특발성 치은증식증의 치험례 (IDIOPATHIC GINGIVAL HYPERPLASIA)

  • Idiopathic gingival hyperplasia is a rare condition of undetermined etiology. The enlargement is usually associated with the emergence of the teeth into the oral cavity and may regress after extraction. The enlarged gingiva may be primarily attributed to hyperplasia of the subepithelial layer that is relatively avascular and consists of densely arranged collagen bundles and numerous fibroblasts. The recommended time for treatment is after completion of eruption of permanent teeth. But the most important thing is the patient's psychological and esthetic needs. Lately, Schluger has proposed modified gingivectomy procedure with horizontal, internal beveled incision for thinning of the flap resulting in less pain and bleeding after treatment, minimal opportunity of infection. The purpose of this report is to document a case of 8-year-old girl who had registered in Dept. of Pediatric Dentistry of Seoul National University dental hospital for treatment of her gingival hyperplasia and delayed tooth eruption

  • 본 증례에서는 척추측만증과 척추후만증과 이에 동반한 증상에 대해 음양균형장치와 한약 처방을 통해 증상이 호전되는 결과를 관찰하였다. 음양균형장치를 통해 턱관절을 조절하는 기능적뇌척주요법의 치료가 척추의 구조적 불균형을 개선시킴을 관찰하였다.

  • Objectives : The purpose of this study is to report the effect of muscle energy technique(MET) on idiopathic scoliosis with double curved thoracolumbar spine. Methods : Two patients with idiopathic scoliosis with double curved thoracolumbar spine were treated by MET. To investigate the outcome of the patients' improvement, we observed the standing X-ray of cervical, thoracic, lumbar spine. And Cobb's angle, correctability and visual analog scale(VAS) were used to measure changes during treatment. Results : After treatment, Cobb's angle, correctability and VAS were improved significantly. Conclusions : MET showed significant improvement to idiopathic scoliosis with double curved thoracolumbar spine. There is limitation on this study due to insufficient number of cases. Further research should be followed.

특발성 파킨슨병 환자의 전정유발근육전위 (Vestibular Evoked Myogenic Potential in Idiopathic Parkinson's Disease)

  • Background: Idiopathic Parkinson's disease (IPD) is closely related to Lewy body pathology. Pathological changes in medullar oblongata and pontine tegmentum have been reported in patients with subclinical motor symptom. Vestibular evoked myogenic potential (VEMP) is mediated by vestibular nuclei in lower brainstem and reflects the function of lower brainstem. The purpose of our study is to estimate the lower brainstem function in IPD patients. Methods: Ten patients with idiopathic Parkinson's disease underwent VEMP test. The patients were divided into Hohn-Yahr (H-Y) stage I (unilateral motor involvement) group and H-Y stage II or more severe (bilateral motor involvement) group. VEMP results were compared between groups using Mann-Whitney U test. Results: Among patients, 6 patients showed abnormal VEMP (unilateral abnormality 2, bilateral abnormalities 4). Between H-Y stage I group and H-Y II,III group, there was no statistical difference in the results of VEMP. Conclusions: We concluded that the lower brainstem dysfunction reflected in VEMP could occur in IPD regardless of the progression of the disease.

매선요법을 병행한 특발성 동안신경마비 한방치험 1례 (A Case of Idiopathic Oculomotor Nerve Palsy Treated with Korean Medicine Including Needle-embedding Therapy)

  • Objectives : The aim of this study is to report a case of idiopathic oculomotor nerve palsy improved by Korean Medicine therapy including needle-embedding therapy. Methods : We treated a 69-year old male patient who has Rt. ptosis, Rt. eye movement impairment, and dizziness with various Korean medical therapy including acupuncture, Bojungikgitanggami, and Hominis Placenta pharmacopuncture. To promote recovery, we additionally performed needle-embedding therapy to periocular acupoints. Results : Ptosis started to remarkably improve after performing needle-embedding therapy. After 54 days of treatment, ptosis and eye movement impairment of Rt. eye were significantly improved similar to the normal eye. Dizziness was cured in a few days after administration. Conclusions : This case suggests Korean medicine including needle-embedding therapy is effective against oculomotor nerve palsy symptoms especially for ptosis.

  • Objective : Oculomotor nerve palsy is a disorder which causes eyeball movement trouble, diplopia, dizziness and eyelid ptosis. It is caused by aneurysm, mass, trauma, infection and so on. But sometimes we can't establish the cause. We observed 3 cases of idiopathic oculomotor nerve palsy patient treated with hominis placenta pharmacopuncture. Method & Result : We treated three idiopathic oculomotor nerve palsy patients with hominis placenta pharmacopuncture and electroacupuncture. As the result, symptoms of three patients were improved remarkably. Conclusion : In this cases, the hominis placenta pharmacopuncture is effective on idiopathic oculomotor nerve palsy. We need further study about idiopathic oculomotor nerve palsy and hominis placenta pharmacopuncture.

