인터넷의 비약적인 발전, 네트웍 인프라의 향상으로 인해 인터넷을 이용한 문서 처리는 날로 증가하고 있으며, 이 기종간의 문서 정보 교환 역시 계속해서 증가하고 있다. 또한 문서의 표현 방법 및 문서량의 증대로 사용자의 요구는 계속 다양해져 가고 있다. 기존의 웹에서 사용되고 있는 HTML(Hyper-Text Markup Language)의 경우 그 표현력의 한계 때문에 사용자가 직접 구조적인 문서를 생성할 수가 없었다. 이에 W3C에서 XML (eXtensible Markup Language)을 발표하여 사용자가 직접 논리적인 문서의 작성하는 것이 가능하도록 했다. 이러한 XML 문서의 논리적인 구조를 표현하기 위해서는 문서의 원형을 정의할 수 있는 DTD(Document Type Definition)가 필요하며, 본 논문은 이러한 XML 문서의 원형을 정의하기 위해 필요한 DTD를 보다 효율적이며 쉽게 작성할 수 있는 DTD 편집기의 구현 및 설계에 대하여 논술한다.

In an attempt to eliminate some negative aspects of conventional extensive laminectomy, 4 cases of multiple level of cervical compression myelopathy, 1 OPLL(ossification of posterior longitudinal ligament) and 3 cervical stenosis, were treated with a technique of expansive laminoplasty. Operative results in all patients were satisfactory without surgical complications and all patients had a neurological improvement. We suggest that our technique is more effective one for cervical canal enlargement with preservation of stability for treating multiple level of cervical canal stenosis, OPLL, and spondylosis than conventional extensive laminectomy.

Incontinentia pigmenti is characterized by irregular linear blisters with erythematous lesions which are developed from birth or later, verrucous papules on the extremities which are noted after a few weeks or months, and the appearance of streaks, spots and swirls of hyperpigmentation. This disorder is known as a systemic disorder caused by a defect at the developmental stage of organs originated from ectoderm or mesoderm. We experienced an incontinentia pigmenti in a male newborn infant who did not have any family history and had cutaneous lesions, which were in bullous and verucous stages, ophthalmic problems, and neurologic abnormalities.

Toxic epidermal necrolysis is a reactive erythema of nonstaphylococcal origin characterized by a scalded appearance of the skin. The TEN is widely regarded as a variant of severe erythema multiforme because of its acute course, its freguent common cause, its freguent overlap with Stevens-Johnson disease, and its histologic identity. I present a case of TEN with severe mucosal involvement resembled Stevens-Johnson disease.

Idiopathic hypoparathyroidism is frequently associated with intracranial calcification and neuropsychiatric abnormalities. The most commonly recognized central nervous system manifestations of chronic hypoparathyroidism are seizure, alteration of mental function and extrapyramidal signs. We present a case of hypoparathyroidism, demonstrating extensive intracranial calcification, not only basal ganglia, but also outside the extrapyramidal system. An 58-year-old woman presented with 30 year history of seizure and memory disturbance. The physical examination and several laboratory studies disclosed normal. However hypocalcemia, hyperphosphatemia with hypocalciuria and decreased parathormone level were demonstrated. Clinical symptoms and signs showed improvement after supplementary calcium and Vit $D_3$ therapy.

A 25 years old male patient was experienced Trough method operation to correct of Tracheal stenosis following Tracheostomy. After admission, a No.3 Magill plastic portex tube had been placed in the previous tracheostomy opening. Stenotic narrowing portion was noted low trachea (about 3.5cm above carina) Despite of narrowing of the lesion, anesthetic course was uneventful with oxygen, halothane and non depolarized muscle relaxant by controlled respiration. Postoperative coures were also favorable With adequate respiratory cares and blood gas analyzed.

Myopathy in primary aldosteronism is relatively rare disease in Korea. A 42-year-lod woman with hypokalemic periodic paralysis, proximal muscle weakness and hypertension was found to have myopathy associated with adenoma in the right adrenal gland. She showed marked elevation of muscle enzymes and myopathic pattern in EMG.

Osteogenesis imperfecta has been categorized as a heritable disorder of connective tissue affecting both bone and soft tissues and is characterized by fragility of bone, blue sclerae, and deafness, less frequently by dentinogenesis imperfecta and laxity of ligament. The goals of orthopedic management with osteogenesis imperfecta are the treatment of acute fractures and long-term rehabilitation in an effort to maintain ambulation. We report a case of osteogenesis imperfecta with anterior bowing of tibia which was successfully corrected by multiple osteotomies and intramedullary fixation by Sofield method and reviewed literatures.

위장관에서 분비되는 많은 국소호르몬 중에서 이미 널리 알려진 것을 외에 구조적인 공통점을 가진 새로운 group인 pancreatic polypeptide family에 대한 정보를 요약하여 보았다. PP, NPY 및 PYY는 그 구조적 유사점들에도 불구하고 분비되는 장소가 판이하여, PP는 위와 췌장의 국소호르몬으로서, NPY는 중추 및 말초신경의 신경충동전달체로서, 또 PYY는 하부위장관 점막에서부터 분비되어 전신 호르몬처럼 작용하고 있다. 이들 NPY와 PYY의 혈관수축작용과 혈압상승작용은 우리의 흥미를 끌기에 충분하다. 특히 PYY는 주로 하부장관 특히 결장과 직장의 분비조직에서 많이 발견된다는 점과 돼지의 뇌혈관을 수축시켰다는 점이 주목할 만한 것이다. 우리나라에는 많은 고혈압 환자가 있으며, 고혈압을 비롯한 여러가지 원인에 의한 뇌졸중(뇌출혈 포함) 환자도 많이 발생하고 있다. 민간의 속설에 의하면 출혈성 뇌졸증이 기온이 낮은 계절에 보다 자주 발생하는 경향이 있으며, 이른아침 화장실에서의 배변시 자주 발생한다고 한다. 물론 이러한 속설을 뒷받침할 만한 자료가 없으므로 학문적 견지에서의 신빙성 여부를 논할 가치는 없겠으나, 배변시 비교적 강한 자극을 받고 활발한 운동을 하는 결장과 직장에 PYY의 분비세포가 집결되어 있다는 점과 PYY의 뇌혈관수축작용 및 혈압 상승작용 등을 고려해 볼 때, 출혈성 뇌졸증과 PYY의 상관관계는 추구해볼 만한 사항이라 사료된다.

The efficacy of injection sclerotherapy for treatment of acute esophageal variceal bleeding is well established. But several complications of endoscopic sclerotherapy have been reported. One of the complications is mesenteric venous thrombosis which develops when vasopressin is used for the sclerotherapy. We report a case of superior mesenteric venous thrombosis which developed after endoscopic sclerotherapy for control of esophageal variceal bleeding.
