Exposure to Blood and Body Fluids and Influencing Factors on Compliance with Standard Precautions among Nurses in General Hospital (종합병원 간호사의 혈액 및 체액 노출과 표준주의지침 이행도에 대한 영향 요인)

  • The purpose of this study was to identify blood and body fluids exposure, and factors influencing compliance with standard precautions among nurses. Participants consisted of 144 nurses in three general hospitals located in C province. Data were collected using a questionnaire that consisted of awareness, compliance, safety environment and attitude toward standard precautions. A total of 40.3% of participants had been injured by syringes or sharp tools that had already been used for patients within the last 1 year 1.01 times. Moreover, 22.9% of participants had been exposed to blood and body fluid of patients 1.06 times within the last 1 year. The mean score for compliance with standard precautions was lower than its awareness. There was a significant correlation between compliance and awareness and a safe environment. Multiple regression revealed an adjusted $R^2$ of 0.166 with the awareness score serving as the major predictor variable for compliance of standard precautions. Therefore, it is necessary to develop strategies to apply to systemic and continuous education regarding standard precautions and to foster a safe environment to enhance nurses' compliance with standard precautions.

  • The Asian ladybird, Harmonia axyridis, might be distinguished in biological control agents for the several aphid species in the greenhouses as well as in the fields. Therefore, feeding behavior and predacious capability characteristics were observed. When ladybirds are searching for aphids as their preys, they use the maxillary palpus as contact reception. Otherwise, most of aphids escape from walking or chewing ladybird to other place (mainly move to near-by leaf, or fall down to the under-leaf or ground) instead of plant sucking. Consumed time of ladybirds for searching and feeding prey were significantly different and dependent on their development stage. The ladybird adults ate about 120 green peach aphids and 257 cotton aphids per day. The 3rd instar of ladybirds ate 2,420 and 2,750 eggs of greenhouse whitefly on the cucumber and tomato leaf, respectively, as well. However, ladybird instars taken greenhouse whitefly eggs could not continuously develope to adult, on the other hand, ladybirds taken aphids were normal condition.

A Clinical and Epidemiologic Study on Varicella in Children (수두의 임상 및 유행 양상에 대한 연구)

  • Purpose : Varicella is a common contagious illness and vaccine against it has been used in healthy Korean children. We studied clinical and epidemiologic features of varicella and the effect of vaccination. Methods : We analyzed clinical and epidemiologic features of varicella and vaccination effect against it in children treated in Hangang Sacred Heart Hospital from March 1994 to December 1996. Results : 1) Total 73 cases of varicella, 46 male and 27 female patients were included. Annual incidence was 31, 20 and 22 cases from '94 to '96 respectively, with an increase during the period June to July and December to January. 2) Mean age was $46{\pm}37.2$ months and 84.9% of the patients were younger than 7 years old including 16.4% of infants. 3) Varicella occurred after a contact with varicella patients in 40(54.8%) patients. 4) Fever developed in 44(60%) patients with mean $38.4{\pm}0.8^{\circ}C$ and persisted for mean $3{\pm}2.4$ days. Nine(12.3%) patients had complications and eighteen(25%) patients were admitted for mean $4{\pm}2.1$ days but all recovered. 5) Twenty(27.8%) patients were affected despite preceding vaccination against varicella but the number of vesicles decreased significantly in vaccinees(P<0.01). 6) The rate of secondary cases in healthy siblings was 27 out of 35(77.1%) and was not affected by preceding vaccination. Conclusion : Varicella occurred mainly in children younger than 7 years old and showed seasonality. Because 27.8% of cases developed despite preceding vaccination and the rate of secondary cases was not affected by vaccination, further study for vaccine efficacy is necessary.

