Mini-implant with additional retentive structure by using digital method (부가적인 유지구조를 가진 미니 임플란트의 디지털 수복 증례)

  • Mini-implant has been reported as an alternative method to conventional diameter implants in the treatment of mandibular anterior region with narrow ridge. However, one-piece type mini-implant showed biological and technical complications, such as peri-implantitis by residual cement and prosthetic detachment. Recently, one-piece type mini-implant with additional retentive structure has been introduced to increase the retention and stability of the mini-implant. The mini-implant is based on spreading the upper retentive structure with a spreader and connected it with the inner surface of the zirconia prosthesis. In two cases, we tried to reduce these complications through the mini-implant on the narrow mandibular anterior region, and it was possible to reduce the overall treatment period through a digital system. In this case report, restoration using the digital mini-implant system showed improvement of patient satisfaction on the aspect of function and esthetic in the narrow mandibular anterior region.

  • TMJ Osteoarthritis is a degenerative disease caused by overload on joint tissue, and often accompany with local tenderness on the joint, crepitus by mandibular movement, restriction of mandibular movement, and anterior openbite. In general, panoramic radiography, TMJ panoramic radiography, and transcranial radiography are conducted to diagnose osteoarthritis after clinical examination, however, these radiographic evaluations are limited in detecting minute bony changes of early pathologic lesion. The aim of this pilot study was to evaluate the limitation and usefulness of several TMJ imaging techniques, so we compare the findings from clinical examination, plain film radiographs, tomograph, and bone scan from 81 patients with temporomandibular joint disorder. The proportion of patients showing same findings in plain film radiographs, TMJ tomograph, and bone scan was high, however, it is desirable that clinician should conduct phased imaging examinations according to the clinical findings due to the possibilities of false negative findings in diagnosis of osteoarthritis.

Supernumerary teeth in the mandibular incisor region associated with fused primary teeth : two case reports (유치의 융합과 연관된 하악 전치부의 과잉치 : 증례 보고)

  • Odontogenic anomalies can occur as a result of conjoining defects. These include fusion, gemination and concrescence. The presence of fused teeth can lead to various clinical problems, including the aplasia of permanent successor, the supernumerary teeth and delayed eruption of permanent teeth. In general, the supernumerary teeth associated with fusion is mainly found in the maxillary anterior region. This report describes rare cases of supernumerary tooth associated with fused teeth of mandibular primary lateral incisor and canine. In the first case, fused teeth was extracted. The supernumerary teeth was erupted at canine space. The second case is still being observed. Extraction of fused teeth and observation of supernumerary teeth is planned.

  • Our Goal of modified segmental osteotomy on maxilla accompanied by mandibular anterior subapical osteotomy was to get the best results from patients of bi-alveolar or bi-maxillary protrusion. In this study, cephalometric and photo (en face, profile) analysis have been compared to define the soft tissue change after surgery. Lip protrusion was efficiently reduced and nasolabial angle was much obtused. Although nose was a little widened, it was quite acceptable. Blood circulation on osteotomized segment was well maintained. This relatively simple methods could be successfully applied to many patients.

  • Adenomatoid odontogenic tumor (AOT) is a tumor of odontogenic epithelium with varying degrees of inductive changes in the connective tissue. The common radiographic appearance of AOT is a unilocular radiolucency associated with an unerupted tooth. Detectable radiopacities have been reported in many cases. We present a case of AOT in a 9-year old-female patient. Cystic lesion with numerous, punctuate radiopaque foci was observed on the anterior region of the mandible. These radiopacities were situated mostly on the buccal side of impacted tooth on the multi-planar images of cone beam computed tomograph. Characteristic duct like structures and amyloid like material were observed on histopathologic finding. (Korean J Oral Maxillofac Radiol2009; 39 : 99-102)

Gingival recession of lower anterior incisors in orthodontic treatment (교정치료시 하악 전치부 치은퇴축의 고려)

  • Purpose: These case reports show the orthodontic treatment of lower anterior incisors with gingival recession. Materials and Methods: Three cases were treated by an orthodontist and a periodontist. Each case had lingually tilted lower anterior incisors, anterior crossbite and skeletal Cl III pattern. Results: A variety of etiological factors were thought to cause gingival recession: aging, oral hygiene, tooth malpositioning, occlusal trauma. Conclusion: Due to the interaction among many possible contributing factors, it is difficult to predict whether further gingival recession may occur at a given site. The position and the movement of the lower anterior incisors with gingival recession are important factors in diagnosis and orthodontic treatment planning.

