A Case of Prismatic Correction for Cyclovertical Heterophoria (회선수직사위의 프리즘 교정 증례)

  • Purpose: The purpose of this case study is to prescribe a prismatic correction in uncommon case of cyclovertical heterophoria. Methods: The prescribed prism was used to balance the vertical vergence break values by the binocular vision evaluation such as phoria, vergence, accommodative function and the others. Results: Although asymptomatic lateral phoria was changed, the vertical prismatic correction improved the symptoms such as eyestrain, headaches, diplopia and others. And cyclophoria was disappeared by prismatic correction. Conclusions: The prismatic correction for this case was useful for alleviating symptoms of cyclovertical deviations.

Subjective Visuoperception to Vertical Yoked Prisms (수직동향프리즘의 자각적 시감각에 관한 연구)

  • Purpose: Even refractive error is perfectly corrected by glasses power, the glasses wearer can feel imbalance and uncomfortable by prism effects. The purpose of this study was to investigate subjective imbalance to vertical yoked prism in visually normal subjects. Methods: Visually normal 37 subjects (aged 20 to 31 y) were fully corrected by soft contact lens and worn vertical yoked prism, base up and base down 1, 2, 4, 6, 8 prism diopter(pd) at random order. A rating scale questionnaire was administered to assess quantitatively subjective imbalance to the yoked prism. The near phoria tests were done using Howell-Kim phoria card at 40 cm distance. Results: For the subjective response of imbalance, base up yoked prism was higher than base down yoked prism (t-test: t=4.67, p=0.00) in over 2 prism diopters. The frequency of subjects who feel imbalance by base up vertical yoked prism is higher for near esophoric group than for exophoric group. Conclusions: To reduce subjective imbalance caused by glasses such as dizzy, it needs to make the minimum prism effect, and base down yoked prism is more effective than base up yoked prism in prism effects.

Clinical Evaluation of Prism-stabilized Toric Soft Contact Lens Fit (프리즘 안정화 디자인 토릭 소프트 콘택트렌즈 피팅에 대한 임상 평가)

  • Purpose: This study was to evaluate the clinical performance of 1-Day disposable toric soft contact lenses (TSCL) and to compare the visual performance with glasses and TSCL. Methods: In a two-week investigation of 'prism ballast' designed TSCL, occular refractions in 15 (30 eyes) healthy subjects were within the range of -2.75 D TO -8.00 D spherical power and -0.75 DC to -2.25 DC cylinderical power (with-the-rule). Subjects were assessed the clinical performance (orientation, centration, movement, tightness, rotation recovery, symptoms) of TSCL and compared binocular vision and contrast sensitivity with corrected glasses and TSCL at initial and two-week follow-up visits. Results: The prism-ballast design TSCL achieved better orientation, rotation recovery and symptoms over 2 week period. There was no statistically significant difference between spectacles and TSCL with respect to measuring binocular vision and contrast sensitivity. Conclusions: The clinical performance was good with prism-ballast design toric soft contact lenses. Evaluation of binocular vision and contrast sensitivity did not show the differences between spectacles and TSCL correction. This study suggests that TSCL wearers can be achieved similar visual performance for spectacles wearers.

Induced Prism by the Categories of Spectacle Frames (안경테 종류에 따른 유발프리즘)

  • Purpose: One of the critical aspects on dispensing glasses is to match the center of pupils to the optical center of lenses as the mismatched glasses are able to induce uncomfortable effects called prism which has been known to induce phoria, a main cause for asthenopia in many cases. Therefore, we investigated the induced prism occurred by mismatching centers between the center of pupils and the optical center of lenses. Methods: In this study, total 103 subjects were examined whether the center of pupils and the optical center of lenses are matched in horizontal and vertical directions, and then, the data was categorized into 4 groups based on the structural components of glasses. Total amount of prism was compared to show the effect of the glasses frame on the prism induction, and the value of measured prism was compared with the German RAL-RG 915 regulations. Results: The results in respect to the horizontal component showed that the induced prism was not found in 10.7% of total subjects. 73.8% of total subjects were influenced by induced prism, the range of prism was in a tolerance level. However, the 15.5% of total subjects seemed to be influenced by prism which is out of the criterion of tolerance. In case of vertical component, 23.3% of total subjects showed no effects of prism while early adopting glasses, 54.4% of total subjects showed a little prism effect within the criterion of tolerance, and 22.3% of total subjects showed the prism effect out of the tolerance range. This data indicates that group A and B that are less likely adjustable by fitting induce more prism than group C and D in horizontal and vertical components. Conclusions: In higher refractive error condition, it was found that aligning the optical center of lenses with the center of pupils by pre-fitting of glasses frame minimized prism induction in horizontal and vertical components, which ameliorates ocular fatigue. Therefore, appropriate optometric dispensing through fitting by opticians and precise design about monocular PD and monocular Oh are necessary to improve visual perception.

