이다. 경사진 안창의 경사조건은 내/외측 각각 $5^{\circ},\;8^{\circ},\;15^{\circ}$이며, 경사가 없는 안창을 포함해 총 일곱 가지 안창이 실험에 사용되었다. 실험참여자로는 20대 성인 남성 10명이 참여하였다. 실험결과 앞발 누름기에서 MWI와 LWI는 모두 COP를 내측으로 이동시키고 외측 압력을 감소시켰다. 뒤축 접지기에서 MWI는 COP를 내측으로 이동 시키고 내측압력을 증가시키는 반면, LWI는 COP를 외측으로 이동시키고 외측압력을 증가시켰다. 그리고 RVF 분석결과 MWI와 LWI는 모두 앞발 부위 힘의 크기를 감소시켰고, LWI만이 발 뒤축 부위 힘의 크기를 증가시켰다.

  • Neuromyelitis optica (NMO) is an autoimmune inflammatory disorder of the central nervous system characterized by optic neuritis and longitudinally extensive myelitis. Painful tonic spasm is a common complication of NMO, but there have been no reports about it in Korean medicine studies. In this case, we treated a 53-year-old woman diagnosed with NMO with paraplegia, painful tonic spasm, and decreased visual acuity using Korean medicine treatment, including acupuncture, herbal medicine, cupping, and moxibustion. We measured changes in clinical symptoms using the manual muscle testing (MMT), the numeric rating scale (NRS), the modified Barthel Index (MBI), and the Functional Independence Measure (FIM). After treatment, clinical symptoms were improved. The results indicate that Korean medicine treatment may be effective in the treatment of an NMO patient with paraplegia and painful tonic spasm.

  • Recently, meniscal allograft transplantation has been regarded as a successful procedure in terms of pain relief and functional improvement for the symptomatic patients previously underwent subtotal or total meniscectomy. However, the likelihood of a successful outcome would be reduced by various complications including graft tear, shrinkage, extrusion, infection, progressive chondral injury, and granuloma due to nonabsorbable suture material. Therefore, knee surgeons need to be all aware of the complications and to make an effort to minimize them. The purpose of this article is to review the current literatures regarding clinical results and complications after meniscus allograft transplantation.

Lumbar Spinal Stenosis Cases Managed by Yinyang Balance Appliance of FCST, a TMJ Therapy for the Balance of Meridian and Neurological System (FCST의 음양균형장치를 활용한 요추 척추관 협착증의 증례보고)

  • Therapeutic effect of Yinyang Balancing Appliance of functional cerebrospinal therapy (FCST) for meridian and neurologic yinyang balance was observed in a lumbar spinal stenosis case. One lumbar spinal stenosis case was managed with the Yinyang Balancing Appliance on temporomandibular joint (TMJ), combined with acupuncture. Clinical outcome measurement was based on subjective measures and clinical observations. The patient showed positive changes even after the initial treatment and this effect maintained over the follow-up period. further clinical and biological research on FCST is expected.

The Experience of Life Experiences among Patients with Chronic Low Back and Extremity Pain (만성 요$\cdot$하지통 환자의 생활세계 경험)

  • Purpose: The purpose of this study was to understand the meaning and nature of pain experienced among patients with chronic pain. The present study adopted a hermeneutic phenomenological method which was developed by van Manen. Method: The participants for this study were 4 men and 5 women, who were over the age of 20 with chronic pain more than 6 months. Data was collected by using in-depth interviews and observations from September, 2004 to December, 2004. The contents of the interviews were tape-recorded with the consent of the subject. Result: The essential themes that fit into the context of the 4 existential grounds of body, time, space and other people were 'untamed and unremitting pain', 'the body as an obstacle', 'continuity of suffering time as if the moment would never end', 'a narrow radial range of action' and 'separating from other people'. Conclusion: Patients with chronic pain experienced and perceived the world through the filter of their pain. It is necessary for nurses to understand the experiences of chronic pain patients and to provide more empathic, supportive care. Further research is needed on nursing interventions that could help chronic patients cope with and find the meaning in their suffering.

