Evaluation of Usefulness of Automatic Exposure Control (AEC) by Comparison Analysis of Entrance Surface Dose (ESD) and Entropy in Clinical Application of Digital Radiography (DR) (디지털 방사선 시스템의 노출 유형에 따른 임상 적용 시 입사표면선량 및 Entropy 비교분석을 통한 자동노출제어장치의 유용성 평가)

  • The purpose of this study is to evaluate the usefulness of automatic exposure control (AEC) by analyzing entrance surface dose (ESD) and entropy on using automatic exposure and manual exposure. The experimental method was to measure the dose by placing a semiconductor dosimeter on the Rando Phantom for the Pelvis, Abdomen, Skull, and Chest regions. The DICOM file was simultaneously acquired and then entropy was analyzed by using Matlab. As a result, when using the automatic exposure control, dose of all sites was lower than manual exposure's dose and entropy was high. In addition, paired t-test was performed for each item and p<0.05 was found in each item. In conclusion, the use of automatic exposure control can be a useful method to contribute to the optimization of the exposure dose and the image quality by reducing the amount of unnecessary radiation amount and information loss that can occur in X-ray examination.

Dose Distribution in Solid Phantom by TLD with a Metal Plate of Various Thicknesses (다양한 두께의 금속판을 얹은 TLD를 이용하여 구한, 고체 팬텀 내에서의 선량분포)

  • Purpose: TLD experiments were set up to measure the dose distribution and to analyze the influence on dose measurement of thin metal plate and solid water phantom. The aim of the present study was to investigate the build-up effect of metal plate loaded on TLD chip and depth dose in the controlled environment of phantom measurements. Materials and Methods: Measurements were done by using LiF TLD-100 loaded by a thin metal plate with the same surface area (3.2$\times$3.2 $\textrm{mm}^2$) as TLD chip. TLD chips loaded with one metal plate from three different metal plate (Tin, Copper, Gold) of different thicknesses (0.1, 0.15, 0.2, 0.3 mm) were used respectively to measure radiation dose. Using the TLD loaded with one metal plate, surface dose and the depth dose at the build-up maximum region were investigated. Results: Using a metal plate on TLD chip increased the surface dose. Surface dose curve shows the dose build-up against equivalent thickness of metal to water. The values of TL reading obtained by using metal plate at depth of build-up maximum are about 8% to 13% lower than those obtained by normal TLD chip. Conclusion: The metal technique used for TLD dosimetry could provide clinicals information about the build-up of dose up to 4.2mm depth in addition to a depth dose distribution. The results of TLD with a metal plate measurements may help with decisions to boost or bolus certain areas of the skin.

A Comparison of Density and Patient Doses According to kVp and mAs Changes in General Radiography (일반촬영에서 kVp와 mAs의 변화에 따른 농도와 환자 선량 비교)

  • Low energy x-rays that occur in the low tube voltage radiography of general radiography are absorbed strongly in the body and do not aid image quality enhancement. This study maintains titer in general radiography while using tube current that are proportional to density and the tube voltage 15% principle according to density to reduce patient exposure doses, and area doses and entrance surface doses were measured to compare patient exposure doses. In hand, knee, abdomen, and skull radiography, kVp was increased to 115% and mAs was decreased to 50% and kVp was decreased to 85% while mAs was increased to 200% and area doses and entrance surface doses were measured to compare relative doses. Also, 5 places in each image were set, density was measured, and Kruskal wallis H test was conducted to observe significance probabilities between groups. To fix density, kVp was increased to 115% and mAs was decreased to 50% and after measurements of mean area doses and entrance surface doses were made by each part, each decreased to 58.68% and 59.85% when standard doses were set to 100%, and each increased to 147.28% and 159.9% when kVp was decreased to 85% and mAs was increased to 200%. Comparisons of density changes showed that hand, knee, abdomen, and skull radiography all displayed significance probabilities>0.05, showing no changes in concentration. Radiography that increases kVp and lowers mAs through reasonable calculations within ranges that don't affect resolution and contrast seems to be a simple way to decrease patient exposure doses.

Characteristics of Detectors for Measurements of Photon Depth Doses in Build-Up Region (선량보강(Build-up) 영역에서의 광자선 깊이선량률 측정을 위한 검출기의 특성 비교)

  • To determine the appropriate method out of various available methods to measure build-up doses, the measurements and comparisons of depth doses of build-up region including the surface dose were executed using the Attix parallel-plate ionization chamber, the Markus chamber, a cylindrical ionization chamber, and a diode detector. Based on the measurements using the Attix chamber, discrepancies of the Markus chamber were within $2\%$ for the open field and increased up to $3.9\%$ in the case of photon beam containing the contaminant electrons. The measurements of an cylindrical ionization chamber and a diode detector accord with those of the Attix chamber within $1.5\%\;and\;1.0\%$ and after those detectors were completely immersed in the water phantom. The results suggest that the parallel-plate chamber is the best choice to measure depth doses in the build-up region containing the surface, however, using cylindrical ionization chamber or diode detector would be a reasonable choice if no special care is necessary for the exact surface dose.

