Superficial Radial Neuropathy due to Anatomic Variation: A Case Report (해부학적 변이로 인한 표재성 요골 신경병증: 증례 보고)

  • Superficial radial neuropathy is a purely sensory neuropathy, usually caused by nerve entrapment in the distal forearm. We report a case of superficial radial neuropathy caused by the anomalous course of the superficial radial nerve, which was found to be spirally encircling the brachioradialis tendon in the distal forearm. To the best of our knowledge, this is the first report of an anatomical variant of the superficial radial nerve that causes neuropathy.

Antifungal activity of essential oil from Cryptomeria japonica against dermatophytic fungi (삼나무 정유의 표재성 진균에 대한 항진균 활성)

  • In this study, the antifungal activity of Cryptomeria japonica essential oil against superficial and allergic fungi, Trichophyton schoenleinii, Trichophyton tonsurans, Trichophyton mentagrophytes, Trichophyton rubrum, Epidermophyton floccosum, and Aspergillus fumigatus, was evaluated for determining the potential compound as dermatitis treatment. Minimum Inhibitory Concentration (MIC) measurement, TLC bioassay and agar dilution methods were used for determining the antifungal activity of crude essential oil and its fractions from C. japonica. Also, their major constituents were analyzed by GC/MS. The MICs were below 500 ppm at all superficial fungi, and spot 1 of C. japonica essential oil showed highly effective antifungal activity by TLC bioassay. In antifungal activity by agar dilution methods, crude oil showed high antifungal effect at more than 500 ppm and fraction D was significantly effective at even 100 ppm except for A. fumigatus. The major compounds of spot 1 and fraction D of C. japonica oil determined by GC/MS were elemol, ${\gamma}$-eudesmol, and ${\beta}$-eudesmol, which could be used as atopic dermatitis treatment material.

  • Malignant tenosynovial giant cell tumor (TsGCT) is a rare disease that can arise as a recurrent lesion or co-exist with a benign TsGCT lesion. Here we report a rare case of malignant TsGCT in a 73-year-old male with a history of lymphoma. The tumor appeared as a superficial soft-tissue mass in the subcutaneous fat tissue of the left knee.

  • A dog with a history of long-term phenobarbital administration presented to Seoul National University Hospital for Animals with clinical signs of footpad ulceration and facial ulcerative dermatitis. Skin lesions (facial/footpad dermatitis), histopathologic signs (parakeratotic hyperkeratosis, edema and hyperplasia of the epidermis), and liver involvement (increased plasma liver enzymes and nodular lesions) suggested a diagnosis of superficial necrolytic dermatitis (SND). The patient died suddenly at home of unknown cause after 8 months medical treatment, but intravenous amino acid supplementation and supportive hepatic medication proved as effective for the skin conditions.

The Effects of Reducing a Dose on the Genital Gland at a CT Scan on the Whole Abdomen According to the Shielding Material (Whole Abdomen CT촬영 시 차폐 재료에 따른 생식선 선량 감쇠 효과)

  • The purpose of this study is to produce a shielding material to reduce a dose on the genital gland, one of the superficial organs, at a CT scan on the whole abdomen and hardly affect picture quality and examine its utility. This research made 22 mm silicone and 7.3 mm aluminum having the similar material quality and effect of previous bismuth. By using the non-shield, bismuth, 22 mm silicone, and 7.3 mm aluminum shielding materials, this author conducted a comparative experiment measuring the decay rate of the genital gland's exposure to radiation, change of the CT number and noise in the image, and the CT number, noise, and uniformity in the AAPM phantom. According to the results, exposure to radiation is reduced in bismuth as 29.96%, silicone 22 mm as 13.10%, and 7.3 mm aluminum as 18.27%. In bismuth, however, the image's CT number varies a lot, and uniformity is measured to be inappropriate in the AAPM phantom scan; therefore, it indicates great change in terms of picture quality in superficial organs like the genital gland. Concerning superficial organs like the genital gland, if 22 mm silicone and 7.3 mm aluminum are used as shielding materials, it will be helpful in reducing variation in picture quality and also decreasing radiation exposure to radiation.

  • Intravesical instillation of the bacillus Calmentte-Gu$\acute{e}$rin(BCG), an attenuated strain of Mycobacterium bovis, is an approved method for the treatment of superficial bladder cancer. Because BCG is a living organism, the potential for infection exists. BCG is generally well tolerated, with complications in less than 5% of those treated with use of current practices. The most frequent symptoms of toxicity associated with intravesical BCG immunotherapy include bladder irritation, frequency, and dysuria. Systemic reactions are less common but more serious than local side effects, and include fever, chills, malaise, rash, hepatitis, pneumonitis, arthritis and sepsis. In rare cases, BCG treatment can result in a systemic infection that requires antituberculous therapy. The pulmonary toxicity that results from intravesical BCG treatment is generally characterized by one of two types : systemic allergic reaction with pulmonary reticulonodular opacities depicted on chest radiographs with cellular findings consisting of activated lymphocytes, and actual BCG mycobacteremia with a miliary pattern depicted on chest radiographs and granuloma formation which rarely results in positive acid-fast stain or culture results. Recently we experienced two types of pulmonary complications following intravesical BCG immunotherapy in patients with superficial bladder cancer. We report two cases with a review of literatures.

T1-Staging for Urinary Bladder Cancer with the Stalk and Inchworm Signs with 3.0 Tesla MRI (3.0 테슬러 자기공명영상에서 Stalk 및 Inchworm Sign이 있는 방광암의 T1 병기 진단)

  • Purpose To evaluate the diagnostic utility of the stalk and the inchworm sign on preoperative MRI for detecting superficial bladder cancers, and to compare the diagnostic performance between the stalk and the inchworm sign. Materials and Methods We retrospectively reviewed 240 patients (505 tumors) who had undergone radical cystectomy. The tumors were classified as follows: superficial or invasive tumors indicated by the stalk or inchworm sign on 3.0 Tesla MRI. We evaluated the diagnostic accuracy of the stalk and inchworm signs, by comparing each finding with the postoperative pathologic T stage. We compared diagnostic performance between them statistically. Results The stalk and inchworm signs showed high specificity (93% and 91%, respectively), positive predictive values (89% and 90%, respectively), and acceptable accuracy (70% and 74%, respectively), but low sensitivity (54% and 61%, respectively) and negative predictive values (60% and 63%, respectively). There was no statistically significant difference between the two signs (p > 0.05). Conclusion Superficial bladder cancers could be differentiated from invasive tumors using the stalk or inchworm sign on MRI.

Clinical investigations of canine superficial pyoderma (개에서 표재성 세균성 피부염에 관한 임상적 고찰)

  • Fifteen dogs with superficial pyoderma were investigated clinically. Dermatological signs were mainly consisted of papule (66.6%), pustule (86.6%), epidermal collrarette and patchy (40%), and hyperpigmentation (53.3%). Distribution of skin lesion were consisted of back (35.5%), abdomen (29.0%), axillary (6.4%), leg (3.2%), neck (3.6%) and foot (16.1%), respectively. In pustular cytology PMN cells and cocci were examined. Cephalexin was very effective antibiotics on superficial pyoderma at administration of 30mg/kg bid P.O. for 3 weeks. Hyperadrenocorticism and atopy were diagnosed as a primary cause on pyoderma in 2 dogs.