A study on the measurement and tendency of heterophoria using Von Graefe test and Maddox rod test (프리즘 분리법과 마독스 로드 검사법을 이용한 사위측정과 경향 연구)

  • To investigate the difference in the value of horizontal heterophoria measured in phoria test of about normal 72 college students, using both Von Graefe test and Maddox rod test. We used two different methods which are Von Graefe test and Maddox rod test on full correction condition at the long distance. Using Von Graefe test of horizontal heterophoria Measurement, We measured 21 patients(29%) for orthophoria, 36 patients(50%) for exophoria and 15 patients(21%) for esophoria at distance. Using Maddox rod test of horizontal heterophoria Measurement, We measured 36 patients(50%) for orthophoria, 26 patients(36%) for exophoria and 10 patients(14%) for esophoria at distance. Each test average were $0.93{\Delta}$ BI for Von Graefe test, $0.96{\Delta}$ BI for Maddox rod test. We could not find the definite difference about the value of horizontal heterophoria aby testing method. Furthermore, heterphoria AC/A ratio was found to vary from 1.0 to 8.8 and its relationship to refractive error could not be determined.

Research on Visual Function Before and After Prescription of Vertical Prism (수직사위 교정 전·후 시기능 변화에 대한 연구)

  • Purpose: This study was undertaken to study on visual function before and after prescription of vertical prism. Methods: 80 subjects (10-30ages) who had been precribed vertical prism and 1 year after they were represcribed vertical prism. Results: Phoria (p=0.000), supra vergence (p=0.000), NPC (p=0.003), NPA (right eye) (p=0.005), NPA(left eye) (p=0.000) showed a significant change. Conclusion: Vertical prism prescription was reduced during revisit. Supra vergence, NPC, NPA(right eye), NPA(left eye) showed a significant change. Binocular Accommodative lag was not associated with vertical prism prescription and binocular vision findings.

Phoria Changes Caused by Lens Adaptation after Wearing of Horizontal Prism Lens and Spherical Lens (수평 프리즘렌즈와 구면렌즈 착용 후 렌즈적응에 의한 사위도의 변화)

  • Purpose: To investigate an individual phoria compensating effect by measuring the lens adaptation after wearing horizontal prism or spherical lenses at near. Methods: 103 subjects (63 males and 40 females) aged $22.43{\pm}2.07years$ participated for this study. Refractive errors of subjects were fully corrected, and subjects were classified into three groups; esophoria, orthophoria and exophoria. The adaptation test of prism and spherical lenses was performed immediately, 15 minutes and 30 minutes after wearing lenses with $OU\;2\;{\Delta}\;BO$ and S+1.00 D for an esophoria group and $OU\;2\;{\Delta}\;BI$ and S-1.00 D for orthophoria and exophoria groups, respectively. Each measured phoria was compared to phoria measured with fully corrected condition, and the phoria changing effects about both lenses was analyzed. Results: The mean of phoria by prism lenses significantly decreased for every group. The mean of phoria by spherical lenses also significantly decreased in orthophoria and exophoria groups. The esophoria group showed a decreasing tendency but has no significance. The decreased phoria value by prism lens was classified into three steps; 90~100%, 0~10% and 50% or more. According to results of individual distribution ratio, it was 5, 64 and 24 subjects at 30 minutes after wearing lenses, respectively. The decreased phoria value by spherical lens was classified into three steps; 90~100%, 0~10% and 50% or more. According to results of individual distribution ratio, it was 37, 53 and 41 subjects at 30 minutes after wearing lenses, respectively. The subjects having no phoria changed by both prism and spherical lenses were 37.9%. Conclusions: When prescribing to compensate the near horizontal phoria using prism or spherical lens, the pretest for lens adaptation should be taken before prescription.

