Study on the Dental Attrition of the Patients with Craniomandibular Disorders using Age Estimation (연령감정을 이용한 두개하악장애환자의 치아 교모도에 관한 연구)

  • 두개하악장애는 저작근, 측두하악관절 및 그와 관련된 구조물의 많은 임상적인 문제를 포함하는 포괄적인 용어이다. 두개하악장애의 원인은 다양하고 종종 다인성이며 위험을 증가시키는 여러가지 소인들이 존재한다. 이갈이나 이악물기 같은 이상기능습관은 흔하며 또한 임상적으로 많은 관련이 있는 것으로 사료된다. 그러나 이전의 연구에서는 연령증가에 따른 기능적 마모를 고려한 치아교모에 관한 연구가 시행되지 않고 단순한 교모도의 평가비교만이 시행되었다. 본 연구는 법치의학적 연령감정에서 널리 사용되고 있느 치아교모도를 이용한 연령감정법을 이용하여 두개하악장애환자의 치아교모도에 관한 연구를 시행하였다. 연령은 기능적마모에 따른 정도를 평가하기 위해서 고려되었다. 단국대학교 치과병원 구강내과에서 두개하악장애로 진단된 환자18명과 대조군으로 외래환자 14명을 대상으로 하였다. 연령차는 연령감정법의 추정연령과 환자의 실제나이의 차이를 구해서 비교분석하였다. 이상의 결과로 두개하악장애환자의 중절치, 측절치, 제1소구치, 제2소구치의 치아교모도가 정상군보다 유의성 있게 높았다.

  • Dental statistics for Koreans are far from complete and the majority of previous researches have adopted techniques such as radiological analysis and sectioning of teeth for morphological information, which are time-consuming, less accurate and destructive. Thus, a new nondestructive method is necessary to get precise dental standardization data for Koreans. For the above purpose, each of the first premolars was scanned by a micro-CT (SkyScan, Belgium) with a resolution of $21.31{\mu}m$ at an interval of 0.022mm along the plane horizontally parallel to an occlusion plane. Internal and external morphological sections were measured and compared to the values in the average tooth size table for permanent teeth presented by G. V Black.

Effect of maxillary premolar extraction on transverse arch dimension in Class III surgical-orthodontic treatment (III급 부정교합의 수술-교정 치료시 상악 소구치 발치가 치열궁 폭경 변화에 미치는 영향)

  • Collective changes caused by orthodontic tooth movement evaluated in a specific treatment modality could give suggestive information on the specific treatment strategy. The aim of this study was to investigate retrospectively the characteristics of the orthodontic tooth movement during surgical-orthodontic treatment in order to provide an effective presurgical orthodontic treatment planning for the maxillary premolar extraction modality In the skeletal Class III malocclusion patient. Pre- and post-treatment dental casts of skeletal Class III malocclusion patients with nonextraction (N=:24) and the maxillary premolar extraction (N=31) were collected. The angulation and inclination measuring gauge(Invisitech Co. Seoul, Korea) was used to evaluate the orthodontic tooth movement. The changes in the maxillary and mandibular dental arch widths were also measured from the canines to the second molars. As a result, more palatal inclination change in the maxillary dentition was found with the premolar extraction modality than with the nonextraction modality. Linear regression analysis showed that the inter-arch width coordination was mainly due to the inclination changes of maxillary posterior teeth We conclude that the indications and proper treatment planning for surgical-orthodontic treatment in skeletal Class III malocclusion with maxillary premolar extraction could depend partly on the magnitude of the transverse inter-arch coordination especially in the maxillary dentition.

