  • 최근 XML 문서가 여러 분야의 시스템에서 정보 교환과 메시지 전송을 위한 표준으로 자리 잡으면서 의료계에서도 응용 프로그램 간의 메시지 전송 혹은 병원 간 환자의료정보를 교환하기 위한 XML기반의 정보 교환 프로토콜 표준화가 계속 진행 중에 있다. 하지만 이러한 환자의료정보가 인터넷을 통해 병원간에 이동할 때, 여러 보안상의 위협에 노출될 수 있다. 이에 본 논문에서는 기존의 보안 메커니즘의 한계점을 보완하면서 XML 기반의 정보를 보호하기 위한 가장 효율적인 메커니즘으로 인정받은 XML 보안기술을 적용한 환자의뢰시스템을 시스템을 구현하고 보안 메커니즘 적용 시 부득이하게 발생할 수 있는 성능 저하 정도를 알아보기 위해 성능 평가 테스트를 실시한다.

  • 본 논문에서는 병원간의 정보를 교환하기 위한 목적으로 정의된 의료 분야의 표준기관인 HL7에서 제안하는 Clinical Document Architecture(CDA)문서를 관리하기 위한 시스템을 제안한다. CDA Manager라는 이 시스템은 각 병원의 관계형 데이터베이스인 CDA local repository에서 CDA문서를 저장, 검색, 수정의 기능을 제공한다. 본 논문에서는 XML로 기술된 CDA문서의 특성을 살려 테이블을 설계하고 검색속도를 향상시켰다.

  • 병원 이외의 환경, 특히 댁내 기반의 헬스 모니터링 관련 연구는 최근 헬스케어 시스템 관련 연구자 및 개발자들의 많은 관심을 받고 있다. 심장병이나 당뇨병 같은 만성질환을 앓고 있는 경우에는 생리학적 요소들을 일상 생활 중 지속적으로 모니터링 하는 과정이 특히 중요하다. 본 논문에서는 댁내 환경에서 서비스되고 있는 헬스케어 시스템과 병원에서 운용되고 있는 헬스케어 시스템과의 연동을 지원하고, 클라우드 환경을 활용한 생리학적 데이터 분석을 지원하는 통합 헬스케어 시스템의 구조를 제안한다. 또한, 제안된 통합 헬스케어 시스템의 구조를 활용한 질병진단 시나리오도 제시한다.

  • 위암은 전 세계적인 주요 건강문제이며, 근치적 위절제술은 위암의 표준치료이다. 근치적 위절제술 후 치료목표는 합병증 발생을 낮춰 병전 상태로 빠르게 회복하는 데 있다. 따라서, 근치적 위절제술 후 합병증 발생 여부를 선별하여 예측할 수 있는 성능이 좋은 모델을 개발하는 것은 위암환자의 회복에 매우 중요하다. 랜덤포레스트 모델은 여러 개의 결정트리를 활용한 배깅 방식의 대표적인 알고리즘으로 의료 데이터를 기반으로 한 예측에 있어 뛰어난 성능을 보여 주었다. 그러나 실제 데이터는 불균형이 빈번하게 발생하여 모델의 예측 성능에 영향을 미치므로, 최적의 분류 임계값을 설정하여 다수 클래스에 대한 편향을 줄이는 것이 중요하다. 따라서, 본 연구는 최근 10년 간 일개 대학병원의 전자의무기록 데이터를 활용하여 근치적 위절제술 후 합병증 발생을 예측하는 랜덤포레스트 모델을 개발하고, 임계값 설정을 통해 불균형 데이터에 대한 모델의 성능을 평가하고자 한다.

  • The existing emergency medical systems are unable to obtain accurate clinical information, so most of the specialized first aid is being done within the hospital. In this paper, designed medical information transfer structure based on medical information standards HL7 for accurate first aid is proposed. The designed system can share clinical information and emergency medical information in terms of standard HL7 messages. Therefore, the correct first aid is available in steps taken by the hospital. In addition, the system of this paper has been implemented based on the OSGi service platform for the efficient integration with other health-related services.

  • Coronavirus disease (COVID-19) has threatened public health as a global pandemic. Chest CT and radiography are crucial in managing COVID-19 in addition to reverse transcription-polymerase chain reaction, which is the gold standard for COVID-19 diagnosis. This is a review of the current status of the use of chest CT and radiography in COVID-19 diagnosis and management and anㄷ introduction of early representative studies on the application of artificial intelligence to chest CT and radiography. The authors also share their experiences to provide insights into the future value of artificial intelligence.

A Research on State Infection Control for Medical Technician in Small and Medium-sized Hospitals (중소병원 의료기사의 감염관리 실태에 관한 연구)

  • This study aims to identify the awareness and performance level of hospital infection control in medical technicians working in small and medium-sized hospitals. The experiment was conducted in 235 medical technicians (medical laboratory technologist, radiological technologist, and physical therapist), and for statistical processing, SPSS Ver.23.0 was used to conduct mean and standard deviation, analysis of variance, and regression analysis. As a result, it was found that the major cause of improper infection control was lack of experts ($3.17{\pm}1.18$) and performance level was statistically significantly lower than awareness of infection control. "I took no measures after infection accident" was responded the highest at 40.0%(94 persons), and in "I reported to the head of department after disinfection, examination, and/or vaccination," the standardized regression coefficient ${\beta}$ was -0.265, p=0.025, which suggested that there was a significant negative (-) impact. Infection control level showed a significant positive correlation with training and infection control instructions. Therefore, it was found that medical technicians in small and medium-sized hospitals required high skilled manpower in infection control, training, infection control instructions, etc. Through this study, more effective infection control is considered to be possible.

  • The objective of this study was to predict the changes in the duration of hospital stay, hospital admission fee, costs of drugs, changes in laboratory cost, material cost, total medical cost, adjusted amount of treatment and the efficacy of obstetrics and gynecology DRG system. The cost of drugs showed the greatest change and was followed by materials for medical examinations and the change in methods of medical examinations. In the analysis of the quantity of medical service the profit of medical examinations were influenced mostly by the duration of hospital stay. The results and data in this study could be used as a basis of future DRG system protocols and will be utilized so that hospitals can build a efficient medical system.

  • Medical Gateway(MG), that integrates medical devices with its unique data format in various hospital information systems, provides effectiveness in the cost of the management unlike existing systems that uses PC as a gateway to hospital information systems because MG enables to interface medical devices with itself. However, in spite of using MG for integrating medical devices, we need additional programs or systems to interwork with EMR system of each hospital in accordance with the configuration of DB. In this paper, we design and implement the format of medical information and server system for interworking MG with EMR system. The proposed system makes easy exchanging medical data between MG and EMR system. Thus, the proposed system can contribute to a national project to export abroad advanced hospital setup converged with IT.

  • Renal artery injury is a rare complication in blunt trauma, but can cause devascularization of the kidney, leading to renal failure. It requires early diagnosis and management. The treatment of renal artery injury still remains controversial, but recent studies have reported the successful treatment outcome with endovascular stent placement. Nevertheless, there is no standard treatment strategy in cases of pediatric patients. We report a case of a 16-year-old girl with right renal artery occlusion associated with a grade IV liver laceration. She was treated with only balloon angioplasty, and the kidney showed marked improvement of parenchymal perfusion with normalized renal function. Treatment with only balloon angioplasty can be a treatment option in pediatric patients with renal artery injury.