Ankle Taping and Vertical Drop Landing (발목관절의 테이핑과 수직 착지동작)

  • 발목에 대한 유착성 테이핑의 적용은 물리치료 분야와 운동 트레이닝 분야에서의 일반적인 치료 접근 방법이라 할 수 있다. 즉 발목 염좌 같은 손상을 예방하기 위한 방법과 재활의 부가적인 치료 형태로 이용될 수 있는 것이다. 본 연구는 발목의 테이핑과 트레드밀에서의 30분간 보행이 수직 착지 동작 동안 하지의 운동 역학적 요소들에 어떠한 영향을 주는지 알아보기 위해 시행되었다. 14명의 신체 건강한 대상자들 (남:10, 여:4)이 본 연구에 참여하였다. 한발로 서있는 동안 40 cm의 높이로 뛰게 하였다. 가자미근, 전경골근, 안쪽넓은근, 넙다리 두갈래근에 근전도 전극이 부착되었다. 테이핑된 신체 부위로 착지하는 동안 근전도와 힘판(force plate) 자료가 500 Hz의 주파수로 수집되었다. 대상자들의 우세쪽 하지 발목에 테이핑을 하였고, 대상자는 한발로 뛰어 내리는 동작을 반복하였다. 트레드밀에서 30분 동안 발목에 테이핑을 한 채로 보행하였고 이후 한발로 뛰어 내리는 동작을 반복하여 수행하였다. 테이핑을 적용한 상태와 테이핑한 채로 30분 동안 트레드밀에서 걷기 운동을 한 조건에서 수직 지면 반발력의 통계학적으로 유의미한 증가가 있었다. 착지 동안에 네 가지 상황에서의 최대 내측 가속도의 유의한 차이가 있었고, 그 차이는 테이핑 전과 테이핑을 한 채로의 운동 조건에서 유의한 차이가 있었다. 테이핑한 상태에서 가자미근은 접촉되는 착지 순간부터 최대 수직 지면 반발력이 나타나는 동안의 평균 근전도의 수치에 있어서 유의한 감소가 있었다. 반면에 다른 근육들에서는 아무런 변화를 보이지 않았다. 비록 발목 테이핑이 발목 손상의 위험을 줄이는데 효과적이라고 일반적으로 인식되고 있으나 이번 실험의 결과는 30분 동안의 트레드밀에서의 보행 후 테이핑은 발목의 움직임을 제한하는 효과는 있지만 한편으로는 가자미근의 활동을 감소시키는 것을 보여주었다. 따라서 이 연구의 결과는 테이핑을 하면 점프와 착지로 이어지는 동작 중에 발목관절 손상 위험을 증가시킬 가능성이 있음을 보여주었다.

A Study on Family Involvement in Therapeutic Process of the Mentally Ill (정신장애인 가족의 치료참여에 관한 연구)

  • The main premise of this study is that family involvement which is based on professional-family collaborative partnership is very important for successful treatment and rehabilitation of the mentally ill. Accordingly, this study aims to find out factors which are related to family involvement in therapeutic process. This study conducted the survey with 275 service providers(i.e. mental health professionals) and with 124 service consumers(i.e. family members) in order to discover in what extent do related factors affect on the family involvement. The major findings of the study are; 1) From the service providers standpoint, using multiple regression, job characteristics, perception of family culpability, frequency of contact with family, functioning of their organization affect significantly on the family involvement by 33.0% of explanatory power. Using a step-wise multiple regression, the most powerful influenced factors on family involvement are frequency of contact with family, type of agency, type of profession, and their career. 2) From the service consumers standpoint, using multiple regression, socioeconomic level, attitudes of professionals, optimistic perception of prognosis, functioning of their family, diagnosis affect significantly on the family involvement by 40.9% of explanatory power. Using a step-wise multiple regression, the most powerful influenced factors on family involvement are economic level, attitudes of professionals, optimistic perception of prognosis, educational level. In conclusion, family accessibility to the therapeutic system must be allowed and further, politically encouraged by the related legislation. Also this study recommends clinician to use an empowerment model for the families with the mentally ill.