A STUDY OF TOOTH NUMBER ANOMALY USING PANORAMIC RADIOGRAPHS (파노라마 X선사진을 이용한 치아수 이상에 관한 연구)

  • The purpose of this study was to evaluate the prevalence and distribution of tooth number anomaly by means of the analysis of panoramic radiographs in 6,531 patients visited the Dental Infirmary of Kyungpook National University Hospital from January 1983 to May 1992. The results were as follows: 1. The prevalence of congenitally missing teeth except third molar was revealed to be 10.8%, and there was a hihger prevalence in females(44.6%) than in males(55.4%). Mandibular 2nd premolars(23.2 %) were absent most frequently, followed by maxillary lateral incisors(18.4 %), mandibular lateral incisors(18.3%), and maxillary second premolars(15.4 %) in descending order of frequency. As to the number of congenitally missing teeth, the percentage of missing one tooth was 48%, missing two teeth was 35.4 %, missing three teeth was 6.6%. 2. he prevalence of congenitally missing third molars was revealed to be 39.7%. There was a higher prevalence n the maxilla(60.3%) than in the mandible(39.7%). Maxillary right 3rd molars(30.6%) were absent most frequently, followed by maxillary left 3rd molar(29.7%), mandibular right 3rd molar(202%), mandibular left 3rd molar(19.5%) in descending order of frequency. 3. The prevalence of supernumerary teeth was revealed to be 4.2%, and there was a higher prevalence in males(65.7%) than in females(34.3 ). They were ound most frequently in maxillary central incisor area(64.8%), followed by maxillary lateral incisor area(132%), posterior area of maxillary third molar(8.7%) in descending order of frequency. As to the number of supernumerary teeth; The percentage of one supernumerary tooth was 79.9%, two supernumerary teeth was 8.9%, three supernumerary teeth was 1.2 %.

ECTOPIC ERUPTION OF MANDIBULAR FIRST PERMANENT MOLAR : A CASE REPORT (하악 제1대구치 이소맹출의 치험례)

  • Ectopic eruption is caused by an abnormal direction of eruptive path, most common in maxillary first molar, mandibular lateral incisor, and maxillary canine, and sometimes mandibular first molar. Ectopic eruption of first molar leads to abnormal root resorption of second deciduous molar, which, if left untreated, could cause premature loss of second deciduous molar; mesial tilting and rotation of first permanent molar; lack of space for eruption of second premolar; and occlusal problems. Therefore early treatment is advised when diagnosed as ectopic eruption. Treatment of ectopic eruption in the first permanent molar involves providing proper guidance for the direction of eruption using interproximal wedging and distal tipping methods while preserving second deciduous molar. This case report shows satisfactory results of the ectopic eruption of mandibular first molars in young patients who were treated with Humphrey appliance and Halterman appliance.

TREATMENT OF RADICULAR CYST USING DECOMPRESSION (감압술을 이용한 치근단 낭의 처치)

  • Radicular cyst is a true epithelium lined cyst formed when epithelium at the apex of a nonvital tooth is stimulated by inflammation. It is the most common type of cyst in the oral cavity, but its frequency is low in the primary dentition. Treatment of large-sized cyst aims at conservation of adjacent structure and allowance of proper eruption of the successive permanent teeth in the primary dentition. Considering these two aspects, marsupialization or enucleation following decompression is recommended as a treatment means for large-sized radicular cyst. In this case, 8-year old boy visited the pediatric dentistry department. Yonsei University Dental Hospital, with the chief complaint of pain on the lower right area. Clinical and radiographic examinations revealed periapical radiolucent lesion on #84, which had previous pulp treatment and restorated with the stainless steel crown. Dislocation of its successive tooth, #44, was also observed. #84 was extracted and sent for biopsy. Through histologic examination, it was diagnosed as radicular cyst. Following its extraction, removable space maintainer was delivered, which was also used as a decompressor. During periodic check-up for next 22 months, favorable healing of the lesion and eruption of the successive tooth were observed.

