Compare the Clinical Tissue Dose Distributions to the Derived from the Energy Spectrum of 15 MV X Rays Linear Accelerator by Using the Transmitted Dose of Lead Filter (연(鉛)필터의 투과선량을 이용한 15 MV X선의 에너지스펙트럼 결정과 조직선량 비교)

  • Recent radiotherapy dose planning system (RTPS) generally adapted the kernel beam using the convolution method for computation of tissue dose. To get a depth and profile dose in a given depth concerened a given photon beam, the energy spectrum was reconstructed from the attenuation dose of transmission of filter through iterative numerical analysis. The experiments were performed with 15 MV X rays (Oncor, Siemens) and ionization chamber (0.125 cc, PTW) for measurements of filter transmitted dose. The energy spectrum of 15MV X-rays was determined from attenuated dose of lead filter transmission from 0.51 cm to 8.04 cm with energy interval 0.25 MeV. In the results, the peak flux revealed at 3.75 MeV and mean energy of 15 MV X rays was 4.639 MeV in this experiments. The results of transmitted dose of lead filter showed within 0.6% in average but maximum 2.5% discrepancy in a 5 cm thickness of lead filter. Since the tissue dose is highly depend on the its energy, the lateral dose are delivered from the lateral spread of energy fluence through flattening filter shape as tangent 0.075 and 0.125 which showed 4.211 MeV and 3.906 MeV. In this experiments, analyzed the energy spectrum has applied to obtain the percent depth dose of RTPS (XiO, Version 4.3.1, CMS). The generated percent depth dose from $6{\times}6cm^2$ of field to $30{\times}30cm^2$ showed very close to that of experimental measurement within 1 % discrepancy in average. The computed dose profile were within 1% discrepancy to measurement in field size $10{\times}10cm$, however, the large field sizes were obtained within 2% uncertainty. The resulting algorithm produced x-ray spectrum that match both quality and quantity with small discrepancy in this experiments.

  • Purpose : To obtain the actual dose distribution from measured data by doconvolution method using the measured ion chamber response function. Materials and Methods : The chamber response functions for 2 ionization chambers (diameter 5mm, 6.4mm) were measured. and dose Profiles were measured for $10{\times}20cm^2$ field size using two different detectors. The deconvolution of chamber response function from the measured data were performed for these Profiles. The same procedures were repeated for 4MV, 6MV and 1 SMV photon energies. Results : Different dose Profiles were obtained for the same field with the chambers which have the different response functions. Nearly the same results could be obtained with deconvolution for the profiles from various detectors. Conclusion : The effect of the chamber response function can be extracted by deconvolution method. Deconvolved dose profile using various ionization chambers gave better dose distributions. Technical improvements are needed for practical application.

웹에 기반한 선량교정 프로그램 개발

  • 목적 : 선량측정의 정확성을 향상시키기 위하여 물 흡수선량 표준에 토대를 두고 있는 표준측정법, AAPM TG-51과 IAEA TRS-398, 이 발표되어 선량측정의 파라다임이 변화하고 있다. 본 연구에서는 이들 대표적인 표준측정법에 대하여 사용자 편의성을 고려하여 웹에 기반한 선량교정 프로그램을 개발하고자 한다. 대상 및 방법 : 미국의학물리학회 AAPM TG-51과 국제원자력기구 IAEA TRS-398 표준측정법에 선량교정 프로그램은 Microsoft IIS 6.0 웹서버와 .NET 플렛폼상에서 Visual Studio. NET 도구를 사용하여 개발하였다. 개발언어로는 C# 언어를 사용하였고 각 표준측정법에 대한 선량교정 작업서는 ASP. NET 페이지로 작성하였다. 웹페이지와 산량 교정 모듈을 분리하여서 개발 후 유지보수가 쉽게 설계하였다. 또한 기준점에서의 선량 계상에 사용하는 모든 물리적인 파라미터와 데이터는 데이터베이스에 저장하였다. 이로써 향후 수식체계의 변화 또는 물리적인 데이터의 변화로 인한 프로그램 수정이 최소화하도록 하였다. 결과 : 이들 표준측정법은 모두 물 흡수선량 교정인수에 토대를 두고 있으나 측정 조건 및 물리적인 자료 에 있어 약간의 차이를 보이고 있다. 그러므로 각 표준측정법간의 유사점 및 차이점을 비교 분석하기가 용이하였다. 그리고 개발된 프로그램을 이용하여 표준측정법에서 제시된 선량교정 작업서에 따라 선량 교정을 수행한 교정 결과 데이터를 XML 파일 형식으로 저장하여 이전의 측정 자료를 관리할 수 있게 하였다. 이 과거 측정 자료를 사용하여 출력 선량 교정의 변화 및 기타 중요한 물리적인 데이터 값의 변화를 분석할 수 있다. 결론 : 두 표준측정법에 대한 선량교정 프로그램은 사용자가 선호하는 표준측정법을 선택할 수 있고, 웹에 토대를 두고 있어 프로그램으로 전국 방사선종양학과의 방사선치료기기의 출력 및 물리적인 변화에 대한 자료를 비교 분석하기 용이하고 수작업으로 인해 발생할 수 있는 실수 및 오차를 줄일 수 있다. 또한 개발된 프로그램의 활용을 통하여 국내 실정에 적합한 물 흡수선량 표준에 기반한 표준측정법 개발에 토대를 마련하는데 있어 기여할 것으로 사료된다.