Effect of Vision Training using Fresnel Prism Lens on Fusional Vergence and Accommodation (프레넬 프리즘렌즈를 이용한 시기능 훈련이 조절과 융합버전스에 미치는 영향)

  • Purpose: Of the various methods of vision training, the essay aims to explore the effective ways of using the Fresnel prism lens in order to expand the positive fusional vergence for the patient having specific condition of convergence insufficiency or basic exophoria. Methods: 15 students of city of Daejeon university without an eye disease (average age $22.73{\pm}1.68$) were selected and underwent the subjective refraction test and binocular vision test, and recording their test results before vision training and replacing an identical frame with a lens of same quality after the full calibration, the lens was then adhered with the Fresnel prism lens and continued to train for thirty minutes daily during two weeks. Afterwards, the binocular vision test was reattempted. The observation of the change in the results of the binocular vision test in use of the fresnel prism lens in the vision training test was researched. Results: After training, the positive fusional vergence had increased to a number of $22.27{\pm}2.26$ $\Delta$, to 7.80 $\Delta$, at near, the fused cross cylinder test increased to an average of $0.55{\pm}0.09$ D, 0.40 D after training, showing a normal result. The value of negative relative accommodation after training had an average of $2.22{\pm}0.08$D, showing that 0.42 D had increased. The value of near point of convergence after training had an average of $6.13{\pm}0.53$ cm, showing that 2.80 cm had decreased. To patients who had convergence insufficiency or basic exophoria, the value of the near vision test that used the Fresnel prism lens which was able to expand BO positive fusional vergence had increased without phoria. Conclusions: The changes were tested and the effectiveness of the Fresnel prism lens, due to the nature of the lens itself, helped with both cosmetic effects and cost. It also allows good optical correction effects, in addition to these clinical effects indicated before. Therefore, it may be determined that the Fresnel prism lens binocular vision therapy for patients is more popular and highly recommended.

Analysis of far & near distance of lateral phoria by various testing methods (검사 방법에 따른 원·근거리 수평사위의 분석)

  • This Study on three different methods which are Von Graefe test, Maddox Rod test and Howell card test of measuring far and near lateral phoria investigated. It reviewed that 420 subjects aged from 15 to 35 years old. The result were as follows; 1. Using Von Graefe lateral phoria measurement, they were measured 6% for orthophoria, 62% for exophoria, 32% for esophoria at far distance. 2. Using Von Graefe lateral phoria measurement, they were measured 2% for orthophoria, 74% for exophoria, 24% for esophoria at near distance. 3. Using Maddox Rod lateral phoria measurement, they 59% for exophoria, 37% for esophoria at far distance. 4. Using Maddox Rod lateral phoria measurement, they 67% for exophoria, 30% for esophoria at near distance. 5. Using Howell card lateral phoria measurement, they 60% for exophoria, 32% for esophoria at far distance. 6. Using Howell card lateral phoria measurement, they 69% for exophoria, 27% for esophoria at near distance.

  • To investigate the subjective refractive error with Von Graefe test. Maddox Rod test and polarizing lens test of Horizontal Heterophoria Measurement at distance on normal 109 subjects aged from 18 to 30 years old. The results were as follows: 1. Using Von Graefe Horizontal Heterophoria Measurement, they were measured 15% for orthophoria, 65% for exophoria and 20% for esophoria at distance. 2. Using Maddox Rod Horizontal Heterophoria Measurement, they were measured 14% for orthophoria, 57% for exophoria and 29% for esophoria at distance. 3. Using polarizing lens test of Horizontal Heterophoria Measurement, they were measured 15% for orthophoria, 72% for exophoria and 13% for esophoria at distance. 4. Each test average were $1.32{\Delta}BI$ for Von Graefe test, $0.88{\Delta}BI$ for Maddox Rod test and $1.76{\Delta}BI$ for polarizing lens test. 5. Patient's preference distribution were 44% for Von Graefe test, 28% for Maddox Rod test and 28% for polarizing lens test.