Study of horizontal skeletal pattern and dental arch in skeletal Class III malocclusion patients (골격성 III급 부정교합자의 횡적인 골격과 악궁 형태에 관한 연구)

  • Objective: The purpose of this study was to investigate the horizontal skeletal pattern and dental arch differences between Class III malocclusion patients and normal occlusion patients. Methods: Twenty skeletal Class III malocclusion patients and ten normal occlusion patients were selected and 3D facial CT were taken to analyze the horizontal skeletal differences between the two groups. Results: In the horizontal comparison of the maxilla, skeletal width and perimeter were significantly smaller in skeletal Class III patients on ANS and A point reference planes. The difference between maxillary width of ANS and A point reference planes showed that there was greater constriction of the first and second premolar in skeletal Class III patients. In the horizontal comparison of the mandible, the widths of the canine and premolar area were significantly larger in skeletal Class III patients on B point reference plane. The differences between width of the upper and lower jaws (comparison of A and B reference planes) were significantly large in the canine and premolar area. Conclusions: From this study, the characteristics of Class III malocclusion patients were shown through horizontal constriction of the maxilla. But to make clear further detailed characteristics of Class III malocclusion patients, additional studies are necessary.

A Survey Research on the Actual Condition for Intra-oral Treatment in Patients with a Visit to Dental Hospital in K Region (K지역 치과 병원에 내원한 일반 성인의 구강 내 치료 상태 실태조사)

  • This study carried out the survey of the actual condition targeting 303 patients with execution of treatment after medical examination among patients who visited from January 2, 2008 to December 31, 2008 at dental hospital where is located in K region, and analyzed by using a program of SPSS13.0. The ratio of treatment in posterior was high with 76.9% in central incisor, 62.3% in the 1st bicuspids, and 45.3% in the first permanent molars as for health in the right teeth of maxillary, and with 77.9% in central incisor, 64.4% in the 1st bicuspids, and 47.6% in the first permanent molars as for the left teeth. In the lower jaw, the ratio of treatment in posterior was high with 92.1% in central incisor, 73.9% in the 1st bicuspids, and 42.6% in the first permanent molars as for health in the right teeth, and with 92.1% in central incisor, 75.9% in the 1st bicuspids, and 43.2% in the first permanent molars as for the left teeth. The upper left cuspid was treated in older patients by age(${\chi}^2=32.33$, p=0.000), in the more patients with high blood pressure(${\chi}^2=12.60$, p=0.000), and in case of systemic disease. The significant difference was shown(${\chi}^2=6.37$, p=0.012). The older patients led to being treated cuspid among right teeth in the lower jaw. The significant difference was shown according in the more patients with high blood pressure(${\chi}^2=8.26$, p=0.004). The left cuspid in the lower jaw was treated in the older patients by age the statistically significant difference was shown(${\chi}^2=12.10$, p=0.007).

Mixed dentition analysis using a multivariate approach (다변량 기법을 이용한 혼합치열기 분석법)

  • Objective: To develop a mixed dentition analysis method in consideration of the normal variation of tooth sizes. Methods: According to the tooth-size of the maxillary central incisor, maxillary 1st molar, mandibular central incisor, mandibular lateral incisor, and mandibular 1st molar, 307 normal occlusion subjects were clustered into the smaller and larger tooth-size groups. Multiple regression analyses were then performed to predict the sizes of the canine and premolars for the 2 groups and both genders separately. For a cross validation dataset, 504 malocclusion patients were assigned into the 2 groups. Then multiple regression equations were applied. Results: Our results show that the maximum errors of the predicted space for the canine, 1st and 2nd premolars were 0.71 and 0.82 mm residual standard deviation for the normal occlusion and malocclusion groups, respectively. For malocclusion patients, the prediction errors did not imply a statistically significant difference depending on the types of malocclusion nor the types of tooth-size groups. The frequency of prediction error more than 1 mm and 2 mm were 17.3% and 1.8%, respectively. The overall prediction accuracy was dramatically improved in this study compared to that of previous studies. Conclusions: The computer aided calculation method used in this study appeared to be more efficient.