  • Purpose : To develop a practical film dosimetry system for routine Quality Assurance (QA). Materials and Methods :An One Click Film (OCF) Dosimetry system was designed to perform swift routine QA with functions including automatic fog value elimination, angle adjustment, automatic symmetry calculation, and realtime profile generation with the ability to display realtime three-dimensional dose distributions. Results : The most frequently used functions for routine QA, such as the elimination of the fog value, conversion into an H&D curve, symmetry, and isodose distribution, can be achieved with only one click. Conclusion : Reliable results were achieved with the OCF dosimetry with simpler steps than other commercially available film dosimetry systems for routine QA. More research on the refined user interface will make this system be clinically useful.

Commissionning of Dynamic Wedge Field Using Conventional Dosimetric Tools (선량 중첩 방식을 이용한 동적 배기 조사면의 특성 연구)

  • Purpose : To collect beam data for dynamic wedge fields using conventional measurement tools without the multi-detector system, such as the linear diode detectors or ionization chambers. Materials and Methods : The accelerator CL 2100 C/D has two photon energies of 6MV and 15MV with dynamic wedge an91es of 15o, 30o, 45o and 60o. Wedge transmission factors, percentage depth doses(PDD's) and dose Profiles were measured. The measurements for wedge transmission factors are performed for field sizes ranging from $4\times4cm^2\;to\;20\times20cm^2$ in 1-2cm steps. Various rectangular field sizes are also measured for each photon energy of 6MV and 15MV, with the combination of each dynamic wedge angle of 15o 30o. 45o and 60o. These factors are compared to the calculated wedge factors using STT(Segmented Treatment Table) value. PDD's are measured with the film and the chamber in water Phantom for fixed square field. Converting parameters for film data to chamber data could be obtained from this procedure. The PDD's for dynamic wedged fields could be obtained from film dosimetry by using the converting parameters without using ionization chamber. Dose profiles are obtained from interpolation and STT weighted superposition of data through selected asymmetric static field measurement using ionization chamber. Results : The measured values of wedge transmission factors show good agreement to the calculated values The wedge factors of rectangular fields for constant V-field were equal to those of square fields The differences between open fields' PDDs and those from dynamic fields are insignificant. Dose profiles from superposition method showed acceptable range of accuracy(maximum 2% error) when we compare to those from film dosimetry. Conclusion : The results from this superposition method showed that commissionning of dynamic wedge could be done with conventional dosimetric tools such as Point detector system and film dosimetry winthin maximum 2% error range of accuracy.

Development of Quality Assurance Software for $PRESAGE^{REU}$ Gel Dosimetry ($PRESAGE^{REU}$ 겔 선량계의 분석 및 정도 관리 도구 개발)

  • The aim of this study is to develop a new software tool for 3D dose verification using $PRESAGE^{REU}$ Gel dosimeter. The tool included following functions: importing 3D doses from treatment planning systems (TPS), importing 3D optical density (OD), converting ODs to doses, 3D registration between two volumetric data by translational and rotational transformations, and evaluation with 3D gamma index. To acquire correlation between ODs and doses, CT images of a $PRESAGE^{REU}$ Gel with cylindrical shape was acquired, and a volumetric modulated arc therapy (VMAT) plan was designed to give radiation doses from 1 Gy to 6 Gy to six disk-shaped virtual targets along z-axis. After the VMAT plan was delivered to the targets, 3D OD data were reconstructed from 512 projection data from $Vista^{TM}$ optical CT scanner (Modus Medical Devices Inc, Canada) per every 2 hours after irradiation. A curve for converting ODs to doses was derived by comparing TPS dose profile to OD profile along z-axis, and the 3D OD data were converted to the absorbed doses using the curve. Supra-linearity was observed between doses and ODs, and the ODs were decayed about 60% per 24 hours depending on their magnitudes. Measured doses from the $PRESAGE^{REU}$ Gel were well agreed with the TPS doses at central region, but large under-doses were observed at peripheral region at the cylindrical geometry. Gamma passing rate for 3D doses was 70.36% under the gamma criteria of 3% of dose difference and 3 mm of distance to agreement. The low passing rate was resulted from the mismatching of the refractive index between the PRESAGE gel and oil bath in the optical CT scanner. In conclusion, the developed software was useful for 3D dose verification from PRESAGE gel dosimetry, but further improvement of the Gel dosimetry system were required.