Study of the re-osseointegration of implant fixture after mechanical unscrewing (임플란트 고정체의 의원성 동요 후 골 유착 반응에 관한 연구)

  • Purpose: The purpose of this study was to investigate whether the re-osseointegration of the implants that had mechanical unscrewing possibly occurred or not. Furthermore, if it happened, the degree of re-osseointegration was evaluated by comparing with previous osseointegration. Materials and methods: The smooth implant (commercial pure titanium 99%) specimens, whose diameter and length was 3.75 mm, 4 mm, respectively were produced. Two implants were inserted into each tibia of 7 New Zealand female white rabbits weighing at least 3.0 kg. The torque removal force for each implant after 6 weeks of implants placement was measured and included in group I. The torque removal forces were assessed after the fixtures were re-screwed to original position and the subjects were allowed to have 4 more weeks for healing and included in group II. One rabbit was sacrificed after first measurement and produced 4 slide specimens in group I, and two rabbits were sacrificed after 2nd measurement, 7 slide specimens, in group II for histomorphologic investigations. All slide specimens were assessed based on the proportion of BIC (bone-implant contact) as well as CBa (Bone area in the cortical passage) value produced by counting the screw threads embedded in the compact bones under the optical microscopic analysis (${\times}20$). Statistical analysis was conducted to evaluate the torque removal force, BIC and CBa between group I and II. Results: As for the torque removal force, the result was $10.8{\pm}3.6$ Ncm for group I and $20.2{\pm}9.7$ Ncm for group II. Furthermore, the torque removal force of group II increased by 98.1% in average compared to group I (P<.05). On the other hand, histomorphologic analysis displayed that there was no statistical significance in BIC and CBa values between group I and the group II (P>.05), and RT/BIC and RT/CBa between group I and group II were statistically significant (P<.05). Conclusion: It is possible to obtain more substantial re-osseointegration within shorter periods than the period needed for the initial osseointegration in case of iatrogenically unscrewed implants.

Occlusal Analysis of the Subjects with Chewing Side Preference Using the T-Scan II System (T-Scan II 시스템을 이용한 습관적 편측저작자들의 교합 분석)

  • While orofacial pain or various dental factors are generally considered as the primary cause of unilateral chewing tendency, there exist several studies indicating that dental factors did not affect the preferred chewing side. The aim of this study was to examine difference of occlusal scheme between the subjects with and without chewing side preference. The difference between the chewing and non-chewing sides in the unilateral chewing group was investigated as well. Computerized, T-Scan II system was used for occlusal analysis. 20 subjects for the unilateral chewing group (mean age of $25.25{\pm}2.84$ years) and 20 subjects for the bilateral chewing group (mean age of $27.00{\pm}5.07$ years) were selected by a questionnaire on presence or absence of chewing side preference and those with occlusal problem or pain and/or dysfunction of jaw were excluded. T-Scan recordings were obtained during maximum intercuspation and excursion movement. The number of contact points, relative occlusal force ratio between right and left sides, tooth sliding area and elapsed time throughout the maximum intercuspation were calculated. Elapsed time for excursion was also investigated. The results of this study shows that the unilateral chewing group had the smaller average tooth contact areas compared with those of the bilateral group (p<0.005). In the unilateral chewing group, the contact areas of non-chewing side are smaller than those of chewing side (p<0.005). The contact areas on their preferred sides were not significantly different with those of right or left side of the subjects without chewing side preference. There was no significant difference in the elapsed time during maximum intercuspation and lateral excursion, the sliding areas and relative of right-to-left occlusal force ratio between the two groups. From the results of this study, it is likely that individuals prefer chewing on the side with more contact areas for efficient chewing.

The Comparison Research on Walking Pattern of Rehabilitation Training Program Participants in Stroke Patients (재활운동에 참가한 뇌졸중 환자들의 걷기형태 비교 연구)

  • The purpose of this study was to analyse the gait characteristics of stroke patients walking on a Zebris system, through quantitative three-dimensional biomechanical analysis. They underwent a continuous rehabilitation training program (RTP). A comparison was made between 3 month and 6 month RTP participants. Their ages were between 60 and 65. The data were analyzed by t-test. The result of comparative analysis of the two groups can be summarized as below. Temporal-spatial data, sagittal plane angular kinematics data, and peak ground reaction force and max pressure data showed that there were no significant differences between the 3 month RTP group and the 6month RTP group (Table 2, Table3, Table 4). It can be suggested that patients with hemiplegia after stroke can improve their walking function through continuous RTP participation.