  • 저작은 치아, 악골, 저작근 뿐만 아니라 근 신경계, 고위 중추까지 복합적으로 관여되는 기능적 행위이다. 저작 형태는 다양한 모양을 가지나 두 가지 전형적인 군, 즉, 전방에서 관찰 시 그 양상이 수직적이며 절단 (chopping) 운동을 하는 군과 주로 측방으로 이루어지며 연마 (grinding)를 하는 군으로 나눌 수 있다. 본 연구의 목적은 저작 형태의 차이가 교합면 마모에 미치는 영향을 조사하는 것이다. 두개 하악 관절과 저작 습관에 이상이 없으며 교합면에 수복물이 없는 치과 대학생으로 하악 운동 궤적 기록기를 이용하여 상기의 전형적인 2가지 저작 형태를 보이는 각 15명씩을 피검자로 선택하였다. 각 피검자에 대한 임상 검사를 통해 ordinal scale로 교합면 마모의 등급을 조사하여, 평균 치아 마모도와 부위에 따른 치아 마모도를 비교 조사하였다. 각 피검자에 대한 인상 채득 후 모형을 제작하고 arbitrary scale로 교합면 마모의 등급을 조사하여, 평균 치아 마모도와 부위에 따른 치아 마모도를 비교 조사하고 저작측과 비저작측 마모를 비교 조사하였으며 수평 마모면과 수직 마모면을 비교 조사하였다. 1. 평균 치아 마모도는 ordinal scale로 측정하였을 때, 절단형과 연마형간에 유의할 만한 차이가 없었다.(p >0.05) 2. 부위에 따른 치아의 마모도는 ordinal scale로 측정하였을 때, 절단형과 연마형간에 유의할 만한 차이가 없었다.(p >0.05) 3. 평균 치아 마모도는 arbitrary scale로 측정하였을 때, 절단형에 비교하여 연마형에서 높은 마모도를 보였다.(p<0.05). 4. 절단형에 비교하여 연마형은 arbitrary scale로 측정하였을 때, 구치부에서는 높은 마모도를 보였으며 전치부에서는 유의할 만한 차이가 없었다(p<0.05) 5. 구치 평균 치아 마모도와 부위에 따른 치아의 마모도는 균형측에서 절단형과 연마형간에 유의할 만한 차이를 보이지 않았다(p>0.05). 6. 전치 평균 치아 마모도와 부위에 따른 치아의 마모도는 수평 마모면과 수직 마모면 비교시, 절단형과 연마형간에 유의할 만한 차이를 보이지 않았다.(p>0.05)

  • The radicular cyst is the most common odontogenic cyst which is caused by pulpal inflammation, pulp death, and secondary to trauma or dental caries. Usually, the radicular cyst is asymptomatic, but a secondary inflammation can cause pain, swelling and redness. Getting larger, the radicular cyst can cause facial asymmetry and paresthesia by pressure on nerves. It requires conservative endodontic treatment or surgical approach. When the size of cyst is large or invasion of the adjacent tissue is not expected, cyst enucleation is carried out. And most of the case can be completely cured and shows low recurrence. In these radicular cysts cases, by cyst enucleation or apicoectomy after root canal treatment simultaneously, the infected teeth can be preserved successfully.

  • As patients' esthetic expectations increase, there is an increase in demand for cosmetic restorations of anterior and posterior teeth that resemble the natural tooth morphology and color. An example of high-strength all-ceramic systems is zirconia with CAD/CAM application. This case report describes a full-mouth rehabilitation using zirconia all-ceramic crowns supported by upper and lower implants and by a minimal increase in the occlusal vertical dimension in a patient with severe abrasion due to loss of posterior teeth.

  • A proper vertical dimension and a harmonious occlusal plane are essential to satisfy a patient esthetically and functionally. A maxillomandibular occlusal vertical dimension is determined by the elevators which repeatedly contracts to a certain length, and a tooth location is determined by a maxillomandibular vertical dimension. The patient of this case came in with the incongruity of the lips and the occlusal plane. The result of clinical test showed the lack of length of the lower anterior due to the reduction of vertical dimension, the deep overbite of anterior, the excessive attrition of anterior, and the incongruity of occlusal plane. After the diagnostic wax-up, the temporary restoration was installed, and final prosthesis was installed after 6 months. As a result, the patient obtained a functionally and esthetically satisfying result.
