The purpose of this study is to develop a critical pathway (CP) for the prevention of adverse reactions to contrast media for computed tomography. The CP was developed and implemented by a multidisciplinary group in Seoul National University Hospital. The CP was applied to CT patients. Patients who underwent CT scanning were included in the CP group from March in 2004. The satisfaction of the patients with CP was compared with non-CP groups. We also investigated the degree of satisfaction among the radiological technologists and nurses. The degree of patient satisfaction with the care process increased patient information(24%), prevention of adverse reactions to contrast media(19%), pre-cognitive effect of adverse reactions to contrast media(39%) and information degree of adverse reactions to contrast media(19%). This CP program can be used as one of the patient care tools for reducing the adverse reactions to contrast media and increasing the efficiency of care process in CT examination settings.

Critical pathway (CP) defines the optimal care process, sequencing and timing of intervention by multidisciplinary health care teams for a particular diagnosis and procedure. The aim of the study was to evaluate the clinical usefulness and the satisfaction of patients and dental staff after implementation of a critical pathway for the dental treatment of disabled children and adolescents under conscious sedation. Thirty patients are divided in two groups (Pre-CP and CP) at the department of Pediatric Dentistry of Pusan National University Dental Hospital. The satisfaction levels of patients/guardians and the staff members were collected by survey questionnaire. The parents' satisfaction was significantly improved after the implementation of CP. Also, medical/dental staff members were highly satisfied with its usefulness. The application of a critical pathway for disabled children and adolescents might be useful and improve the satisfaction of the parents and medical/dental staff members.

의료분야에서의 통신망 이용은 의료분야 전산화의 진전과 함께 계속 증가하고 있으며, 이의 일환으로 외국에서는 의료용 화상정보 표현, 전송 및 검색을 위한 표준 프로토콜인 DICOM을 이용한 PACS를 개발, 시험 또는 상용화 하고 있는 상황이다. 한편 국내에서는 이러한 PACS 시스템의 도입이 미미한 실정으로, 삼성의료원을 비롯한 몇몇 대형 의료기관에서 PACS의 일부를 도입, 운영하고 있으며, 보험청구 및 환자정보 관리를 중심으로 한 의료분야 전산화가 꾸준히 이루어지면서 많은 병원에서도 PACS 시스템의 도입 및 원격 진료 서비스 제공에 많은 관심을 가지고 있는 상황이다. 또한 이러한 의료용 화상정보 뿐만 아니라 일반적인 문자기반 의료정보의 표현, 전송 및 검색을 위한 표준 프로토콜인 HL7(health level 7)을 이용하여 의료보험 등과 같이 직접적인 의료행위 이외의 기타 부가정보를 의료기관 사이 또는 의료기관과 비의료기관 사이에서 공유하고 있다. 이러한 의료분야에서의 통신망 이용기술과 함께, 현재 고속 통신망에서 많이 개발되어지고 있는 화상회의 시스템의 발전에 기인하여 원격 진료 서비스에 관한 연구가 활발히 이루어지고 있는데, 이는 원거리에 위치한 의료기관간에 표준 프로토콜을 이용한 의료용 화상 또는 문자정보 공유가 가능해지고, 고속 통신망을 이용한 화상회의 및 긴급성을 가진 대용량 화상정보의 실시간 전송이 가능한 점에 기인한다. 본 고에서는 이러한 원격 진료 시스템의 발전 배경 및 전반적 시스템의 구조와 특성을 고찰하고, 국내외의 원격 진료 시스템 활용 현황에 관하여 기술한다.

In general, all physicians have some standardized diagnosis and treatment methods. However, there are differences in the precise order and examination depending on the hospital size, system, medical equipment, etc. To reduce this difference, the interest about standardized guidelines recently increased and a variety of research is being conducted. The uniform guideline cannot reflect the differences of each situation and environment to meet the hospitals. Therefore, standardized medical guidelines(=clinical pathway) should provide customized guidelines based on the relevant medical data to ensure the quality of the medical service and the doctor's autonomy. In this paper, we will analyze medical data made by two thyroid specialists in the same hospitals. Moreover, this paper mentions the implement of automatic generating clinical pathway system which consider its real hospital situation and result.

본 연구는 지역사회간호학회가 실시하는 전국의 보건진료원의 인력조사 연구의 일환으로 경상남도 지역의 전체 보건진료원 259명의 일반적 상황 및 업무수행정도를 파악하기 위하여 1986년 8월 25일부터 11월 25일 사이에 설문조사한 결과 자료수집이 가능한 195명의 설문지를 분석하였다. 대상자 195명의 진료원이 회신한 자료의 분석은 총점, 평균치, 표준편차, 백분율 및 범위로 분석하였으며 업무수행소요시간과의 관련은 Pearson상관계수로 분석한 연구 결과는 다음과 같다.