Load capacity simulation of PTO gears for a small cultivator during rotary ditching operation (구굴 작업에 따른 소형 관리기의 PTO 기어 부하 용량 시뮬레이션)

  • 지속적인 고령화 추세에 따라 여성과 고령층의 노동력이 차지하는 비율이 높아지고 있어, 사용하기 편리한 소형농기계의 요구가 증가하고 있는 실정이다. 본 연구에서 사용한 소형 관리기는 정식 작업 전 경운, 정지 등 다양한 작업이 가능한 농기계이다. 본 연구에서는 소형 관리기에 토크 측정 시스템을 구성하였으며, 작업 중 가장 큰 부하를 받는 구굴 작업 부하를 측정하였다. 또한, 작업 시 가장 직접적으로 영향을 받는 PTO (Power Take Off) 기어의 부하 용량(load capacity)을 기어 해석 소프트웨어를 이용하여 분석하였다. PTO 기어의 부하 용량은 안전율, 피로수명을 대상으로 평가하였다. 측정된 부하 데이터는 변동 하중이므로, 부하 크기와 빈도수의 규칙적인 신호로 단순화하기 위하여 레인플로우 카운팅 방법을 사용하였으며, SWT (Smith-Watson-Topper) 방법을 이용하여 공칭 토크를 계산하였다. PTO 기어의 안전율은 ISO 6336, 피로 수명은 마이너 법칙(Miner rule)을 이용하여 계산하였다. PTO의 변속 단수 총 2단이며, 5개의 스퍼 기어로 구성되어 있다. 시뮬레이션 결과, 소형 관리기의 주행 속도 또는 PTO의 회전속도 증가에 따라 PTO에서 발생하는 평균 부하가 크게 나타났다. 또한 주행 단수 및 PTO 기어 단수가 증가할수록 기어의 안전율과 피로 수명이 감소하였으며, 특히 PTO 기어의 안전율은 접촉 응력에서의 안전율보다 굽힘 응력에서의 안전율이 급격하게 감소하였다. 소형 관리기의 PTO 수명은 주행 단수 2단, PTO 단수 2단 일 때 가장 적게 나타났다. 따라서 소형 관리기의 주행 속도와 PTO 회전 속도를 저속으로 작업하는 것이 PTO 기어의 수명에 더 유리할 것으로 판단된다.

An Experimental Study on the Treatment of Organic Wastewater by Soil Microbes Using Three-phase Fluidized Bed (삼상유동층 반응기에서 토양미생물에 의한 유기성 폐수처리에 관한 연구)

  • In this study, isolated and cultured nitrogen fixed microbes were seeded in the three-phase fluidized bed in which gas, solid and liquid were contacted directly. Input velocity was varied from 8.12 cm/hr to 16.32 cm/hr. And upflow gas pressure was fixed to 80 psi. Return ratios were from 0.2 to 0.6 with the each experimental condition. According to these condition, movement of media, growth of biofilm and removal efficiency were measured. As the results, in case of briquette ash, biofilm was developed to $170{\mu}m$ when velocity was 8.12 cm/hr and return ratio was 0.6. In this condition, COD removal efficiency was 97% and $NH_4$-N removal efficiency was 83%. At the same condition, biofilm thickness of glass bead was $17.59{\mu}m$ and its COD and $NH_4$-N removal efficiency was 83% and 72%. Nitrogen fixed microbes have following characters: it formed dark-brownish sludge, excellent adhesive force, easy solid-liquid separation and low oxygen uptake ratio, but sensitive to DO concentration. Not only it endured shock loading, but required short time to steady state.