  • Pediatrics are more sensibility to radiation than adults and because they are organs that are not completely grown, they have a life expectancy that can be adversely affected by exposure. Therefore, the management of exposure dose is more important than the case of adult. The purpose of this study was to determine the suitability of the 10 year old phantom for the 5 year old pediatric's recommendation and the incident surface dose, and to measure the organ absorbed dose. This study is compared the organ absorbed dose and the entrance surface dose in the clinical conditions at 5 and 10 years old pediatric. Clinical 5 year old condition was slightly higher than recommendation condition and 10 year old condition was very high. In addition, recommendation condition ESD was found to be 43% higher than the ESD of the 5 year old group and the ESD of the 10 year old group was 126% higher than that of the 5 year old group. The recommended ESD at 5 years old and the ESD according to clinical imaging conditions were 31.6%. There was no significant difference between the 5 year old recommended exposure conditions and the organ absorbed dose due to clinical exposure conditions, but there was a large difference between the Chest and Pelvic. However, it was found that there was a remarkable difference when comparing the organ absorbed dose by 10 year clinical exposure conditions. Therefore, more detailed standard exposure dose for the recommended dose of pediatric should be studied.

Characteristics of 23 MV Photon Beam from a Mevatron KD 8067 Dual Energy Linear Accelerator (Mevatron KD 8067 선형가속기의 23 MV 광자선의 특성)

  • The characteristics of 23 MV photon beam have been presented with respect to clinical parameters of central axis depth dose, tissue-maxi mum ratios, scatter-maximum ratios, surface dose and scatter correction factors. The nominal accelerating potential was found to be $18.5\pm0.5$ MV on the central axis. The half-value layer (HVL) of this photon beam was measured with narrow beam geometry from central axis, and it has been showed the thickness of $24.5\;g/cm^2$. The tissue-maximum ratio values have been determined from measured percentage depth dose data. In our experimental dosimetry, the surface dose of maximum showed only $9.6\%$ of maximum dose at $10\times10\;cm^2$, 100 cm SSD, without blocking tray in. The TMR'S of $0\times0$ field size have been determined to get average $2.3\%$ uncertainties from three different methodis; are zero effective attenuation coefficient, non-ilnear least square fit of TMR's data and effective linear attenuation coefficient from the HVL of 23 MV photon beams of dual energy linear accelerator.

  • Beta ray $(^{90}Sr+^{90}Y)$ absorbed dose at tissue surface was measured from the distance of 30cm by use of extrapolation chamber. In the measurement, following factors were considered: effective area of collecting electrode, polarity effect, ion recombination and window attenuation. The measured absorbed dose rate at tissue surface was $1.493{\mu}Gy/sec$ with ${\pm}2.9%$.

  • In this study, maximum exposure rate at DAW(Dry Active Waste) drum surface which is satisfying regulation limit was calculated for conceptual design of IP(Industrial Package). DAW can be classified as combustible and non-combustible waste and the calculation was conducted for single and mixed radionuclide for each type of waste. In case of combustible waste that mixed radionuclide is uniformly distributed, the maximum exposure rates at drum surface were 3.60E-01, 8.85E-01 and 1.27E+01 mSv/hr for IP Type 1, 2-a and 2-b, respectively. and 3.60E-01, 8.85E-01, 1.27E+01 mSv/hr for single radionuclide(Co-60). In case of non-combustible waste that mixed radionuclide is uniformly distributed, the maximum exposure rates at drum surface were 7.14E-01, 1.83E+00, 2.69E+01 mSv/hr for IP Type 1, 2-a and 2-b, respectively. and 7.13E-01, 1.81E-01, 2.62E+01 mSv/hr for single radionuclide(Co-60). Through this study, the maximum amount of DAW can be transported by IP was suggested as maximum exposure rate at drum surface and the calculation for the other types of waste will be conducted.

The Enhancement of Skin Sparing by Tray Materials for High Energy Photon Beam (고에너지 광자선치료에서 고정판 흡수물질을 이용한 피부보호효과의 향상)

  • The skin sparing effect associated with high energy x-ray or gamma ray beams may be reduce or lost under certain conditions of treatment. Current trends in using large fields. Shield carrying trays, compensating filters, and isocentric methods of treatment have posed problems of increased skin dose which sometimes become a limiting factor in giving adquate tumor doses. We used the shallow ion chamber to measure the phantom surface dose and the physical treatment variables for Co-60 gamma ray, 4MV and 10 MV x-ray beam. The dependence of percent surface dose on field sizes, atomic number of the shielding tray materials and its distance from the surface for 4, 10MV x-rays and Co-60 gamma ray is qualitatively similar. The use of 2 mm thick tin filter is recommended for situations where a low atomic number tray is introduced into the beam at distances less than 15 cm from the surface and with the large field sized for 4 MV x-ray beam. In case of Co-60 gamma ray, the lead glass tray is suitable for enhancement of skin sparing. Also, the filter distance should be as large as possible to achieve substantial skin sparing.