Evaluation of Beam Modeling Using Collapsed Cone Convolution Algorithm for Dose Calculation in Radiation Treatment Planning System (방사선치료계획시스템의 Collapsed Cone Convolution 선량계산 알고리듬을 이용한 빔 모델링의 정확성 평가)

  • This study aims to evaluate the accuracy of the collapsed cone convolution (CCC) algorithm for dose calculation in a treatment planning system (TPS), CorePLAN$^{TM}$. We implemented beam models for various setup conditions in TPS and calculated radiation dose using CCC algorithm for 6 MV and 15 MV photon beam in $50{\times}50{\times}50cm^3$ water phantom. Field sizes were $4{\times}4cm^2$, $6{\times}6cm^2$, $10{\times}10cm^2$, $20{\times}20cm^2$, $30{\times}30cm^2$ and $40{\times}40cm^2$ and each case was classified as open beam cases and wedged beam cases, respectively. Generated beam models were evaluated by comparing calculated data and measured data of percent depth dose (PDD) and lateral profile. As a result, PDD showed good agreement within approximately 2% in open beam cases and 3% in wedged beam cases except for build-up region and lateral profile also correspond within approximately 1% in field and 4% in penumbra region. On the other hand, the discrepancies were found approximately 4% in wedged beam cases. This study has demonstrated the accuracy of beam model-based CCC algorithm in CorePLAN$^{TM}$ and the most of results from this study were acceptable according to international standards. Although, the area with large dose difference shown in this study was not significant region in clinical field, the result of our study would open the possibility to apply CorePLAN$^{TM}$ into clinical field.

Simplistic QA for an Enhanced Dynamic Wedge using the Reversed Wedge Pair Method (역방향 조사방식을 통한 동적쐐기의 품질관리)

  • A simplistic quality assurance (QA) method was designed for a Linac built-in enhanced dynamic wedge (EDW), which can be utilized to make wedged beam distributions. For the purpose of implementing the EDW symmetry QA, a film dosimetry system, low speedy dosimetry film, film densitometer and 3D RTP system were used, and the films irradiated by means of a 60$^{\circ}$ Reversed wedge pair (REWP) method. The profiles were then analyzed in terms of their symmetries, including partial treatment, which is the case of stopping it abruptly during EDW irradiation, and the measured and calculated values compared using the Cad Plan Golden Segmented Treatment Table (Golden STT). The result of this experiment was in good agreement, within 1 %, of the 'reversed wedge pair counterbalance effect'. For the QA of the effective wedge factor (EWF), the authors measured EWFs in relation to the 10$^{\circ}$, 15$^{\circ}$, 20$^{\circ}$, 25$^{\circ}$, 30$^{\circ}$, 45$^{\circ}$ and 60$^{\circ}$ EDW, which were compared with the calculated values using the correction factor derived from the Golden STT and the log files produced automatically during the process of EDW irradiation. By means of this method it was capable of check up the safety of effective wedge factor without any other dosimetry system. The EDW QA was able to be completed within 1 hour from irradiation to analysis as a consequence of the simplified QA procedure, with maximized effectiveness. Unlike the metal wedge system, the EDW system was heavily dependent on the dose rates and jaw movements; therefore, its features could potentially cause inaccuracy. The frequent simplistic QA for the EDW is essential, and could secure against the flaw of dynamic treatment that uses the EDW.

  • DICOM (Digital Imaging and Communications in Medicine) standards are generally introduced as de facto and de jure standards in modern medical imaging devices to store and to transmit medical image information. DICOM Dose Structured Report (DICOM dose SR) is implemented to report radiation exposure information in image acquiring process. and DIOCM Modality Performed Procedure Step (DICOM MPPS) is also partly used to report this exposure with the information in its DICOM tag. This article is focused on three type of radiation exposure information of DICOM standards, 1) DICOM dose SR, 2) DICOM MPPS and 3) Radiation Exposure Monitoring(REM) profile by Integrating the Healthcare Enterprise(IHE), to study on radiation exposure reporting. Healthcare facility and its staff of medical imaging related to radiation exposure should have a deep understanding of radiation exposure, and it required a standards to enhance the quality control of medical imaging and the safety of patients and staffs. Staff member have to pay attention on radiation exposures and controling processes from the purchasing stage of X-ray devices.

Image Reconstruction of Sinogram Restoration using Inpainting method in Sparse View CT (Sparse view CT에서 inpainting 방법을 이용한 사이노그램 복원의 영상 재구성)

  • Sparse view CT has been widely used to reduce radiation dose to patient in radiation therapy. In this work, we performed sinogram restoration from sparse sampling data by using inpainting method for simulation and experiment. Sinogram restoration was performed in accordance with sampling angle and restoration method, and their results were validated with root mean square error (RMSE) and image profiles. Simulation and experiment are designed to fan beam scan for various projection angles. Sparse data in sinogram were restored by using linear interpolation and inpainting method. Then, the restored sinogram was reconstructed with filtered backprojection (FBP) algorithm. The results showed that RMSE and image profiles were depended on the projection angles and restoration method. Based on the simulation and experiment, we found that inpainting method could be improved for sinogram restoration in comparison to linear interpolation method for estimating RMSE and image profiles.