  • Although the atlantoaxial joint is the most common site of rheumatoid arthritis, our patient had no symptoms or signs suggestive of rheumatoid arthritis. The atlantoaxial joint is frequently involved in degenerative osteoarthritis, especially in elderly patients. An 81-year old man presented with severe intermittent electric shock like, lancinating pain from the occipital to the temporal and parietofrontal areas. He also had neck pain and a limited range of motion. After many examinations and laboratory tests, at the department of neurology and neurosurgery, he was diagnosed with idiopathic neuralgia. The diagnosis of atlantoaxial joint syndrome was confirmed, and treated successfully with atlantoaxial joint block.

정형 물리치료와 근막 이완요법이 특발성 척추 측만증의 개선에 미치는 영향 (The Effect of Orthopedic Physiotherapy and Fascia Relaxation Treatment on the Improvement of Idiopathic Scoliosis)

  • Background: The purpose of this study is to find out the effect on the improvement in idiopathic scoliosis when applying the orthopedic manual physical therapy and myofascial release technique. Methods: 20 patients in Cheon-An ${\bigcirc}{\bigcirc}$ hospital were selected as subjects whose Cobb's angle were higher than $10^{\circ}$ according to the radiograph. They were treated for 60 minutes a day, three days a week, and 8 weeks so that the total time of visit would be 24. We performed five tests (Moire test, X-ray, standing height test, Cobb's angle, pelvic level test, VAS) to indentifiy the therapeutic effects. Results: Four test results were improved more remarkably after those treatment than before(p<.001), except pelvic level test (p>.001). Conclusion: we confirmed that there were correcting effects on most of the valuation index from the primary analysis, which divided the subjects into 2 groups by Cobb's angle, but there were more effective in patients having lower than $25^{\circ}$. There were similarly correcting effects on most of the valuation index according to the secondary analysis, but there were particularly more effective in children.

청소년기 특발성 척추측만증의 중재에 관한 연구 (A Study of Intervention for Adolescent Idiopathic Scoliosis)

  • Many methods have been described for the early intervention of adolescent idiopathic scoliosis. Adolescent idiopathic scoliosis is lateral and rotational spinal curvature in absence of associated congenital or neurologic abnormalities, the most common type of scoliosis observed in child and young adults, and refers to curves that develop after the age of $10{\sim}18$. The curves of adolescent idiopathic scoliosis have the potential to progress rapidly during growth. Curves are currently universally measured by the Cobb's method and Ferguson method. Some curves do not remain small, these may be mildly or severely progressive and the ribs on the convex side of the curve separate, and those on the concave side ribs approximate so rib undergoes deformation with rib humping. The latter may make angles that can affect vestibular system, balance, sensory, especially cardipulmonary function. Intervention for adolescent idiopathic scoliosis is based on the patient's age, the angular value of the curve, the maturity of their skeleton, and the topography. The purpose of intervention for adolescent idiopathic scoliosis consists of knowing how to go to the best approach the correction of the lateral curve and rotational deformity holding the achieved for the remainder of spinal growth, preventing significant cosmetic abnormality, pain and cardiopulmonary complication, control the muscle imbalance and proprioceptive postural disturbances, be less need for radical surgery to avoid early surgery.

특발성 식도비후환자에서 EEA Stapler를 사용한 식도위문합 후에 발생한 식도협착 -1례 보고- (A case Report of Esophageal Stricture after Esophagogastric Anastomosis with EEA Stapler in a patient of Idiopathic Muscular Hypertrophy of Esophagus)

  • Since Steichen and Ravich`s pioneer work in 1972 proved that staples reduced anastomotic leaks and operative time, the use of EEA stapler`s in esophagogastrostomy have gained acceptance and popularity. But overriding these benefits are the high stricture rate, which leads to the reappearance of dysphagia. The mechanism for the development of stricture in stapled anastomosis is likely to be due to the lack of mucosa to mucosa apposition and presence of necrotic tissue between the luminal edge and the rows of the stapler. All strictures were easily dilated. Recently, we encountered a patient who suffered from an esophageal stricture that slowly developed 10 months after an esophagogastric anastomosis with a EEA 425 was performed due to severe muscular hypertrophy of esophagus. Because the stricture failed to respond to the Bougienage, we reoperated using a EEA 28 this time. We feel that this case review helps to show that despite the very low leakage rate in small size EEA stapler, there is also a very high risk of stricture.
