최근 유헬스 산업을 꽃피우기 위한 노력이 다양한 형태로 전개되고 있지만, 국민의 건강 복지에 기여하는 효율성 제고와 새로운 고용 창출이라는 두 가지 큰 명제를 달성하기 위해 최소한 필수 국제표준은 채택(Adoption) 해야겠다는 업계의 의지가 부족한 것도 사실이다. 유헬스는 장비, 소프트웨어, 서비스가 다양한 형태의 결합으로 전개될 가능성이 아주 높고, 여러 이해 당사자들의 각기 다른 생각들 때문에 이해 관계가 상충하는 아주 복잡한 영역(Complex Domain)이므로, 표준 기술에 근거한 개발 노력을 경주해야 한다. 유헬스 서비스 전담 병원을 짓고, 필요한 장비, 소프트웨어, 서비스를 일체형으로 수출할 수 있는 기반을 닦기 위해서는 하루빨리 필수 표준기술을 확보하고, 표준기술 적합성(Conformance)을 검증하여 경쟁력 있는 유헬스 산업을 육성할 수 있도록 하는 경쟁력 있는 유헬스 산업을 육성할 수 있도록 하는 정책적 지원이 절실하다. 본고에서는 유헬스 산업 육성을 위한 필수 표준들과, 표준 구현 기술이 Conformant한지를 인증하고 있는 국제적 활동에 대해 기술하고자 한다.

Purpose: This study was performed to find the current problems of positron emission tomography/computed tomography(PET/CT) data on CD for inter-hospital transfer. Materials and Methods: The subjects were 746 consecutive $^{18}F$-fluorodeoxyglucose PET/CT data CDs from 56 hospitals referred to our department for image interpretation. The formats and contents of PET/CT data CDs were reviewed and the email questionnaire survey about this was performed. Results: PET/CT data CDs from 21 of 56 hospitals(37.5%) included all transaxial CT and PET images with DICOM standard format which were required for authentic interpretation. PET/CT data from the others included only secondary capture images or fusion PET/CT images. According to this survey, the main reason of limited PET/CT data on CD for inter-hospital transfer was that the data volume of PET/CT was too large to upload to the Picture Archiving and Communication System. Conclusion: The majority of hospitals provided limited PET/CT data on CD for inter-hospital transfer, which could be inadequate for accurate interpretation and clinical decision making. It is necessary to standardize the format of PET/CT data on CD for inter-hospital transfer including all transaxial CT and PET images with DICOM standard format.

수년 전부터 환자와 병원 간 의료정보 교환의 표준에 관한 연구가 국내외에서 꾸준히 진행되고 있으며, 앞으로도 활발한 연구가 진행되어 많은 산출물들이 도출될 것이다. 현재 대부분의 연구에서는 병원정보시스템(HIS)을 기반으로 병원 내 의료 장비들 간의 메시지 표준화를 위한 것으로 환자와 병원 간 정보 교환을 위해 스마트 기기, Open Service Gateway Initiative(OSGi) 플랫폼 등에서 오픈소스를 이용한 HL7 메시지 교환 시스템을 구성하는 연구는 아직 미비한 실정이다. 본 논문에서는 Open eHealth Integration Platform(IPF)을 이용해 안드로이드와 OSGi 플랫폼에서 환자와 병원 간의 HL7(Health Level Seven) 메시지의 교환, 검증 그리고 모니터링 위한 시스템을 구현하였다.

This study was a descriptive investigation to identify the influence of nurses' moral sensitivity and safety of their environment on compliance with standard precautions. Participants consisted of 214 nurses in one general hospital and data were collected using a questionnaire that consisted of moral sensitivity, safety of environment and compliance with standard precautions from 1-15 August 2017. Data were evaluated by the independent t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis. The mean scores of moral sensitivity, safety of environment and compliance with standard precautions were 5.05, 5.76 and 4.50, respectively. Moreover, compliance with standard precautions was significantly positively correlated with moral sensitivity and safety of environment. Additionally, multiple regression revealed an $R^2$ of 0.253 with the department (ER, ICU), age and safety environment serving as the major predictor variables for compliance with standard precautions. However, the influence of moral sensitivity on compliance was not identified. Therefore, to increase compliance with standard precautions among nurses, it is necessary to provide continuous educational programs for new nurses, customized programs considering departmental characteristics and administrative support of institutions to enhance safety of the environment.

It is needed to provide a supplicated and standardized training program for hospital coordinators to boost their competencies as professionals. The purpose of this study was to propose a standardized training program for hospital coordinators especially trained at private institutions. Using DACUM method, with 11 professionals, we first developed competencies required for hospital coordinators. They were service mind, attitude, MOT(Moment of Truth) & phone-call etiquette, communication skill, customer behavior, basic medical terminology, insurance, computer skills, etc. Finally, we proposed a standardized training program for hospital coordinators which covers 16 subject areas.

This study is to propose the characteristics that a desirable quality, especially usability, evaluation method of hospital information systems(HIS) should possess. This study reviews the literature recently published in the academic journals which are in the Science Citation Index or registered by the Korean Research Foundation. Especially, the International Journal of Medical Informatics was the valuable source, providing almost 20 literature. The results reveal the need to develop a domain-specific usability evaluation method that reflects the uniqueness of hospital industry. The results show user-centeredness and context-orientedness are the indispensible characteristics of a HIS quality evaluation method too.

본 논문에서는 향후 의료분야에서 활용하게 되는 표준화 기반기술을 개발함을 주된 목표로 HL7 메시지 서버 구축에 필요한 소프트웨어 프레임워크의 디자인 및 구현에 대해서 기술하였다. HL7 표준은 의료 환경에서의 전자적 데이터 교환을 위한 이벤트 중심의 어플리케이션 프로토콜로서, 메시지 단위로 정보의 전송이 이루어진다. 이와 같은 의료정보 교환의 기본 단위인 HL7 메시지를 만들기 위해서 연구진들은 중간 계층의 인터페이싱 방식의 유효성을 고려하여 인터페이스 엔진에 해당하는 HL7 메시지 서버를 구축하였다. HL7 메시지 서버는 병원점보시스템에 연결되어 약속된 이벤트가 발생할 때마다 해당하는 HL7 메시지를 자동적으로 생성하는 서버 시스템으로서, 현재 운영되고 있는 국내병원정보시스템에 HL7 표준을 쉽게 적용시킬 수 있는 최적의 솔루션을 제공해 줄 것이다.

본 논문에서는 의료정보 연동을 위한 보안 시스템을 설계하기 위해 의료정보 표준과 XML을 조사 및 분석하고, 이를 토대로 보안 시스템이 연동되는 서비스 흐름도 도출 및 시스템 설계를 하였다. 특히, 병원에서 질환관련, 식이관련, 운동관련 데이터를 제공하고, EMR 통합 Gateway를 통해 환자 데이터를 전송 받아 병원 간 개인건강정보 관리 모듈과 임상정보 관리 모듈을 통해 HL7표준을 이용하여 환자 정보를 XML 형태로 교환 하는 경우, 보안 시스템이 고려된다면 향후 다양한 응용 서비스에 안전하게 사용될 수 있을 것이다.