A case report of patient who has spinal cord infarction with monoplegia and pain of lower extremity and dysuria (하지 단마비와 통증 및 배뇨장애를 주소로 하는 척수경색환자의 한방치료 치험 1례)

  • Objective : The purpose of this study is to report the improvement after oriental medical treatment about a patient who has spinal cord infarction with monoplegia and pain of lower extremity and dysuria. Methods : We diagnosed that symptoms as Wejeung, and employed oriental medical treatment, acupuncture, herbal medicine and moxibustion. This study was measured by MMT(Manual Muscle Testing), VAS(Visual Analogue Scale) and the number of Nelaton Catherization. Results : After oriental medical treatment, power and pain of lower extremity and dysuria were improved. Conclusions : There is no specific treatment for spinal cord infarction in western medicine and the prognosis is not so good. But in the case of this patient, power and pain of lower extremity and dysuria were improved remarkably through oriental medical treatment. Further clinical studies are needed to verify the findings.

Warm Sensation of Left lower Extremity as a Complication of Left Stellate Ganglion Block (좌측성상교감신경절차단후(左側星狀交感神經節遮斷後)에 합병증(合倂症)으로 온 좌측하지온감(左側下肢溫感))

  • A case of the left stellate ganglion block (SGB) with a warm serration of the left lower extremity in a 25-year-old male soldier is presented. During the Korean War, this patient received a penetrating gun shot wound from the right knee through the left abdominal wall, left upper arm and left thumb. He was evacuated to the a marine corps surgical hospital where amputation of the left thumb and an end-to-end anatomosis of the left brachial artery were performed. After surgery, left ulnar and median nerve paralysis and causalgia developed and about 9 months later an upper thoracic ganglionectomy was proposed at the Chin-Hae Navel Hospital. Before the ganglionectomy a stellate ganglion block for diagnostic and prognostic purposes was requested by the surgeon. This block was performed by the supraclavicular anterior approach using 10 ml of 2% procaine. The effect of the block including Horner's syndrome was confirmed 5 minute later in this patient. This patient returned to the ward by walking unassisted 10 minutes after the block, and complained of a warm sensation in the left lower extremity 20 minutes later as well as the left upper arm. This warm sensation in the lower extremity following ipsilateral stellate ganglion block indicates that the local anesthetics solution injected tinto the neck spread down to lumbar sympathetic ganalgion along the fascial membrane of the sympathetic chain as a consequence of the 10 minutes walk.

  • To make ones life meaningful is one of human's most valuable goal. Nevertheless, for cancer patients who are in the last phase of their lives obtaining this goal may be threatened by physical, psychological, and social difficulties. Music therapy ran be utilized to effectively fill in these physical, psychological, and social needs. In the form of a creative art, music therapy can be effective in easing the pain and tension as well as assisting the patient to discover meanings of life through psychological comfort. Further, music is itself a verbal/non-verbal communication tool and thus connects the patients with their families with understanding and sympathy. This study aims to examine the need and role of music therapy in hospice.

Effect of Tai Chi Exercise Program on Muscle Strength, Grip Strength, Flexibility, Pain, Depression and Self-efficacy in Patients with Osteoarthritis (골관절염 환자에게 적용한 타이치운동이 하지근력, 악력, 유연성, 통증, 우울 및 자기효능감에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of Tai Chi exercise on muscle strength, grip strength, flexibility, pain, depression and self-efficacy in patients with osteoarthritis. Method: With a quasi-experimental design, 46 persons with osteoarthritis were assigned into an experimental group (n=22) or a control group (n=24). Muscle strength, grip strength, flexibility, pain, depression and self-efficacy were measured both before and after the exercise. Data were analyzed with SPSS WIN using descriptive statistics, t-test, and ANCOVA. Results: The mean age of the subjects was 68 years in the experimental group and 64 years in the control group. After controlling for age and pre-test outcome variables, muscle strength scores (F=-14.8, p= .00), grip strength scores (Rt: F=8.40, p= .01), and self-efficacy (F=47.3, p= .00) improved more in the experimental group than the control group. Joint pain scores (F=15.0, p= .00) and depression scores (F=14.7, p= .00) decreased in the experimental group. Conclusion: Tai Chi exercise is likely to be an effective nursing intervention to improve muscle strength, grip strength, and self-efficacy and reduce pain and depression for patients with osteoarthritis.
